Total hip arthroplasty through the direct anterior approach with and without the use of a traction table: a matched-control, retrospective, single-surgeon study.
Hip surgeons performing total hip arthroplasty (THA) through the direct anterior approach (DAA) commonly use a traction table to facilitate exposure. Even though performing THA through DAA without a traction table could be technically more demanding, this technique offers the advantage of intraoperative leg length comparison. Therefore, this study aimed to compare clinical outcomes, complication rates, component positioning, and leg length discrepancy (LLD) after THA through the DAA performed with or without a traction table.
A single-surgeon continuous series of 75 patients who underwent DAA THA performed with a traction table was matched for gender, age, and BMI with 75 patients who underwent DAA THA performed without a traction table (male, 62; female, 88, with an average age of 68 years old). Clinical and radiological outcomes, intra- and postoperative complications, and LLD were retrospectively assessed.
No statistically significant difference was detected in surgical time, hospital stay, Harris Hip Score (HHS), complication rates, and implant positioning between the two groups. Leg length restoration was significantly more accurate in the group performed without a traction table (2.4 ± 2 mm vs. 3.7 ± 3.1 mm; p value ≤ 0.05). No LLD > 10 mm was reported in the group performed without a traction table, whereas two cases (2.7%) were reported in those performed with a traction table.
Performing THA through DAA without a traction table was associated with a significantly more accurate leg length restoration without a significant increase in the rates of intra- and postoperative complications
Towards higher order lattice Boltzmann schemes
In this contribution we extend the Taylor expansion method proposed
previously by one of us and establish equivalent partial differential equations
of DDH lattice Boltzmann scheme at an arbitrary order of accuracy. We derive
formally the associated dynamical equations for classical thermal and linear
fluid models in one to three space dimensions. We use this approach to adjust
relaxation parameters in order to enforce fourth order accuracy for thermal
model and diffusive relaxation modes of the Stokes problem. We apply the
resulting scheme for numerical computation of associated eigenmodes and compare
our results with analytical references
Geology of the d'Entrecasteaux - New Hebrides arc collision zone : results from a deep submersible survey
International audienceDuring the SuBPSOl cruise, seven submersible dives were conducted between water depths of 5350 and 900 m over the collision zone between the New Hebrides island arc and the d'Entrecasteaux Zone (DEZ). The DEZ, a topographic high on the Australian plate, encompasses the North d'Entrecasteaux Ridge (NDR) and the Bougainville guyot, both of which collide with the island-arc slope. In this report we use diving observations and samples, as well as dredging results, to analyse the geology of the Bougainville guyot and the outer arc slope in the DEZ-arc collision zone, and to decipher the mechanisms of seamount subduction. These data indicate that the Bougainville guyot is a middle Eocene island arc volcanocapped with reef limestones that appear to have been deposited during the Late Oligocene to Early Miocene and in Miocene-Pliocene times. This guyot possibly emerged during the Middle and Late Miocene, and started to sink in the New Hebrides trench after the Pliocene. The rocks of the New Hebrides arc slope, in the collision zone, consist primarily of Pliocene-Recent volcaniclastic rocks derived from the arc, and underlying fractured island-arc volcanic basement, possibly of Late Miocene age. However, highly sheared, Upper Oligocene to Lower Miocene nannofossil ooze and chalk are exposed at the toe of the arc slope against the northern flank of the NDR. Based on a comparison with cores collected at DSDP Site 286, the ooze and chalk can be interpreted as sediments accreted from the downgoing plate. East of the Bougainville guyot an antifonn that developed in the arc slope as a consequence of the collision reveals a 500-m-thick wedge of strongly tectonized rocks, possibly accreted from the guyot or an already subducted seamount. The wedge that is overlain by less deformed volcaniclastic island-arc rocks and sediments includes imbricated layers of Late Oligocene to Early Miocene reef and micritic limestones. This wedge, which develops against the leading flank of the guyot, tends to smooth its high-drag shape. A comparison between the 500-m-thick wedge of limestones that outcrops southeast of the guyot and the absence of such a wedge over the flat top of the guyot, although the top is overthrust by island-arc rocks and sediments, can be interpreted to suggest that the wedge moves in the subduction zone with the guyot and facilitates its subduction by streamlining
Cytoplasmic PML promotes TGF-β-associated epithelial–mesenchymal transition and invasion in prostate cancer
Epithelial–mesenchymal transition (EMT) is a key event that is involved in the invasion and dissemination of cancer cells. Although typically considered as having tumour-suppressive properties, transforming growth factor (TGF)-β signalling is altered during cancer and has been associated with the invasion of cancer cells and metastasis. In this study, we report a previously unknown role for the cytoplasmic promyelocytic leukaemia (cPML) tumour suppressor in TGF-β signalling-induced regulation of prostate cancer-associated EMT and invasion. We demonstrate that cPML promotes a mesenchymal phenotype and increases the invasiveness of prostate cancer cells. This event is associated with activation of TGF-β canonical signalling pathway through the induction of Sma and Mad related family 2 and 3 (SMAD2 and SMAD3) phosphorylation. Furthermore, the cytoplasmic localization of promyelocytic leukaemia (PML) is mediated by its nuclear export in a chromosomal maintenance 1 (CRM1)-dependent manner. This was clinically tested in prostate cancer tissue and shown that cytoplasmic PML and CRM1 co-expression correlates with reduced disease-specific survival. In summary, we provide evidence of dysfunctional TGF-β signalling occurring at an early stage in prostate cancer. We show that this disease pathway is mediated by cPML and CRM1 and results in a more aggressive cancer cell phenotype. We propose that the targeting of this pathway could be therapeutically exploited for clinical benefit
Interest of colchicine for the treatment of cystic fibrosis patients. Preliminary report.
Cystic fibrosis (CF) lung disease is characterized by persistent inflammation. Antiinflammatory drugs, such as corticosteroids and ibuprofen, have proved to slow the decline of pulmonary function although their use is limited because of frequent adverse events. We hypothesized that colchicine could be an alternative treatment because of its antiinflammatory properties and upregulatory effect on cystic fibrosis transmembrane regulator (CFTR) closely related proteins. We herein present results obtained in an open study of eight CF children treated with colchicine for at least 6 months. Clinical status was better in all patients and respiratory function tests significantly improved in five. Median duration of antibiotherapy decreased significantly. These preliminary results support our hypothesis of a beneficial effect of colchicine in CF patients and stress the need for a controlled therapeutic trial
Variational Principles for Stellar Structure
The four equations of stellar structure are reformulated as two alternate
pairs of variational principles. Different thermodynamic representations lead
to the same hydromechanical equations, but the thermal equations require, not
the entropy, but the temperature as the thermal field variable. Our treatment
emphasizes the hydrostatic energy and the entropy production rate of luminosity
produced and transported. The conceptual and calculational advantages of
integral over differential formulations of stellar structure are discussed
along with the difficulties in describing stellar chemical evolution by

Prehistory of Transit Searches
Nowadays the more powerful method to detect extrasolar planets is the transit
method. We review the planet transits which were anticipated, searched, and the
first ones which were observed all through history. Indeed transits of planets
in front of their star were first investigated and studied in the solar system.
The first observations of sunspots were sometimes mistaken for transits of
unknown planets. The first scientific observation and study of a transit in the
solar system was the observation of Mercury transit by Pierre Gassendi in 1631.
Because observations of Venus transits could give a way to determine the
distance Sun-Earth, transits of Venus were overwhelmingly observed. Some
objects which actually do not exist were searched by their hypothetical
transits on the Sun, as some examples a Venus satellite and an infra-mercurial
planet. We evoke the possibly first use of the hypothesis of an exoplanet
transit to explain some periodic variations of the luminosity of a star, namely
the star Algol, during the eighteen century. Then we review the predictions of
detection of exoplanets by their transits, those predictions being sometimes
ancient, and made by astronomers as well as popular science writers. However,
these very interesting predictions were never published in peer-reviewed
journals specialized in astronomical discoveries and results. A possible
transit of the planet beta Pic b was observed in 1981. Shall we see another
transit expected for the same planet during 2018? Today, some studies of
transits which are connected to hypothetical extraterrestrial civilisations are
published in astronomical refereed journals. Some studies which would be
classified not long ago as science fiction are now considered as scientific
