Comparative effectiveness of cognitive behavioural therapy combined with exercise versus exercise in the management of non-specific chronic low back pain: A systematic review with meta-analysis
Copyright © 2018 Paapa Kwesi Ampiah et al. Aims: To systematically review the evidence for the effectiveness of combining cognitive behavioural therapy (CBT) and exercise versus exercise alone in the management of patients with non-specific chronic low back pain (NSCLBP).
Methods: Electronic search of CINAHL, PUBMED, Sports Discuss, SCOPUS, AMED, MEDLINE, Cochrane Central Register of Controlled Trials, and EMBASE, between 1990 – July 2017; complemented by hand searching of citation lists and citation tracking. Two independent reviewers screened titles and abstracts from the retrieved search results. Studies were considered based on PRISMA guidelines. Data was extracted based on Cochrane the Handbook of Systematic Reviews guidelines; the Cochrane Data Collection Form for Intervention Reviews (RCTs only), was customized and utilized. Risk of bias assessment was undertaken utilizing the Cochrane Back Review Group recommendations employing two independent reviewers. Meta-analysis was used to produce a weighted average for primary outcome measures, namely pain and disability. Results: Four studies were included (n = 406 participants); all studies provided post intervention results on pain and disability. Meta-analysis showed no significant difference between groups for both pain and disability, post intervention in the short term (pain; SMD -0.02, 95% CI -0.23 to 0.19; disability; SMD 0.06, 95% CI -0.15 to 0.27); medium term (pain; SMD -0.01, 95% CI -0.24 to0.22; disability; SMD 0.00, 95% CI -0.23 to 0.23); and long term (pain; SMD 0.06, 95% CI -0.18 to 0.29; disability; SMD -0.06, 95% CI -0.39 to 0.27).

Conclusion: The findings from this review reveal that there is no significant difference between groups; that is, there is moderate level evidence that the addition of CBT to exercise for patients with NSCLBP does not improve pain and disability outcomes in either the short, medium, or long term; however, both interventions (CBT plus exercise and exercise alone) produce favourable outcomes
Structural basis of a redox-dependent conformational switch that regulates the stress kinase p38α
Many functional aspects of the protein kinase p38α have been illustrated by more than three hundred structures determined in the presence of reducing agents. These structures correspond to free forms and complexes with activators, substrates, and inhibitors. Here we report the conformation of an oxidized state with an intramolecular disulfide bond between Cys119 and Cys162 that is conserved in vertebrates. The structure of the oxidized state does not affect the conformation of the catalytic site, but alters the docking groove by partially unwinding and displacing the short αD helix due to the movement of Cys119 towards Cys162. The transition between oxidized and reduced conformations provides a mechanism for fine-tuning p38α activity as a function of redox conditions, beyond its activation loop phosphorylation. Moreover, the conformational equilibrium between these redox forms reveals an unexplored cleft for p38α inhibitor design that we describe in detail.© 2023. The Author(s)
Impact of Tumor LINE-1 Methylation Level and Neoadjuvant Treatment and Its Association with Colorectal Cancer Survival
Recent studies suggest that long-interspersed nucleotide element-1 (LINE-1) hypomethylation is commonly found in colorectal cancer (CRC), and is associated with worse prognosis. However, the utility of LINE-1 methylation on the prognosis of CRC is still controversial, and may be due to the fact that some clinical and pathological features may affect LINE-1 methylation. Thus, the aim of this study was to assess the prognostic value of tumor LINE-1 methylation in CRC, through their association with the CRC clinical and pathological characteristics. Survival of sixty-seven CRC patients was evaluated according to the median of tumor LINE-1 methylation, as well as pathological and oncological variables. We also studied the association between LINE-1 methylation and pathological features, and finally, we assessed the overall and disease-free survival of LINE1 methylation, stratified by neoadjuvant treatment and further checked by multivariate Cox regression to assess the statistical interactions. LINE-1 was hypomethylated in the CRC tumor with respect to the tumor adjacent-free area (p < 0.05), without association with any other clinical and oncological features, nor with overall and disease-free survival rates for CRC. Relevantly, in neoadjuvant treatment, LINE-1 methylation was associated with survival rates. Thus, disease-free and overall survival rates of treated CRC patients were worse in the hypomethylated LINE-1 tumors than those with normal LINE-1 methylation (p = 0.004 and 0.0049, respectively). Indeed, LINE-1 was hypermethylated more in the treated patients than in the non-treated patients (p < 0.05). The present study showed that tumor LINE-1 hypomethylation was associated with worse survival rates in only treated patients. Our data suggest an interactive effect of neoadjuvant treatment and tumor LINE-1 methylation, which could be a specific-tissue biomarker to predict survival of the treated patients, and help to personalize treatment in CRC
Genomic diversity of human papillomavirus-16, 18, 31, and 35 isolates in a Mexican population and relationship to European, African, and Native American variants
AbstractCervical cancer, mainly caused by infection with human papillomaviruses (HPVs), is a major public health problem in Mexico. During a study of the prevalence of HPV types in northeastern Mexico, we identified, as expected from worldwide comparisons, HPV-16, 18, 31, and 35 as highly prevalent. It is well known that the genomes of HPV types differ geographically because of evolution linked to ethnic groups separated in prehistoric times. As HPV intra-type variation results in pathogenic differences, we analyzed genomic sequences of Mexican variants of these four HPV types. Among 112 HPV-16 samples, 14 contained European and 98 American Indian (AA) variants. This ratio is unexpected as people of European ethnicity predominate in this part of Mexico. Among 15 HPV-18 samples, 13 contained European and 2 African variants, the latter possibly due to migration of Africans to the Caribbean coast of Mexico. We constructed phylogenetic trees of HPV-31 and 35 variants, which have never been studied. Forty-six HPV-31 isolates from Mexico, Europe, Africa, and the United States (US) contained a total of 35 nucleotide exchanges in a 428-bp segment, with maximal distances between any two variants of 16 bp (3.7%), similar to those between HPV-16 variants. The HPV-31 variants formed two branches, one apparently the European, the other one an African branch. The European branch contained 13 of 29 Mexican isolates, the African branch 16 Mexican isolates. These may represent the HPV-31 variants of American Indians, as a 55% prevalence of African variants in Mexico seems incomprehensible. Twenty-seven HPV-35 samples from Mexico, Europe, Africa, and the US contained 11 mutations in a 893-bp segment with maximal distances between any two variants of only 5 mutations (0.6%), including a characteristic 16-bp insertion/deletion. These HPV-35 variants formed several phylogenetic clusters rather than two- or three-branched trees as HPV-16, 18, and 31. An HPV-35 variant typical for American Indians was not identifiable. Our research suggests type specific patterns of evolution and spread of HPV-16, 18, 31, and 35 both before and after the worldwide migrations of the last four centuries. The high prevalence of highly carcinogenic HPV-16 AA variants, and the extensive diversity of HPV-18, 31, and 35 variants with unknown pathogenic properties raise the possibility that HPV intra-type variation contributes to the high cervical cancer burden in Mexico
Magnesium in Kidney Function and Disease—Implications for Aging and Sex—A Narrative Review
Magnesium (Mg) has a vital role in the human body, and the kidney is a key organ in the metabolism and excretion of this cation. The objective of this work is to compile the available evidence regarding the role that Mg plays in health and disease, with a special focus on the elderly population with chronic kidney disease (CKD) and the eventual sex differences. A narrative review was carried out by executing an exhaustive search in the PubMed, Scopus, and Cochrane databases. Ten studies were found in which the role of Mg and sex was evaluated in elderly patients with CKD in the last 10 years (2012–2022). The progression of CKD leads to alterations in mineral metabolism, which worsen as the disease progresses. Mg can be used as a coadjuvant in the treatment of CKD patients to improve glomerular filtration, but its use in clinical applications needs to be further characterized. In conclusion, there’s a need for well-designed prospective clinical trials to advise and standardize Mg supplementation in daily clinical practice, taking age and sex into consideration
Use of colonoscopy as a primary screening test for colorectal cancer in average risk people
The use of colonoscopy as a primary screening test for colorectal
cancer (CRC) in average risk adults is a subject of controversy. Our primary
objective was to build a predictive model based on a few simple variables that
could be used as a guide for identifying average risk adults more suitable for
examination with colonoscopy as a primary screening test. METHODS: The prevalence
of advanced adenomas was assessed by primary screening colonoscopy in 2210
consecutive adults at least 40 yr old, without known risk factors for CRC. Age,
gender, and clinical and biochemical data were compared among people without
adenomas, those with non-advanced adenomas, and those with any advanced neoplasm.
A combined score to assess the risk of advanced adenomas was built with the
variables selected by multiple logistic regression analysis. RESULTS: Neoplastic
lesions were found in 617 subjects (27.9%), including 259 with at least one
neoplasm that was 10 mm or larger, villous, or with moderate-to-severe dysplasia,
and 11 with invasive cancers. Advanced lesions were more frequent among men,
older people, and those with a higher body mass index (BMI). These three
variables were independent predictors of advanced adenomas in multivariate
analysis. A score combining age, sex, and BMI was developed as a guide for
identifying individuals more suitable for screening colonoscopy. CONCLUSIONS:

Diagnostic value of distal colonic polyps for prediction of advanced proximal neoplasia in an average-risk population undergoing screening colonoscopy
For colorectal cancer screening, the predictive value of distal
findings in the ascertainment of proximal lesions is not fully established. The
aims of this study were to assess distal findings as predictors of advanced
proximal neoplasia and to compare the predictive value of endoscopy alone vs.
combined endoscopic and histopathologic data. METHODS: Primary colonoscopy
screening was performed in 2210 consecutive, average-risk adults. Age, gender,
endoscopic (size, number of polyps), and histopathologic distal findings were
used as potential predictors of advanced proximal neoplasms (i.e., any adenoma >
or =1 cm in size, and/or with villous histology, and/or with severe dysplasia or
invasive cancer). Polyps were defined as distal if located in the descending
colon, the sigmoid colon, or the rectum. Those in other locations were designated
proximal. RESULTS: Neoplastic lesions, including 11 invasive cancers, were found
in 617 (27.9%) patients. Advanced proximal neoplasms without any distal adenoma
were present in 1.3% of patients. Of the advanced proximal lesions, 39% were not
associated with any distal polyp. Older age, male gender, and distal adenoma were
independent predictors of advanced proximal neoplasms. The predictive ability of
a model with endoscopic data alone did not improve after inclusion of
histopathologic data. In multivariate logistic regression analysis, the
predictive ability of models that use age, gender, and any combination of distal
findings was relatively low. The proportion of advanced proximal neoplasms
identified if any distal polyp was an indication for colonoscopy was only 62%.
CONCLUSIONS: A strategy in which colonoscopy is performed solely in patients with
distal colonic findings is not effective screening for the detection of advanced
proximal neoplasms in an average-risk populatio

We present the results of the treatment with ursodeoxycholic acid (UDCA, 7-9 mg/kg body weight daily) of 17 patients with primary biliary cirrhosis (8 in stages I-II; 9 in stages III-IV). At two months the mean values of alkaline phosphatase, gammaglutamiltranspeptidase, alanine and aspartate aminotransferase were reduced (p less than 0.001, p less than 0.001, p less than 0.01 and p less than 0.01 respectively). This improvement persisted without increase during the first year. At two months the total bilirubin value was reduced (p less than 0.01) associated with a reduction in the conjugated fraction (p less than 0.05). Cholesterol and gammaglobulin mean values also decreased at two months (p less than 0.05). We found no changes in IgM levels and antimitochondrial antibody titers. The improvement was similar in both groups (early I-II and advanced III-IV stages) and the treatment showed no undesirable effects either in early or advanced stages.
