    Exploring the moral imagination through relational pedagogies in pre-service teacher ethics

    Whilst there is significant research examining the pedagogical development of pre-service teachers’ knowledge and skills after their internship experience, there has been little examination of their experience of ethical tension and little investigation into ways to further enhance pre-service teachers’ ethical reasoning. This paper documents some of the ways that pre-service teachers reason about ethically charged situations. It aims to extend conceptions of the moral imagination and its place in the teaching of ethics to pre-service teachers by discussing findings from a teaching project pilot study designed to investigate the ways in which pre-service teachers experience and respond to ethical tensions. Whilst recognising common difficulties in responding to ethically charged situations, our analysis utilises pre-service teacher dialogue in the form of an assessment strategy based on a ‘community of inquiry’ model and examines the ways in which pre-service teachers utilise the moral imagination to reason through ethical issues. This paper indicates some ways professional development could provide opportunities more aligned with pre-service teachers’ learning needs in the development of their ethical reasoning, and argues that the moral imagination is a fundamental feature of ethical practice and decision-making

    Delayed-onset endophthalmitis associated with corneal suture infections

    BACKGROUND: The purpose of the current study was to report the microbiology, risk factors, and treatment outcomes in patients with delayed-onset endophthalmitis associated with corneal suture infections. For this retrospective consecutive case series, a search of the ocular microbiology department database was performed to identify all patients with positive corneal and intraocular cultures (anterior chamber and/or vitreous) between 01 January 1995 and 01 January 2010. A subset of patients with a history of corneal suture infections and delayed-onset endophthalmitis was identified. RESULTS: Over the 15-year period of the study, 68 patients were identified to have both positive corneal and intraocular cultures. Among them, six patients were identified to have a culture-proven, delayed-onset endophthalmitis that developed from a culture-positive corneal suture infection. All of the patients in the current study were using topical corticosteroids at the time of diagnosis. In four of six patients, there was documented manipulation of a suture before the development of endophthalmitis. Streptoccocus was identified as the causative organism in five of six patients in the current study. All of the Streptoccocus isolates were sensitive to vancomycin. The single case of Serratia marcescens endophthalmitis was sensitive to amikacin, ceftazidime, ciprofloxacin, gentamicin, and tobramycin. Treatment modalities varied and were guided by the attending ophthalmologist depending upon clinical presentation. One patient with severe Streptococcus pyogenes keratitis and endophthalmitis underwent a primary enucleation after developing a wound dehiscence. Of the remaining five patients, all received topical and intravitreal antibiotics. Therapeutic penetrating keratoplasty was performed in three patients. Pars plana vitrectomy was performed in two patients. Visual acuity outcomes ranged from 20/150 to no light perception. CONCLUSIONS: In the current study, Streptococcus was isolated in nearly all patients with delayed-onset endophthalmitis associated with corneal suture infections. Topical steroid use and suture manipulation were identified as associated factors for developing endophthalmitis. Visual acuity outcomes were poor despite the prompt recognition of endophthalmitis and appropriate antibiotic therapy

    Diatom responses and geochemical feedbacks to environmental changes at Lake Rauchuagytgyn (Far East Russian Arctic)

    This study is based on multiproxy data gained from a 14C-dated 6.5 m long sediment core and a 210Pb-dated 23 cm short core retrieved from Lake Rauchuagytgyn in Chukotka, Arctic Russia. Our main objectives are to reconstruct the environmental history and ecological development of the lake during the last 29 kyr and to investigate the main drivers behind bioproduction shifts. The methods comprise age-modeling, accumulation rate estimation, and light microscope diatom species analysis of 74 samples, as well as organic carbon, nitrogen, and mercury analysis. Diatoms have appeared in the lake since 21.8 ka cal BP and are dominated by planktonic Lindavia ocellata and L. cyclopuncta. Around the Pleistocene–Holocene boundary, other taxa including planktonic Aulacoseira, benthic fragilarioid (Staurosira), and achnanthoid species increase in their abundance. There is strong correlation between variations of diatom valve accumulation rates (DARs; mean 176.1×109 valves m2 a1), organic carbon accumulation rates (OCARs; mean 4.6 g m−2 a−1), and mercury accumulation rates (HgARs; mean 63.4 µg m−2 a−1). We discuss the environmental forcings behind shifts in diatom species and find moderate responses of key taxa to the cold glacial period, postglacial warming, the Younger Dryas, and the Holocene Thermal Maximum. The short-core data likely suggest recent change of the diatom community at the beginning of the 20th century related to human-induced warming but only little evidence of atmospheric deposition of contaminants. Significant correlation between DAR and OCAR in the Holocene interglacial indicates within-lake bioproduction represents bulk organic carbon deposited in the lake sediment. During both glacial and interglacial episodes HgAR is mainly bound to organic matter in the lake associated with biochemical substrate conditions. There were only ambiguous signs of increased HgAR during the industrialization period. We conclude that if increased short-term emissions are neglected, pristine Arctic lake systems can potentially serve as long-term CO2 and Hg sinks during warm climate episodes driven by insolation-enhanced within-lake primary productivity. Maintaining intact natural lake ecosystems should therefore be of interest to future environmental policy

    Folic acid supplementation before and during pregnancy in the Newborn Epigenetics STudy (NEST)

    <p>Abstract</p> <p>Background</p> <p>Folic acid (FA) added to foods during fortification is 70-85% bioavailable compared to 50% of folate occurring naturally in foods. Thus, if FA supplements also are taken during pregnancy, both mother and fetus can be exposed to FA exceeding the Institute of Medicine's recommended tolerable upper limit (TUL) of 1,000 micrograms per day (μg/d) for adult pregnant women. The primary objective is to estimate the proportion of women taking folic acid (FA) doses exceeding the TUL before and during pregnancy, and to identify correlates of high FA use.</p> <p>Methods</p> <p>During 2005-2008, pre-pregnancy and pregnancy-related data on dietary supplementation were obtained by interviewing 539 pregnant women enrolled at two obstetrics-care facilities in Durham County, North Carolina.</p> <p>Results</p> <p>Before pregnancy, 51% of women reported FA supplementation and 66% reported this supplementation during pregnancy. Before pregnancy, 11.9% (95% CI = 9.2%-14.6%) of women reported supplementation with FA doses above the TUL of 1,000 μg/day, and a similar proportion reported this intake prenatally. Before pregnancy, Caucasian women were more likely to take FA doses above the TUL (OR = 2.99; 95% = 1.28-7.00), compared to African American women, while women with chronic conditions were less likely to take FA doses above the TUL (OR = 0.48; 95%CI = 0.21-0.97). Compared to African American women, Caucasian women were also more likely to report FA intake in doses exceeding the TUL during pregnancy (OR = 5.09; 95%CI = 2.07-12.49).</p> <p>Conclusions</p> <p>Fifty-one percent of women reported some FA intake before and 66% during pregnancy, respectively, and more than one in ten women took FA supplements in doses that exceeded the TUL. Caucasian women were more likely to report high FA intake. A study is ongoing to identify possible genetic and non-genotoxic effects of these high doses.</p

    Unexpectedly high burden of rotavirus gastroenteritis in very young infants

    <p>Abstract</p> <p>Background</p> <p>The highest incidence of rotavirus gastroenteritis has generally been reported in children 6-24 months of age. Young infants are thought to be partially protected by maternal antibodies acquired transplacentally or via breast milk. The purpose of our study was to assess the age distribution of children with confirmed community-acquired rotavirus gastroenteritis presenting to an urban referral hospital.</p> <p>Methods</p> <p>Children presenting to The Children's Hospital of Philadelphia with acute gastroenteritis have been monitored for the presence of rotavirus antigen in the stool by ELISA (followed by genotyping if ELISA-positive) since the 1994-95 epidemic season.</p> <p>Results</p> <p>Over the last 12 rotavirus seasons prior to the introduction of the pentavalent rotavirus vaccine in 2006, stool specimens from 1646 patients tested positive for community-acquired rotavirus infection. Gender or age was not recorded in 6 and 5 cases, respectively. Overall, 58% of the cases occurred in boys. G1 was the predominant VP7 serotype, accounting for 72% of cases. The median (IQR) age was 11 (5-21) months. A total of 790 (48%) cases occurred in children outside the commonly quoted peak age range, with 27% in infants <6 months of age and 21% in children >24 months of age. A total of 220 (13%) cases occurred during the first 3 months of life, and the highest number of episodes per month of age [97 (6%)] was observed during the second month of life.</p> <p>Conclusions</p> <p>The incidence of community-acquired rotavirus gastroenteritis monitored over 12 seasons in the prevaccine era at a major university hospital was nearly constant for each month of age during the first year of life, revealing an unexpectedly high incidence of symptomatic rotavirus disease in infants <3 months old. A sizeable fraction of cases occurred in children too young to have been vaccinated according to current recommendations.</p
