Paraneoplastic brainstem encephalomyelitis and atypical form of chronic inflammatory demyelinating polyneuropathy in patient with testicular germinal tumorโ€”Is this an overlap syndrome? A case report

    Get PDF
Paraneoplastic neurologic syndromes are diagnosed when neurologic symptoms are associated with neoplasm and other causative factors are excluded. They may precede or be simultaneous to various types of neoplasms, mainly malignant. In men up to 45โ€“50 years old the most common cancer causing the paraneoplastic syndrome is testicle tumor, manifesting usually as limbic/brain stem encephalitis and myelitis. Usually effective treatment of underlying neoplasm brings resolution of neurologic symptoms. But corticosteroids and intravenuous immunoglobulins are also used. In the presented case a 37-year-old man was primarily diagnosed and treated for progressive tetraparesis with signs of both upper and lower motor neuron dysfunction, associated with bulbar symptoms. Having various diagnostic procedures performed an atypical form of chronic inflammatory demyelinating polyradiculoneuronopathy was primarily suspected, but eventually a discovery of endodermal sinus tumor in the testicle enabled to state the diagnosis of possible paraneoplastic syndrome. In spite of chemotherapy the patient died shortly after the diagnosis because of infectious complications. Histopathology displayed intense inflammatory changes in the brain stem as well as in cranial nerves and cervical spinal cord. The same immunological process evoked by various pathogenetic factors (infection vs. neoplasm) may cause similar clinical picture and hinder the diagnosis. Most importantly it may delay the proper way of treatment

The aim of this study was to evaluate the ability of two synthetic GnRH analogues, goserelin and leuprolide, to induce ovulation in rabbit does using intravaginal administration. A total of 252 primiparous lactating does were randomly divided into five groups that, at the time of insemination, received the following treatments for ovulation induction: 1 ยตg of buserelin administered intramuscularly (control group), 5 ยตg of goserelin added to the semen dose (Group G5), 10 ยตg of goserelin added to the semen dose (Group G10), 5 ยตg of leuprolide added to the semen dose (Group L5), and 10 ยตg of leuprolide added to the semen dose (Group L10). The kindling rate was 80.5% in Group G10 and 75.0% in Group L10; these values are comparable to the kindling rate obtained in the control group (85.9%). The kindling rates in Groups G5 and L5 were significantly lower than in the control group (60.0%, 54.2% and 85.9%, respectively). The number of live-born rabbits was not significantly affected by the ovulation induction treatment. As regards the total number of rabbits born the only significant difference was observed between Groups G5 and L5. This study shows the possibility of inducing ovulation in rabbits by adding goserelin and leuprolide directly to the semen dose

The article marks main components for formation of a subsystem of managerial accounting and reporting, develops methods of formation of reporting in the system of managerial accounting in a small business and also analyses units of managerial information in accordance with requirements of users and develops an organisational scheme of formation of economic information about production, realisation and financial flows in the system of accounting and analytical provision of activity of a small business (using example of an industrial enterprise).

The compressive strength of fly ash concrete and its mineralogy

    Get PDF
Includes bibliographical references. The use of fly ash as a cement extender in portland cement concrete is well established. Strict requirements are set for the fly ash on its physical properties and chemical composition to ensme its successful application as a partial replacement material for cement. An investigation was undertaken into the effectiveness and properties of a high carbon clinker ash when used as a cement extender at a 30 direct mass to mass substitution for portland cement. The clinker ash came from the Van Eck power station in Windhoek, Namibia and was milled to pass a 63micron sieve. For comparison fly ashes from the Escom power stations of Lethabo, Duvha and Matla were used. Both concrete and pure paste specimens were prepared for the evaluations
