    Factors associated with non adherence to regulations on sale of alcohol by alcohol outlets operators in Thika Municipality, Kiambu County

    Objective: To determine factors associated with non-adherence of existing alcohol regulations and policies on alcohol sale in Thika municipality Kiambu County.Design: A descriptive cross-sectional study.Setting: Thika Municipality, Kiambu County, KenyaSubjects: Eighty nine (89) participants were enrolled in the study. Ten (10) Key Informant Interviews were conducted among the security personnel, health officers, managers and supervisors.Results: Out of the 89 outlet operators enrolled in the study, (70.8%) among therespondents were not aware of the alcohol control act. Association between marital status and non adherence of the act was significant (p=0.003). There was low knowledge of 5.6%, who were aware of when the alcohol act was implemented, majority who totaled to 70.8% were not aware on when the alcohol control policy came into existence. Majority of the respondent 62.9% reported not to adhere to the alcohol control act of 2010; also the study showed that 57.3% of the responded were not aware of measures to control illicit brew. From the In-depth interviews most of the respondents interviewed confirmed that many of the operators were not aware of the act and it was difficult to implement thus subjecting the consumers into alcohol abuse. They also confirmed that with lack of employment, many women engaged into the alcohol operations which also contributed to the breakdown of their families given the mode of operations.Conclusion: This study concludes that there is failure in enforcement and adherence of the 2010 alcohol regulations. The study recommends the review of the alcohol control act of 2010 to ensure tough measures and penalties are adapted to anyone who fails to adhere to the act. Also there is need for awareness to be initiated to ensure members are informed and adhere to the policy. Setting up of an independent unit that will be mandated to enforcement and adherence of the 2010 alcoholic act

    Use of rapid diagnostic tests in malaria school surveys in Kenya: does their under-performance matter for planning malaria control?

    Malaria rapid diagnostic tests (RDTs) are known to yield false-positive results, and their use in epidemiologic surveys will overestimate infection prevalence and potentially hinder efficient targeting of interventions. To examine the consequences of using RDTs in school surveys, we compared three RDT brands used during a nationwide school survey in Kenya with expert microscopy and investigated the cost implications of using alternative diagnostic approaches in identifying localities with differing levels of infection. Overall, RDT sensitivity was 96.1% and specificity was 70.8%. In terms of classifying districts and schools according to prevalence categories, RDTs were most reliable for the 40% categories and least reliable in the 1-4.9% category. In low-prevalence settings, microscopy was the most expensive approach, and RDT results corrected by either microscopy or polymerase chain reaction were the cheapest. Use of polymerase chain reaction-corrected RDT results is recommended in school malaria surveys, especially in settings with low-to-moderate malaria transmission

    Modern contraceptive prevalence and its predictors among non-refugee and refugee Somali women in Nairobi city, Kenya; a comparative view

    Introduction and methodsThis study sought to determine the prevalence and predictors of modern contraceptive use among non-refugee and refugee Somali women in Nairobi City, Kenya. The analysis was based on 976 currently married Somali women aged 15–39 years (non-refugees; 523, refugees; 415) who were interviewed in a 2021 household survey conducted in Kamukunji, Embakasi, and Ruaraka sub-counties of Nairobi City. The analysis was stratified by refugee status and multivariable logistic regression were run to determine predictors of modern contraceptive use in each group.ResultsThe prevalence of modern contraceptives was 34% for the total sample and 43% and 24% for non-refugees and refugees, respectively. The main methods of contraception among non-refugees were injectables, implants, and daily pills, while refugees mainly used male condoms, implants, and injectables. Stratified multivariable analysis showed that residence in formal vs. informal settlements was associated with significantly higher odds of modern contraceptive use among non-refugees but significantly lower odds among refugees, after controlling for other factors. Interaction terms confirmed that the strength of the associations of these variables with the odds of modern contraceptive use varied significantly by refugee status.ConclusionUse of modern methods of contraception was lower among non-refugee and refugee Somali women compared to the national average and refugee status moderated the association of some predictor variables with the odds of modern contraceptive use. To increase use of modern contraceptives in urban areas, it is recommended that the Ministry of Health, refugee agencies, and county governments engage with the Somali community and implement appropriate interventions to empower refugee women economically and promote their access to and use of voluntary contraception services as soon as they settle in urban areas

    Malaria in school-age children in Africa: an increasingly important challenge.

    School-age children have attracted relatively little attention as a group in need of special measures to protect them against malaria. However, increasing success in lowering the level of malaria transmission in many previously highly endemic areas will result in children acquiring immunity to malaria later in life than has been the case in the past. Thus, it can be anticipated that in the coming years there will be an increase in the incidence of both uncomplicated and severe malaria in school-age children in many previously highly endemic areas. In this review, which focuses primarily on Africa, recent data on the prevalence of malaria parasitaemia and on the incidence of clinical malaria in African school-age children are presented and evidence that malaria adversely effects school performance is reviewed. Long-lasting insecticide treated bednets (LLIN) are an effective method of malaria control but several studies have shown that school-age children use LLINs less frequently than other population groups. Antimalarial drugs are being used in different ways to control malaria in school-age children including screening and treatment and intermittent preventive treatment. Some studies of chemoprevention in school-age children have shown reductions in anaemia and improved school performance but this has not been the case in all trials and more research is needed to identify the situations in which chemoprevention is likely to be most effective and, in these situations, which type of intervention should be used. In the longer term, malaria vaccines may have an important role in protecting this important section of the community from malaria. Regardless of the control approach selected, it is important this is incorporated into the overall programme of measures being undertaken to enhance the health of African school-age children

    Market intelligence for informing crop-breeding decisions by CGIAR and NARES

    Crop breeding by the Consultative Group on International Agricultural Research (CGIAR), in partnership with national agricultural research and extension systems (NARES) and privately owned seed companies, forms the backbone of the research and development (R&D) effort needed to ensure future food security in developing regions. Over the past decades, varieties derived from CGIAR germplasm have helped provide food for hundreds of millions living in harsh and vulnerable environments. However, the capacity to deliver increased benefits from crop breeding will depend on effective strategies to address the stubbornly slow rate of uptake of improved varieties. Meeting the challengewill require, among other inputs, deeper insights on the evolving preferences and requirements of farmers, processors and consumers. In short, breeding will need to be led more by demand. Key to the success of demand-led breeding will be the availability of reliable and timely intelligence on the current and future preferences and requirements of farmers, processors and consumers. Although ‘market intelligence’ has existed in various forms in CGIAR’s social science research agenda, an opportunity exists for increased relevance based on shared approaches and tools and new partnerships, both within CGIAR and between CGIAR and its partners. Beginning in 2022, a coordinated effort through the CGIAR Initiative on Market Intelligence looks to build cross-functional and transdisciplinary teams to provide market intelligence in support of crop breeding and seed-system development

    Quantifying previous SARS-CoV-2 infection through mixture modelling of antibody levels.

    As countries decide on vaccination strategies and how to ease movement restrictions, estimating the proportion of the population previously infected with SARS-CoV-2 is important for predicting the future burden of COVID-19. This proportion is usually estimated from serosurvey data in two steps: first the proportion above a threshold antibody level is calculated, then the crude estimate is adjusted using external estimates of sensitivity and specificity. A drawback of this approach is that the PCR-confirmed cases used to estimate the sensitivity of the threshold may not be representative of cases in the wider population-e.g., they may be more recently infected and more severely symptomatic. Mixture modelling offers an alternative approach that does not require external data from PCR-confirmed cases. Here we illustrate the bias in the standard threshold-based approach by comparing both approaches using data from several Kenyan serosurveys. We show that the mixture model analysis produces estimates of previous infection that are often substantially higher than the standard threshold analysis

    The mode of inheritance in tetraploid cut roses

    Tetraploid hybrid tea roses (Rosa hybrida) represent most of the commercial cultivars of cut roses and form the basis for breeding programmes. Due to intensive interspecific hybridizations, modern cut roses are complex tetraploids for which the mode of inheritance is not exactly known. The segregation patterns of molecular markers in a tetraploid mapping population of 184 genotypes, an F1 progeny from a cross of two heterozygous parents, were investigated for disomic and tetrasomic inheritance. The possible occurrence of double reduction was studied as well. We can exclude disomic inheritance, but while our observations are more in line with a tetrasomic inheritance, we cannot exclude that there is a mixture of both inheritance modes. Two novel parental tetraploid linkage maps were constructed using markers known from literature, combined with newly generated markers. Comparison with the integrated consensus diploid map (ICM) of Spiller et al. (Theor Appl Genet 122:489–500, 2010) allowed assigning numbers to each of the linkage groups of both maps and including small linkage groups. So far, the possibility of using marker-assisted selection in breeding of tetraploid cut roses and of other species with a tetrasomic or partly tetrasomic inheritance, is still limited due to the difficulties in establishing marker-trait associations. We used these tetraploid linkage maps to determine associations between markers, two morphological traits and powdery mildew resistance.
