    Objetivo: Conocer las características de la cultura de seguridad clínica en cirugía ortopédica y traumatología, valorando la influencia de una intervención formativa en su tendencia evolutiva. Población y metodología: Estudio observacional ecológico con intervención formativa. La población objeto de estudio fueron los profesionales del Servicio de Cirugía Ortopédica y Traumatología del Hospital Clínic Universitario de Barcelona. Se utilizó la versión española del cuestionario HSOPS para valorar la cultura de seguridad clínica. Resultados: El porcentaje de respuestas válidas obtenidas fue superior al 60% en las dos mediciones de la cultura realizadas. Las 12 dimensiones consideradas mejoraron su tendencia. El trabajo en equipo dentro del servicio se identificó como una dimensión de fortaleza del modelo. Conclusiones: La intervención formativa ha consolidado una tendencia favorable. La mejora de la cultura de seguridad clínica requiere tiempo y actuaciones periódicas

    The role of temperature in the magnetic irreversibility of type-I Pb superconductors

    Evidence of how temperature takes part in the magnetic irreversibility in the intermediate state of a cylinder and various disks of pure type-I superconducting lead is presented. Isothermal measurements of first magnetization curves and magnetic hysteresis cycles are analyzed in a reduced representation that defines an equilibrium state for flux penetration in all the samples and reveals that flux expulsion depends on temperature in the disks but not in the cylinder. The magnetic field at which irreversibility sets in along the descending branch of the hysteresis cycle and the remnant magnetization at zero field are found to decrease with temperature in the disks. The contributions to irreversibility of the geometrical barrier and the energy minima associated to stress defects that act as pinning centers on normal-superconductor interfaces are discussed. The differences observed among the disks are ascribed to the diverse nature of the stress defects in each sample. The pinning barriers are suggested to decrease with the magnetic field to account for these results

    Destination Therapy with Left Ventricular Assist Devices in Non-transplant Centres: The Time is Right

    For almost half a century, cardiac transplant has been the only long-term treatment for patients with end-stage heart failure. Implantable left ventricular assist devices (LVADs) have emerged as a new treatment option for advanced heart failure as destination therapy for patients either too old or not suitable for transplant. A meta-analysis presenting head-to-head comparisons of cardiac transplant versus LVAD as destination therapy (LVAD-DT) found no difference in 1-year mortality rates between LVAD-DT and cardiac transplant (OR 1.49; 95% CI [0.48–4.66]; I2=82.8%). Moreover, a recent subanalysis from the Interagency Registry for Mechanically Assisted Circulatory Support found similar outcomes after LVAD-DT implantation in both transplant and non-transplant centres. The time is right for LVAD-DT in non-transplant centres, provided multidisciplinary heart failure teams and expertise are in place

    The stocking of antidotes in various care settings is a subject of debate. A group of experts in hospital pharmacy and both adult and pediatric clinical toxicology was formed to review the Spanish and international literature on the subject with the aim of updating recommendations about which antidotes to stock and in what amounts in hospitals of different levels of complexity. The experts also considered the usual indications for the use of antidotes and dosing. The amount to have on hand was defined as enough to treat an adult weighing 70 kg for 24 hours (or 3 such adults in an A-level hospital; ie, a highly complex center or the reference hospital for an area). The group also listed 34 antidotes that B-level hospitals should stock and 22 that other types of hospital should stock. The recommendations also provide a guide for using the antidotes in the forms of presentation available in Spain in 2015, a list of their main indications and recommended doses, and specific notes on each
