Generic Medications and Blood Pressure Control in Diabetic Hypertensive Subjects
OBJECTIVE
To investigate temporal improvements in blood pressure (BP) control in subjects with diabetes and policy changes regarding generic antihypertensives.
RESEARCH DESIGN AND METHODS
In a cross-sectional study we used logistic regression models to investigate the temporal relationship between access to generic antihypertensive medications and BP control (<130/80 mmHg) in 5,375 subjects (mean age, 66 ± 9 years; 61% African American) with diabetes and hypertension (HTN) enrolled in the national Results from the REasons for Geographic And Racial Differences in Stroke (REGARDS) cohort study between 2003 and 2007. At enrollment, BP was measured and medications in the home determined by medication label review by a trained professional. Generic antihypertensive medication status was ascertained from the U.S. Food and Drug Administration.
RESULTS
The percentage of subjects accessing generically available antihypertensive medications increased significantly from 66% in 2003 to 81% in 2007 (P < 0.0001), and the odds of achieving a BP <130/80 mmHg in 2007 was 66% higher (odds ratio 1.66 [95% CI 1.30–2.10]) than in 2003. Nevertheless, <50% of participants achieved this goal. African American race, male sex, limited income, and medication nonadherence were significant predictors of inadequate BP control. There was no significant relationship between access to generic antihypertensives and BP control when other demographic factors were included in the model (0.98 [0.96–1.00]).
CONCLUSIONS
Among African American and white subjects with HTN and diabetes, BP control remained inadequate relative to published guidelines, and racial disparities persisted. Although access to generic antihypertensives increased, this was not independently associated with improved BP control, suggesting that poor BP control is multifactorial
Magnetoconductivity of Hubbard bands induced in Silicon MOSFETs
Sodium impurities are diffused electrically to the oxide-semiconductor
interface of a silicon MOSFET to create an impurity band. At low temperature
and at low electron density, the band is split into an upper and a lower
sections under the influence of Coulomb interactions. We used
magnetoconductivity measurements to provide evidence for the existence of
Hubbard bands and determine the nature of the states in each band.Comment: In press in Physica
Processing Images from the Zwicky Transient Facility
The Zwicky Transient Facility is a new robotic-observing program, in which a
newly engineered 600-MP digital camera with a pioneeringly large field of view,
47~square degrees, will be installed into the 48-inch Samuel Oschin Telescope
at the Palomar Observatory. The camera will generate ~petabyte of raw
image data over three years of operations. In parallel related work, new
hardware and software systems are being developed to process these data in real
time and build a long-term archive for the processed products. The first public
release of archived products is planned for early 2019, which will include
processed images and astronomical-source catalogs of the northern sky in the
and bands. Source catalogs based on two different methods will be
generated for the archive: aperture photometry and point-spread-function

SPIRITS 15c and SPIRITS 14buu: Two Obscured Supernovae in the Nearby Star-Forming Galaxy IC 2163
SPIRITS---SPitzer InfraRed Intensive Transients Survey---is an ongoing survey
of nearby galaxies searching for infrared (IR) transients with Spitzer/IRAC. We
present the discovery and follow-up observations of one of our most luminous
( mag, Vega) and red (
mag) transients, SPIRITS 15c. The transient was detected in a dusty spiral arm
of IC 2163 ( Mpc). Pre-discovery ground-based imaging revealed an
associated, shorter-duration transient in the optical and near-IR (NIR). NIR
spectroscopy showed a broad ( km s), double-peaked
emission line of He I at m, indicating an explosive origin. The NIR
spectrum of SPIRITS 15c is similar to that of the Type IIb SN 2011dh at a phase
of days. Assuming mag of extinction in SPIRITS 15c
provides a good match between their optical light curves. The IR light curves
and the extreme color cannot be explained using only a standard
extinction law. Another luminous ( mag) event, SPIRITS
14buu, was serendipitously discovered in the same galaxy. The source displays
an optical plateau lasting days, and we suggest a scenario similar
to the low-luminosity Type IIP SN 2005cs obscured by mag.
Other classes of IR-luminous transients can likely be ruled out in both cases.
If both events are indeed SNe, this may suggest of nearby
core-collapse SNe are missed by currently operating optical surveys.Comment: 19 pages, 7 Figures, 4 Table
SPIRITS: Uncovering Unusual Infrared Transients with Spitzer
We present an ongoing, five-year systematic search for extragalactic infrared transients, dubbed SPIRITS—SPitzer InfraRed Intensive Transients Survey. In the first year, using Spitzer/IRAC, we searched 190 nearby galaxies with cadence baselines of one month and six months. We discovered over 1958 variables and 43 transients. Here, we describe the survey design and highlight 14 unusual infrared transients with no optical counterparts to deep limits, which we refer to as SPRITEs (eSPecially Red Intermediate-luminosity Transient Events). SPRITEs are in the infrared luminosity gap between novae and supernovae, with [4.5] absolute magnitudes between −11 and −14 (Vega-mag) and [3.6]–[4.5] colors between 0.3 mag and 1.6 mag. The photometric evolution of SPRITEs is diverse, ranging from 7 mag yr^(−1). SPRITEs occur in star-forming galaxies. We present an in-depth study of one of them, SPIRITS 14ajc in Messier 83, which shows shock-excited molecular hydrogen emission. This shock may have been triggered by the dynamic decay of a non-hierarchical system of massive stars that led to either the formation of a binary or a protostellar merger
Methodological approaches, challenges, and opportunities in the application of Mendelian randomisation to lifecourse epidemiology: A systematic literature review
Diseases diagnosed in adulthood may have antecedents throughout (including prenatal) life. Gaining a better understanding of how exposures at different stages in the lifecourse influence health outcomes is key to elucidating the potential benefits of disease prevention strategies. Mendelian randomisation (MR) is increasingly used to estimate causal effects of exposures across the lifecourse on later life outcomes. This systematic literature review explores MR methods used to perform lifecourse investigations and reviews previous work that has utilised MR to elucidate the effects of factors acting at different stages of the lifecourse. We conducted searches in PubMed, Embase, Medline and MedRXiv databases. Thirteen methodological studies were identified. Four studies focused on the impact of time-varying exposures in the interpretation of "standard" MR techniques, five presented methods for repeat measures of the same exposure, and four described methodological approaches to handling multigenerational exposures. A further 127 studies presented the results of an applied research question. Over half of these estimated effects in a single generation and were largely confined to the exploration of questions regarding body composition. The remaining mostly estimated maternal effects. There is a growing body of research focused on the development and application of MR methods to address lifecourse research questions. The underlying assumptions require careful consideration and the interpretation of results rely on select conditions. Whilst we do not advocate for a particular strategy, we encourage practitioners to make informed decisions on how to approach a research question in this field with a solid understanding of the limitations present and how these may be affected by the research question, modelling approach, instrument selection, and data availability
"The contribution of chronic diseases to the prevalence of dependence among older people in Latin America, China and India: a 10/66 Dementia Research Group population-based survey"
<p>Abstract</p> <p>Background</p> <p>The number of older people is set to increase dramatically worldwide. Demographic changes are likely to result in the rise of age-related chronic diseases which largely contribute to years lived with a disability and future dependence. However dependence is much less studied although intrinsically linked to disability. We investigated the prevalence and correlates of dependence among older people from middle income countries.</p> <p>Methods</p> <p>A one-phase cross-sectional survey was carried out at 11 sites in seven countries (urban sites in Cuba, Venezuela, and Dominican Republic, urban and rural sites in Peru, Mexico, China and India). All those aged 65 years and over living in geographically defined catchment areas were eligible. In all, 15,022 interviews were completed with an informant interview for each participant. The full 10/66 Dementia Research Group survey protocol was applied, including ascertainment of depression, dementia, physical impairments and self-reported diagnoses. Dependence was interviewer-rated based on a key informant's responses to a set of open-ended questions on the participant's needs for care. We estimated the prevalence of dependence and the independent contribution of underlying health conditions. Site-specific prevalence ratios were meta-analysed, and population attributable prevalence fractions (PAPF) calculated.</p> <p>Results</p> <p>The prevalence of dependence increased with age at all sites, with a tendency for the prevalence to be lower in men than in women. Age-standardised prevalence was lower in all sites than in the USA. Other than in rural China, dementia made the largest independent contribution to dependence, with a median PAPF of 34% (range 23%-59%). Other substantial contributors were limb impairment (9%, 1%-46%), stroke (8%, 2%-17%), and depression (8%, 1%-27%).</p> <p>Conclusion</p> <p>The demographic and health transitions will lead to large and rapid increases in the numbers of dependent older people particularly in middle income countries (MIC). The prevention and control of chronic neurological and neuropsychiatric diseases and the development of long-term care policies and plans should be urgent priorities.</p
