Performance of screening for aneuploidies by cell-free DNA analysis of maternal blood in twin pregnancies
Objectives
To report clinical implementation of cellâfree DNA (cfDNA) analysis of maternal blood in screening for trisomies 21, 18 and 13 in twin pregnancies and examine variables that could influence the failure rate of the test.
Methods
cfDNA testing was performed in 515 twin pregnancies at 10â28 weeks' gestation. The failure rate of the test to provide results was compared with that in 1847 singleton pregnancies, and logistic regression analysis was used to determine which factors among maternal and pregnancy characteristics were significant predictors of test failure.
Results
Failure rate of the cfDNA test at first sampling was 1.7% in singletons and 5.6% in twins. Of those with a test result, the median fetal fraction in twins was 8.7% (range, 4.1â30.0%), which was lower than that in singletons (11.7% (range, 4.0â38.9%)). Multivariable regression analysis demonstrated that twin pregnancy, higher maternal weight and conception by inâvitro fertilization provided significant independent prediction of test failure. Followâup was available in 351 (68.2%) of the twin pregnancies and comprised 334 with euploid fetuses, 12 discordant for trisomy 21 and five discordant for trisomy 18. In all 323 euploid cases with a result, the risk score for each trisomy wasâ<â1:10â000. In 11 of the 12 cases with trisomy 21 and in the five with trisomy 18, the cfDNA test gave a highârisk result, but in one case of trisomy 21, the score wasâ<â1:10â000.
Conclusion
In twin pregnancies screening by cfDNA testing is feasible, but the failure rate is higher and detection rate may be lower than in singletons
Moderate beer consumption does not change early or mature atherosclerosis in mice
BACKGROUND: Although the consumption of wine in particular has been associated with a lower risk of atherothrombotic cardiovascular disease, systematic reviews differ as to the relative protective effect of beer, wine and spirits. Two previous studies showed that red wine reduces fatty streak formation (early atherosclerosis) but not mature atherosclerosis in apolipoprotein (apo) E-deficient (apoE-/-) mice. AIM OF THE STUDY: To determine whether a moderate beer intake would affect early and mature atherosclerotic lesion formation using control C57BL/6 and apoE-/- mice, respectively, as models. METHODS: Control C57BL/6 and apoE-/- mice were randomized to receive either water, ethanol, mild beer, dark beer or ethanol-free beer. The level of beer was designed to approximate the alcohol intake currently believed to be beneficial in reducing human vascular risk. Control C57BL/6 mice were fed a Western diet for 24 weeks, and apoE-/- mice a chow diet for 12 weeks. At the end of the trial period, mice were euthanized and atherosclerotic lesions quantified. Plasma lipid concentrations were also measured. RESULTS: The amount of atherosclerosis and average number of lesions in the proximal aortic region did not differ among groups in control C57BL/6 mice (p = 0.32 and p = 0.29, respectively) and apoE-/- mice (p = 0.19 and p = 0.59, respectively). No consistent differences were observed in plasma lipid and lipoprotein concentrations among water, ethanol and beer groups. CONCLUSIONS: Moderate beer consumption does not change the development of early or mature atherosclerosis in mice. Our findings do not support the hypothesis of an anti-atherogenic effect of beer. Other potential protective actions of moderate beer consumption such as plaque stabilization, a reduction in plaque intrinsic thrombogenicity, or a reduction in the systemic propensity to thrombosis, remain to be studied

Keratoacanthoma-like nodules as first manifestation of metastatic epithelioid trophoblastic tumor
Cutaneous metastases are rarely the initial manifestation of a previously undiagnosed malignancy and keratoacanthoma-like lesions are a notoriously unusual presentation pattern of cutaneous dissemination of a primary tumor. Herein, we report a 40-year-old woman presenting to our dermatology department with multiple keratoacanthoma-like scalp nodules. Subsequent investigation determined it to be the first manifestation of a disseminated endometrial epithelioid trophoblastic tumor, eventually causing the patient's death. Epithelioid trophoblastic tumor, a rare form of gestational trophoblastic disease, is a recently described neoplasm whose cutaneous metastasis has not been previously reported in the literature
Mitigating risk through R&D+Innovation: Chileâs national strategy for disaster resilience
With an annual average expense of more than 2,800 million USD, Chile leads the OECD countries with the
largest percentage of the GDP spent in disaster losses per year (almost 1.2% GDP). This was the motivation of the
Chilean President in 2015 to ask a group of experts to prepare a national Research, Development, and Innovation
(R&D+i) strategy for disaster resilience. The strategy was developed by a group, called by the acronym CREDEN,
and involved more than 80 experts representing different national stakeholders from the academia, public and
private sectors, NGOs, and the armed forces. The work of CREDEN finished December 2016 and produced an
R&D+i roadmap composed of five enabling conditions and 14 tasks. The implementation of this strategy demands
a total investment of 914 million USD in 20 years, which is expected to have a benefit-cost ratio of 2.32, and
annual savings of about $106 million USD. The first stage in this process is the design of a National Institute of
R&D+i for Disaster Resilience (ITReND), which will oversee the implementation of the strategy. ITReNDâs
design was completed in 2017 and its implementation is expected to begin in 2018. This strategy can be considered
as an example of how to position R&D+i in the basis of public policy for disaster resilience. Both, the contents of
this strategy and its implementation process, have unique aspects and may help guide other disaster-prone countries
in their pursuit of larger resilience to the increasing occurrence rate of extreme natural events
Research on nonlinear and quantum optics at the photonics and quantum information group of the University of Valladolid
We outline the main research lines in Nonlinear and Quantum Optics of the Group of Photonics and Quantum Information at the University of Valladolid. These works focus on Optical Solitons, Quantum Information using Photonic Technologies and the development of new materials for Nonlinar Optics. The investigations on optical solitons cover both temporal solitons in dispersion managed fiber links and nonparaxial spatial solitons as described by the Nonlinear Helmholtz Equation. Within the Quantum Information research lines of the group, the studies address new photonic schemes for quantum computation and the multiplexing of quantum data. The investigations of the group are, to a large extent, based on intensive and parallel computations. Some associated numerical techniques for the development of the activities described are briefly sketched
Survey of H-alpha emission from thirty nearby dwarf galaxies
Measurements of the H-alpha flux from 30 neighboring dwarf galaxies are
presented. After correction for absorption, these fluxes are used to estimate
the star formation rate (SFR). The SFR for 18 of the galaxies according to the
H-alpha emission are compared with estimates of the SFR from FUV magnitudes
obtained with the GALEX telescope. These are in good agreement over the range
log[SFR] = [-3,0]M sun/yr.Comment: 18 pages, 10 figures, 3 table
TMAO and Gut Microbial-Derived Metabolites TML and ÎłBB Are Not Associated with Thrombotic Risk in Patients with Venous Thromboembolism
Background: The present work evaluates the association between circulating concentrations of Trimethylamine-N-oxide (TMAO), gamma butyrobetaine (gamma BB), and trimetyllisine (TML) in controls and patients with venous thromboembolism (VTE) with coagulation parameters. Methods: The study involved 54 VTE patients and 57 controls. Platelet function, platelet hyperreactivity, platelet adhesiveness, thrombosis-associated parameters, and thrombin generation parameters were studied. Plasma TMAO, gamma BB, and TML determination was performed using an ultra-high-performance liquid chromatography system coupled with mass spectrometry. Results: No differences were found for TMAO, gamma BB, or TML concentrations between controls and VTE patients. In thrombin generation tests, TMAO, gamma BB, and TML showed a positive correlation with lag time and time to peak. TMAO, gamma BB, and TML negatively correlated with peak height. No significant differences were observed regarding TMAO, gamma BB, and TML concentrations between the two blood withdrawals, nor when the control and VTE patients were analyzed separately. No correlation was observed between these gut metabolites and platelet function parameters. Conclusions: No differences were found regarding TMAO, gamma BB, and TML concentrations between the control and VTE groups. Some correlations were found; however, they were mild or went in the opposite direction of what would be expected if TMAO and its derivatives were related to VTE risk
Burden of herpes zoster requiring hospitalization in Spain during a seven-year period (1998â2004)
<p>Abstract</p> <p>Background</p> <p>A thorough epidemiological surveillance and a good understanding of the burden of diseases associated to VZV are crucial to asses any potential impact of a prevention strategy. A population-based retrospective epidemiological study to estimate the burden of herpes zoster requiring hospitalization in Spain was conducted.</p> <p>Methods</p> <p>This study was conducted by using data from the national surveillance system for hospital data, Conjunto MĂnimo BĂĄsico de Datos (CMBD). Records of all patients admitted to hospital with a diagnosis of herpes zoster (ICD-9-MC codes 053.0â053.9) during a 7-year period (1998â2004) were selected.</p> <p>Results</p> <p>A total of 23,584 hospitalizations with a primary or secondary diagnosis of herpes zoster in patients â„ 30 years of age were identified during the study period. Annually there were 13.4 hospitalizations for herpes zoster per 100,000 population in patients â„ 30 years of age. The rate increases with age reaching a maximum in persons â„ 80 years of age (54.3 admissions per 100,000 population >80 years of age). The mean cost of a hospitalization for herpes zoster in adult patients was 3,720 âŹ. The estimated annual cost of hospitalizations for herpes zoster in patients â„ 30 years of age in Spain was 12,731,954 âŹ.</p> <p>Conclusion</p> <p>Herpes zoster imposes an important burden of hospitalizations and result in large cost expenses to the Spanish National Health System, especially in population older than 50 years of age</p
