Trastorno de la personalidad: un desafío en la relación médico-paciente

ResumenEl paciente con trastorno de la personalidad límite o con trastornos afectivos suele obstaculizar la relación médico-paciente debido a sus conflictos emocionales. Es importante que el médico familiar reconozca de manera oportuna dichos trastornos y desarrolle habilidades tanto para la comunicación como para la empatía; con el propósito de favorecer una adecuada relación médico-paciente y referir oportunamente al enfermo a un servicio de atención especializado. De este modo se podrán prevenir la cronicidad, la iatrogenia y la estigmatización del paciente, así como la frustración e impotencia del equipo médico.SummaryPatients with limit personality disorder or affective disorders often hinder the doctor-patient relationship due to their emotional conflicts. It is important that the Family Physician recognized in a timely manner such disorders and develop skills for communication and empathy; in order to promote a proper doctor-patient relationship and promptly refer the patient to a specialist. Thus this will prevent chronicity, iatrogenic and stigmatization of the patient, as well as frustration and a sense of helplessness of the medical team

Aplicación de la acupuntura en la inducción del parto en Cuba

Se realizó un estudio experimental  en el Hospital General Provincial Docente “Roberto Rodríguez Fernández” de Morón durante el período comprendido del 1ro. de enero  al 31 de diciembre       del       2013, para evaluar el comportamiento de la inducción del parto con tratamiento acupuntural en embarazos a términos y pósterminos.  La muestra de estudio estuvo constituida por 80 pacientes.  A todas las gestantes se le aplicó 3 sesiones de acupuntura utilizando agujas filiformes  que se estimularon manualmente y se aplicaron en los puntos Ig4, Vc4, E36 y B6. Se evaluaron las siguientes variables: Edad, paridad, motivo de la inducción, relación entre madurez cervical y respuesta a la inducción,  Inicio del trabajo de parto, su duración, así como las características del parto, además de las complicaciones; donde predominó el Grupo de edad de 20 a 29 años,  las  nulíparas y la vía de parto más frecuente fue la transpelviana, el embarazo prolongado fue el principal motivo de la inducción del parto, el mayor porciento, independientemente de Bishop favorable o desfavorable, inició dinámica uterina útil entre 3 y 5 horas, el   trabajo de parto así como la duración del mismo no sobrepasó las  8 h y todos los recién nacidos fueron vigorosos y no existió complicaciones maternas ni neonatales

Piel de cíclidos con utilidad en las quemaduras: perspectivas en la Medicina

Las lesiones por quemaduras constituyen una de las patologías que con mayor frecuencia ocupan las consultas de urgencia, siendo la variedad de trauma que más frecuentemente deja en el paciente graves y permanentes secuelas, y dependiendo de la magnitud de la injuria, puede constituir uno de los más formidables desafíos para el médico de urgencia. Por mucho tiempo se han utilizado numerosos fármacos para su tratamiento con la desventaja que implican los efectos secundarios, se experimenta también en la utilización de productos naturales con la ventaja de poder disminuir los efectos mencionados, utilizándose la piel animal para el tratamiento de quemaduras, especialmente la piel de cerdo y otros animales domésticos. Actualmente se realizan investigaciones sobre la viabilidad y el costo de la piel de los cíclidos con énfasis en la piel de la tilapia, en comparación con los tratamientos tradicionales. Según los estudios realizados contiene una proteína llamada colágeno tipo 1 y tiene un grado de humedad que propicia adecuada y rápida cicatrización de las heridas, aspecto que resulta novedoso en la práctica médica

A Literature Review on Adolescence and Its Impact on Pubertal Changes

Aim: Puberty and adolescence are stages of life where within adolescence is puberty which will be characterized by a range of age and has its difference in boys and girls and begins with different changes in the body. Material and method: This stage of puberty is determined by the activation of certain axes and by the inhibition of some proteins that will provide the reaction of several body systems to favour the development of pubertal characters, so the objective of this study is to describe the processes involved in puberty and adolescence to improve the prognosis of time and development in the child. Statistics and Result: The descriptive study method was used by means of a bibliographic review of diverse scientific literature, which includes the results of research with different modalities, but all of them related to the subject and which evidences this problem. The results of the study showed that the clinical manifestations are important to know in which stage of puberty the individual is and to be able to delimit if puberty is developing in a normal way or if there is some type of delay or advancement

Current challenges and future trends in the field of communication architectures for microgrids

[EN] The concept of microgrid has emerged as a feasible answer to cope with the increasing number of distributed renewable energy sources which are being introduced into the electrical grid. The microgrid communication network should guarantee a complete and bidirectional connectivity among the microgrid resources, a high reliability and a feasible interoperability. This is in a contrast to the current electrical grid structure which is characterized by the lack of connectivity, being a centralized-unidirectional system. In this paper a review of the microgrids information and communication technologies (ICT) is shown. In addition, a guideline for the transition from the current communication systems to the future generation of microgrid communications is provided. This paper contains a systematic review of the most suitable communication network topologies, technologies and protocols for smart microgrids. It is concluded that a new generation of peer-to-peer communication systems is required towards a dynamic smart microgrid. An Algorithm for the Efficient Management of the Power Converters Connected to the DC Bus of a Hybrid Microgrid Operating in Grid-connection Mode

[EN] In this paper a centralized control strategy for the efficient power management of the power converters conforming a hybrid distributed generation microgrid is explained. The microgrid is based on a DC and an AC bus. The study is focused on the converters connected to the DC bus. The proposed power management algorithm is implemented in a microgrid central processor. This algorithm is based on assigning several operation functions to each of the generators, loads and energy storage systems in the microgrid. A communication system is used to assign the operation functions to each of the microgrid elements. The power flows between the DC and AC buses are studied in several operation scenarios, in which the proposed control can be verified.
