    Gemcitabine and S-1 Combination Chemotherapy in Patients with Advanced Biliary Tract Cancer: A Retrospective Study

    Background:The aim of this study was to investigate the efficacy and safety of gemcitabine and S-1 combination chemotherapy in patients with advanced biliary tract cancer. Patients and Methods: A retrospective study was performed on 15 consecutive patients. Gemcitabine was administered intravenously at 1,000 mg/m2 on days 8 and 15. Oral S-1 (60 mg/m2 in 2 divided doses) was given daily for the first 2 weeks, followed by 1 week of rest. This 3-week course of treatment was repeated. The primary endpoint was response rate, and the secondary endpoints were overall survival, progression-free survival, and safety. Results: The overall response rate was 26.7%, and the disease control rate was 73.4%. The overall survival was 12.0 months (95% CI, 9.5–14.5 months), and the progression-free survival was 8.0 months (95% CI, 4.3–11.7 months). Adverse events of grade 3 or 4 occurred in 33.3%, and the major grade 3/4 toxicities were anemia (20.0%), leukopenia (13.3%), and anorexia (13.3%). Conclusion:Gemcitabine and S-1 combination chemotherapy is effective and safe in patients with advanced biliary tract cancer

    Effects of extended context discrimination training and context extinction on transfer of context dependency of conditioned flavor aversion

    We trained rats in a context discrimination paradigm by pairing a sucrose solution with lithium chloride in one context (conditioning context) and simple exposure to the same fluid in a second (neutral) context to establish a context-dependent aversion to the conditioned fluid. We then investigated whether transfer of the context dependency to a test fluid (a sodium chloride solution) was affected by two post-discrimination training treatments, an extended context discrimination training, and non-reinforced exposure to the conditioning context (context extinction). We found that the context-dependent flavor aversion that had been specific to sucrose transferred to the test fluid after the extensive training (Experiment 1). Context extinction eliminated the transfer effect that had been observed immediately after the context discrimination training (Experiment 2). In addition, an aversion acquired by sucrose through a simple conditioning of sucrose-LiCl pairings did not generalize to the test fluid (Experiment 3). These results emphasize the importance of a Pavlovian excitatory association between the conditioning context and nausea as a primary source of transfer of the context dependency, rather than a generalization of aversion acquired by the conditioned fluid to the test fluid. © 2014 Elsevier B.V

    Unsupervised Predominant Sense Detection and Its Application to Text Classification

    This paper focuses on the domain-specific senses of words and proposes a method for detecting predominant sense depending on each domain. Our Domain-Specific Senses (DSS) model is an unsupervised manner and detects predominant senses in each domain. We apply a simple Markov Random Walk (MRW) model to ranking senses for each domain. It decides the importance of a sense within a graph by using the similarity of senses. The similarity of senses is obtained by using distributional representations of words from gloss texts in the thesaurus. It can capture large semantic context and thus does not require manual annotation of sense-tagged data. We used the Reuters corpus and the WordNet in the experiments. We applied the results of domain-specific senses to text classification and examined how DSS affects the overall performance of the text classification task. We compared our DSS model with one of the word sense disambiguation techniques (WSD), Context2vec, and the results demonstrate our domain-specific sense approach gains 0.053 F1 improvement on average over the WSD approach

    Role of the Pathotype-Specific ACRTS1 Gene Encoding a Hydroxylase Involved in the Biosynthesis of Host-Selective ACR-Toxin in the Rough Lemon Pathotype of Alternaria alternata

    The rough lemon pathotype of Alternaria alternata produces host-selective ACR-toxin and causes Alternaria leaf spot disease of the rootstock species rough lemon (Citrus jambhiri) and Rangpur lime (C. limonia). Genes controlling toxin production were localized to a 1.5-Mb chromosome carrying the ACR-toxin biosynthesis gene cluster (ACRT) in the genome of the rough lemon pathotype. A genomic BAC clone containing a portion of the ACRT cluster was sequenced which allowed identification of three open reading frames present only in the genomes of ACR-toxin producing isolates. We studied the functional role of one of these open reading frames, ACRTS1 encoding a putative hydroxylase, in ACR-toxin production by homologous recombination-mediated gene disruption. There are at least three copies of ACRTS1 gene in the genome and disruption of two copies of this gene significantly reduced ACR-toxin production as well as pathogenicity; however, transcription of ACRTS1 and production of ACR-toxin were not completely eliminated due to remaining functional copies of the gene. RNA-silencing was used to knock down the remaining ACRTS1 transcripts to levels undetectable by reverse transcription-polymerase chain reaction. The silenced transformants did not produce detectable ACR-toxin and were not pathogenic. These results indicate that ACRTS1 is an essential gene in ACR-toxin biosynthesis in the rough lemon pathotype of A. alternata and is required for full virulence of this fungus. </jats:p

    Localized Subcutaneous Insulin-Derived Amyloidosis Excised after Evaluation Using Ultrasonography in a Patient with Type 2 Diabetes Mellitus

    A 62-year-old man with type 2 diabetes mellitus, who had been on insulin therapy for the past 20 years, was found to have subcutaneous mass formation in the abdomen during a workup of worsened glycemic control. Because of suspected amyloid deposition, he was advised to avoid injections to the mass, which led to improvement of glycemic control. However, he strongly requested mass excision and was hospitalized. After evaluation using ultrasonography and computed tomography, a total mass excision was performed, and a diagnosis of insulin-derived amyloidosis was made. Comparison of the ultrasonographic and histopathological findings demonstrated that the location of the amyloid deposition nearly corresponded to the hypoechoic region. This case highlights that ultrasonography, which is a noninvasive imaging modality, can be useful for detection of insulin-derived amyloidosis

    Soluble oligomeric amyloid β (oAβ) causes synaptic dysfunction and neuronal cell death, which are involved in the pathogenesis of Alzheimer's disease (AD). The hematopoietic growth factor granulocyte-colony stimulating factor (G-CSF) is expressed in the central nervous system (CNS) and drives neurogenesis. Here we show that G-CSF attenuated oAβ neurotoxicity through the enhancement of the enzymatic activity of Aβ-degrading enzyme neprilysin (NEP) in neurons, while the NEP inhibitor thiorphan abolished the neuroprotection. Inhibition of MEK5/ERK5, a major downstream effector of G-CSF signaling, also ablated neuroprotective effect of G-CSF. Furthermore, intracerebroventricular administration of G-CSF enhanced NEP enzymatic activity and clearance of Aβ in APP/PS1 transgenic mice. Thus, we propose that G-CSF may be a possible therapeutic strategy against AD
