    Eosinophilic esophagitis: A relevant entity for the otolaryngologist

Its diagnosis represents a challenge for different medical specialties, among which ENT specialists play an important role. Clinical suspicion in a patient with recurrent food impaction or a child with eating disorders and history of hypersensitivity constitutes the first warning sign of a possible EE.The purpose of this review is to highlight EE as a possible differential diagnosis in patients with deglutition disorders and describe the possible clinical symptoms that should alert the ENT specialist to perform appropriate diagnostic tests and procedures. The transnasal esophagoscopy, performed in-office by the ENT, is ideal for reducing possible underdiagnosed cases.Given the fact that an ENT specialist will evaluate a great many patients with deglutition disorders, it is paramount for possible EE cases to be suspected and recognised so that a correct multidisciplinary approach involving not only ENT specialists but also paediatricians, gastroenterologists, allergologists and pathologists can be established. Identifying the dietary component responsible for the esophageal inflammation and removing that food from the patient's diet is the key in the treatment of this immune-mediated disease.La esofagitis eosinof铆lica (EE) es una entidad cl铆nico patol贸gica reconocida recientemente y con una prevalencia que va en aumento desde su descripci贸n inicial. Su diagn贸stico representa un reto para diferentes especialistas, entre los que tiene un rol destacado el otorrinolaring贸logo. La sospecha cl铆nica ante un paciente que presenta episodios recidivantes de impactaci贸n de alimentos no punzantes o ante un ni帽o con trastornos de la alimentaci贸n y antecedentes de atopia constituyen el primer signo de alerta de una posible EE. El objetivo de esta revisi贸n persigue destacar el papel de la EE en el diagn贸stico diferencial de los pacientes con trastornos de la degluci贸n, as铆 como dar a conocer las manifestaciones cl铆nicas que deben alertar al otorrinolaring贸logo para proseguir la realizaci贸n de las pruebas encaminadas al diagn贸stico de esta enfermedad. La esofagoscopia transnasal, realizada por el otorrinolaring贸logo en consulta, ayudar谩 a disminuir el n煤mero de casos infradiagnosticados. Dado que gran parte de los pacientes afectos de trastornos de la degluci贸n van a ser evaluados por el otorrinolaring贸logo, se hace imprescindible el reconocimiento de la EE, as铆 como el manejo diagn贸stico-terap茅utico por un equipo multidisciplinar en el que se involucren, adem谩s del otorrinolaring贸logo, pediatras, digest贸logos, alerg贸logos y pat贸logos familiarizados con la enfermedad.     Dysphagia and dysphonia are commonly associated with neuromuscular disorders. The symptoms and complications arise from the sensorimotor dysfunction of the oral and pharyngeal phases of swallowing. These symptoms may be unapparent due to compensation strategies, absent sensory cough reflex, or altered cognition. The risks of dysphagia are malnutrition, dehydration, and aspiration. Finding patients at risk is key to avoiding unnecessary complications. Dysphagia could be the first sign of a neuromuscular disease. Videoendoscopy, videofluoroscopy, manometric and electrophysiological studies are complementary tests and should be selected according to patient complaints and availability. In general, neuromuscular diseases tend to affect most phases of deglutition with variable expression and progression among patients. The association with old age of many degenerative conditions further impairs swallowing. Knowledge and awareness of neuromuscular diseases, the signs and symptoms that could affect speech and swallowing, along with familiarity performing a detailed neurological pharyngolaryngeal examination, is essential in understanding the mechanisms altering swallowing and in planning the best treatment. This chapter gives an overview of the physiopathology, the signs and symptoms, the evaluation, and the medical and surgical treatments of the most common neuromuscular conditions
