    Association of Khat and alcohol use with HIV infection and age at first sexual initiation among youths visiting HIV testing and counseling centers in Gamo-Gofa Zone, South West Ethiopia

    BACKGROUND: HIV/AIDS is the major problem and an obstacle to both the health and development of people in Ethiopia today. It is also indicated that the use of substances have dramatically increased despite the serious concern about HIV infection. METHODS: Unmatched case control study was conducted in South West Ethiopia using a sample of 105 cases and 305 controls. Multivariate logistic regression was used to assess the degree of association between dependent and independent variables. RESULT: HIV infection was positively associated with being in the age of 20 – 24 years [OR & (95% CI) = 2.892 (1.266, 6.607)], being female [OR & (95% CI) = 2.013 (1.061, 3.822)], alcohol use [OR & (95% CI) = 5.883 (3.034, 11.408)], having no education [OR & (95% CI) = 3.193 (1.523, 6.695)] and primary education level [OR & (95% CI) = 3.160 (1.351, 7.388)]. Early sexual initiation was also positively associated with being not employed Adj. HR & (95% CI) = 7.372 (1.455, 37.357)], not having comprehensive knowledge on HIV/AIDS [Adj. HR & (95% CI) = 8.247 (2.121, 32.067)], alcohol use [Adj. HR & (95% CI) = 3.815 (1.315, 11.070)] and khat use [Adj. HR & (95% CI) = 7.241 (1.871, 28.016)]. CONCLUSION: Strategies should be designed to control the use of alcohol and khat which were found to be predictors of HIV infection and early sexual initiation in this study

    Prevalence and Associated Factors of Hypertension among Civil Servants Working in Arba Minch Town, South Ethiopia

    Despite Hypertension is a global public health challenge and a leading modifiable risk factor for cardiovascular disease and death attention was not given in developing countries. Therefore measuring the prevalence and identifying predictors of Hypertension is very important. Institution based cross sectional study design was employed from March–April, 2016 by taking 319 randomly selected civil servants working in in Arba Minch town. Data was collected using structured questionnaire and standardized instruments for physical examination by 5 trained nurses. SPSS version 20 was used for data analysis. Bi-variable and Multivariate logistic regression was employed for analysis of risk factors. The mean SBP and DBP of study participants were 120.87 + 14.15 mmHg and 80.28 + 8.8 mmHg, respectively. The prevalence of hypertension was found to be 27.8% (95% CI = 22.9-32.7%). Civil servants of age 50 years and above [AOR = 13.3], age 40-49 years [AOR = 5], age 30-39 years [AOR = 3.5], abdominal obesity [AOR=12.2], general obesity [AOR = 4.2], stress status [AOR = 12.3], current alcohol drink [AOR = 3.3], ex-drinker [AOR = 8.9] and family history of hypertension [AOR = 5.6] were found to be significantly associated with hypertension. The prevalence indicates that it is hidden epidemic in this population; therefore for screening and risk reduction program are needed

    Community Perception and Attitude towards people with Depression among Adults Residing in Arba Minch Health and Demographic Surveillance Site (AM-HDSS), Southern Ethiopia

    BACKGROUND: Negative perception and attitude of community prevents many people with depression and their caregivers from seeking help and receiving adequate treatment due to fear of social reaction and may try to hide the illness. The reasons for negative attitudes are not consistent across communities or cultures. Therefore, understanding the level of community perception and attitude towards people with depression is important to develop an intervention to reduce the impact of mental illness.METHODS: A community-based cross-sectional study was conducted among 617 randomly selected adults. The data was collected using structured, pre-tested, and interviewer-administered questionnaires. Descriptive statistics like frequency, mean, and median were performed. Bi-variable and multivariable logistic regression analyses were performed to identify the factors that affect the community attitude towards people with depression.RESULTS: Of the study population, 325(52.7%) had a good perception and 246(39.9%) had a positive attitude towards people with depression. The majority of study participants frequently identified as the perceived cause of depression was substance misuse, loss of loved one, and conflict within a family. In addition, psychosocial treatment was the most preferred treatment for people with depression in the study area. Marital status and educational status were significantly associated with the community attitude towards people with depressionCONCLUSION: Giving special attention to people with substance misuse, loss of loved one, and conflict within a family is very vital for the prevention of depression. In addition, future mental health promotion activities should focus on cause and common manifestation of depression to improve the attitude toward people with depression.&nbsp

    Determinants of delay in care seeking for diarrheal diseases among mothers/caregivers with under-five children in public health facilities of Arba Minch town, southern Ethiopia; 2019.

    BACKGROUND:Timely and appropriate health care seeking for diarrhea of under-five children is important to reduce severe and life-threatening complications. However, different findings indicate that mothers of under-five children often delay in seeking care which in turn contributes to the death of large number of children without ever reaching a health facility. Therefore, a proper pinpointing of determinants of delay in seeking care informs intervention strategies for health service planners. OBJECTIVES:Of this study was to identify the determinants of delay in care seeking for diarrheal disease among mothers/caregivers of under-five children in public health facilities of Arba Minch town, South Ethiopia, 2019. METHODS:Facility based case control study was conducted from March 4 to April 30, 2019. Total sample size was 400. Cases were selected by systematic random sampling technique while controls were mothers of under-five children with signs and symptoms of diarrhea who came to the same health facility within 24 hours following cases. Data was collected by using pretested structured questionnaire by three data collectors and entered into EpiData V4 and exported to SPSS V23 for further analysis. Bivariable logistic regression was done to identify variables candidate for Multivariable LR at p-value<0.25. Multivariable logistic regression was done and p-value <0.05 and 95%CI of AOR was used to declare statistical significance. RESULT:Female sex[AOR = 1.93, (95%CI: 1.11,3.36)], child age <24 months[AOR = 4.47,95%CI:2.51,7.97)], mothers'/caregivers without formal education[AOR = 6.90, (95%CI:3.10,15.37)], and attended primary school [AOR = 3.12,(95%CI:1.44,6.73)], poorest household wealth index category[AOR = 2.81, (95%CI:1.20,6.58) and poor household wealth index category [AOR = 2.61,(95%CI: 1.12, 6.09)], mothers/caregivers who did not visit health facility to first episode diarrhea [AOR = 4.55, (95%CI:2.41,8.59)], mothers/caregivers who were satisfied in the last six month visit [AOR = 0.29, (95%CI:0.15,0.55)], and poor perceived health care professionals respect[AOR = 4.91, (95%CI:2.64,9.15)] were important determinants of delay in seeking care. CONCLUSIONS:Sex and age of the child, educational status of the mother/caregiver, poor wealth index category, not visiting health facility at first response, satisfaction with the care and examination, and respect of health care professionals were important determinants of delay in seeking care among mothers/caregivers of under-five children with diarrhea illness. All concerned body should focus interventions on poor and less educated mothers/caregivers with emphasis on female children and <24 months. Health workers are needed to provide respectful service to promote satisfaction level of clients

    The Prevalence and Associated Factors of Hypertension among Adults in Southern Ethiopia

    Background. Hypertension is a growing public health problem in many developing countries including Ethiopia. Determining the prevalence of hypertension and identifying the associated factors is crucial. Objective. To assess the prevalence of hypertension and associated factors, among adult population of Arba Minch town, Gamo Zone, Southern Nations, Nationalities and Peoples Region, Ethiopia. Methods. A cross-sectional study design was conducted from December 1 to 30, 2017 among adults. Study participants were selected using a multistage systematic sampling method. Data were collected by face-to-face interview after getting written informed consent by using a structured questionnaire. Additionally, weight, height, and blood pressure of participants were measured following standard procedures. Data were entered into a computer using EPI INFO 7 and exported into SPSS version 20 for analysis. Bivariate and multivariable analyses were performed to explore the association between hypertension and associated factors. Multivariable logistic regressions were fitted to control the effect of confounders. Results. A total of 784 study participants were included in this study. The overall prevalence of hypertension in Arba Minch Town was 35.2%, (95% CI: 32.4%, 38.4%). Nearly 90% of hypertensive patients were screened for the first time. Age ≥55 years [AOR=7.74; 95% CI: 2.19, 27.23], income level which is greater than 2501 Ethiopian Birr [AOR=9.5; 95% CI: 4.5, 20.20], working hour less than seven hours per day [AOR=12.5; 95% CI: 4.3, 36.1], and chewing “khat” [AOR=11.06: 95% CI: 4.3, 27.7] were the independently associated factors with hypertension. Conclusion. The prevalence of hypertension is found to be high. Increasing awareness on control use of “khat,” increasing physical activity, and strengthening community-based periodic screening programs of high-risk populations are recommended

    Predictors of time-to-recovery from severe acute malnutrition treated in an outpatient treatment program in health posts of Arba Minch Zuria Woreda, Gamo zone, Southern Ethiopia: A retrospective cohort study.

    BackgroundAccess to outpatient therapeutic feeding programs for all children who had uncomplicated severe acute malnutrition (SAM) in need is a global health priority. In Ethiopia SAM is treated in hospitals, health centers and health posts. Health extension workers (HEWs) manage SAM that is uncomplicated at the health posts through the outpatient therapeutic feeding programs (OTP). Identifying predictors that predict time-to-recovery of children on OTP is thus vital to optimizing therapeutic success. However, the factors affecting children's' recovery time at this peripheral health institutions were not well documented. Therefore, this study aimed to identify predictors of time-to-recovery from SAM among children treated at an OTP in health posts of Arba Minch Zuria woreda, Gamo Zone, Southern Ethiopia.MethodsA retrospective cohort study was conducted on 402 children enrolled in an OTP in the health posts of Arba Minch Zuria woreda based on data abstracted from their medical records. The study children were selected using systematic random sampling method using a list of their medical record numbers. Both descriptive and analytic analyses were performed. Median time of recovery was estimated by using the Kaplan-Meier survival curve. Furthermore, bivariate and multivariable Cox proportional hazard regression analyses were used to identify factors significantly associated with outcome variable.ResultThe median time-to-recovery from severe acute malnutrition among children was 49 days (Interquartile range [IQR]: 42-56). Among the participants, 70.40% with 95% CI: (74.2-85.0%) recovered from severe acute malnutrition. The Cox-proportional hazard analysis showed that children's age at admission (Adjusted hazards ratio [AHR] = 3.15; 95% confidence interval [CI]: 1.85, 5.03), diagnosis with edema (AHR = 1.75, 95%CI: (1.27, 2.43), co-morbidity of diarrhea (AHR = 0.22, 95% CI(0.13, 0.39), and anemia (AHR = 0.64, 95% CI:(0.42, 0.98) were found to be predictors of time to recovery from SAM.ConclusionsThe median time-to-recovery at the health posts in this study was in the accepted time period for the maximum Ethiopian standard protocol set for the management of SAM. However, the nutritional recovery rate was lower than the minimum acceptable threshold for the Sphere International Standards. Therefore, early screening of co-morbidity like diarrhea, anemia and edemaand timely intervention would increase the chance of recovery of children
