    Methodology for assessing the social effectiveness of exhibition activity

    Improving the exhibition activity efficiency is a rather complex and multi-purpose task that requires taking into account many factors and using various methods and tools. To achieve success in this field, it is necessary to apply an integrated approach, considering the relationship between the economic and social effectiveness of exhibition activity, as well as to use innovative technologies and the latest research methods. The article presents the methodology developed by the authors for the quantitative assessment of social efficiency, which is complex and also includes the calculation of the selected elements’ sensitivity of social efficiency to different changes. The indicators’ calculation of the exhibition social significance the makes it possible to assess its impact on society and determine how much it contributes to the development of culture, coverage of topical issues and familiarization of the general population with cultural and scientific achievements. The quantitative assessment methodology of the exhibition activity effectiveness based on the calculation of the social efficiency integral indicator can be supplemented with methods of organizational support of the exhibition process, meta-planning and risk assessment

Acest studiu a fost efectuat pentru a determina lățimea optima disponibila a zonei donatoare de lambou care nu va prejudicia Ăźnchiderea primara a plăgii donore, fiind suturata primar plan cu plan.Scop. Evaluarea disponibilității tegumentare fiziologice a fiecărei regiuni donatoare de la nivelul gambei. Material și metode. Studiul a fost efectuat pe un lot de 30 de voluntari, pe membrele sănătoase. Limitele de vĂąrsta in lot au fost de 20 - 60 de ani, divizate pe categorii a cĂąte 10 ani, fiecare subcategorie incluzĂąnd minim 5 voluntari. Disponibilitatea fiziologica s-a calculat pentru lambourile: safen, sural, peronier superficial, suprameleolar, fibular, tibial posterior si tibial anterior. Tegumentul s-a plicaturat in centrul teritoriului donator (cetripet), pana cĂąnd părtile plicaturate contactau formĂąnd duplicatura (Ld). Grosimea duplicaturii s-a măsurat, fiind trecuta pe linia milimetrica. Ulterior, plica cutanata era desfăcuta la parametrii normali ai tegumentului cu extensia acelorași puncte in exterior (centrifug), fixĂąndu-se lungimea in milimetri (Le). Disponibilitatea tegumentara fiziologica (Dt) s-a calculat după formula: Dt =Le – Ld. Rezultate. Cea mai mare disponibilitate fiziologica la nivelul gambei s-a determinat in regiunea lamboului safen (5.05±0.29 cm; n=30; p=0.033), urmata de regiunea lamboului sural (4.83±0.25 cm; n=30; p=0.028) si regiunea lamboului tibial anterior (4.28±0.20 cm; n=30; p=0.03). Cea mai mica disponibilitate fiziologica s-a determinat Ăźn regiunea lamboului supramaleolar (3.04±0.21 cm; n=30; p=0.018). Concluzii. În urma studiului efectuat am constatat ca ridicarea unui lambou cu lățimea disponibila fiziologica a regiunilor donatoare la gamba nu va produce probleme de Ăźnchidere a plăgii donore, fiind suturata primar plan cu plan.Introduction. This study was conducted to determine the optimal, available width of the flap’s donor site that will not harm the primary closure of donor wound. Purpose. To assess the skin physiological availability of each donor region of the calf. Material and methods. The study was conducted on a group of 30 volunteers, assessing healthy limbs. Age limits in group were 20-60 years, divided into categories of 10 years, including at least 5 volunteers each category. Physiological availability was calculated for flaps: saphenous, sural, superficial peroneal, suprameleolar, fibular, posterior and anterior tibial. The skin was fold in the center of donor territory (centripetal) until the folded parts was contacting, forming a folding (Ld). Folding’s thickness was measured, being passed on millimeter line. Subsequently, skin fold was unfolded to normal skin with the extension of the same points outside (centrifugal), attaching the length in millimeters (Le). Skin physiological availability (Dt) was calculated using the formula: Dt = Le - Ld. Results. Highest physiological availability was determined in region of saphenous flap (5.05±0.29 cm; n = 30; p = 0.033), followed by the sural flap (4.83±0.25 cm; n=30; p= 0.028) and region of anterior tibial flap (4.28±0.20 cm, n=30; p=0.03). The smallest physiological availability was determined in the region of supramalleolar flap (3.04±0.21 cm, n = 30; p = 0.018). Conclusions. From this study we found that raising a flap with physiological available width of donor regions of calf will not cause problems in donor wound’s closure, this being sutured primary

    Megan Coyer’s chapter engages with periodical print as a vehicle for an improving medical culture in Scotland, concentrating on the second series of the Scots Magazine. Coyer demonstrates how the Scottish press often complemented improving civic initiatives like the Edinburgh Lunatic Asylum campaign. She focuses attention on the distinctive national dynamics associated with medical improvement efforts in early nineteenth-century Scotland, with the Scots Magazine ‘providing a public forum for the expression of a national medical identity’. This identity, as Coyer shows, had an ideology of improvement at its core. This work recovers the cultural significance of the Scots Magazine as ‘the third major player in popular periodical culture in Romantic-era Scotland’; a status overshadowed by the recent critical attention devoted to the second Edinburgh Review and Blackwood’s in Scottish Romantic studies. Coyer also shows how the efforts of public health reformers highlight the complexity of improvement as both a material and moral process. She argues that print efforts dedicated to improving public health represent a ‘discursive strand in the magazine identifying a lack of cleanliness 
 as a moral and material blight on an otherwise improving Scottish society’. This bringing together of moral and practical aspects of improvement in the Scots also finds expression in the magazine’s series of Scottish medical biographies, whose narratives, Coyer notes, provide ‘ideal exemplars of lives dedicated to a culture of improvement’

    Human NK cells internalize recombinant major stress protein HSP70

    Heat shock proteins 70 kDa (HSP70) protect intracellular proteins from the damaging effects of stress factors of various natures. Moreover, HSP70 play an important role in the vital activity of cells under normal physiological conditions, performing chaperone functions. These functions are realized in the intracellular space; however, in some cases, these proteins are also found on the cell surface and in the extracellular environment. The causes and mechanisms of HSP70 translocation to the cell surface and secretion into the extracellular space have not yet been well understood, but such an unusual localization of HSP70 activates the immune system. The surface HSP70 and their extracellular pool stimulate the cytotoxic activity of NK cells. However, direct experimental evidence for the internalization of HSP70 molecules by NK cells has not yet been demonstrated. This paper presents the results of the interaction of the extracellular HSP70 pool with NK cells from the peripheral blood. The results demonstrated the confirmation of the internalization of exogenous HSP70 molecules by NK cells. To this end, fluorescently labeled recombinant stress-inducible human HSP70 were obtained. The electrophoretic data indicated the absence of protein degradation during the labeling process, the purity and stability of the modified protein. To assess the interaction of HSP70 with NK cells, the fluorescently labeled HSP70 was added to an in vitro culture of NK cells isolated by magnetic separation from the peripheral blood mononuclear fraction and analyzed by confocal microscopy. This analysis indicated that living NK cells internalize extracellular HSP70 with localization both in lysosomes and in phagosomes. Our experiments illustrated for the first time the process of penetration of the extracellular form of HSP70 into these cells. The results suggest that the activation of NK cells under the action of exogenous HSP70 could be associated with the internalization of these protein molecules

    Revision for adverse local tissue reaction following metal-on-polyethylene total hip arthroplasty is associated with a high risk of early major complications

    AimsFretting and corrosion at the modular head/neck junction, known as trunnionosis, in total hip arthroplasty (THA) is a cause of adverse reaction to metal debris (ARMD). We describe the outcome of revision of metal-on-polyethylene (MoP) THA for ARMD due to trunnionosis with emphasis on the risk of major complications.Patients and MethodsA total of 36 patients with a MoP THA who underwent revision for ARMD due to trunnionosis were identified. Three were excluded as their revision had been to another metal head. The remaining 33 were revised to a ceramic head with a titanium sleeve. We describe the presentation, revision findings, and risk of complications in these patients.ResultsThe patients presented with pain, swelling, stiffness, or instability and an inflammatory mass was confirmed radiologically. Macroscopic material deposition on the trunnion was seen in all patients, associated with ARMD. Following revision, six (18.2%) dislocated, requiring further revision in four. Three (9.1%) developed a deep infection and six (18.2%) had significant persistent pain without an obvious cause. One developed a femoral artery thrombosis after excision of an iliofemoral pseudotumor, requiring a thrombectomy.ConclusionThe risk of serious complications following revision MoP THA for ARMD associated with trunnionosis is high. In the presence of extensive tissue damage, a constrained liner or dual mobility construct is recommended in these patients. Cite this article: Bone Joint J 2018;100-B:720–4.</jats:sec

    How can healthcare professionals provide guidance and support to parents of adolescents? Results from a primary care-based study

    Background: This study explored the rewards and difficulties of raising an adolescent and investigated parents' level of interest in receiving guidance from healthcare providers on parenting and adolescent health topics. Additionally, this study investigated whether parents were interested in parenting programs in primary care and explored methods in which parents want to receive guidance. Methods: Parents of adolescents (ages 12-18) who attended an outpatient pediatric clinic with their adolescent were contacted by telephone and completed a short telephone survey. Parents were asked open-ended questions regarding the rewards and difficulties of parenting and rated how important it was to receive guidance from a healthcare provider on certain parenting and health topics. Additionally, parents reported their level of interest in a parenting program in primary care and rated how they would like to receive guidance. Results: Our final sample included 104 parents, 87% of whom were interested in a parenting program within primary care. A variety of parenting rewards and difficulties were associated with raising an adolescent. From the list of parenting topics, communication was rated very important to receive guidance on (65%), followed by conflict management (50%). Of health topics, parents were primarily interested in receiving guidance on sex (77%), mental health (75%), and alcohol and drugs (74%). Parents in the study wanted to receive guidance from a pediatrician or through written literature. Conclusions: The current study finds that parents identify several rewarding and difficult aspects associated with raising an adolescent and are open to receiving guidance on a range of parenting topics in a variety of formats through primary care settings. Incorporating such education into healthcare visits could improve parents' knowledge. Healthcare providers are encouraged to consider how best to provide parenting support during this important developmental time period

    Pediatric supracondylar fractures of the distal humerus

    Supracondylar fractures of the humerus are a common pediatric elbow injury that are historically associated with morbidity due to malunion, neurovascular complications, and compartment syndrome. True anteroposterior and lateral radiographs are essential not only for an accurate diagnosis, but also for creating a treatment plan for these injuries. A staging system (based on the lateral radiograph) for classifying the severity of the fracture helps guide definitive management. Nondisplaced fractures are treated initially with a posterior splint, followed by a long-arm casting. Closed reduction and percutaneous pinning is the preferred treatment for displaced or unstable fractures. If there is any question about fracture stability, patients should be seen within 5 days postoperatively for repeat radiographs to ensure that the reduction and pin fixation has been maintained. Understanding the anatomy, radiographic findings, management options, and complications associated with this fracture allow physicians to limit the morbidity associated with this relatively common pediatric injury

    Therapeutic effect of exogenous Hsp70 in mouse models of Alzheimer's disease

    Brain deterioration resulting from "protein folding" diseases, such as the Alzheimer's disease (AD), is one of the leading causes of morbidity and mortality in the aging human population. Heat shock proteins (Hsps) constitute the major cellular quality control system for proteins that mitigates the pathological burden of neurotoxic protein fibrils and aggregates. However, the therapeutic effect of Hsps has not been tested in a relevant setting. Here we report the dramatic neuroprotective effect of recombinant human Hsp70 in the bilateral olfactory bulbectomy model (OBX mice) and 5XFAD mouse models of neurodegeneration. We show that intranasally-administered Hsp70 rapidly enters the afflicted brain regions and mitigates multiple AD-like morphological and cognitive abnormalities observed in model animals. In particular, in both cases it normalizes the density of neurons in the hippocampus and cortex which correlates with the diminished accumulation of amyloid-ÎČ (AÎČ) peptide and, in the case of 5XFAD mice, reduces AÎČ plaque formation. Consistently, Hsp70 treatment also protects spatial memory in OBX and 5XFAD mice. These studies demonstrate that exogenous Hsp70 may be a practical therapeutic agent for treatment of neurodegenerative diseases associated with abnormal protein biogenesis and cognitive disturbances, such as AD, for which neuroprotective therapy is urgently needed
