    Introdução: Um estudo epidemiológico sobre causas externas, utilizando taxas de mortalidade, foi realizado no Município de São José do Rio Preto, Estado de São Paulo, Brasil. O período considerado foi o triênio 1996-1998. Objetivos: Descrever as taxas de mortalidade por causas externas no Município de São José do Rio Preto, no período de 1996-1998. Metodologia: O estudo foi baseado em dados oficiais do Sistema de Informação de Mortalidade do Ministério da Saúde, e de investigação baseada nas informações dos médicos legistas (IML).A análise dos dados foi feita através dos softwares Epinfo 6.0 e Tabwin. Resultados: Os resultados foram comparados com os valores, obtidos na literatura, de algumas cidades e estados do Brasil. No caso de São José do Rio Preto, observou-se, no peírodo, diminuição no número dos óbitos por causas externas, particularmente nas mortes devidas a acidentes de trânsito, que são mais altas em relação aos valores observados no Brasil e no Estado de São Paulo. O estudo também detectou aumento no número absoluto de homicídios, principalmente nas faixas etárias de 15 a 39 anos de idade. Conclusão: Os resultados demonstram a necessidade de iniciar programas de prevenção à mortalidade pelas citadas causas externas e monitorizar, em nível local, suas ocorrências .Introduction: An epidemiological study of the mortality rates by injuries in São José do Rio Preto, State of São Paulo, Brazil, has been carried out. The period considered is 1996-1998. Objectives: To show injuries mortality rates in São José do Rio Preto-SP in 1996-98. Methods: The study was based on official data from the Mortality Information Service of the Brazilian Ministry of Health, and in a survey based on data from medical forensic examiners (IML).Software Epinfo6.0 and Tabwin was utilized for data analises. Results:The results were compared to values obtained in the literature for the other cities of São Paulo and Brazil. In the case of São José do Rio Preto, was observed a decrease in the number of deaths associated with external causes in the period, but particularly in deaths due to traffic accidents with motor vehicles an increase in cases of pedestrians killed by hit-and-run drivers, that is much larger than the corresponding values for Brazil and for the State of São Paulo. The study showed increase in rates of homicides, mainly in the 15 to 39 years age group . Conclusion: This result suggests that is important to start programs to reduce the number of deaths by injuries, as well as the necessity of monitoring locally all these data

    Socioeconomic inequalities in dental health services in Sao Paulo, Brazil, 2003-2008

    __Background:__ Access to, and use of, dental health services in Brazil have improved since 2003. The increase of private health care plans and the implementation of the "Smiling Brazil" Program, the largest public oral health care program in the world, could have influenced this increase in access. However, we do not yet know if inequalities in the use of dental health services persist after the improvement in access.     Use, access, and equity in health care services in São Paulo, Brazil

    The study analyzed how socioeconomic factors are associated with seeking, access, use, and quality of health care services in São Paulo, Brazil. Data were obtained from two household health surveys in São Paulo. We used logistic regression to analyze associations between socioeconomic factors and seeking, access, use, and quality of health care services. Access to health care services was high among those who sought it (94.91% in 2003 and 94.98% in 2008). The proportion of access to and use of health care services did not change significantly from 2003 to 2008. Use of services in the public sector was more frequent in lower socioeconomic groups. There were some socioeconomic differences in seeking health care and resolution of health problems. The study showed almost universal access to health care services, but the results suggest problems in quality of services and differences in quality experienced by lower socioeconomic groups, who mostly use the Brazilian Unified National Health System (SUS)

    Self-reported diabetes in older people: comparison of prevalences and control measures

    OBJECTIVE The objective of this study was to analyze the prevalence of diabetes in older people and the adopted control measures. METHODS Data regarding older diabetic individuals who participated in the Health Surveys conducted in the Municipality of Sao Paulo, SP, ISA-Capital, in 2003 and 2008, which were cross-sectional studies, were analyzed. Prevalences and confidence intervals were compared between 2003 and 2008, according to sociodemographic variables. The combination of the databases was performed when the confidence intervals overlapped. The Chi-square (level of significance of 5%) and the Pearson’s Chi-square (Rao-Scott) tests were performed. The variables without overlap between the confidence intervals were not tested. RESULTS The age of the older adults was 60-69 years. The majority were women, Caucasian, with an income of between > 0.5 and 2.5 times the minimum salary and low levels of schooling. The prevalence of diabetes was 17.6% (95%CI 14.9;20.6) in 2003 and 20.1% (95%CI 17.3;23.1) in 2008, which indicates a growth over this period (p at the limit of significance). The most prevalent measure adopted by the older adults to control diabetes was hypoglycemic agents, followed by diet. Physical activity was not frequent, despite the significant differences observed between 2003 and 2008 results. The use of public health services to control diabetes was significantly higher in older individuals with lower income and lower levels of education. CONCLUSIONS Diabetes is a complex and challenging disease for patients and the health systems. Measures that encourage health promotion practices are necessary because they presented a smaller proportion than the use of hypoglycemic agents. Public health policies should be implemented, and aimed mainly at older individuals with low income and schooling levels. These changes are essential to improve the health condition of older diabetic patients

    Avaliação terapêutica do praziquantel (Embay 8440) na infecção humana pelo S. mansoni

    Foram tratados com praziquantel, dose oral única de 40 ou 50 mg/kg, 200 indivíduos portadores de esquistossomose mansoni, matriculados na Clínica de Doenças Infecciosas e Parasitárias do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. As idades dos pacientes variavam de seis a 63 anos, sendo que 33 (16,5%) eram menores de 15 anos. Os principais efeitos colaterais consistiram em tontura 27,5%; sonolência 21,0%; eólica 17,5%; náusea 16,0%; diarréia 9,0%; cefaléia 7,5%; vômito 4,0%; febre 2,0%; disenteria 1,0%; tremor 1,0%; exantema 1,0% e urticaria 0,5%. A toxicidade do medicamento foi investigada mediante a realização, pré e pós-tratamento, de exames hematimétricos, bem como de função renal (uréia e creatinina), hepática (enzimas de liberação hépato-canalicular e bilirrubinas), cardíaca (ECG) e neuropsiquiátrica (EEG). Não foram encontradas nesses controles alterações relevantes que repercutissem clinicamente. O controle de cura verificou-se em 115 indivíduos, através de oito coproscopias, no período de seis meses, subseqüente ao tratamento, utilizando-se duas técnicas (HOFFMAN e KATO/KATZ) para cada amostra de fezes. Dos 82 indivíduos que tiveram as oito coproscopias negativadas, 62 realizaram biópsia retal. Essa mostrou-se positiva em duas oportunidades, indicando um percentual de 3,2% de achados falsos negativos com relação às coproscopias. Os índices de cura variaram de 65,2%, nos pacientes menores de 15 anos, a 75% nos acima dessa idade. Para todas as faixas etárias a eficácia foi de 71,3%. Os resultados obtidos demonstram ter o praziquantel, nas doses empregadas, relativa eficácia no tratamento da esquistossomose mansoni, bem como ser determinante de baixos efeitos tóxico-colaterais
