Rodzinna częściowa lipodystrofia w diagnostyce różnicowej zespołu policystycznych jajników

  According to current diagnostic criteria, polycystic ovary syndrome (PCOS) is effective as a diagnosis of exclusion. Here, we present a case of a 31-year-old woman with a history of oligomenorrhoea and hirsutism, who, despite a “muscular” appearance and a normal body mass index (22.27 kg/m2), was found to have an extreme insulin resistance and diabetes accompanied by hyperandrogenism and polycystic ovaries. An autoimmune screen for possible latent autoimmune diabetes in adults was negative. She was subsequently found to have familial partial lipodystrophy (FPLD2, OMIM #151660) caused by an R482Q mutation in the LMNA gene encoding lamin A/C. This mutation results in arginine to glutamine substitution at the protein level, while phenotypically this condition presents with a loss of body fat, insulin resistance, dyslipidaemia, and other features mimicking PCOS. Interestingly her mother, with a history of myocardial infarction and diabetes at the age of 46 but no oligomenorrhoea, was also found to harbour the same mutation (LMNA R482Q). Conclusions: Our case highlights the importance of assessment of adipose tissue distribution, as well as a significance of assessment of glucose tolerance and insulin resistance in the differential diagnosis of PCOS. Furthermore, patients with atypical adipose tissue distribution should be referred for formal genetic testing. W pracy przedstawiono przypadek 31-letniej kobiety z wywiadem zaburzeń miesiączkowania o typie rzadkich miesiączek oraz hirsutyzmu, u której mimo „muskularnego” wyglądu i prawidłowego wskaźnika masy ciała (22,27 kg/m2) stwierdzono bardzo wysoką insulinooporność, cukrzycę i hiperandrogenizm ze współistniejącą morfologią policystycystyczną jajników. Badania autoimmunologiczne w kierunku możliwej autoimmunologicznej cukrzycy u dorosłych (tzw. LADA) były ujemne. Przeprowadzono diagnostykę genetyczną, rozpoznając rodzinną lipodystrofię częściową spowodowaną przez mutację laminy A/C genu R842Q (FPLD2, OMIM # 151660). Powyższa mutacja prowadzi do podstawienia argininy glutaminą (R482Q) na poziomie białka, natomiast fenotypowo objawia się utratą tkanki tłuszczowej, insulinoopornością, dyslipidemią, jak również cechami naśladującymi PCOS. Co ciekawe u matki pacjentki również rozpoznano tę samą mutację (lamina A/C genu R842Q). U matki wystąpił również zawał serca i cukrzyca w wieku 46 lat, jednak nie było u niej zaburzeń miesiączkowania o typie oligomenorrhoea. Wnioski: Opisany przypadek podkreśla znaczenie oceny dystrybucji tkanki tłuszczowej, jak również znaczenie oceny tolerancji glukozy i insulinooporności w diagnostyce różnicowej PCOS. Autorzy pracy zalecają również, aby pacjentki z nietypowym rozkładem tkanki tłuszczowej były kierowane do dalszej diagnostyki genetycznej. (Endokrynol Pol 2015; 66 (6): 550–554)

Kinetic studies of novel inhibitors of endomorphin degrading enzymes

Endomorphins (EMs), two endogenous μ-opioid receptor selective ligands, are attractive lead compounds for opioid-based pain management studies. However, these peptides are quickly degraded by peptidases, in particular by dipeptidylpeptidase IV (DPP IV) and aminopeptidase M (APM). Targeting enzymatic degradation is one approach to prolong endomorphin activity. In this study we characterized the action of two new inhibitors of similar to endomorphins structure, Tyr-Pro-Ala-NH2 (EMDB-2) and Tyr-Pro-Ala-OH (EMDB-3), which were designed earlier in our laboratory. The presented data give evidence that EMDB-2 and EMDB-3 are potent inhibitors of enzymes responsible for endomorphin cleavage. These compounds are stable and easily synthesized. EMDB-2 and EMDB-3 are competitive inhibitors of both, DPP IV and APM, with Ki values in micromolar range. They are less potent than diprotin A in protecting EMs against DPP IV but more potent than actinonin in protecting these peptides against APM

Synthesis, Selected Transformations, and Biological Activity of Alkoxy Analogues of Lepidilines A and C

Condensation of diacetyl monooxime with formaldimines derived from alkoxyamines in glacial acetic acid at room temperature leads to corresponding 2-unsubstituted imidazole N-oxides bearing an alkoxy substituent at the N(1) atom of the imidazole ring. Subsequent O-benzylation afforded, depending on the type of alkylating agent, either symmetric or nonsymmetric alkoxyimidazolium salts considered as structural analogues of naturally occurring imidazole alkaloids, lepidilines A and C. Some of the obtained salts were tested as precursors of nucleophilic heterocyclic carbenes (NHCs), which in situ reacted with elemental sulfur to give the corresponding N-alkoxyimidazole-2-thiones. The cytotoxic activity of selected 4,5-dimethylimidazolium salts bearing either two benzyloxy or benzyloxy and 1-adamantyloxy groups at N(1) and N(3) atoms was evaluated against HL-60 and MCF-7 cell lines using the MTT (3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyltetrazolium bromide) assay. Notably, in two cases of alkoxyimidazolium salts, no effect of the counterion exchange (Br− → PF6−) on the biological activity was observed

The role of morphine in regulation of cancer cell growth

Morphine is considered the “gold standard” for relieving pain and is currently one of the most effective drugs available clinically for the management of severe pain associated with cancer. In addition to its use in the treatment of pain, morphine appears to be important in the regulation of neoplastic tissue. Although morphine acts directly on the central nervous system to relieve pain, its activities on peripheral tissues are responsible for many of the secondary complications. Therefore, understanding the impact, other than pain control, of morphine on cancer treatment is extremely important. The effect of morphine on tumor growth is still contradictory, as both growth-promoting and growth-inhibiting effects have been observed. Accumulating evidence suggests that morphine can affect proliferation and migration of tumor cells as well as angiogenesis. Various signaling pathways have been suggested to be involved in these extra-analgesic effects of morphine. Suppression of immune system by morphine is an additional complication. W pracy ukazane są pogańskie i chrześcijańskie zwyczaje Hucułów. Praca omawia pochodzenie Hucułów oraz opisuje tereny przez nich zamieszkiwane. Osnową pracy jest powieść Mychajła Kociubyńskiego "Cienie zapomnianych przodków". Dzięki powieści czytelnik poznaje magiczny huculski świat, zaklęcia i zamówienia w nim obecne. Terminologia wprowadzona w pracy pozwala na rozróżnienie elementów świata chrześcijańskiego od przedchrześcijańskiego.The work shows the pagan and Christian customs of Hutsuls. The work discusses the origin of Hutsuls and describes the territories inhabited by them. The basis of this work is a novel written by Mykhailo Kotsiubynsky "Shadows of forgotten ancestors". Thanks to the novel, the reader learns the magical Hutsul world, spells and orders are present in it. The terminology introduced in the work makes it possible to distinguish between elements of the Christian world and the pre-Christian world
