    Terapia por pressão negativa (vácuo) no preparo do leito da ferida em pacientes diabéticos: série de casos

    CONTEXT: Complications from diabetes mellitus affecting the lower limbs occur in 40 to 70% of such patients. Neuropathy is the main cause of ulceration and may be associated with vascular impairment. The wound evolves with necrosis and infection, and if not properly treated, amputation may be the end result. Surgical treatment is preferred in complex wounds without spontaneous healing. After debridement of the necrotic tissue, the wound bed needs to be prepared to receive a transplant of either a graft or a flap. Dressings can be used to prepare the wound bed, but this usually leads to longer duration of hospitalization. Negative pressure using a vacuum system has been proposed for speeding up the treatment. This paper had the objective of analyzing the effects of this therapy on wound bed preparation among diabetic patients. CASE SERIES: Eighty-four diabetic patients with wounds in their lower limbs were studied. A commercially available vacuum system was used for all patients after adequate debridement of necrotic tissues. For 65 patients, skin grafts completed the treatment and for the other 19, skin flaps were used. Wound bed preparation was achieved over an average time of 7.51 days for 65 patients and 10 days for 12 patients, and in only one case was not achieved. CONCLUSIONS: This experience suggests that negative pressure therapy may have an important role in wound bed preparation and as part of the treatment for wounds in the lower limbs of diabetic patients.CONTEXTO: Complicações do diabetes mellitus que afetam os membros inferiores ocorrem em 40 a 70% dos pacientes. A neuropatia é a principal causa de ulceração e pode estar associada com problemas vasculares. A ferida evolui com necrose e infecção, e se não for corretamente tratada poderá terminar em amputação. O tratamento cirúrgico é preferido em feridas complexas, quando não há cicatrização espontânea. Após desbridamento cirúrgico do tecido necrótico do leito da ferida este precisa ser preparado para receber um transplante, seja um enxerto ou um retalho. Curativos podem ser usados para o preparo do leito da ferida, mas frequentemente levam a um longo tempo de hospitalização. A pressão negativa usada através de um sistema vácuo foi proposta para acelerar o tempo de tratamento. O presente trabalho teve como finalidade analisar os efeitos desta terapia no preparo do leito de feridas em pacientes diabéticos. SÉRIE DE CASOS: Oitenta e quatro pacientes diabéticos com feridas em membros inferiores foram estudados. Um sistema vácuo de uso comercial foi utilizado em todos os pacientes após adequado desbridamento de tecidos necróticos. Em 65 pacientes enxertos de pele completaram o tratamento e em outros 19 retalhos cutâneos. O preparo do leito da ferida foi conseguido, em média, em 7,51 dias em 65 pacientes, em 10 dias para 12 pacientes e em somente um caso não foi efetivo. CONCLUSÃO: A experiência sugere que a terapia por pressão negativa possa ter um papel importante no preparo do leito e como parte do tratamento de feridas nos membros inferiores de pacientes diabéticos

    The role of potassium channels in the endothelial dysfunction induced by periodontitis

    Objective: Periodontitis is associated with endothelial dysfunction, which is clinically characterized by a reduction in endothelium-dependent relaxation. However, we have previously shown that impairment in endothelium-dependent relaxation is transient. Therefore, we evaluated which mediators are involved in endothelium-dependent relaxation recovery. Material and methods: Rats were subjected to ligature-induced experimental periodontitis. Twenty-one days after the procedure, the animals were prepared for blood pressure recording, and the responses to acetylcholine or sodium nitroprusside were obtained before and 30 minutes after injection of a nitric oxide synthase inhibitor (L-NAME), cyclooxygenase inhibitor (Indomethacin, SC-550 and NS- 398), or calcium-dependent potassium channel blockers (apamin plus TRAM- 34). The maxilla and mandible were removed for bone loss analysis. Blood and gingivae were obtained for C-reactive protein (CRP) and myeloperoxidase (MPO) measurement, respectively. Results: Experimental periodontitis induces bone loss and an increase in the gingival MPO and plasmatic CRP. Periodontitis also reduced endothelium-dependent vasodilation, a hallmark of endothelial dysfunction, 14 days after the procedure. However, the response was restored at day 21. We found that endothelium-dependent vasodilation at day 21 in ligature animals was mediated, at least in part, by the activation of endothelial calcium-activated potassium channels. Conclusions: Periodontitis induces impairment in endothelial-dependent relaxation; this impairment recovers, even in the presence of periodontitis. The recovery is mediated by the activation of endothelial calcium-activated potassium channels in ligature animals. Although important for maintenance of vascular homeostasis, this effect could mask the lack of NO, which has other beneficial properties

    iFloW: an integrated logistics software system for inbound supply chain traceability

    Visibility plays an important role in supply chain management. Such visibility is not only important for better planning, but especially for real-time execution related with the traceability of goods. In inbound supply chain management, logistics planners need to trace raw materials from their requests in order to properly plan a plant’s production. The iFloW (Inbound Logistics Tracking System) integrates logistics providers IT applications and Global Positioning System (GPS) technology to track and trace incoming freights. The Estimated Time of Arrival (ETA) is updated in real-time allowing an improved materials planning process.     Using scrum together with UML models: a collaborative university-industry R&D software project

    Conducting research and development (R&D) software projects, in an environment where both industry and university collaborate, is challenging due to many factors. In fact, industrial companies and universities have generally different interests and objectives whenever they collaborate. For this reason, it is not easy to manage and negotiate the industrial companies’ interests, namely schedules and their expectations. Conducting such projects in an agile framework is expected to decrease these risks, since partners have the opportunity to frequently interact with the development team in short iterations and are constantly aware of the characteristics of the system under development. However, in this type of collaborative R&D projects, it is often advantageous to include some waterfall practices, like upfront requirements modeling using UML models, which are not commonly used in agile processes like Scrum, in order to better prepare the implementation phase of the project.     Correlações entre a hemodinâmica testicular e as características espermáticas em carneiros

    A artéria testicular é responsável pelo fluxo de sangue que chega aos testículos e tem grande importância na termorregulação. Mudanças vasculares nos testículos podem levar à queda da produção espermática, refletindo na motilidade e morfologia. O objetivo deste trabalho foi avaliar as correlações entre a vascularização testicular e as características espermáticas. Foram utilizados oito carneiros adultos Santa Inês com diferentes status reprodutivos. A vascularização testicular e as características seminais foram avaliadas por um período de 90 dias. A ultrassonografia Doppler colorida foi utilizada para avaliar a hemodinâmica testicular. Foram calculados os índices de resistência (RI) e os índices de pulsatilidade (PI) com o modo espectral do Doppler. O modo colorido do Doppler foi utilizado para analisar o fluxo sanguíneo do plexo pampiniforme e do parênquima testicular. As características seminais avaliadas foram o volume, concentração, motilidade e morfologia. Os dados foram submetidos ao teste de correlação linear de Person (P < 0.05 foi considerado significativo). Não foram encontradas relações entre motilidade e a hemodinâmica testicular. A porcentagem de defeitos totais correlacionou-se positivamente com o escore de vascularização dos parênquimas direito e esquerdo, e com o RI e PI esquerdos. Também o escore de vascularização dos plexos se correlacionou positivamente com a média de pixels e negativamente com o RI e PI, de ambos os lados. Este trabalho mostrou que o aumento de defeitos espermáticos pode estar correlacionado com o aumento do fluxo sanguíneo nos testículos; contudo, mais estudos são necessários.The testicular artery is responsible for the blood supply that reaches the testis and has great importance in heat radiation. Vascular changes in the testis may lead to damage in sperm production, reflected in sperm motility and morphology. The aim of the present study was to evaluate correlations between testicular vascularity and sperm characteristic. Eight adult Santa Ines rams showing different reproductive status were used. The testicular vascularity and sperm characteristics were evaluated fortnightly during 90 days. Color Doppler ultrasonography was used to evaluate the testicular hemodynamic. Resistance index (RI) and pulsatility index (PI) of the testicular artery were evaluated by spectral-Doppler mode. The color-Doppler mode was used to evaluate the blood flow of the pampiniform plexus and testicular parenchyma. The semen analyses assessed were volume, concentration, motility, and morphology. The data were submitted to Pearson´s linear correlations test (p < 0.05 was considered significant). No correlations were found between motility and testicular hemodynamic. The percentage of total sperm defects was positively correlated to left and right parenchymal score and to left RI and PI. On the other hand, the pampiniform plexus score was positively correlated to the number of colored pixels and negatively correlated to the RI and PI, for both sides. This study showed that the increase of sperm defect can be related to increase of testicular blood flow; however, more studies are need

    Magnetoliposomes containing multicore nanoparticles and a new antitumor thienopyridine compound with potential application in chemo/thermotherapy

    Multicore magnetic nanoparticles of manganese ferrite were prepared using carboxymethyl dextran as an agglutinating compound or by an innovative method using melamine as a cross-coupling agent. The nanoparticles prepared using melamine exhibited a flower-shape structure, a saturation magnetization of 6.16 emu/g and good capabilities for magnetic hyperthermia, with a specific absorption rate (SAR) of 0.14 W/g. Magnetoliposome-like structures containing the multicore nanoparticles were prepared, and their bilayer structure was confirmed by FRET (Förster Resonance Energy Transfer) assays. The nanosystems exhibited sizes in the range of 250–400 nm and a low polydispersity index. A new antitumor thienopyridine derivative, 7-[4-(pyridin-2-yl)-1H-1,2,3-triazol-1-yl]thieno[3,2-b]pyridine, active against HeLa (cervical carcinoma), MCF-7 (breast adenocarcinoma), NCI-H460 (non-small-cell lung carcino-ma) and HepG2 (hepatocellular carcinoma) cell lines, was loaded in these nanocarriers, obtaining a high encapsulation efficiency of 98% ± 2.6%.     O papel dos profissionais da atenção básica na triagem auditiva neonatal: vantagens, desafios e impacto na identificação precoce de deficiências auditivas

    A surdez infantil representa uma preocupação significativa, impactando não apenas aspectos de saúde, mas também sociais e econômicos. A triagem auditiva neonatal (TAN) surge como uma ferramenta crucial para detectar precocemente problemas auditivos. Apesar de sua importância, a abordagem efetiva da saúde auditiva na atenção primária enfrenta desafios consideráveis no Brasil. Fatores como a escassez de profissionais especializados, vasta extensão territorial, custos dos equipamentos e carência de recursos humanos contribuem para essa realidade. Neste contexto, torna-se evidente o papel fundamental dos profissionais da atenção básica na implementação da triagem auditiva neonatal. Diante desses desafios e da importância desses profissionais, esta revisão tem como objetivo avaliar o engajamento e a eficácia dos profissionais da atenção básica na realização da triagem auditiva neonatal. A presente pesquisa tratou-se de uma revisão integrativa de literatura. Os resultados desta pesquisa foram construídos por meio da análise e interpretação de 10 artigos científicos nesse seguimento. Por fim, os estudos ressaltam a importância da colaboração entre os profissionais de saúde, a sensibilização da população sobre a TAN desde o pré-natal, e a necessidade de uma abordagem integrada para garantir o sucesso da triagem auditiva neonatal e o cuidado efetivo das crianças
