    Ensemble programme for early intervention in informal caregivers of psychiatric adult patients: a protocol for a randomised controlled trial.

    Informal caregivers play a major role in the support and maintenance of community patients with severe psychiatric disorders. A pilot study showed that an individualised brief intervention such as the Ensemble programme leads to significant improvements in psychological health state and optimism. This randomised controlled trial aims to compare the efficacy of using Ensemble in improving informal caregivers' psychological health states and the ability to play an active role in their situations with that of support as usual. Improvements on the psychological health global index will be measured three times (T0-pre, T1-post and T3 2 months follow) with standardised questionnaires (the Global Severity Index of Brief Inventory Symptoms, the Life Orientation Test-Revised, the 36-item Medical Outcome Study Short-Form Health Survey and the French Zarit Burden Interview). Differences between groups in post-test and pretest values will be examined using an analysis of covariance for each outcome variable. The severity of illness measured by the Social and Occupational Functioning Assessment Scale will also be collected at T0 and T2 to compare eventual patient improvements. At the end of the programme, the experiences of the 20 patients participating in the Ensemble programme will be evaluated qualitatively. The research protocol received full authorisation from the Human Research Ethics Committee of the Vaud state, Switzerland. The principal paper will concern the results of the experimental design used to test the Ensemble programme. The research team will prioritise open access publications. NCT04020497

    HD 178892 - a cool Ap star with extremely strong magnetic field

    We report a discovery of the Zeeman resolved spectral lines, corresponding to the extremely large magnetic field modulus =17.5 kG, in the cool Ap star HD 178892. The mean longitudinal field of this star reaches 7.5 kG, and its rotational modulation implies the strength of the dipolar magnetic component Bp>=23 kG. We have revised rotation period of the star using the All Sky Automated Survey photometry and determined P=8.2478 d. Rotation phases of the magnetic and photometric maxima of the star coincide with each other. We obtained Geneva photometric observation of HD 178892 and estimated Teff=7700+/-250 K using photometry and the hydrogen Balmer lines. Preliminary abundance analysis reveals abundance pattern typical of rapidly oscillating Ap stars.Comment: Accepted by Astronomy & Astrophysics; 4 pages, 4 figure

    Simulating NIRS and MRS Measurements During Cerebral Hypoxia-Ischaemia in Piglets Using a Computational Model

    We present a group analysis of the changes in cerebral haemodynamics, and the oxidation state of cytochrome-c-oxidase measured using broadband near-infrared spectroscopy (NIRS) and intracellular pH measured by phosphorous ((31)P) magnetic resonance spectroscopy (MRS) during and after cerebral hypoxia-ischaemia (HI) in 15 piglets. We use a previously published computational model of cerebral metabolism in the piglet [1] to integrate these measurements and simulate HI. We successfully simulate changes in cellular metabolism including shifts in intracellular pH observed in the piglet brain during HI. In this process, we optimise physiological parameters in the model identified through sensitivity analysis (such as the rate of glucose metabolism and intracellular lactate concentration), to fit simulated and measured data. The model fits the data reasonably and suggests a 20 % drop in glucose consumption, a ~65 % increase in lactate concentration and ~35 % drop in the cerebral metabolic rate of oxygen (CMRO₂) during HI

    On the ground states of the Bernasconi model

    The ground states of the Bernasconi model are binary +1/-1 sequences of length N with low autocorrelations. We introduce the notion of perfect sequences, binary sequences with one-valued off-peak correlations of minimum amount. If they exist, they are ground states. Using results from the mathematical theory of cyclic difference sets, we specify all values of N for which perfect sequences do exist and how to construct them. For other values of N, we investigate almost perfect sequences, i.e. sequences with two-valued off-peak correlations of minimum amount. Numerical and analytical results support the conjecture that almost perfect sequences do exist for all values of N, but that they are not always ground states.     The gait and balance of patients with diabetes can be improved: a randomised controlled trial

    Aims/hypothesis: Gait characteristics and balance are altered in diabetic patients. Little is known about possible treatment strategies. This study evaluates the effect of a specific training programme on gait and balance of diabetic patients. Methods: This was a randomised controlled trial (n = 71) with an intervention (n = 35) and control group (n = 36). The intervention consisted of physiotherapeutic group training including gait and balance exercises with function-orientated strengthening (twice weekly over 12weeks). Controls received no treatment. Individuals were allocated to the groups in a central office. Gait, balance, fear of falls, muscle strength and joint mobility were measured at baseline, after intervention and at 6-month follow-up. Results: The trial is closed to recruitment and follow-up. After training, the intervention group increased habitual walking speed by 0.149m/s (p < 0.001) compared with the control group. Patients in the intervention group also significantly improved their balance (time to walk over a beam, balance index recorded on Biodex balance system), their performance-oriented mobility, their degree of concern about falling, their hip and ankle plantar flexor strength, and their hip flexion mobility compared with the control group. After 6months, all these variables remained significant except for the Biodex sway index and ankle plantar flexor strength. Two patients developed pain in their Achilles tendon: the progression for two related exercises was slowed down. Conclusions/interpretation: Specific training can improve gait speed, balance, muscle strength and joint mobility in diabetic patients. Further studies are needed to explore the influence of these improvements on the number of reported falls, patients' physical activity levels and quality of life.
