    Serotonin transporter in the temporal lobe, hippocampus and amygdala in SUDEP

    Several lines of evidence link deficient serotonin function and SUDEP. Chronic treatment with serotonin reuptake inhibitors (SRIs) reduces ictal central apnoea, a risk factor for SUDEP. Reduced medullary serotonergic neurones, modulators of respiration in response to hypercapnia, were reported in a SUDEP post-mortem series. The amygdala and hippocampus have high serotonergic innervation and are functionally implicated in seizure-related respiratory dysregulation. We explored serotonergic networks in mesial temporal lobe structures in a surgical and post-mortem epilepsy series in relation to SUDEP risk. We stratified 75 temporal lobe epilepsy patients with hippocampal sclerosis (TLE/HS) into high (16), medium (11) and low risk (48) groups for SUDEP based on generalised seizure frequency. We also included the amygdala in 35 post-mortem cases, including SUDEP (17), epilepsy controls (10) and non-epilepsy controls (8). The immunohistochemistry labelling index (LI) and axonal length (AL) of serotonin transporter (SERT)-positive axons were quantified in 13 regions of interest with image analysis. SERT LI was highest in amygdala and subiculum regions. In the surgical series, higher SERT LI was observed in high risk than low risk cases in the dentate gyrus, CA1 and subiculum (p<0.05). In the post-mortem cases higher SERT LI and AL was observed in the basal and accessory basal nuclei of the amygdala and peri-amygdala cortex in SUDEP compared to epilepsy controls (p<0.05). Patients on SRI showed higher SERT in the dentate gyrus (p<0.005) and CA4 (p<0.05) but there was no difference in patients with or without a psychiatric history. Higher SERT in hippocampal subfields in TLE/HS cases with SUDEP risk factors and higher amygdala SERT in post-mortem SUDEP cases than epilepsy controls supports a role for altered serotonergic networks involving limbic regions in SUDEP. This may be of functional relevance through reduced 5-HT availability

    The clinical utility of lung clearance index in early cystic fibrosis lung disease is not impacted by the number of multiple-breath washout trials

    © ERS 2018. The lung clearance index (LCI) from the multiple-breath washout (MBW) test is a promising surveillance tool for pre-school children with cystic fibrosis (CF). Current guidelines for MBW testing recommend that three acceptable trials are required. However, success rates to achieve these criteria are low in children aged &lt; 7 years and feasibility may improve with modified pre-school criteria that accepts tests with two acceptable trials. This study aimed to determine if relationships between LCI and clinical outcomes of CF lung disease differ when only two acceptable MBW trials are assessed. Healthy children and children with CF aged 3–6 years were recruited for MBW testing. Children with CF also underwent bronchoalveolar lavage fluid collection and a chest computed tomography scan. MBW feasibility increased from 46% to 75% when tests with two trials were deemed acceptable compared with tests where three acceptable trials were required. Relationships between MBW outcomes and markers of pulmonary inflammation, infection and structural lung disease were not different between tests with three acceptable trials compared with tests with two acceptable trials. This study indicates that pre-school MBW data from two acceptable trials may provide sufficient information on ventilation distribution if three acceptable trials are not possible

    Monitoring Leachate Migration in Compacted Soil Using Digital Image Technique

    As leachate has been a source of groundwater contamination worldwide, this paper examines the phenomenon of leachate migration on different gradations of compacted laterite soil used as sanitary landfill liners. Three different soil gradations (30%, 40% and 50% with respect to fines content) used in this study were compacted in circular acrylic columns to provide a clear visualization of leachate migration into the soils. Digital image technique was used in capturing photos at successive time intervals to monitor the leachate migration. The captured digital images were fed into Matlab and converted into hue-saturation-intensity (HSI) format. Surfer software then read the HSI and generated 2D contour plots. The results of the experiments showed that the leachate moves downward faster in the soil gradation with the least fines content. Hydraulic conductivity values decrease with increase in time duration and equally with increase in fines content. The hydraulic conductivities of the leachate for 30%, 40% and 50% fines were 3.64×10-9m/s, 2.40×10-9m/s, and 1.24×10-9m/s respectively. This reveals that for tropical laterite soils, gradation containing 50% fines content provides better hydraulic conductivity. The use of noninvasive digital image technique can enable designers/engineers to monitor and visualize the leachate migration in compacted soils in waste containment application systems

    The Quantized O(1,2)/O(2)×Z2O(1,2)/O(2)\times Z_2 Sigma Model Has No Continuum Limit in Four Dimensions. II. Lattice Simulation

    A lattice formulation of the O(1,2)/O(2)×Z2O(1,2)/O(2)\times Z_2 sigma model is developed, based on the continuum theory presented in the preceding paper. Special attention is given to choosing a lattice action (the ``geodesic'' action) that is appropriate for fields having noncompact curved configuration spaces. A consistent continuum limit of the model exists only if the renormalized scale constant βR\beta_R vanishes for some value of the bare scale constant~β\beta. The geodesic action has a special form that allows direct access to the small-β\beta limit. In this limit half of the degrees of freedom can be integrated out exactly. The remaining degrees of freedom are those of a compact model having a β\beta-independent action which is noteworthy in being unbounded from below yet yielding integrable averages. Both the exact action and the β\beta-independent action are used to obtain βR\beta_R from Monte Carlo computations of field-field averages (2-point functions) and current-current averages. Many consistency cross-checks are performed. It is found that there is no value of β\beta for which βR\beta_R vanishes. This means that as the lattice cutoff is removed the theory becomes that of a pair of massless free fields. Because these fields have neither the geometry nor the symmetries of the original model we conclude that the O(1,2)/O(2)×Z2O(1,2)/O(2)\times Z_2 model has no continuum limit.Comment: 32 pages, 7 postscript figures, UTREL 92-0

    Are people who participate in cultural activities more satisfied with life?

    The influence of various aspects of life on wellbeing has been extensively researched. However, despite little empirical evidence, participation in leisure activities has been assumed to increase subjective wellbeing. Leisure is important because it is more under personal control than other sources of life satisfaction. This study asked whether people who participate in cultural leisure activities have higher life satisfaction than people who do not, if different types of leisure have the same influence on life satisfaction and if satisfaction is dependent on the frequency of participation or the number of activities undertaken. It used data from UKHLS Survey to establish associations between type, number and frequency of participation in leisure activities and life satisfaction. Results showed an independent and positive association of participation in sport, heritage and active-creative leisure activities and life satisfaction but not for participation in popular entertainment, theatre hobbies and museum/galleries. The association of reading hobbies and sedentary-creative activities and life satisfaction was negative. High life satisfaction was associated with engaging in a number of different activities rather than the frequency of participation in each of them. The results have implications for policy makers and leisure services providers, in particular those associated with heritage recreation. Subjective wellbeing measures, such as life satisfaction, and not economic measures alone should be considered in the evaluation of services. The promotion of leisure activities which are active and promote social interaction should be considered in programmes aimed at improving the quality of life

    Assessment of material loss of retrieved magnetically controlled implants for limb lengthening

    PURPOSE: We aimed to understand wear from the telescopic component of PRECICE nails, which are used for distraction osteogenesis of the femur or tibia. We also aimed to identify any correlation between implant performance and patient factors. METHODS: This retrieval study involved 11 magnetically controlled intramedullary nails from nine patients who had achieved the targeted leg length. All the nails were assessed macroscopically and microscopically for wear. All implants were radiographed to assess the internal mechanism. A Talyrond 365 (Taylor Hobson, Leicester, UK) roundness measuring machine was used to generate three-dimensional surface maps of the telescopic components to allow for measurement of wear. RESULTS: Visual assessment of all the nails showed evidence of wear from the telescopic component. The radiographs revealed that all the nails had intact internal mechanism and no evidence of fractured pins. The roundness measuring machine showed that the quantity of wear was lowest in the latest design of the PRECICE nail. There was no significant correlation between wear and the two patient factors (duration of the lengthening phase, the time of implantation) included in this study. CONCLUSION: This study is the first to investigate the performance of the three different designs of the PRECICE system with a focus on wear. We found that the latest design had the best implant performance. We are confident of the continued success of the PRECICE system and reassure surgeons and patients that they are unlikely to encounter problems with the implant related to wear

    In the nonextensive Tsallis scenario, Page's conjecture for the average entropy of a subsystem[Phys. Rev. Lett. {\bf 71}, 1291(1993)] as well as its demonstration are generalized, i.e., when a pure quantum system, whose Hilbert space dimension is mnmn, is considered, the average Tsallis entropy of an mm-dimensional subsystem is obtained.
