Synthesis of new 1- 2-azido-2-(2,4-dichlorophenyl)ethyl -1H/-imidazoles and in vitro evaluation of their antifungal activity

Les dérivés azolés, inhibiteurs de la 14 a[alpha]-déméthylase largement utilisés dans le traitement des infections fongiques, se heurtent à l'augmentation des résistances de la part de microchampignons pathogènes chez l'homme. Aussi, une étude de la relation structure/activité des dérivés azolés en série 1-[2-(2,4-dichlorophényl)éthyl]-1H-imidazole sur une souche modèle de C. glabrata Cg-5579R, de sensibilité réduite aux azolés (excepté le tioconazole) a été réalisée par modification de la chaîne latérale. Certains groupements, tels qu'un azoture, une isoxazoline ou un thiophène (présent dans le tioconazole), induisaient une forte activité sur Cg-5579R. Plusieurs séries de dérivés azolés ayant pour structure de base le 1-(1H-azol-1-yl)-2-(2,4-dichlorophényl)propane ont alors été développées en intégrant ces éléments d'activité en position 2 ou 3. Parmi ces séries originales, des dérivés large spectre actifs sur C. glabrata, Cg-5579R, C. albicans et A. fumigatus ont pu être identifiés.Azole derivatives, inhibitors of the 14 a[alpha]-demethylase largely used in the treatment of the fungic infections, come up against the increase in resistances of various pathogenic strains. The structure/activity relationships of 1-[2-(2,4-dichlorophenyl)ethyl]-1H-imidazol compounds was then studied on a model stock of C. glabrata Cg-5579R with reduced sensitivity to azoles except to tioconazole. During this study, the structure of the side chain was modified and some particular moieties, such as an azide, an isoxazoline or a thiophene, induced a strong activity on Cg-5579R. Some original 1-(1H-azol-1-yl)-2-(2,4-dichlorophenyl)propane derivatives integrating these elements of activity were then synthesized. Among these new azole compounds, some broad spectrum derivatives with an activity on C. glabrata, Cg-5579R, C. albicans and A. Preoperative hepatic insufficiency and type III endoleak: a confirmed potential fatal association following endovascular treatment.

International audienceConsumptive coagulopathy is known to occur in patients with aneurysm, especially in the thoracic localization. Compared to open chest surgery, the endovascular treatment leaves in place a large thrombosed aneurysmal sac, which might induce and/or exacerbate the coagulopathy. Although exceptional, some recent reports have raised the potential disastrous issue related to this complication. We report the case of a 74-year-old patient treated for an asymptomatic thoracic aorta aneurysm by endoprosthesis who developed a fatal disseminated intravascular coagulopathy. This complication has been related to a type III endoleak associated with a preoperative hepatic insufficiency
