Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients. Conversely, if the ART strategy is similar or inferior to the current evidence-based strategy (ARDSNet), this should also change current practice as many institutions routinely employ recruitment maneuvers and set PEEP levels according to some titration method.Hospital do Coracao (HCor) as part of the Program 'Hospitais de Excelencia a Servico do SUS (PROADI-SUS)'Brazilian Ministry of Healt

An analytical method to determine tin concentrations in natural samples, by coupling atomic fluorescence spectrometry, hydride generation and flow injection system was developed. The sample digestion procedure and the conditions of hydride generation were fully optimized. The best conditions for the generation of SnH4 were obtained with NaBH4 4% (m v-1) and HCl 0.6 mol L-1. The optimized flow rates were 1.2 mL min-1 for sample, 1.2 mL min-1 for HCl and 1.6 mL min-1 for NaBH4 solution. The sample volume was 500 µL. The best acid volume for digestion of solid samples was stablished to be 3 mL HNO3 + 1 mL HCl. The accuracy of the method was established by the analysis of certified reference materials and by comparative determinations, using inductively coupled optical emission spectrometry. The approach developed was shown to offer a robust method with excellent accuracy and precision for the determination of Sn in environmental samples. Finally, the method was applied to analyse the soil, sediment and water samples, collected in area under anthropogenic activities: processing area of cassiterite in Corumbataí/SP. The obtained data indicated that there was alteration in the natural quality of the environment as a result of the activities developed in that sit

Tensor-Based Subspace Tracking for Time-Delay Estimation in GNSS Multi-Antenna Receivers

Although Global Navigation Satellite Systems (GNSS) receivers currently achieve high accuracy when processing their geographic location under line of sight (LOS), multipath interference and noise degrades the accuracy considerably. In order to mitigate multipath interference, receivers based on multiple antennas became the focus of research and technological development. In this context, tensor-based approaches based on Parallel Factor Analysis (PARAFAC) models have been proposed in the literature, providing optimum performance. State-of-the-art techniques for antenna array based GNSS receivers compute singular value decomposition (SVD) for each new sample, implying into a high computational complexity, being, therefore, prohibitive for real-time applications. Therefore, in order to reduce the computational complexity of the parameter estimates, subspace tracking algorithms are essential. In this work, we propose a tensor-based subspace tracking framework to reduce the overall computational complexity of the highly accurate tensor-based time-delay estimation process

Speciation analysis of inorganic arsenic in river water by Amberlite IRA 910 resin immobilized in a polyacrylamide gel as a selective binding agent for As(v) in diffusive gradient thin film technique

    No full text
In this study, a method is proposed for the selective retention of As(v) using diffusive gradient in thin film (DGT) samplers containing a strongly basic anion exchange resin (Amberlite IRA 910) supported on a polyacrylamide gel. In addition, the total arsenic content is determined by ferrihydrite gel discs. Subsequently, the concentration of As(III) was obtained by determining the difference between the total As and As(V). DGT experiments showed linear accumulation of As(V) (up to 280 ng) until a deployment time of 8 h deployment (R-2 > 0.99). The retention of As(V) was appropriate (97.9-112.3%) between pH 5 and 9. For a solution with an ionic strength ranging from 0.001 to 0.05 mol L-1, the As(V) uptake ranged from 90-120%. The proposed method was applied for the speciation of arsenic in river water. For the analysis of spiked samples collected at the Furnas stream, the recoveries of total arsenic content ranged between 103.9% and 118.8%. However, the recoveries of As(III) and As(V) were 43.3-75.2% and 147.3-153.4%, respectively. These differences were probably because of the oxidation of As(III) to As(V) during deployments. For spiked samples collected at the Ribeirao Claro, the recoveries of dissolved As(III), As(V) and As(T) were 103.1%, 108.0% and 106.3%, respectively. Thus, the DGT technique with Amberlite IRA 910 resin as the binding phase can be employed for the in situ redox speciation of inorganic arsenic.

The experiment was carried out in a greenhouse with the objective of evaluating the dry matter partition of capim-xaraés (Brachiaria brizantha) and capim-massai (Panicum maximum x Panicum infestum) submitted to four treatments of nitrogen (0, 40, 80, and 120 mg/dm3). A randomized design with four replications was applied. Both species were sowed in trays containing commercial agricultural substratum and 15 days after the emergence of five plants they were transferred to 5.8-dm3 vases. When the plants reached an average height of 35 cm, they were cut 5 cm from the soil to achieve a uniform cut. The nitrogen was applied four times. After forty-five days of free development the plants were cut and evaluated for dry matter of the leaf, stem, dead material, and root. The shoot to root and lamina to stem ratios were also evaluated. It was observed, according to the results that the nitrogen affected the dry matter partition of capim-xaraés, favoring the development of the shoot, but without effect on the roots. The total dry matter production of xaraes and massai shoot, as well as the lamina to stem ratio increased linearly with the doses of N, without affecting the root dry matter. It was also observed that nitrogen favors the lamina to stem relation and increases the dead material deposit in xaraes grass and massai grass
