Chronic Pain Program Development: Old Town Clinic, Central City Concern
Jessica Gonzales and Jennifer Theusch began their partnership with the Old Town Clinic in the spring of 2009 as part of a pilot fieldwork occupational therapy program sponsored by Pacific University. Jennifer and Jessica worked under the guidance of a supervising occupational therapist for their unique level II mental health fieldwork rotation. Together they performed a needs assessment and gap analysis to determine a strategic fit for occupational therapy at the clinic. The students worked to solidify an occupational therapy program at the clinic that would integrate into the existing system and begin the process of providing services for the clients.
As the relationship between Pacific University and The Old Town Clinic continued the clinic expressed an interest with assistance to develop an interdisciplinary chronic pain program. Jessica and Jennifer choose to return to the clinic during their third and final year of graduate studies for their innovative practice project.
Jessica and Jennifer delved into evidenced based research regarding current chronic pain programming. The students visited a chronic pain rehabilitation institute to meet with an occupational therapist and discuss trends and gain insight in the arena of chronic pain. While researching chronic pain programming Jessica and Jennifer determined appropriate models for practice in this area. These models helped to frame program and evaluation design.
The clinic expressed interest in a comprehensive evaluation tool for chronic pain clients. During their research Jessica and Jennifer determined typical areas of assessment for individuals with chronic pain. Once these areas were determined the students began to review common assessments used to evaluate these specific regions. The assessment list was narrowed to those which were accessible and were supported by evidence. Once the assessments had been chosen work on drafting the evaluation tool began.
The evaluation tool was developed over two and a half months. It took seven drafts and many hours of research. The tool was evaluated during two focus groups and piloted twice with Old Town Clinic clients. The students received professional insight and feedback from the clinic medical director, occupational therapists, and their supervising professor. Once the evaluation tool was completed the students began the process of determining how the information obtained in the tool could be best expressed in an evaluation tool summary template. After two drafts and feedback from the supervising professor the evaluation tool and summary template were ready for use at the clinic.
The clinic determined the chronic pain evaluation tool would be used for all chronic pain clients. Results from the tool would be used to screen clients to different pathways for intervention. It was determined one of these pathways would be occupational therapy individual or group intervention. Jessica and Jennifer returned to research to assist with the determination of appropriate evidence based group topics for chronic pain treatment. Approximately seventeen group topics were researched as possible occupational therapy group interventions. These topics are currently being used to educate chronic pain clients to improve occupational performance and enhance occupational functioning in daily activities

SUBSTANCE USE COUNSELORS\u27 PERCEPTIONS OF EFFECTIVE TREATMENT MODALITIES FOR WOMEN WITH CHILDREN
The U.S Department of Health and Human Services (HHS) conducted a study and determined that approximately one-third to two-thirds of child neglect cases had some form of substance abuse related to the case. Further, it is reported that women who use alcohol or drugs are two times more likely to lose custody of their children than non- using mothers. The purpose of this study is to examine which treatment modalities substance use counselors find most effective when treating women with children. This study utilized a qualitative design asking eight open ended questions to fourteen substance use counselors employed at Prototypes in Pomona, CA. The substance use counselors were asked questions regarding what treatments they offered at their facility, what they believed the most effective treatment modalities are when treating women with children, and what barriers they faced when treating women with children.
Findings from this study found the holistic and client centered approaches to be the most effective treatment modalities when treating women with children. The holistic approach considers every aspect of the client’s life and the client centered approach allows the counselors to develop care plans that are specific to their client’s unique needs. Social workers should strive to keep women with their children while they are in treatment. Further research is needed to gain a better understanding of this population and to provide appropriate treatment, services, and resources to women and their children
Factores clÃnicos, bioquÃmicos y de imágenes predictores de mortalidad en obstrucción intestinal por adherencias: un artÃculo de revisión
La obstrucción intestinal (OI) es un sÃndrome quirúrgico de presentación frecuente. Las adherencias son la principal causa de OI en pacientes post-operados. El impacto en la salud de dichos pacientes, generalmente es favorable. Los desenlaces fatales podrÃan prevenirse si se contará con factores que pronostiquen los resultados en este grupo especial de pacientes.
El equipo investigador revisó la literatura disponible hasta el presente año, con objeto de resaltar los factores clÃnicos, bioquÃmicos y de imágenes predictores de mortalidad más importantes evidenciados.
En el estudio se concluyó que existe alta evidencia cientÃfica para considerar edad avanzada, estado general, albúmina, urea sérica alteradas y lÃquido libre en cavidad abdominal como factores predictores independientes de mortalidad en pacientes con obstrucción intestinal por adherencias. Los otros factores revisados necesitan mayor evidencia para considerarlos significativos.Intestinal obstruction (IO) is a frequently occurring surgical syndrome. Adhesions are the main cause of IO in post-operative patients. The impact on the health of these patients is generally favorable. Fatal outcomes could be prevented if factors are expected to predict outcomes in this special group of patients.
Research team reviewed the literature available up to this year, in order to highlight the most important clinical, biochemical and imaging predictors of mortality evidenced.
Study concluded that there is high scientific evidence to consider advanced age, general condition, albumin, altered serum urea and free fluid in abdominal cavity as independent predictors of mortality in patients with intestinal obstruction by adhesions. Metagenomic deep sequencing of aqueous fluid detects intraocular lymphomas.
IntroductionCurrently, the detection of pathogens or mutations associated with intraocular lymphomas heavily relies on prespecified, directed PCRs. With metagenomic deep sequencing (MDS), an unbiased high-throughput sequencing approach, all pathogens as well as all mutations present in the host's genome can be detected in the same small amount of ocular fluid.MethodsIn this cross-sectional case series, aqueous fluid samples from two patients were submitted to MDS to identify pathogens as well as common and rare cancer mutations.ResultsMDS of aqueous fluid from the first patient with vitreal lymphoma revealed the presence of both Epstein-Barr virus (HHV-4/EBV) and human herpes virus 8 (HHV-8) RNA. Aqueous fluid from the second patient with intraocular B-cell lymphoma demonstrated a less common mutation in the MYD88 gene associated with B-cell lymphoma.ConclusionMDS detects pathogens that, in some instances, may drive the development of intraocular lymphomas. Moreover, MDS is able to identify both common and rare mutations associated with lymphomas
La afectación en los procesos de alimentos en los niños y adolescentes del AA.HH Huascar, del distrito de San Juan de Lurigancho, en el periodo diciembre 2014- diciembre 2015
El presente trabajo de investigación, se enmarca dentro del derecho civil y en él
se describe como en la actualidad se viene dando el incumplimiento de la pensión
alimenticia a favor de los menores de edad de la local idad de Huáscar, dado que
se atenta contra el derecho fundamental a los alimentos establecido en la ley.
Se describirá como se viene dando la afectación de los derechos de alimentos en
los niños y adolescentes del AA.HH. Huáscar del distrito de San Juan de
Lurigancho, en el periodo diciembre 2014 - diciembre 2015, y si dicha afectación
se enmarca dentro de las regulaciones que protegen el interés superior del niño
con respecto a los alimentos
Las microfinanzas y su contribución en el crecimiento económico de las mypes post Fenómeno el Niño en el distrito de Casa Grande provincia de Ascope en el año 2017
La presente investigación se ha desarrollado en el distrito de Casa Grande, buscando resolver
el siguiente problema:
¿De qué manera las microfinanzas contribuyen en el crecimiento económico de las mypes
post fenómeno El Niño en el distrito de Casa Grande provincia de Ascope en el año 2017?
El objetivo general de esta investigación ha sido determinar la contribución de las
microfinanzas en el crecimiento económico de las Mypes post fenómeno El Niño en el
distrito de Casa Grande provincia de Ascope en el año 2017. La hipótesis que se ha planteado
es la siguiente: Las Microfinanzas contribuyen favorablemente en el crecimiento económico
de las mypes post Fenómeno El Niño en el distrito de Casa Grande provincia de Ascope en
el año 2017. Se han utilizado métodos y técnicas de recolección de datos como hojas de
registro y entrevistas, las cuales nos han permitido conocer la situación económica de las
mypes, mostrando que en el distrito existe una cultura de emprendimiento convirtiendo al
mismo en la principal actividad económica de la provincia. Además el fenómeno El Niño
ocurrido en el presente año perjudicó a las mypes con problemas de liquidez recurriendo a
las microfinancieras, principalmente a tres que son: Crediscotia, Financiera Compartamos y
Cooperativa de ahorro y crédito San José de Cartavio. Las conclusiones que obtuvimos en
esta investigación a través de un análisis económico reflejados en los gráficos de los estados
financieros de cada mype en estudio es que el 100% de estas mypes han accedido a un
financiamiento del sistema microfinanciero, encontrándose una relación positiva entre el
crecimiento económico para el periodo enero a julio 2017 y las microfinanzas, lo que quiere
decir que las microfinanzas poseen un efecto positivo en cuanto al crecimiento económico
de las mypes, y mediante la realización de un análisis financiero de estas 5 mypes evaluadas
muestran aspectos positivos en los indicadores de liquidez, solvencia, apalancamiento y
rentabilidad, confirmando que las microfinancieras contribuyen favorablemente en el
crecimiento económico de las mypes en estos últimos meses post fenómeno El Niño.The present investigation has been developed in the district of Casa Grande, seeking to
solve the following problem:
How does microfinance contribute to the economic growth of the mypes post El Niño
phenomenon in the district of Casa Grande province of Ascope in the year 2017? The
general objective of this research has been to determine the contribution of microfinance
in the economic growth of the Mypes after El Niño phenomenon in the district of Casa
Grande province of Ascope in the year 2017. The hypothesis that has been raised is the
following: Microfinance contributes favorably to the economic growth of the mypes post
El Niño phenomenon in the district of Casa Grande province of Ascope in the year 2017.
Methods and techniques of data collection have been used as record sheets and interviews,
which have allowed us to know the economic situation of the mypes, showing that in the
district there is a culture of entrepreneurship making it the main economic activity of the
province. In addition, the El Niño phenomenon that occurred this year hurt the MEPs with
liquidity problems by using microfinance institutions, mainly three: Crediscotia, Financiera
Compartamos and San Jose de Cartavio savings and credit cooperative. The conclusions
that we obtained in this research through an economic analysis reflected in the graphs of
the financial statements of each mype under study is that 100% of these mypes have agreed
to a financing of the microfinance system, finding a positive relationship between growth
economic activity for the period January to July 2017 and microfinance, which means that
microfinance has a positive effect on the economic growth of the mypes, and by carrying
out a financial analysis of these 5 evaluated MEPs they show positive aspects in the
indicators of liquidity, solvency, leverage and profitability, confirming that the
microfinance institutions contribute favorably to the economic growth of the mypes in the

Knowledge and self-care of arteriovenous fistula in patients with chronic kidney disease in a Type III Hospital in Callao, 2022.
El autocuidado de fÃstula arteriovenosa es un procedimiento necesario en pacientes que presentan enfermedad renal crónica, a fin de realizar los procedimientos de hemodiálisis necesarios para reemplazar la actividad que los riñones ya no pueden llevar a cabo. Su importancia es de alto impacto, pues si no se cuenta con los conocimientos adecuados para el autocuidado de la fÃstula arteriovenosa podrÃan producirse infecciones, sangrado, interrupción de hemodiálisis u otros eventos adversos que perjudicarÃan su tratamiento o complicar su estado de salud. Objetivo: relacionar conocimiento y autocuidado de fÃstula arteriovenosa en pacientes con enfermedad renal crónica de un hospital tipo III del Callao, 2022. Material y método: será una investigación cuantitativa, de tipo básica, observacional- no experimental, transversal con alcance correlacional. La muestra será de 60 pacientes. El instrumento 1 será el cuestionario para medir la variable conocimiento, elaborado por Jesús K, de validez adecuada, 99.4 y confiabilidad de 0.724 medida por Alfa de Cronbach. El instrumento 2 es el cuestionario para medir la variable autocuidado de la fÃstula arteriovenosa elaborado por Mamani D, con una confiabilidad de 0,85 medido por Alfa de Cronbach. Informe de experiencia profesional en el cuidado de paciente post operado de Cáncer de Colon Portador de ColostomÃa del Servicio 3ero "A" CirugÃa de Colon y Esófago del Hospital Nacional Edgardo Rebagliati Martins - 2016
Las colostomÃas constituyen un procedimiento común de las prácticas
cotidianas de los hospitales, debido al alto Ãndice de obstrucción intestinal,
siendo mayor en nuestro medio a causa del aumento de los últimos años
de cáncer de colorrectal. Siendo la ostomÃa un procedimiento quirúrgico,
mediante el cual se crea una comunicación artificial entre una vÃscera
hueca y el intestino a la pared abdominal exterior con el objetivo de
evacuación, denominándose colostomÃa cuando la intervención coge
parte del colon; el paciente la considera como una gran carga limitante en
diferentes aspectos de la vida y debe aprender a conllevarlo.
Como profesión sanitaria, la enfermerÃa ofrece cuidados de calidad según
las necesidades del paciente; en pacientes colostomizados, el cuidado
para la pronta recuperación del posoperatorio es de gran importancia, a
fin de disminuir la ansiedad y mejorar la comunicación, pues muchos
pacientes desconocen sobre su estado y refieren: ¿Cómo hare ahora con
esto? ¿Por ahà saldrán las heces? ¿Quién me ayudara a eliminarlas? ¿La
herida se infectara con esto? ¿Qué debo comer ahora? Sin embargo, es
fundamental la intervención de enfermerÃa en la educación del paciente
para favorecer su autocuidado, a fin de disminuir el riesgo de
