    Practical points of attention beyond instructions for use with the Zenith fenestrated stent graft.

    Fenestrated stent grafting for endovascular repair (F-EVAR) aims to treat patients with abdominal aortic aneurysms that are unsuitable for standard EVAR because of a short or absent infrarenal neck. F-EVAR has been used initially in patients with higher surgical risk with pararenal abdominal aortic aneurysms, but F-EVAR is now increasingly considered a treatment alternative to open surgery in anatomically suitable patients. F-EVAR has benefitted from ongoing technical refinements and accumulating clinical experience but remains a relatively complex procedure. Correct indication, accurate preoperative planning, and meticulous execution are the key to long-term success. Considering the growing interest in F-EVAR worldwide, including the United States, we discuss current indications and provide advice for planning and technical execution on the basis of the senior authors' 13 years of experience

    Monitoring Breast Cancer Response to Neoadjuvant Chemotherapy Using Ultrasound Strain Elastography

    © 2019 The Authors Strain elastography was used to monitor response to neoadjuvant chemotherapy (NAC) in 92 patients with biopsy-proven, locally advanced breast cancer. Strain elastography data were collected before, during, and after NAC. Relative changes in tumor strain ratio (SR) were calculated over time, and responder status was classified according to tumor size changes. Statistical analyses determined the significance of changes in SR over time and between response groups. Machine learning techniques, such as a naïve Bayes classifier, were used to evaluate the performance of the SR as a marker for Miller-Payne pathological endpoints. With pathological complete response (pCR) as an endpoint, a significant difference (P < .01) in the SR was observed between response groups as early as 2 weeks into NAC. Naïve Bayes classifiers predicted pCR with a sensitivity of 84%, specificity of 85%, and area under the curve of 81% at the preoperative scan. This study demonstrates that strain elastography may be predictive of NAC response in locally advanced breast cancer as early as 2 weeks into treatment, with high sensitivity and specificity, granting it the potential to be used for active monitoring of tumor response to chemotherapy

    Dedicated core-on-anvil production of bladelet-like fakes in the Acheulean at Thomas Quarry I - L1 (Casablanca, Morocco)

    The ability to produce large cutting tools (LCTs) is considered as the technological marker of the Acheulean and the indicator of a greater technological complexity compared to the previous Oldowan. Although Acheulean techno-complexes are also composed of a concurrent core-and-flake technology, the iconic handaxes have attracted more attention than any other lithic component. Consequently, little is known of the small and medium-sized flake productions (small flaking), especially starting from 1 Ma, when handaxe and cleaver manufacture becomes intensive and widespread across Africa, including the Atlantic coastal regions of Morocco. Research at Thomas Quarry I yielded a rich early Acheulean lithic assemblage, mainly composed of quartzite LCTs and small flaking, together with a small-sized flint production. Here, we report a particular aspect of this flint assemblage, i.e. a flint bladelet-like flake production. This process represents a discrete technical behaviour among those related to small flaking both in quartzite and flint: pebbles were flaked using the bipolar-on-anvil technique repeatedly employing a specific method to produce bladelet-like flakes. This production represents the oldest dated occurrence of bladelet-like technology in Africa and reveals technical competencies hitherto unknown for these periods, providing further elements for the techno-economic diversification of the African Acheulean

    Application of Plant Surfactants as Cleaning Agents in Shampoo Formulations

    With the increased demand for sustainable, hypoallergenic products, plant surfactants are a promising, eco-friendly option for cleaning products due to their low toxicity or even the absence of toxicity. In the present study, surfactant-rich extracts from Chenopodium quinoa, Glycine max, and Malpighia emarginata were assessed for their stability, antioxidant capacity, toxic potential, and cleaning potential in shampoo formulations. The surfactants in the extracts were isolated and characterized by NMR, UV-Vis, and FTIR spectroscopy. The results demonstrated that the extracts remained stable within the temperature and pH ranges tested. The antioxidant properties were also determined. In the analysis of irritation potential, G. max and C. quinoa exhibited low toxicity and no toxicity, respectively. The cleaning potential analysis confirmed that the extracts could be used as primary surfactants. Seven shampoo formulations were developed, which showed potential to reduce surface tension to the range of 27.1-31.7 mN/m and interfacial tension to the range of 5.4-7.3 mN/m. The wettability, percentage of solids, density, pH, and dirt dispersion of the formulas were within standard ranges, and the sebum removal capacity of the seven formulations was similar to or even better than that of a commercial shampoo

    Shaping the Internet: 10 Years of IXP Growth

    Over the past decade, IXPs have been playing a key role in enabling interdomain connectivity. Their traffic volumes have grown dramatically and their physical presence has spread throughout the world. While the relevance of IXPs is undeniable, their long-term contribution to the shaping of the current Internet is not fully understood yet. In this paper, we look into the impact on Internet routes of the intense IXP growth over the last decade. We observe that while in general IXPs only have a small effect in path shortening, very large networks do enjoy a clear IXP-enabled path reduction. We also observe a diversion of the routes, away from the central Tier-1 ASes supported by IXPs. Interestingly, we also find that whereas IXP membership has grown, large and central ASes have steadily moved away from public IXP peerings, whereas smaller ones have embraced them. Despite all this changes, we find though that a clear hierarchy remains, with a small group of highly central network
