    Photodynamic Therapy Combined with Antibiotics or Antifungals against Microorganisms That Cause Skin and Soft Tissue Infections: A Planktonic and Biofilm Approach to Overcome Resistances

    The present review covers combination approaches of antimicrobial photodynamic therapy (aPDT) plus antibiotics or antifungals to attack bacteria and fungi in vitro (both planktonic and biofilm forms) focused on those microorganisms that cause infections in skin and soft tissues. The combination can prevent failure in the fight against these microorganisms: antimicrobial drugs can increase the susceptibility of microorganisms to aPDT and prevent the possibility of regrowth of those that were not inactivated during the irradiation; meanwhile, aPDT is effective regardless of the resistance pattern of the strain and their use does not contribute to the selection of antimicrobial resistance. Additive or synergistic antimicrobial effects in vitro are evaluated and the best combinations are presented. The use of combined treatment of aPDT with antimicrobials could help overcome the difficulty of fighting high level of resistance microorganisms and, as it is a multi-target approach, it could make the selection of resistant microorganisms more difficult

    INTRODUCTION Leprosy is a chronic granulomatous infection caused by Mycobacterium leprae with varied clinical presentations. Owing to its low incidence, together with its myriad clinical presentations, leprosy poses a diagnostic challenge and can easily be confused with other infective and noninfective granulomatous dermatoses.1 Dermoscopy is a simple and noninvasive technique that is widely used for the diagnosis and monitoring of melanoma and nonmelanoma skin cancers and many inflammatory disorders. However, the utility of this technique for the diagnosis of granulomatous diseases has only recently been demonstrated.2 Although dermoscopy shows diagnostically useful patterns in infections and inflammatory diseases with granulomatous presentations, the dermoscopic features of leprosy are poorly documented in the literature. CASE REPORT A 45-year-old Moroccan man who lived in Spain for many years, but frequently visited his country of origin, was seen at the dermatology service for skin lesions on the trunk and upper and lower extremities. The lesions, which appeared 5 years earlier, were progressive in nature, caused intense itching, and were treated with topical corticosteroids and oral antihistamines without improvement.     Follow-up Study to Evaluate the Long-term Safety and Efficacy of Darvadstrocel (Mesenchymal Stem Cell Treatment) in Patients With Perianal Fistulizing Crohn’s Disease: ADMIRE-CD phase 3 randomized controlled trial

    BACKGROUND: Darvadstrocel is an expanded allogeneic adipose-derived mesenchymal stem cell therapy for the treatment of complex perianal fistulas in patients with Crohn’s disease. Safety and efficacy outcomes from the clinical trial known as “Adipose derived mesenchymal stem cells for induction of remission in perianal fistulizing Crohn’s disease,” or ADMIRE-CD (NCT01541579), from up to 52 weeks posttreatment were previously reported. Here, the outcomes from an extended 104-week follow-up are reported. OBJECTIVE: The goal of this study was to assess the long-term safety and efficacy of darvadstrocel at 2 years post-treatment in patients with Crohn’s disease and complex perianal fistulas. DESIGN: This was a phase 3 double-blind randomized controlled study (ADMIRE-CD) in patients with perianal fistulizing Crohn’s disease. SETTINGS: This study extension was conducted in multiple hospitals across 7 European countries and Israel. PATIENTS: Forty patients entered the extended follow-up period: 25 patients in the darvadstrocel treatment group and 15 in the control group. INTERVENTIONS: Darvadstrocel or saline solution (control group) was administered once, locally, after fistula tract curettage and internal opening closure (with previous seton placement). All patients were permitted to continue ongoing medical treatments for fistulas. MAIN OUTCOME MEASURES: Treatment-emergent serious adverse events were recorded through week 104. Clinical remission, defined as closure of all treated external openings that were draining at baseline despite gentle finger compression, was assessed at week 104. RESULTS: Of 40 patients, 37 completed the extended follow-up. Through week 104, 7 treatment-emergent serious adverse events were reported, of which 4 occurred between weeks 52 and 104. At week 104, clinical remission was reported in 14/25 (56%) patients in the darvadstrocel group and 6/15 (40%) patients in the control group. LIMITATIONS: Limitations include the small number of patients who entered the extended follow-up period, and no imaging examinations were performed at the 104-week time point. CONCLUSIONS: Darvadstrocel was well tolerated and clinical remission after treatment with darvadstrocel may be sustained for up to 104 weeks in patients with perianal fistulizing Crohn’s disease

    Spanish medical students’ attitudes and views towards Mental Health and Psychiatry: a multicentric cross-sectional study.

    Objective The aim of this study is to investigate the attitudes towards mental illness and psychiatry among fifth year Spanish medical students. Methods The study included 171 students from three medical schools located in different areas of Spain: Cádiz; UCA (n= 113), Madrid; San Pablo-CEU (n=22), and Barcelona; UAB (n=36). They responded, prior to their undergraduate medical course in psychiatry, to the AMI questionnaire to measure the attitudes towards mental illness and to Balon’s adapted questionnaire to investigate their view towards psychiatry. Results The students (93.4 %) had a positive attitude towards mental illness (AMI). Attitudes towards psychiatry were fairly positive with a few negative views, specifically regarding the role of psychiatrists (items 11 and 13) and the prestige of the specialty (item 16). There were some statistically significant differences between the three medical schools in the perception of psychiatry as a medical discipline. A better attitude towards mental illness was associated with a better view of the overall merits of psychiatry. Conclusions Findings suggest that Spanish medical students do not have a negative attitude towards mental illness and they have a good perception of psychiatry, although there are still some misconceptions about this specialty. These student’s attitudes could favor an appropriate management of patients suffering from mental illness

    La terapia con luz es una alternativa en el tratamiento de la hidradenitis supurativa. El objetivo de este estudio fue evaluar la eficacia, la efectividad y la seguridad de las distintas modalidades de terapia con luz en la hidradenitis supurativa mediante una revisión sistemática que actualiza la realizada en 2015 procedente de la Cochrane Library. Se identificaron los estudios mediante una búsqueda electrónica en las bases de datos: MEDLINE, EMBASE, Centre for Reviews and Dissemination database de la Universidad de York, Cochrane Database of Systematic Reviews, Cochrane Skin Group Specialised Skin Register, Centre of Evidence Based Dermatology de la Universidad de Nottingham y TESEO; finalmente se incluyeron 6 series de casos, 3 revisiones sistemáticas y 2 ensayos clínicos de pacientes tratados con terapia fotodinámica convencional o intralesional, láser Nd:YAG, CO2 o diodo y luz intensa pulsada, con un total de 248 pacientes tratados. Treatment options for hidradenitis suppurativa include light-based therapy. This systematic review aimed to update our understanding of the efficacy, effectiveness and safety of these treatments for hidradenitis suppurativa by evaluating the literature published since the 2015 Cochrane review on this topic.     In vitro effect photodynamic therapy with differents photosensitizers on cariogenic microorganisms

    Background: Antimicrobial photodynamic therapy has been proposed as an alternative to suppress subgingival species. This results from the balance among Streptococcus sanguis, Streptococcus mutans and Candida albicans in the dental biofilm. Not all the photosensitizers have the same photodynamic effect against the different microorganims. The objective of this study is to compare in vitro the photodynamic effect of methylene blue (MB), rose Bengal (RB) and curcumin (CUR) in combination with white light on the cariogenic microorganism S. mutans, S. sanguis and C. albicans. Results: Photodynamic therapy with MB, RB and CUR inhibited 6 log 10 the growth of both bacteria but at different concentrations: 0.31-0.62 µg/ml and 0.62-1.25 µg/ml RB were needed to photoinactivate S. mutans and S. sanguis, respectively//1.25-2.5 µg/ml MB for both species//whereas higher CUR concentrations (80-160 µg/ml and 160-320 µg/ml) were required to obtain the same reduction in S. mutans and S. sanguis viability respectively. The minimal fungicidal concentration of MB for 5 log10 CFU reduction (4.5 McFarland) was 80-160 µg/ml, whereas for RB it ranged between 320 and 640 µg/ml. For CUR, even the maximum studied concentration (1280 µg/ml) did not reach that inhibition. Incubation time had no effect in all experiments. Conclusions: Photodynamic therapy with RB, MB and CUR and white light is effective in killing S. mutans and S. sanguis strains, although MB and RB are more efficient than CUR. C. albicans required higher concentrations of all photosensitizers to obtain a fungicidal effect, being MB the most efficient and CUR ineffective

    Skin Cancer Prevalence in Outdoor Workers of Ski Resorts

    Background. Snow reflectivity and altitude increase the exposure of ski resort workers to solar ultraviolet radiation. The aim was to assess the presence of skin cancer in ski resorts workers and compare it with other groups of outdoor workers reviewing published studies. Methods. An observational cross-sectional prospective study was conducted in the three largest ski resorts in Spain: Baqueira Beret, Lleida; Formigal, Huesca and Sierra Nevada, Granada. All outdoor workers including ski instructors were invited to participate in the study. The participants completed a validated questionnaire about sun exposure and underwent a skin examination. Results. 219 workers were included in the study (80% male; mean age 43.8 (SD 11.31) years). Actinic keratosis (AK) but no other skin cancers were detected in 32 participants (14.62%). Those with AK worked in the Southernmost ski resort, were more likely to have light colour hair, and were older and with higher photoaging grade than those without them. Conclusion. Compared to other studies, outdoor workers on ski resorts show a higher prevalence of AK than general population but a lower prevalence than other groups of outdoor workers. © 2020 Yolanda Gilaberte et al

    Respuesta hidrológica del Pirineo central al cambio ambiental proyectado para el siglo XXI

    Streamflows in five Mediterranean mountain headwaters in the central Spanish Pyrenees were projected under various climate and land use change scenarios. Streamflows were simulated using the Regional Hydro-Ecologic Simulation System (RHESSys). The results show that changes in precipitation and temperature could cause a decline of annual streamflow between 13% and 23%, depending on the considered catchment. When the effect of increased forest cover in the basins is added to climate change effects, the decrease in annual streamflow is enhanced up to 19% and 32%. The largest hydrological changes resulting from environmental change are projected mainly in early spring, summer and autumn, when the decline may exceed 40%. Winter is the least affected season by environmental change because of increased runoff as a consequence of reduced storage of water in the snowpack and an earlier onset of the snowmelt, and the lower consumption of water by vegetation during the cold season. The magnitude of hydrological change as a result of the assumed environmental change scenarios may lead to serious impacts on water management and ecology of the studied region, as well as the water availability in the Ebro basin.Se han simulado los caudales de cinco cabeceras de ríos en los Pirineos centrales españoles, considerando diferentes escenarios de cambio climático y de uso del suelo. Los caudales fueron simulados utilizando el modelo hidroecológico RHESSys (Regional Hydro-Ecologic Simulation System). Los resultados muestran que los cambios proyectados por un conjunto de modelos climáticos regionales en precipitaciones y temperaturas en el siglo XXI podrían causar una disminución del caudal anual entre el 13% y el 23%, dependiendo de la cuenca considerada. Cuando se añade a los efectos del cambio climático el efecto del aumento de la cubierta forestal en las cuencas, la disminución de los caudales anuales oscila entre el 19% y el 32%, dependiendo de la cuenca estudiada. Los mayores cambios hidrológicos se producirían a principios de primavera, verano y otoño, cuando la disminución puede superar el 40% respecto a los valores actuales. El invierno es la estación menos afectada como consecuencia del aumento de la escorrentía debido a una reducción del agua acumulada en forma de nieve y a un inicio más temprano de su fusión, así como por que durante los meses fríos el consumo de agua por parte de la vegetación es menor. La magnitud del cambio hidrológico, resultado de los escenarios de cambio ambiental, puede afectar seriamente a la gestión de los recursos hídricos y a las comunidades vegetales del Pirineo central, así como a la disponibilidad de agua en el conjunto de la cuenca del Ebro
