    The present article introduces the third issue of the journal entitled Gradients of liveness: socio-technical shaping of performing arts between online and offline. After situating the issue of liveness within the process of mediatization of society, and the challenges fuelled by the pandemic, we discuss the statute of the field of liveness studies. We then defend the necessity of an approach that understands liveness in terms of the “gradients” with which this phenomenon manifests itself. In the second part, the articles of the issue are presented, showing the multiple branches of the “liveness problem” through heterogeneous performative, technological and theoretical fields, thus highlighting the relevance of this theoretical tool in understanding how the sense of presence and connection is constructed in the context of mediatization. Il presente articolo introduce il terzo numero della rivista dal titolo Gradienti di liveness. Lo shaping socio-tecnico delle arti performative tra online e offline. Dopo aver inquadrato il tema della liveness all’interno del processo di mediatizzazione del sociale e delle sfide alimentate dalla pandemia, viene discusso lo statuto del campo di studi sulla liveness. Si difende quindi e la necessitĂ  di uno sguardo che intende la liveness nei termini di “gradienti” con cui tale fenomeno si manifesta. Nella seconda parte vengono presentati i contenuti del numero, i quali mostrano le molteplici diramazioni della questione liveness attraverso ambiti spettacolari, tecnologici e teorici eterogenei, evidenziando la rilevanza di questo strumento d’indagine nel comprendere la costruzione del senso della presenza e della connessione nel contesto della mediatizzazione.&nbsp

    New totally intracorporeal reconstructive approach after robotic total gastrectomy. Technical details and short-term outcomes

    AIM: To show outcomes of our series of patients that underwent a total gastrectomy with a robotic approach and highlight the technical details of a proposed solution for the reconstruction phase. METHODS: Data of gastrectomies performed from May 2014 to October 2016, were extracted and analyzed. Basic characteristics of patients, surgical and clinical outcomes were reported. The technique for reconstruction (Parisi Technique) consists on a loop of bowel shifted up antecolic to directly perform the esophago-enteric anastomosis followed by a second loop, measured up to 40 cm starting from the esojejunostomy, fixed to the biliary limb to create an enteroenteric anastomosis. The continuity between the two anastomoses is interrupted just firing a linear stapler, so obtaining the Roux-en-Y by avoiding to interrupt the mesentery. RESULTS: Fifty-five patients were considered in the present analysis. Estimated blood loss was 126.55 ± 73 mL, no conversions to open surgery occurred, R0 resections were obtained in all cases. Hospital stay was 5 (3-17) d, no anastomotic leakage occurred. Overall, a fast functional recovery was shown with a median of 3 (3-6) d in starting a solid diet. CONCLUSION: Robotic surgery and the adoption of a tailored reconstruction technique have increased the feasibility and safety of a minimally invasive approach for total gastrectomy. The present series of patients shows its implementation in a western center with satisfying short-term outcomes

    Laparoscopic peritoneal lavage. Our experience and review of the literature

    NTRODUCTION: Over the years various therapeutic techniques for diverticulitis have been developed. Laparoscopic peritoneal lavage (LPL) appears to be a safe and useful treatment, and it could be an effective alternative to colonic resection in emergency surgery. AIM: This prospective observational study aims to assess the safety and benefits of laparoscopic peritoneal lavage in perforated sigmoid diverticulitis. MATERIAL AND METHODS: We surgically treated 70 patients urgently for complicated sigmoid diverticulitis. Thirty-two (45.7%) patients underwent resection of the sigmoid colon and creation of a colostomy (Hartmann technique); 21 (30%) patients underwent peritoneal laparoscopic lavage; 4 (5.7%) patients underwent colostomy by the Mikulicz technique; and the remaining 13 (18.6%) patients underwent resection of the sigmoid colon and creation of a colorectal anastomosis with a protective ileostomy. RESULTS: The 66 patients examined were divided into 3 groups: 32 patients were treated with urgent surgery according to the Hartmann procedure; 13 patients were treated with resection and colorectal anastomosis; 21 patients were treated urgently with laparoscopic peritoneal lavage. We had no intraoperative complications. The overall mortality was 4.3% (3 patients). In the LPL group the morbidity rate was 33.3%. CONCLUSIONS: Currently it cannot be said that LPL is better in terms of mortality and morbidity than colonic resection. These data may, however, be proven wrong by greater attention in the selection of patients to undergo laparoscopic peritoneal lavage

    Robotic total gastrectomy with intracorporeal robot-sewn anastomosis. A novel approach adopting the double-loop reconstruction method

    Gastric cancer constitutes a major health problem. Robotic surgery has been progressively developed in this field. Although the feasibility of robotic procedures has been demonstrated, there are unresolved aspects being debated, including the reproducibility of intracorporeal in place of extracorporeal anastomosis. Difficulties of traditional laparoscopy have been described and there are well-known advantages of robotic systems, but few articles in literature describe a full robotic execution of the reconstructive phase while others do not give a thorough explanation how this phase was run. A new reconstructive approach, not yet described in literature, was recently adopted at our Center. Robotic total gastrectomy with D2 lymphadenectomy and a socalled ‘‘double-loop’’ reconstruction method with intracorporeal robotsewn anastomosis (Parisi’s technique) was performed in all reported cases. Preoperative, intraoperative, and postoperative data were collected and a technical note was documented. All tumors were located at the upper third of the stomach, and no conversions or intraoperative complications occurred. Histopathological analysis showed R0 resection obtained in all specimens. Hospital stay was regular in all patients and discharge was recommended starting from the 4th postoperative day. No major postoperative complications or reoperations occurred. Reconstruction of the digestive tract after total gastrectomy is one of the main areas of surgical research in the treatment of gastric cancer and in the field of minimally invasive surgery. The double-loop method is a valid simplification of the traditional technique of construction of the Roux-limb that could increase the feasibility and safety in performing a full hand-sewn intracorporeal reconstruction and it appears to fit the characteristics of the robotic system thus obtaining excellent postoperative clinical outcome

    Years of life that could be saved from prevention of hepatocellular carcinoma

    BACKGROUND: Hepatocellular carcinoma (HCC) causes premature death and loss of life expectancy worldwide. Its primary and secondary prevention can result in a significant number of years of life saved. AIM: To assess how many years of life are lost after HCC diagnosis. METHODS: Data from 5346 patients with first HCC diagnosis were used to estimate lifespan and number of years of life lost after tumour onset, using a semi-parametric extrapolation having as reference an age-, sex- and year-of-onset-matched population derived from national life tables. RESULTS: Between 1986 and 2014, HCC lead to an average of 11.5 years-of-life lost for each patient. The youngest age-quartile group (18-61 years) had the highest number of years-of-life lost, representing approximately 41% of the overall benefit obtainable from prevention. Advancements in HCC management have progressively reduced the number of years-of-life lost from 12.6 years in 1986-1999, to 10.7 in 2000-2006 and 7.4 years in 2007-2014. Currently, an HCC diagnosis when a single tumour <2 cm results in 3.7 years-of-life lost while the diagnosis when a single tumour 65 2 cm or 2/3 nodules still within the Milan criteria, results in 5.0 years-of-life lost, representing the loss of only approximately 5.5% and 7.2%, respectively, of the entire lifespan from birth. CONCLUSIONS: Hepatocellular carcinoma occurrence results in the loss of a considerable number of years-of-life, especially for younger patients. In recent years, the increased possibility of effectively treating this tumour has improved life expectancy, thus reducing years-of-life lost

    Clinical patterns of hepatocellular carcinoma in nonalcoholic fatty liver disease: A multicenter prospective study

    107noNonalcoholic fatty liver disease (NAFLD) represents the hepatic manifestation of metabolic syndrome and may evolve into hepatocellular carcinoma (HCC). Only scanty clinical information is available on HCC in NAFLD. The aim of this multicenter observational prospective study was to assess the clinical features of patients with NAFLD-related HCC (NAFLD-HCC) and to compare them to those of hepatitis C virus (HCV)-related HCC. A total of 756 patients with either NAFLD (145) or HCV-related chronic liver disease (611) were enrolled in secondary care Italian centers. Survival was modeled according to clinical parameters, lead-time bias, and propensity analysis. Compared to HCV, HCC in NAFLD patients had a larger volume, showed more often an infiltrative pattern, and was detected outside specific surveillance. Cirrhosis was present in only about 50% of NAFLD-HCC patients, in contrast to the near totality of HCV-HCC. Regardless of tumor stage, survival was significantly shorter (P = 0.017) in patients with NAFLD-HCC, 25.5 months (95% confidence interval 21.9-29.1), than in those with HCV-HCC, 33.7 months (95% confidence interval 31.9-35.4). To eliminate possible confounders, a propensity score analysis was performed, which showed no more significant difference between the two groups. Additionally, analysis of patients within Milan criteria submitted to curative treatments did not show any difference in survival between NAFLD-HCC and HCV-HCC (respectively, 38.6 versus 41.0 months, P = nonsignificant) Conclusions: NAFLD-HCC is more often detected at a later tumor stage and could arise also in the absence of cirrhosis, but after patient matching, it has a similar survival rate compared to HCV infection; a future challenge will be to identify patients with NAFLD who require more stringent surveillance in order to offer the most timely and effective treatment.     Trailers as mediatized performances: Investigating the use of promotional videos among Italian contemporary theatre artists

    In the theatre sector, many companies, festivals and theatres have integrated promotional videos into their communication strategies. This recent development is undoubtedly due to the rise of social media and the increasing accessibility of video technologies, but also to the need for theatre companies to publicize their work in a media that combines creative autonomy with economic efficiency. Despite this widespread use, trailers in the performing arts have received little attention in academic literature. This article offers the first, exploratory study on the use of promotional videos in the field of contemporary theatre in Italy and on the connections between the current creation of digital promotional clips and the heritage of the Italian video-theatre. Through in-depth interviews with sixteen of the leading Italian companies, this research aims to bring out the role theatre trailers play for performance artists

    There has been a growing interest in the study of performing arts audiences over the last three decades. This interest is expressed both at an organizational level, by the increasing presence of audience development and audience engagement initiatives, and within the expanding multidisciplinary field of academic research on theatre audiences, which interweaves economics, cultural marketing, the sociology of culture and audience studies applied to theatrical reception. To some extent, such attention is guided by the need for organizations and institutions to compete in increasingly competitive culture markets. It is however also the expression of a rising awareness of the social impacts that art consumption has on the processes of citizenship, community welfare, urban regeneration, political participation and on the struggle against social exclusion. Starting from the introduction of a theoretical framework that unites studies on theatre audiences, audience development and reception processes, the article presents the results of national survey dedicated to the Italian audiences of contemporary and experimental theatre. The purpose of this study is to provide a preliminary profiling of the audience of a branch of performing arts with its own specificities in terms of aesthetic, productive and distributive features. To this aim we also stress the importance of the dialogue between audience research and theatre studies to evaluate the perimeter of the field of the ‘contemporary’. Hence, the present research starts from the process of self-observation of spectators, having asked audience members to define themselves as ‘frequenter of contemporary and experimental theatre’, ‘non-frequenter’ or ‘uncertain about the attribution’. In this way it has been possible to trace homologies and divergences between habitual and occasional audiences, in order to describe attitudes, motivations and attending behavior on the basis of the different degrees of affection and competence towards this cultural sector.The analysis of the data follows the different areas investigated in the questionnaire, presenting a socio-demographic profile of the audience, attending behavior, the attitudes towards the artistic proposals, the relation between the enjoyment of contemporary theatre and other cultural and media consumption routines. It is therefore possible to observe a type of audience with certain specific characteristics: they tend to be competent and willing to accept the challenges of artistic innovation (with certain complaints); they are dynamic and generally omnivorous regarding cultural consumption

    BETWEEN LIVE AND LIFE: EXPLORING THE ONLINE PRESENCE OF PERFORMANCE ARTISTS

    In the last decade, the use of social media in the theatre scene has fueled debates on the possibility to redefine the performer-audience relationship on a wide scale. Scarce attention, however, has been paid on empirically studying how performance artists balance their use of social media between the need to promote their work and the ambition to experiment creatively on the medium affordances. This paper explores the use of social media by Italian contemporary theatre artists and companies. The study aims to explore the relational labour (Baym 2018) of performance artists in tracing the boundaries between online/offline performativity and between personal self-narration and artistic promotion. Through a combination of profile analysis and in-depth interviews, we want to understand how this artistic scene - that always experimented on intermedial and participatory possibilities - is making sense of social media. Preliminary results show how social media interaction and content production is becoming an integrated part of companies working routines. Companies position their social media activity outside a purely promotional logic. This anti-promotional ethos translates to a lack of efficacy in terms of strategic communication. However, it also lays the basis for the use of social media to expand performative practices. Although the interviewed companies do not exclude such a possibility, they lament their current inability in making such an attempt. Lacking resources is the most quoted explanation of this failure, followed by the refusal to compromise with the constraints of online languages, and the distrust of social media to reach wider audiences
