    The passage through high school imposes decisions, challenges and changes upon teenagers. However, there is a lack of areas for dialogues at schools. In this experience – conducted as a monograph defended to obtain the title of specialist in Educommunication in 2015 –, students worked in groups and played different roles in the production of a talk show, experiencing enriching dialogues, as well as the universe of work and research. This process allowed us to evaluate interpersonal relationships, educommunicative practices, and also enabled the critical reading of means of communication and the resignification of the information of the complex world we live in. The educommunicative practices of the research were recorded in a field diary and join forces so that educators can reflect on, renew, and expand communication at schools.A passagem pelo ensino médio impõe aos jovens decisões, desafios e mudanças. No entanto, há um esvaziamento dos espaços de diálogo nas escolas. Nesta experiência – executada como monografia apresentada para obtenção do título de especialista em Educomunicação em 2015 –, alunos atuaram em grupo e desempenharam diferentes papéis na produção de um talk show, desfrutando de diálogos enriquecedores e de uma vivência do universo do trabalho e da pesquisa. Esse processo permitiu avaliar relacionamentos interpessoais, práticas educomunicativas, além de possibilitar a leitura crítica dos meios de comunicação e a ressignificação das informações do mundo complexo em que vivemos. As práticas educomunicativas da pesquisa foram registradas num diário de campo e somam forças para que outros educadores reflitam, renovem e ampliem a comunicação no ambiente escolar

    An OSGi-based production process monitoring system for SMEs

    The present paper proposes an architecture for a product process monitoring system suitable for SMEs (Small-Medium Enterprises). The monitoring system is the main means by which decision-making systems based on intelligent automation technologies are aware of the state of the system on which they will take decisions. Methods and tools from best-practice and best-effort approaches are proposed in the context of SMEs, where the requirements of low cost, low initial level of digitisation and high production flexibility often coexist and contribute to the complexity of management and control problems in these companies. The paper focuses on the design of the monitoring system using an OSGi framework to meet industry standards and Industry 4.0 requirements, taking into account the peculiarities of SMEs as design constraints. The proposed architecture was first tested using a simulation tool and then implemented on a full-scale production line used for data collection

    Microchemical and microscopic characterization of the pictorial quality of egg-tempera polyptych, late 14th century, Florence, Italy

    This paper explores the added value of micro-chemical and microscopic approaches to gather scientific evidence that can technically explain the pictorial quality of an egg-tempera painting, and underpin assessments that otherwise would be based on naked eye observations only. Demonstration is here provided via the interdisciplinary investigation of the original technique used by Giovanni del Biondo in the polyptych Annunciation and Saints (1385 ca), Galleria dell'Accademia, Florence, Italy. The exquisite surface appearance makes this panel painting remarkable compared to artworks by coeval artists. Imaging techniques (UV, IR and IR false color), non-invasive single spot techniques (XRF and FORS spectrometry) and analytical investigations on eight selected micro-samples (ATR-FTIR, GC/MS and Py/GC–MS, ESEM-EDS) were combined to retrieve the palette and identify organic binding media and a superficial coating layer. Stratigraphic and micro-chemical data confirmed the use of a relatively simple egg-tempera technique applied on a ground made of gypsum mixed with animal glue, without complex stratigraphic superimposition of preparation and pictorial layers. Various pigments were identified, among which is the precious lapis lazuli. While Py/GC–MS highlight that the coating is made of dammar resin and honey mixed with animal glue, the results allow us to state that the painting was not intentionally varnished by Giovanni del Biondo. These outcomes shed a new light on the technical knowledge of this polyptych, and prove how challenging is the attempt to categorize egg-tempera recipes used by ancient painters at the turn of the 14th century

    Backgound. The follicular variant (FV) is one of the most common variants of papillary thyroid carcinoma (PTC). Clinically, the FVPTC is considered a low-risk variant of PTC, and the encapsulated forms of FVPTC represent a group of thyroid tumours with an overall good prognosis. Consequently, these neoplasms were very recently reclassified as "non-invasive follicular tumour with papillary-like nuclear features (NIFTP)". From a molecular standpoint, NIFTP appears similar to the follicular pattern thyroid neoplasm; however, limited data are currently available regarding their gene expression profile

    RARE POMC MUTATION IN A PATIENT WITH MYOTONIC DYSTROPHY TYPE 1 AND ADRENOCORTICOTROPIN HYPERRESPONSE TO CORTICOTROPIN-RELEASING HORMONE

    Objective: Myotonic dystrophy (DM) is a monogenic disorder. It is caused by expansion of a cytosine-thymineguanine triplet in the DMPK gene which encodes for myotonic dystrophy protein kinase (DMPK). Methods: A 24-year-old man with DM and the DMPK mutation presented with elevated adrenocorticotropic hormone (ACTH) levels twice (152 and 185 pg/mL; normal value is 10 to 52 pg/mL) with normal cortisol levels (134.6 and 113.0 ng/mL, or 371.3 and 311.7 nmol/L; normal values are 67 to 226 ng/mL or 184.8 to 623.5 nmol/L). ACTH, corticotropin-releasing hormone (CRH) and insulin tolerance test (ITT) demonstrated normal cortisol response to ACTH and partial response to CRH and ITT tests, and ACTH hyperresponse to CRH and ITT. We suspected ACTH and/or ACTH receptor (ACTHR) mutations and evaluated the genetic profile for pro-opiomelanocortin (POMC), melanocortin 2 receptor (MC2R) and follicle-stimulating hormone receptor (FSHR) genes. Results: No mutations were found in either the MC2R or FSHR genes. The patient was heterozygous for the c.614A>G mutation corresponding to a p.53D>G substitution with a glycine instead of an aspartate in position 53 in POMC gene. This mutation was outside the sequence for ACTH (which spans amino acids 138 to 176) but was included in the part originating the N-terminal peptide of pro-opiomelanocortin (also called pro-γ-melanocyte stimulating hormone) which spans amino acids 27 to 102 and is involved in the regulation of adrenal steroidogenesis. Conclusion: The pathologic expansion of the cytosine-thymine-guanine triplet repeat in the 3' noncoding region of DMPK could explain the hyperresponse of ACTH typical of DM. The mutation of pro-γ-melanocyte-stimulating hormone could be associated with the abnormal response of cortisol, compatible with a partial adrenal insufficiency. Other studies are necessary to demonstrate this hypothesis

    Excellent outcome in patients with primary biliary cholangitis in Northwest Italy followed up for up to 30 years

    Objective: Primary biliary cholangitis (PBC) is a rare chronic autoimmune cholangiopathy, characterized by a variable course and response to treatment. We aimed to describe long-term outcomes of PBC patients referred to three academic centres in Northwest Italy. Methods: This is an ambispective cohort study of PBC patients (retrospective component: diagnosis before 1 January 2019; prospective component: thereafter), including 302 patients: 101 (33%) followed up in Novara, 86 (28%) in Turin, 115 (38%) in Genoa. Clinical features at diagnosis, biochemical response to therapy and survival were analyzed. Results: Among the 302 patients (88% women, median age 55 years, median follow-up 75 months), alkaline phosphatase (ALP) levels significantly decreased during treatment with ursodeoxycholic acid (UDCA, P < 0.0001) and obeticholic acid (P < 0.0001). At multivariate analysis, ALP at diagnosis was predictive of 1-year biochemical response to UDCA [odds ratio 3.57, 95% confidence interval (CI) 1.4-9, P < 0.001]. Estimated median survival free of liver transplantation and hepatic complications was 30 years (95% CI 19-41). Bilirubin level at diagnosis was the only independent risk factor for the combined outcome of death, transplantation or hepatic decompensation (hazard ratio, 1.65, 95% CI 1.66-2.56, P = 0.02). Patients presenting with total bilirubin at diagnosis ≥0.6 times the upper normal limit (ULN) had a significantly lower 10-year survival compared to those with bilirubin <0.6 times ULN (63% vs. 97%, P < 0.0001). Conclusion: In PBC, both short-term response to UDCA and long-term survival can be predicted by simple conventional biomarkers of disease severity, obtained at diagnosis

    Long term results of down-staging and liver transplantation for patients with hepatocellular carcinoma beyond the conventional criteria

    The objective of the study is to evaluate 10 years of down-staging strategy for liver transplantation (LT) with a median follow-up of 5 years. Data on long-term results are poor and less information is available for hepatocellular carcinoma (HCC) non-responder patients or those ineligible for down-staging. The outcome of 308 HCC candidates and the long-term results of 231 LTs for HCC performed between 2003 and 2013 were analyzed. HCCs were divided according to tumor stage and response to therapy: 145 patients were T2 (metering Milan Criteria, MC), 43 were T3 successfully down-staged to T2 (Down-Achieved), 20 were T3 not fully down-staged to T2 (Down-not Achieved), and 23 patients were T3 not receiving down-staging treatments (No-Down). The average treatment effect (ATE) of LT for T3 tumors was estimated using the outcome of 535 T3 patients undergoing non-LT therapies, using inverse probability weighting regression adjustment. The 24-month drop-out rate during waiting time was significantly higher in the down-staging groups: 27.6% vs. 9.2%, p &lt; 0.005. After LT, the tumor recurrence rate was significantly different: MC 7.6%, Down-Achieved 20.9%, Down-not Achieved 31.6%, and No-Down 30.4% (p &lt; 0.001). The survival rates at 5 years were: 63% in Down-Achieved, 62% in Down-not Achieved, 63% in No-Down, and 77% in MC (p = n.s.). The only variable related to a better outcome was the effective down-staging to T2 at the histological evaluation of the explanted liver: recurrence rate = 7.8% vs. 26% (p &lt; 0.001) and 5-year patient survival = 76% vs. 67% (p &lt; 0.05). The ATE estimation showed that the mean survival of T3-LT candidates was significantly better than that of T3 patients ineligible for LT [83.3 vs 39.2 months (+44.6 months); p &lt; 0.001]. Long term outcome of T3 down-staged candidates was poorer than that of MC candidates, particularly for cases not achieving down-staging. However, their survival outcome was significantly better than that achieved with non-transplant therapies
