    A numerical study on active control for tiltrotor whirl flutter stability augmentation

    The use of active control to augment whirl flutter stability of tiltrotor aircraft is studied by means of a multibody simulation. The numerical model is based on a 1/5 scale semi-span aeroelastic wind tunnel model of a generic tiltrotor concept and possesses a gimballed, stiff-in-plane rotor that is windmilling. A single-input single-output controller and two types of multi-input multi-output algorithms, Linear Quadratic Gaussian Control and Generalized Predictive Control, are studied. They are using measured wing deflections in order to calculate appropriate swashplate input. Results on the closed-loop behavior of three wing and two gimbal natural modes are given. Robustness analyses with respect to major parameters like wing natural frequencies or structural damping are also briefly discussed. The rotor shear force is shown in the uncontrolled condition and in presence of a controller in order to illustrate the whirl flutter mechanism. The single-input single-output controller yielded substantial gain in stability and turned out to be most suitable for industrial application, whereas the Linear Quadratic Gaussian Regulator yielded even higher damping and still had good robustness characteristics

    Outcomes, controversies and gastric volume after laparoscopic sleeve gastrectomy in the treatment of obesity

    AbstractBackgroundLaparoscopic sleeve gastrectomy is a surgical procedure for the treatment of morbid obesity. However, there are still controversies regarding its efficiency in terms of weight reduction and incidence of complications. In this prospective study, the experience is presented of a referral centre for the treatment of morbid obesity with laparoscopic sleeve gastrectomy.Material and methodsA prospective study on 73 patients subjected to laparoscopic sleeve gastrectomy from February 2009 to September 2013. Patients were followed-up for a period of 12 months, evaluating the development of complications, reduction of gastric volume, and the weight loss associated with the surgery, as well as their impact on the improvement of comorbidities present at beginning of the study.ResultsThere was a statistically a significantly reduction between the preoperative body mass index (BMI) and the BMI at 12 months after laparoscopic sleeve gastrectomy (p<0.001), despite there being an increase in the gastric volume during follow-up, measured at one month and 12 months after surgery (p<0.001). Five patients (6.85%) had complications, with none of them serious and with no deaths in the whole series.ConclusionsLaparoscopic sleeve gastrectomy is a safe and effective technique for the treatment of morbid obesity. Its use is associated with a significant reduction in the presence of comorbidities associated with obesity. Multicentre studies with a longer period of monitoring are required to confirm the efficacy and safety of this surgical technique

    A case of a 29-year-old female, mixed race patient with a history of hypothyroidism and conization due to cervical intraepithelial neoplasia grade II was reported, who performed three pneumothorax within 2 years, being at rest and during the second day of the menstrual cycle. She is admitted to the Pulmonology ward of the General Hospital “Dr. Juan Bruno Zayas Alfonso”, with the probable diagnosis of pneumothorax secondary to pleural endometriosis. He underwent videotoracoscopy and laparoscopy with sampling for histological study. The definitive diagnosis of catamenial pneumothorax was established, chemical pleurodesis was made with talc, hormonal treatment and non-steroidal anti-inflammatory drugs (NSAIDs), which presented a satisfactory evolution.Se reportó un caso de paciente femenina, mestiza de 29 años de edad con antecedentes de hipotiroidismo y conización por neoplasia intraepitelial cervical grado II, que realizó tres neumotórax en un plazo de 2 años, estando en reposo y durante el segundo día del ciclo menstrual. Es ingresada en la sala de Neumología del Hospital General “Dr. Juan Bruno Zayas Alfonso”, con el diagnóstico probable de neumotórax secundario a endometriosis pleural. Se le realizó videotoracoscopia y laparoscopia con toma de muestra para estudio histológico. Se estableció el diagnóstico definitivo de neumotórax catamenial, se le hizo pleurodesis química con talco, tratamiento hormonal y antiinflamatorios no esteroideos (AINES), con lo que presentó una evolución satisfactoria.Foi relatado o caso de uma paciente do sexo feminino, 29 anos, de raça mista, com história de hipotireoidismo e conização por neoplasia intraepitelial cervical grau II, que realizou três pneumotórax em 2 anos, estando em repouso e durante o segundo dia do ciclo menstrual. Ela é internada na enfermaria de Pneumologia do Hospital Geral “Dr. Juan Bruno Zayas Alfonso”, com provável diagnóstico de pneumotórax secundário à endometriose pleural. Foi submetido a videotoracoscopia e laparoscopia com amostragem para estudo histológico.     The article proposes to analyze the implications of a recent trend in the production of intermediate cities in Uruguay, the gated communities. This modality of land use, which is not new in the metropolitan area, begins to have a presence in conglomerates that had remained outside this phenomenon. Specifically, an interdisciplinary look at the process that has been taking place in the city of Salto, on the northwestern coast of the country, will be presented as a result of the proposal for a mega-enterprise called “El Milagro Project”. This private initiative, of an exogenous nature and with traits of urban extractivism, imposes conditions that imply the revision of the local land use plan, which, if it occurs, would have important consequences on spatial, social, economic and environmental dynamics. Based on secondary information from official sources, it will seek to demonstrate the risks that the project entails for territorial sustainability and for the achievement of a democratic and integrated city, in accordance with the principles that govern the country's land use planning policy. &nbsp;El artículo propone analizar las implicaciones de una tendencia reciente en la producción de las ciudades secundarias de Uruguay, las urbanizaciones cerradas. Dicha modalidad de uso del suelo, la cual no es nueva en el área metropolitana, comienza a tener presencia en conglomerados que se habían mantenido ajenos a ese fenómeno. Específicamente se presentará una mirada interdisciplinaria al proceso que se viene dando en la ciudad de Salto, en el litoral noroeste del país, como resultado de la propuesta de un mega emprendimiento denominado “Proyecto El Milagro”. Esta iniciativa privada, de carácter exógeno y con rasgos de extractivismo urbano, impone condiciones que implican la revisión del plan local de ordenamiento territorial lo que, de producirse, tendría importantes consecuencias sobre las dinámicas espaciales, sociales, económicas y ambientales. Sobre la base de información secundaria de fuentes oficiales, se buscará evidenciar los riesgos que el proyecto conlleva para la sostenibilidad territorial y para el logro de una ciudad democrática e integrada, de acuerdo a los principios que rigen la política de ordenamiento territorial del país. &nbsp

    The infections are still a scourge for children, which is evidenced in the second basic cause of infantile mortality in Cuba. It was performed an analytic  research of cases an controls with the objective to identify the influence of risk factors in preterm newborns with precocious newborn infection, hospitalized in the Neonatology Closed Service at Carlos Manuel Cespedes Hospital in Bayamo, Granma, since January 1rst to December 31, 2010. It was performed a univaried analysis, the presence of vaginal (OR 3.07, p=0.001) and urinary sepsis (OR 10.54, p=0.000), the breaking of membranes with more than 18 hours (OR 10.54, p=0.000), the clinic corioamnyonitis was the risk factor because its value was OR 9.70, p=0.000). Concluding, all researched factors influenced in the presence of the precocious neonatal infection. Las infecciones continúan siendo un azote en la infancia, lo cual se evidencia en que representan la segunda causa básica de mortalidad infantil en Cuba. Se realizó un estudio analítico de casos y controles, con el objetivo de identificar la influencia de factores de riesgo en recién nacidos pretérminos con el diagnóstico de  infección  neonatal precoz, ingresados en el Servicio de Neonatología Cerrado, del Hospital Carlos Manuel de Céspedes, Bayamo, Granma, desde el 1 de enero al 31 de diciembre del 2010. Se realizó un análisis univariado, la presencia de sepsis vaginal (OR 3,07, p=0.001) y urinaria (OR 3,02, p=0.0000), la rotura prematura de membranas de más de 18 horas (OR  10,54, p=0.0000),  la corioamnionitis clínica fue factor de riesgo pues se comportó con un valor  de OR 9,70, p=0.0000). Concluimos que todos los factores investigados, influyeron en la aparición de la infección  neonatal precoz

    It is a 53 years old female patient, white, with a personal history of bronchial asthma, high blood pressure, diabetes mellitus type 2, alcoholism, alcoholic polyneuropathy, alcoholic liver disease, tuberculosis contact, multiple admissions for dematological lesions and respiratory sepsis, pathological hip fracture and disseminated osteolytic lesions. She was admitted with diagnosis of deep venous thrombosis, she evolved unfavorably with sepsis at various levels and progressive deterioration and died due to failure and multiple organ damage. The anatomopathological findings showed a systemic miliary tuberculosis that caused multiorgan damage and failure. Mycobacterium tuberculosis is responsible for most of the cases of tuberculosis, several risk factors favor this disease, most of the patients with the human immunodeficiency virus suffer from it. Granuloma is the histological finding that distinguishes it.Se trata de una paciente femenina de 53 años, blanca, con antecedentes patológicos personales de asma bronquial, hipertensión arterial, diabetes mellitus tipo 2, alcoholismo, polineuropatía alcohólica, hepatopatía alcohólica, contacto de tuberculosis, múltiples ingresos por lesiones dematológicas y sepsis respiratorias, fractura patológica de cadera y lesiones osteolíticas diseminadas. Ingresó con diagnóstico de trombosis venosa profunda, evolucionó desfavorablemente con sepsis a varios niveles y deterioro progresivo y falleció por fallo y daño de múltiples órganos. Los hallazgos anatomopatológicos mostraron una tuberculosis miliar sistémica que provocó el daño y el fallo multiorgánicos. El Mycobacterium tuberculosis es responsable de la mayor parte de los casos de tuberculosis, varios factores de riesgo favorecen esa enfermedad, la mayor parte de los pacientes con el virus de inmunodeficiencia humana la padecen. El granuloma es el hallazgo histológico que la distingue
