Influence of firing mechanisms on gain modulation
We studied the impact of a dynamical threshold on the f-I curve-the
relationship between the input and the firing rate of a neuron-in the presence
of background synaptic inputs. First, we found that, while the leaky
integrate-and-fire model cannot reproduce the f-I curve of a cortical neuron,
the leaky integrate-and-fire model with dynamical threshold can reproduce it
very well. Second, we found that the dynamical threshold modulates the onset
and the asymptotic behavior of the f-I curve. These results suggest that a
cortical neuron has an adaptation mechanism and that the dynamical threshold

Estimates of Electronic Medical Records in U.S. Emergency Departments
Background: Policymakers advocate universal electronic medical records (EMRs) and propose incentives for “meaningful use” of EMRs. Though emergency departments (EDs) are particularly sensitive to the benefits and unintended consequences of EMR adoption, surveillance has been limited. We analyze data from a nationally representative sample of US EDs to ascertain the adoption of various EMR functionalities. Methodology/Principal: Findings We analyzed data from the National Hospital Ambulatory Medical Care Survey, after pooling data from 2005 and 2006, reporting proportions with 95% confidence intervals (95% CI). In addition to reporting adoption of various EMR functionalities, we used logistic regression to ascertain patient and hospital characteristics predicting “meaningful use,” defined as a “basic” system (managing demographic information, computerized provider order entry, and lab and imaging results). We found that 46% (95% CI 39–53%) of US EDs reported having adopted EMRs. Computerized provider order entry was present in 21% (95% CI 16–27%), and only 15% (95% CI 10–20%) had warnings for drug interactions or contraindications. The “basic” definition of “meaningful use” was met by 17% (95% CI 13–21%) of EDs. Rural EDs were substantially less likely to have a “basic” EMR system than urban EDs (odds ratio 0.19, 95% CI 0.06–0.57, p = 0.003), and Midwestern (odds ratio 0.37, 95% CI 0.16–0.84, p = 0.018) and Southern (odds ratio 0.47, 95% CI 0.26–0.84, p = 0.011) EDs were substantially less likely than Northeastern EDs to have a “basic” system. Conclusions/Significance: EMRs are becoming more prevalent in US EDs, though only a minority use EMRs in a “meaningful” way, no matter how “meaningful” is defined. Rural EDs are less likely to have an EMR than metropolitan EDs, and Midwestern and Southern EDs are less likely to have an EMR than Northeastern EDs. We discuss the nuances of how to define “meaningful use,” and the importance of considering not only adoption, but also full implementation and consequences

Structural transitions, octahedral rotations, and electronic properties of NiO rare-earth nickelates under high pressure
Motivated by the recent observation of superconductivity with K
in pressurized La3Ni2O7 [Nature 621, 493 (2023)], we explore the structural and
electronic properties in A3Ni2O7 bilayer nickelates (A=La-Lu, Y, Sc) as a
function of hydrostatic pressure (0-150 GPa) from first principles including a
Coulomb repulsion term. At GPa, we observe an
orthorhombic-to-tetragonal transition in LaNiO at variance with
recent x-ray diffraction data, which points to so-far unresolved complexities
at the onset of superconductivity, e.g., charge doping by variations in the
oxygen stoichiometry. We compile a structural phase diagram with particular
emphasis on the ratio, octahedral anisotropy, and octahedral rotations.
Intriguingly, chemical and external pressure emerge as two distinct and
counteracting control parameters. We find unexpected correlations between
and the in-plane Ni-O-Ni bond angles for LaNiO. Moreover, two novel
structural phases with significant octahedral rotations and in-plane bond
disproportionations are uncovered for A=Nd-Lu, Y, Sc that exhibit a surprising
pressure-driven electronic reconstruction in the Ni manifold. By
disentangling the involvement of basal versus apical oxygen states at the Fermi
surface, we identify TbNiO as an interesting candidate for
superconductivity at ambient pressure. These results suggest a profound
tunability of the structural and electronic phases in this novel materials
class and are key for a fundamental understanding of the superconductivity

Large-scale study of the NGC 1399 globular cluster system in Fornax
We present a Washington C and Kron-Cousins R photometric study of the
globular cluster system of NGC 1399, the central galaxy of the Fornax cluster.
A large areal coverage of 1 square degree around NGC 1399 is achieved with
three adjoining fields of the MOSAIC II Imager at the CTIO 4-m telescope.
Working on such a large field, we can perform the first indicative
determination of the total size of the NGC 1399 globular cluster system. The
estimated angular extent, measured from the NGC 1399 centre and up to a
limiting radius where the areal density of blue globular clusters falls to 30
per cent of the background level, is 45 +/- 5 arcmin, which corresponds to 220
- 275 kpc at the Fornax distance. The bimodal colour distribution of this
globular cluster system, as well as the different radial distribution of blue
and red clusters, up to these large distances from the parent galaxy, are
confirmed. The azimuthal globular cluster distribution exhibits asymmetries
that might be understood in terms of tidal stripping of globulars from NGC
1387, a nearby galaxy. The good agreement between the areal density profile of

Transient Oscillation of Currents in Quantum Hall Effect of Bloch Electrons
We consider the quantum Hall effect of two-dimensional electrons with a
periodic potential and study the time dependence of the Hall and longitudinal
currents when the electric field is applied abruptly. We find that the currents
oscillate in time with very large frequencies because of quantum fluctuation
and the oscillations eventually vanish, for their amplitudes decay as 1/t.Comment: 16 pages and 8 figure
Measuring Quality: The Impact of Minimally Invasive Surgery and Operative Time on Surgical Site Infections
Objective: To evaluate the relationship between operative approach, operative time, and SSI rate.Methods: Inpatient database review identified patients undergoing 5 common procedures from 1/2010-12/2011. Patients were stratified into laparoscopic or open approaches. The main outcome measure was the relationship between operative time and SSI by approach.Results: 226,006 patients were evaluated- 28.2% open and 71.8% laparoscopic. Mean overall operative time was significantly shorter laparoscopically (p<0.001). Laparoscopy was associated with significantly lower costs and shorter length of stay (LOS) overall and for each procedure(p<0.0001). Multivariate analysis found SSI increased directly with operative time: for every 30-minute increase, SSI risk increased by 12%. Operative approach was an independent risk factor for SSI: open surgery increased SSI risk by 78%. A direct relationship between open procedures, operative time, and SSI risk was found.Conclusions: Laparoscopy has overall shorter operative time and improved outcomes in SSI rate, LOS, and total costs for common surgical procedures. As operative time and approach were independent risk factors for SSI, the use of laparoscopy and operative time are valuable quality measures
Dynamics of the Globular Cluster System Associated with M87 (NGC 4486). II. Analysis
We present a dynamical analysis of the globular cluster system associated
with M87 (= NGC 4486), the cD galaxy near the dynamical center of the Virgo
cluster. The analysis utilizes a new spectroscopic and photometric database
which is described in a companion paper (Hanes et al. 2001). Using a sample of
278 globular clusters with measured radial velocities and metallicities, and
new surface density profiles based on wide-field Washington photometry, we
study the dynamics of the M87 globular cluster system both globally --- for the
entire cluster sample --- and separately --- for the metal-rich and metal-poor
globular cluster samples. This constitutes the largest sample of radial
velocities for pure Population II tracers yet assembled for any galaxy. We
discuss the implications of our findings for models for the formation of giant
elliptical galaxies, globular cluster systems, and the Virgo cluster.
