    Disordered Eating in a Digital Age: Narratives of Withdrawal From Pro-Ana/Mia Forums by Regular Site Users

    Background & Aims: Studies estimate that over 500 live Pro-Ana websites are in existence (Hansen, 2008). Broadly speaking they advocate that eating ‘disorders’’ are a ‘life-style’ choice rather than a mental health problem requiring treatment (Bardone‐Cone & Cass, 2007). To date, content analyses (Borzekowski, Schenk, Wilson, & Peebles, 2010; Harshbarger, Ahlers‐Schmidt, Mayans, Mayans, & Hawkins, 2009), covert observations (Brotsky & Giles, 2007) and experimental designs (Bardone‐Cone & Cass, 2007; Jett, LaPorte, & Wanchisn, 2010) have been used to investigate the impact of the Pro-Ana movement on individuals. Findings suggest that the sites can maintain and worsen eating difficulties and pose a significant obstacle to recovery. Yet remarkably, no study has investigated accounts of Pro-Ana withdrawal and its associated impact. This study sought to address the literature gap. Methodology: Six women formed the sample (two British, four American). All identified themselves as previously using Pro-Ana sites regularly and also reported experiencing eating difficulties. Using semi structured interviews; Pro-Ana experiences were explored, focusing particularly on site disengagement. Narrative inquiry enabled the content, structure, context and performative aspect of all accounts to be considered. Analysis & Findings: A brief narrative summary was presented for each participant, along with the emerging storylines across accounts. The strongest storyline of coming to Pro-Ana was aspirations for further weight loss and a search for a connection. The strongest storyline of withdrawal involved a quest for recovery, though accounts varied widely in agency surrounding this act and for a small proportion disengagement was actually a sign of things getting worse. Notably, storylines of strengthening different identity aspects and connecting with different social support systems featured heavily in most recovery accounts. The findings are discussed in relation to their clinical implications, the strengths and weaknesses of the project and direction of further research

    Cutting Ties with Pro-Ana: A Narrative Inquiry Concerning the Experiences of Pro-Ana Disengagement from Six Former Site Users.

    Websites advocating the benefits of eating disorders (“Pro-Ana”) tend to reinforce and maintain restrictive eating and purging behaviors. Yet remarkably, no study has explored individual accounts of disengagement from these sites and the associated meanings. Using narrative inquiry, this study sought to address this gap. From the interviews of six women, two overarching storylines emerged. The first closely tied disengagement to recovery with varying positions of personal agency claimed: this ranged from enforced and unwelcomed breaks that ignited change, to a personal choice that became viable through the development of alternative social and personal identities. A strong counternarrative to “disengagement as recovery” also emerged. Here, disengagement from Pro-Ana was storied alongside a need to retain an ED lifestyle. With “recovery” being just one reason for withdrawal from Pro-Ana sites, clinicians must remain curious about the meanings individuals ascribe to this act, without assuming it represents a step toward recovery.Peer reviewedFinal Accepted Versio

    Internet-delivered cognitive behavior therapy for anxiety and insomnia in a higher education context

Background and Objectives: Anxiety and insomnia can be treated with internet-delivered Cognitive Behavioral Therapy (iCBT). iCBT may be well-suited to students who are known to be poor help-seekers and suffer these symptoms. iCBT can offer easy access to treatment and increase service availability. The aim of this study was to evaluate the efficacy of anxiety and insomnia iCBT programs in students. Design: A randomized, controlled study. Methods: Students were randomly allocated to intervention (“Anxiety Relief”: n = 43; “Insomnia Relief”: n = 48; control: n = 47). Interventions lasted six weeks. Outcome measures were the State-Trait Anxiety Inventory and the Pittsburgh Sleep Quality Index. Results: Significant within-group reductions in anxiety (t(31) = 2.00, p =.03) with moderate between-groups (compared to control) effect size (d =.64) and increases in sleep quality (t(31) = 3.46, p =.002) with a moderate between-groups effect size (d =.55) were found for completers of the anxiety program from pre-to post-intervention. Significant within-group increases in sleep quality were found for completers of the insomnia program from pre-to post-intervention (t(35) = 4.28, p >.001) with a moderate between-groups effect size (d =.51). Conclusions: Findings support the use of iCBT for anxiety and insomnia in students, and indicate that further research is needed

    The impact of the initial COVID-19 outbreak on young adults’ mental health: a longitudinal study of risk and resilience factors

    Few studies assessing the effects of COVID-19 on mental health include prospective markers of risk and resilience necessary to understand and mitigate the combined impacts of the pandemic, lockdowns, and other societal responses. This population-based study of young adults includes individuals from the Neuroscience in Psychiatry Network (n = 2403) recruited from English primary care services and schools in 2012–2013 when aged 14–24. Participants were followed up three times thereafter, most recently during the initial outbreak of the COVID-19 outbreak when they were aged between 19 and 34. Repeated measures of psychological distress (K6) and mental wellbeing (SWEMWBS) were supplemented at the latest assessment by clinical measures of depression (PHQ-9) and anxiety (GAD-7). A total of 1000 participants, 42% of the original cohort, returned to take part in the COVID-19 follow-up; 737 completed all four assessments [mean age (SD), 25.6 (3.2) years; 65.4% female; 79.1% White]. Our findings show that the pandemic led to pronounced deviations from existing mental health-related trajectories compared to expected levels over approximately seven years. About three-in-ten young adults reported clinically significant depression (28.8%) or anxiety (27.6%) under current NHS guidelines; two-in-ten met clinical cut-offs for both. About 9% reported levels of psychological distress likely to be associated with serious functional impairments that substantially interfere with major life activities; an increase by 3% compared to pre-pandemic levels. Deviations from personal trajectories were not necessarily restricted to conventional risk factors; however, individuals with pre-existing health conditions suffered disproportionately during the initial outbreak of the COVID-19 pandemic. Resilience factors known to support mental health, particularly in response to adverse events, were at best mildly protective of individual psychological responses to the pandemic. Our findings underline the importance of monitoring the long-term effects of the ongoing pandemic on young adults’ mental health, an age group at particular risk for the emergence of psychopathologies. Our findings further suggest that maintaining access to mental health care services during future waves, or potential new pandemics, is particularly crucial for those with pre-existing health conditions. Even though resilience factors known to support mental health were only mildly protective during the initial outbreak of the COVID-19 pandemic, it remains to be seen whether these factors facilitate mental health in the long term

    An expanding manifold in transmodal regions characterizes adolescent reconfiguration of structural connectome organization

Here, we assessed structural connectome development in a large longitudinal sample ranging from childhood to young adulthood. By projecting high-dimensional connectomes into compact manifold spaces, we identified a marked expansion of structural connectomes, with strongest effects in transmodal regions during adolescence. Findings reflected increased within-module connectivity together with increased segregation, indicating increasing differentiation of higher-order association networks from the rest of the brain. Projection of subcortico-cortical connectivity patterns into these manifolds showed parallel alterations in pathways centered on the caudate and thalamus. Connectome findings were contextualized via spatial transcriptome association analysis, highlighting genes enriched in cortex, thalamus, and striatum. Statistical learning of cortical and subcortical manifold features at baseline and their maturational change predicted measures of intelligence at follow-up. Our findings demonstrate that connectome manifold learning can bridge the conceptual and empirical gaps between macroscale network reconfigurations, microscale processes, and cognitive outcomes in adolescent development
