A randomized comparison trial of two and four-step approaches to teaching Cardio-Pulmonary Reanimation
Background and aim of the work: The treatment of cardiac arrest in an extra-hospital environment improves with the increase in the number of people able to establish an early Cardio-Pulmonary Reanimation (CPR). The main aim of the study was to assess the validity of the two-step method in case of prolonged CPR. Methods: A randomized comparison study was conducted in the University Nursing School of a Northern Italian town, during the 2015/16 academic year, among 60 students, to teach them CPR techniques, through two different teaching methods (4-step and the 2-step of CPR training). The effectiveness of the maneuvers performed on mannequins equipped with skill-meter was verified. Results: Our study did not highlight any significant difference between the two methods of CPR training. The comparison between the two methods regarding their efficacy in practical teaching of CPR, highlighted by this study, proved the validity of both the 4-minute continuous method (1st method) and the 30:2 method (2nd method). Conclusions: The results of the study showed no differences between the 2-step and the 4-step methods, in the effectiveness of cardiac massage. The correct execution of chest compressions during a CPR is the key to increase the patient’s chances of rescue. Research has shown that any interruption in the execution of chest compressions, leads to a progressive reduction of the effectiveness of cardiac massage, with negative consequences on the prognosis of the patient undergoing at CPR
Effectiveness of a video lesson for the correct use in an emergency of the automated external defibrillator (AED)
Background and aim of the work: Every year around 275 thousand people in Europe and 420 thousand
in the United States are affected by sudden cardiac arrest. Early electrical defibrillation before the arrival
of emergency services can improve survival. Training the population to use the AED is essential. The training
method currently in use is the BLSD course, which limits training to a population cohort and may not be
enough to meet the requirements of the proposed Law no. 1839/2019. This study aims to verify the effectiveness
of an online course that illustrates the practical use of the AED to a population of laypeople. Methods: An
observational study was conducted to compare a lay population undergoing the view of a video spot and a cohort
of people who had participated in BLSD Category A courses. The performances of the two groups were
measured immediately after the course and 6 months later. Results: Overall, the video lesson reported positive
results. Six months later the skills were partially retained. The cohort that followed the video lesson showed
significant deterioration in the ability to correctly position the pads and in safety. Conclusions: Although improved
through significant reinforcements, the video spot represents a valid alternative training method for
spreading defibrillation with public access and could facilitate the culture of defibrillation as required by the
new Italian law proposal
MORFEO enters final design phase
MORFEO (Multi-conjugate adaptive Optics Relay For ELT Observations, formerly
MAORY), the MCAO system for the ELT, will provide diffraction-limited optical
quality to the large field camera MICADO. MORFEO has officially passed the
Preliminary Design Review and it is entering the final design phase. We present
the current status of the project, with a focus on the adaptive optics system

How much do periventricular lesions assist in distinguishing migraine with aura from CIS?
Objective To evaluate in clinically isolated syndrome (CIS) and migraine with aura (MA) how the number of periventricular lesions (PVLs) detected at MRI influences diagnostic performance when the Magnetic Resonance Imaging in Multiple Sclerosis (MAGNIMS) or the 2017 revised criteria are applied. Methods In this retrospective study, white matter hyperintensities (WMH) of 84 patients with MA and 79 patients with CIS were assessed using manual segmentation technique. Lesion probability maps (LPMs) and voxel-wise analysis of lesion distribution by diagnosis were obtained. Furthermore, we performed a logistic regression analysis based on lesion locations and volumes. Results Compared to patients with MA, patients with CIS showed a significant overall higher T2WMH mean number and volume (17.9 ± 16.9 vs 6.2 ± 11.9 and 3.1 ± 4.2 vs 0.3 ± 0.6 mL; p < 0.0001) and a significantly higher T2 WMH mean number in infratentorial, periventricular, and juxtacortical areas (p < 0.0001). LPMs identified the periventricular regions as the sites with the highest probability of detecting T2 WMH in patients with CIS. Voxel-wise analysis of lesion distribution by diagnosis revealed a statistically significant association exclusively between the diagnosis of CIS and the PVLs. MAGNIMS criteria demonstrated the highest specificity in differentiating patients with CIS from patients with MA (100% vs 87%) against a predictable lower sensitivity (63% vs 72%). Conclusions PVLs play a key role in the differential diagnosis between MA and CIS, particularly when there are more than 3. Future studies on multiple sclerosis criteria might reconsider the 3 PVLs to minimize the risk of misdiagnosis

Effect of SARS-CoV-2 mRNA vaccination in MS patients treated with disease modifying therapies
Background: In patients with Multiple Sclerosis (pwMS) disease-modifying therapies (DMTs) affects immune response to antigens. Therefore, post-vaccination serological assessments are needed to evaluate the effect of the vaccine on SARS-CoV-2 antibody response.Methods: We designed a prospective multicenter cohort study enrolling pwMS who were scheduled for SARS-Cov-2 vaccination with mRNA vaccines (BNT162b2, Pfizer/BioNTech,Inc or mRNA-1273, Moderna Tx, Inc). A blood collection before the first vaccine dose and 4 weeks after the second dose was planned, with a centralized serological assessment (electrochemiluminescence immunoassay, ECLIA, Roche-Diagnostics). The log-transform of the antibody levels was analyzed by multivariable linear regression.Findings: 780 pwMS (76% BNT162b2 and 24% mRNA-1273) had pre- and 4-week post-vaccination blood assessments. 87 (11.2%) were untreated, 154 (19.7%) on ocrelizumab, 25 (3.2%) on rituximab, 85 (10.9%) on fingolimod, 25 (3.2%) on cladribine and 404 (51.7%) on other DMTs. 677 patients (86.8%) had detectable post-vaccination SARS-CoV-2 antibodies. At multivariable analysis, the antibody levels of patients on ocrelizumab (201-fold decrease (95%CI=128-317), p < 0.001), fingolimod (26-fold decrease (95%CI=16-42), p < 0.001) and rituximab (20-fold decrease (95%CI=10-43), p < 0.001) were significantly reduced as compared to untreated patients. Vaccination with mRNA-1273 resulted in a systematically 3.25-fold higher antibody level (95%CI=2.46-4.27) than with the BNT162b2 vaccine (p < 0.001). The antibody levels on antiCD20 therapies correlated to the time since last infusion, and rituximab had longer intervals (mean=386 days) than ocrelizumab patients (mean=129 days).Interpretation: In pwMS, anti-CD20 treatment and fingolimod led to a reduced humoral response to mRNA-based SARS-CoV-2 vaccines. As mRNA-1273 elicits 3.25-higher antibody levels than BNT162b2, this vaccine may be preferentially considered for patients under anti-CD20 treatment or fingolimod. Combining our data with those on the cellular immune response to vaccines, and including clinical follow-up, will contribute to better define the most appropriate SARS-CoV-2 vaccine strategies in the context of DMTs and MS.
