Incidence and Survival Trends of Pancreatic Cancer in Girona: Impact of the Change in Patient Care in the Last 25 Years.

(1) Background: We investigated the incidence and survival trends for pancreatic cancer (PC) over the last 25 years in the Girona region, Catalonia, Spain; (2) Methods: Data were extracted from the population-based Girona Cancer Registry. Incident PC cases during 1994-2015 were classified using the International Classification of Diseases for Oncology Third Edition (ICD-O-3). Incidence rates age-adjusted to the European standard population (ASRE) and world standard population (ASRW) were obtained. Trends were assessed using the estimated annual percentage of change (EAPC) of the ASRE13. Observed and relative survivals (RS) were estimated with the Kaplan-Meier and Pohar Perme methods, respectively; (3) Results: We identified 1602 PC incident cases. According to histology, 44.4% of cases were exocrine PC, 4.1% neuroendocrine, and 51.1% malignant-non-specified. The crude incidence rate (CR) for PC was 11.43 cases-per-100,000 inhabitants/year. A significant increase of incidence with age and over the study period was observed. PC overall 5-year RS was 7.05% (95% confidence interval (CI) 5.63; 8.84). Longer overall survival was observed in patients with neuroendocrine tumours (5-year RS 61.45%; 95% CI 47.47; 79.55). Trends in 5-year RS for the whole cohort rose from 3.27% (95% CI 1.69-6.35) in 1994-1998 to 13.1% (95% CI 9.98; 17.2) in 2010-2015; (4) Conclusions: Incidence rates of PC in Girona have increased in the last two decades. There is a moderate but encouraging increase in survival thorough the study period. These results can be used as baseline for future research.S

LUZ’CO - Solutions collectives pour développer les légumineuses fourragères

The LUZ’CO project aimed at fostering the emergence, consolidation and multiplication of collective territorial approaches facilitating the development of agroecological systems based on alfalfa. To achieve this, the project partners defined the following objectives: (1) Characterizing the modes of collective organization facilitating the implementation of such systems; (2) Analyzing their economic, environmental and social impact and (3) Creating the conditions allowing to encourage the development of these approaches: support tools and methods, training, networking, etc. Four modes of collective action were thus identified and documented, which allows the strengthening of the contribution of forage legumes for the agroecological transition:Lever n°1: Optimizing harvesting operations through the pool of high-performance equipment and/or hired worker, or through the joint organization;Lever n°2: Drying the fodder with collective dryers;Lever n°3: Engaging local win-win exchanges between producers and users of forage legumes;Lever n°4: Collectively designing, training and experimenting to overcome the obstacles, and to facilitate the development of agroecological systems.From 2015 to 2019, the LUZ’CO team studied and worked with groups of farmers using one or several of these levers, gathered and ranked the existing technical references, produced support tools and methods, as well as designed educational scenarios for the actors of the agricultural development and education sector. All the results are available on the website www.luzco.fr.Le projet LUZ’CO visait à favoriser l'émergence, la consolidation et la démultiplication des démarches collectives territoriales facilitant la mise en place de systèmes agroécologiques à base de luzerne. Pour y parvenir, les partenaires du projet s’étaient donné pour objectifs (1) de caractériser les modalités d’organisation collective facilitant la mise en place de tels systèmes, (2) d’analyser leur impact économique, environnemental et social et (3) de créer les conditions aptes à susciter le développement de ces démarches : outils et méthodes d’accompagnement, formation, réseaux d’acteurs, etc.Quatre modalités d’action collective ont ainsi été identifiées et documentées permettant de renforcer la place des légumineuses fourragères en faveur de la transition agroécologique :Levier n°1 : Optimiser les chantiers de récolte via la mutualisation de matériels performants, de salariés ou de l’entraide ;Levier n°2 : Sécher les fourrages grâce à des unités de séchage collectives ;Levier n°3 : Nouer des échanges gagnants-gagnants à l’échelle locale entre producteurs et utilisateurs de légumineuses fourragères ;Levier n°4 : Concevoir, se former et expérimenter en groupe pour dépasser les freins et faciliter la mise en place de systèmes agroécologiques.De 2015 à 2019, l’équipe de LUZ’CO a étudié et travaillé avec des groupes d’agriculteurs mobilisant un ou plusieurs de ces leviers, rassemblé et hiérarchisé les références techniques existantes, produit des outils et méthodes d’accompagnement et conçu des scénarios pédagogiques pour les acteurs du développement et de l’enseignement agricole.
