    Determinants of fibrinogen in an Italian population suffering from claudication. Lower fibrinogen in the south compared to middle and north of Italy. The ADEP Group.

    Prospective studies have shown that high plasma levels of fibrinogen are independently associated with the risk of cardiovascular complications. In patients suffering from peripheral vascular disease (PVD) fibrinogen has been shown to be an independent predictor of cardiovascular disease but its determinants have never been examined in this clinical setting. DESIGN AND METHODS: Fibrinogen levels were related to clinical and laboratory variables in 2,111 patients suffering from PVD. We also analyzed whether there was a regional distribution of risk factors. RESULTS: The median values of fibrinogen was 312 mg/dL. The clinical variables examined did not differentiate patients with elevated or normal fibrinogen levels. In particular, patients with ankle/arm pressure ratio < 0.8 did not show a higher prevalence of fibrinogen > 312 mg/dL. Conversely, white blood cell (WBC) count and serum cholesterol levels were significantly associated with high fibrinogen levels (p < 0.0001). Multiple logistic regression analysis demonstrated that areas of Italy were differently associated with high plasma fibrinogen levels (p < 0.03): subjects in the north and middle of Italy having significantly higher values of fibrinogen than subjects in the south of Italy (p < 0.01). A similar regional distribution was observed for WBC count and serum cholesterol levels. INTERPRETATION AND CONCLUSIONS: The regional distribution of risk factors raises the question as to whether the already reported large variability of cardiovascular events so in PVD may be attributed to a non homogeneous distribution of risk factors

    Powertrain Modal Analysis for Defining the Requirements for a Vehicle Drivability Study

    The powertrain of a car plays a major role in establishing the vehicle’s offered comfort due to vibrations because it is the heaviest single component installed on the chassis; therefore, when oscillating, it transmits considerable forces to the chassis, inducing unwanted vibrations. For this reason, it is important to identify some associated properties with the powertrain suspension system that describe the performance of its rigid body dynamics. In this way, we could place constraints and requirements on these quantities in order to exclude all the configurations that cause intolerable levels of vibrations, and include all the others in the analysis for further evaluation. The definition of these requirements is critical: a poor setting of requirements excludes potentially good powertrain suspension setups and includes those ones with a drivability index that is too poor. In this paper, we identify a set of quantities that show correlation with the vibration performance of the powertrain setup. A method for testing the specificity of the requirements is also shown in order to evaluate which requirements perform best when it comes to filtering engine suspension setups that provide an acceptable level of vibrations

    Susceptibility mapping of shallow landslides inducing debris flows: a comparison of physics-based approaches

    The assessment of timing and potential locations of rainfallinduced shallow landslides through mathematical models represents a challenge for the assessment of landslide hazard, especially in cases with limited or not available data. In fact, modeling slope hydrological response and stability requires accurate estimates of unsaturated/saturated hydraulic and geotechnical properties of materials involved in landsliding, as well as climate and topography. Such aspect is relevant for the prediction of location and timing of landslide events, which is greatly needed to reduce their catastrophic effects in terms of economic losses and casualties. To such a scope, we present the comparison of results of two physics-based models applied to the assessment of susceptibility to shallow rainfall-induced landslides in Valtellina region (northern Italy). The analyses were carried out considering effects of availability, resolution and type of data concerning spatial distribution, thickness and properties of soils coverings. For such a scope, the Transient Rainfall Infiltration and Grid-Based Regional Slope-Stability (TRIGRS) and the Climatic Rainfall Hydrogeological Modeling Experiment (CHRyME) models were considered. The study emphasizes issues in performing distributed numerical slope stability modeling depending on the availability of spatially distributed soil properties which hamper the quality of physic-based models. Further analyses aimed at the probabilistic assessment of landslide spatial distribution, related to a specific value of rainfall threshold, can be considered as potentially applicable to multi-scale landslide hazard mapping and extendable to other similar mountainous frameworks

    Alternative uses of temporary work and new forms of work organisation

    The purpose of this study is to investigate the relationship between the use of temporary workers and the adoption of new forms of work organisation (NFWO) in production. This study aims to understand to what extent these two forms of human resources flexibility are synergic or mutually exclusive. In order to answer this main goal, we discuss different levels of temporary workers adoption in relation to different levels of use of NFWO, the level of integration of temporary workers within the overall production organisation and the joint and synergistic use of NFWO and temporary work. Evidence drawn from seven case studies in manufacturing plants in northern Italy is provided. Results highlight that, according to the characteristics of the production process, temporary workers and NFWO are not mutually exclusive, that temporary workers can be integrated with other workers in the shop floor, and that NFWO can also be adopted for temporary workers. In addition, NFWO has been proven to be a key enabler to integrate temporary workers within the organisation, thus showing an important synergistic effect between the two human resource flexibility practices

    Electrophysiological profile remodeling via selective suppression of voltage-gated currents by CLN1/PPT1 overexpression in human neuronal-like cells.

    CLN1 disease (OMIM #256730) is an inherited neurological disorder of early childhood with epileptic seizures and premature death. It is associated with mutations in CLN1 coding for Palmitoyl-Protein Thioesterase 1 (PPT1), a lysosomal enzyme which affects the recycling and degradation of lipid-modified (S-acylated) proteins by removing palmitate residues. Transcriptomic evidence from a neuronal-like cellular model derived from differentiated SH-SY5Y cells disclosed the potential negative roles of CLN1 overexpression, affecting the elongation of neuronal processes and the expression of selected proteins of the synaptic region. Bioinformatic inquiries of transcriptomic data pinpointed a dysregulated expression of several genes coding for proteins related to voltage-gated ion channels, including subunits of calcium and potassium channels (VGCC and VGKC). In SH-SY5Y cells overexpressing CLN1 (SH-CLN1 cells), the resting potential and the membrane conductance in the range of voltages close to the resting potential were not affected. However, patch-clamp recordings indicated a reduction of Ba2+ currents through VGCC of SH-CLN1 cells; Ca2+ imaging revealed reduced Ca2+ influx in the same cellular setting. The results of the biochemical and morphological investigations of CACNA2D2/α2δ-2, an accessory subunit of VGCC, were in accordance with the downregulation of the corresponding gene and consistent with the hypothesis that a lower number of functional channels may reach the plasma membrane. The combined use of 4-AP and NS-1643, two drugs with opposing effects on Kv11 and Kv12 subfamilies of VGKC coded by the KCNH gene family, provides evidence for reduced functional Kv12 channels in SH-CLN1 cells, consistent with transcriptomic data indicating the downregulation of KCNH4. The lack of compelling evidence supporting the palmitoylation of many ion channels subunits investigated in this study stimulates inquiries about the role of PPT1 in the trafficking of channels to the plasma membrane. Altogether, these results indicate a reduction of functional voltage-gated ion channels in response to CLN1/PPT1 overexpression in differentiated SH-SY5Y cells and provide new insights into the altered neuronal excitability which may underlie the severe epileptic phenotype of CLN1 disease. It remains to be shown if remodeling of such functional channels on plasma membrane can occur as a downstream effect of CLN1 disease.Peer reviewe

    The impact of country culture on the adoption of new forms of work organization

    Purpose – This paper aims at understanding the relationship between the adoption of new forms of work organizations (NFWOs) and measures of country impact, in terms of national culture and economic development. Design/methodology/approach – The adoption of NFWO practices is measured through data from the fourth edition of the International Manufacturing Strategy Survey, while Hofstede’s measures are adopted for national culture, and gross national income (GNI) per capita is used as an economic development variable. Multivariate linear regression is applied to investigate relationships, using company size as a control variable. A cluster analysis is utilized to identify groups of countries with similar cultural characteristics and to highlight different patterns of adoption of NFWO practices. Findings – The authors show that it is possible to explain different patterns in the adoption of NFWO practices when considering company size and cultural variables. GNI is instead only significant for some practices and does not always positively influence the adoption of NFWO. On the other hand, cultural variables are linked to all the practices, but there is no dominant dimension to explain higher or lower NFWO adoption. Research limitations/implications – Results are limited because only Hofstede’s cultural variables are used and manufacturing performance is not considered. Therefore, it is not possible to discriminate between more or less successful NFWO variations. Practical implications – This paper provides managers with insights on how to take into account cultural variables when transferring organizational models to different countries. Originality/value – This paper contributes to previous studies showing the importance of including several contextual variables, country impact in particular, in the study of operations management

    Elderly patient-centered rehabilitation after cardiac surgery

    The rate of over-70 year post-surgery patients referred to the Cardiac Rehabilitation Units is increasing. Strategies designed to encourage and facilitate participation in rehabilitation programs in the elderly should be developed. Aim of this paper is to present our elderly-centered program, specifically designed on patient’s needs and frailty, and its short- and medium-term results in 160 consecutive over-70 year patients, admitted in our Cardiac Rehabilitation Unit soon after cardiac surgery. The program was safe, well accepted by the patients, and effective in improving objective and subjective functional status

    Multi-parasite infection in an immigrant from Ghana: potential for new epidemic foci.

    Introduction: Imported parasitosis, which do not require an invertebrate vector, are extremely dangerous and can lead to the occurrence of disease in currently parasite free areas. In the present study we report a case of multi-parasitic infection in a young immigrant from Ghana to Italy caused by filaria, Schistosoma sp. and Strongyloides sp. Case presentation: A 27-year-old Ghanaian man attended the Hospital of Nuoro (Sardinia), Italy, at the end of August 2015, claiming pain to the kidney and hypertensive crisis; the patient presented with dyspnea and epistaxis, chronic itchy skin of the back, shoulders, arms and legs, anuria and high creatinine, metabolic acidosis and hypereosinophilic syndrome. Serological test for parasitic infections were done, and showed a marked positivity for filaria, Schistosoma sp. and Strongyloides sp. The patient started the treatment immediately with two doses per day of Bassado Antibiotic (tetracycline) for twenty days and then with a single dose of 3 mg of ivermectin that was repeated after 3 months. Conclusions: Immigrant patients from endemic areas who show clinical signs, such as a general itching on the back, shoulders and arms and legs, should have a thorough history in order to make early diagnosis and prevent further complications. Therefore, general practitioners and doctors in Europe and in other parasitosis non-endemic countries, should consider to test for parasites in any immigrant from endemic countries to aid in establishing the final diagnosis and prevent further complications
