    Teacher training and internships. Proposed guidelines for schools

    This paper presents the results of a study on the first year of direct internships as a part of the Sapienza University of Rome’s Primary Education Sciences degree course (LM 85-bis) in the academic year 2017-2018. Out of this study came a theoretical-operational contribution which focuses on the perception the interns had of the schools hosting them. This work led to the development of proposed guidelines for schools selected for LM 85-bis degree course internships. La formazione al lavoro dei futuri insegnanti attraverso il tirocinio. Una proposta di linee guida per le scuoleL’articolo riporta i risultati di un’indagine sul primo anno di tirocinio diretto nel corso di laurea magistrale (LM 85-bis) in Scienze della formazione primaria dell’Università di dell’Università di Roma "Sapienza" nell’anno accademico 2017-2018. A partire da tale esperienza è stato elaborato un contributo teorico-operativo focalizzato sulla percezione che i tirocinanti hanno delle scuole ospitanti.     Diffusion and transmission of carbapenem-resistant Klebsiella pneumoniae in the medical and surgical wards of a university hospital in Milan, Italy

    Summary: Carbapenem-resistant Klebsiella pneumoniae (CRKP) is emerging as a public health problem worldwide. In Italy, a remarkable increase in CRKP cases has been reported since 2010. In this study, CRKP diffusion, distribution and in-hospital transmission trends were evaluated in a university hospital in Milan, Italy, from January 2012 to December 2013. Isolates from 63 newly detected CRKP-positive patients were genotyped, and possible transmission was determined by combining the molecular results with data concerning the patients' admission and in-hospital transfers. Most of the cases (90.4%) were from general medical and surgery wards, and the remaining 9.6% were from the intensive care unit. Fifteen of the 46 hospital-associated cases (32.6%) were attributable to in-hospital transmission. After the introduction of targeted and hospital-wide control measures, the transmission index significantly decreased from 0.65 to 0.13 (p = 0.01). There was also a decrease in the overall nosocomial case incidence, from 0.37 to 0.17 per 1000 person-days (p = 0.07).Our findings indicate that the spread of CRKP in Northern Italy hospitals may go far beyond high-risk settings (i.e., intensive care units) and that strict surveillance should be extended to general areas of care.     Amyotrophic Lateral Sclerosis, a Multisystem Pathology: Insights into the Role of TNF\u3b1

    Amyotrophic lateral sclerosis (ALS) is considered a multifactorial, multisystem disease in which inflammation and the immune system play important roles in development and progression. The pleiotropic cytokine TNF\u3b1 is one of the major players governing the inflammation in the central nervous system and peripheral districts such as the neuromuscular and immune system. Changes in TNF\u3b1 levels are reported in blood, cerebrospinal fluid, and nerve tissues of ALS patients and animal models. However, whether they play a detrimental or protective role on the disease progression is still not clear. Our group and others have recently reported opposite involvements of TNFR1 and TNFR2 in motor neuron death. TNFR2 mediates TNF\u3b1 toxic effects on these neurons presumably through the activation of MAP kinase-related pathways. On the other hand, TNFR2 regulates the function and proliferation of regulatory T cells (Treg) whose expression is inversely correlated with the disease progression rate in ALS patients. In addition, TNF\u3b1 is considered a procachectic factor with a direct catabolic effect on skeletal muscles, causing wasting.     Detection of ST1702 Escherichia coli blaNDM-5 and blaCMY-42 genes positive isolates from a Northern Italian hospital

    We describe two multi drug-resistant (MDR) carbapenemase-producing Escherichia coli clinical isolates from an acute hospital in Milan. Both strains, isolated from a surgical wound sample and a surveillance rectal swab respectively, were positive for a blaNDM-type gene by Xpert Carba-R test. The whole-genome sequence characterization disclosed several resistance determinants: blaNDM-5, blaCMY-42, blaTEM-198, rmtB, mphA. The two isolates belonged to phylogenetic group A, sequence type (ST) 1702 and serotype O89:H9. PCR-based replicon typing and conjugation assay demonstrated an IncI1 plasmid localization for both blaNDM-5 and blaCMY-42 genes. This is the first report of a ST1702 NDM-5 and CMY-42- producing E. coli clone in Italy

    Mingling of human and veterinary strains of Staphylococcus aureus : an emerging issue in health-care systems

    Aim: Methicillin-resistant Staphylococcus aureus remains a leading cause of hospital and community infections. We report a retrospective molecular characterization of S. aureus strains from different settings: hospital workers and patients, and veterinarian surgeons and pets. Materials and Methods: Eighty-nine S. aureus isolates obtained from nasal swabs of 10 patients, 17 health-care workers (HCWs), 9 pets, and 53 veterinarians were genotypically characterized by means of repetitive extragenic palindromic polymerase chain reaction (Rep PCR) and whole-genome sequencing. Results: Thirteen different sequence types (STs) were detected: ST398, ST22, ST8, ST30, ST15, ST5, ST121, ST45, ST10, ST6, ST34, ST97, and ST1. Two new STs differing from ST22 and ST5 for a single multilocus sequence typing gene were also identified. Rep PCR documented a genetic relationship among isolates obtained from 5 veterinarians and 10 HCWs. Conclusion: The large diversity of S. aureus strains detected may reflect a larger epidemiology within the hospital and community, in which companion animals likely act as a reservoir. We identified the circulation of ST5, ST8, ST15, ST22, ST30, ST45, and ST121 both in the hospital and veterinarian environment.     GPS and Galileo Developments on Board the International Space Station With the Space Communications and Navigation (SCaN) Testbed

    The Space Communications and Navigation (SCaN) is a facility developed by NASA and hosted on board the International Space Station (ISS) on an external truss since 2013.It has the objective of testing navigation and communication experimentations with a Software Defined Radio (SDR) approach, which permits software updates for testing new experimentations.NASA has developed the Space Telecommunications Radio System (STRS) architecture standard for SDRs used in space and ground-based platforms to provide commonality among radio developments to provide enhanced capability. The hardware is equipped with both L band front-end radios and the NASA space network communicates with it using S-band, Ku-band and Ka-band links.In May 2016 Qascom started GARISS (GPS and Galileo Receiver for the ISS), an activity of experimentation in collaboration with ESA and NASA that has the objective to develop and validate the acquisition and processing of combined GPS and Galileo signals on board the ISS SCaN testbed. This paper has the objective to present the mission, and provide preliminary details about the challenges in the design, development and verification of the waveform that will be installed on equipment with limited resources. GARISS is also the first attempt to develop a waveform for the ISS as part of an international collaboration between US and Europe. Although the final mission objective is to target dual frequency processing, initial operations will foresee a single frequency processing. Initial results and trade-off between the two options, as well as the final decision will be presented and discussed. The limited resources on board the SCaN with respect to the challenging requirements to acquire and track contemporaneously two satellite navigation systems, with different modulations and data structure, led to the need to assess the possibility of aiding from ground through the S-band. This option would allow assistance to the space receiver in order to provide knowledge of GNSS orbits and reduce the processing on board. Trade off and various options for telemetry and uplink data are presented and discussed. Finally, integration and validation of the waveform are one of the major challenges of GARISS: The Experiment Development System (EDS) and the the Ground Integration Unit (GIU) for VV will be used prior to conducting the experiment on the ISS. The EDS can be used in lab environment and allows prototyping and verification activities with the simulator, but does not include all hardware components. The GIU on the other side is the flight model which replicates the flying equipment, but has limited flexibility for testing.As conclusion, the project is now approaching the Preliminary Design Review (PDR) and indeed only preliminary results are available. This paper is an opportunity to present the GARISS mission as part of an International cooperation between ESA, NASA and Qascom. The preliminary results include GPS and Galileo processing from space signals, the challenges and trade off decisions, the high level STRS architecture and foreseen experimentation campaign. Detailed results from the test campaigns are expected in 2017
