    Leading and Managing the 21st Century Research University: Creating, Implementing, and Sustaining Strategic Change

In order to evolve in this challenging time, universities must embrace strategies for transformational change. This paper reviews two case studies that illustrate the universal applicability of theories of Change Science for achieving sustainable change in stressful times of prosperity and austerity. Understanding the phases of the Change Process that include Creating Vision, Implementing Vision, and Sustaining Vision can promote sustainable change directly related to the culture and mission of the institution

    A case of perinephric abscess in a child with diabetes mellitus: A rare manifestation of group B Streptococcal infection

    Perinephric abscess is an uncommon infection in children, but it is usually caused by Escherichia coli, Staphylococcus aureus, Proteus mirabilis and Pseudomonas aeruginosa. Group B Streptococcus (GBS) is an uncommon cause of perinephric abscess or serious bacterial infections beyond the neonatal period. Comorbid conditions such as diabetes mellitus and immunosuppression can increase the risk of GBS invasive disease. We describe a 10-year-old female who presented with 1-month of right-sided flank pain and swelling with ultrasound showing large (>3 cm) right perinephric and subcutaneous abscesses. She was additionally diagnosed with new onset type 2 diabetes mellitus (DM) during admission. Abscess cultures obtained after placement of two percutaneous drains and the initiation of broad-spectrum intravenous antibiotics grew beta-hemolytic GBS. Here, we present to our knowledge, the first known documented case of GBS perinephric abscess in a school-aged child with DM. Much of the knowledge of perinephric abscess management is extrapolated from adults, therefore making optimal treatment in the pediatric population challenging. We propose that GBS be considered in the etiology of perinephric abscess in children with DM and other immunosuppressing conditions. In addition, percutaneous drainage of larger abscesses (>3 cm) in conjunction with antibiotic therapy is a reasonable management strategy

    In the Spring of 2006, Southern Illinois University Edwardsville (SIUE) initiated a conversation to define a “Teacher-Scholar” Philosophy appropriate for the campus. In an experience similar to John Bunyan’s protagonist, Christian, in the classic 1678 allegory Pilgrim’s Progress, the SIUE faculty and administrators set off on a 21st century journey of discovery.[2] Our journey, however, was toward shared governance, rather than down the path to salvation. Like Bunyan’s Christian, we traveled metaphorically through the Town of Vanity, the Valley of Humiliation, the Slough of Despond, the Hill of Difficulty, and the Castle of Doubt. Unlike Christian, however, our journey of discovery ended before we crossed the River of Death and entered the Celestial City

    A Theory for Rapid Charging Events on the International Space Station

    The Floating Potential Measurement Unit (FPMU) has detected high negative amplitude rapid charging events (RCEs) on the International Space Station (ISS) at the morning terminator. These events are larger and more rapid than the ISS morning charging events first seen by the Floating Potential Probe (FPP) on ISS in 2001. In this paper, we describe a theory for the RCEs that further elucidates the nature of spacecraft charging in low Earth orbit (LEO) in a non-equilibrium situation. The model accounts for all essential aspects of the newly discovered phenomenon, and is amenable to testing on-orbit. Predictions of the model for the amplitude of the ISS RCEs for the full set of ISS solar arrays and for the coming solar cycle are given, and the results of modeling by the Environments WorkBench (EWB) are compared to the observed events to show that the phenomenon can be explained by solar array driven charging. The situation is unique because the coverglasses have not yet reached equilibrium with the surrounding plasma during the RCEs. Finally, a prescription for further use of the ISS for investigating fundamental plasma physics in LEO is given. Already, plasma and charging monitoring instruments on ISS have taught us much about spacecraft interactions with the dense LEO plasma, and we expect they will continue to yield more valuable science when the Japanese Experiment Module (JEM) is in place

    Column classification/characterisation of strong cation exchange phases for the liquid chromatographic analysis of small molecular weight bases

    A simple, rapid and robust protocol for the characterisation of strong cation exchange columns for the analysis of small molecular weight bases is described. A range of ten different phases were characterised, and the resultant selectivity and retention factors analysed using Principal Component Analysis. The score plots for the first and second principal components described 83% of the variability within the dataset. Score plots highlighted the large chromatographic differences observed between the phases, the validity of which was established using a larger range of bases. All the strong cation exchange materials demonstrated a synergistic mixed mode (i.e. ion exchange and hydrophobic) retention mechanism. Principal Component Analysis also highlighted the potential difficulty in locating suitable strong cation exchange “back-up” columns for the analysis of small molecular weight bases in that the characterised columns all displayed very different selectivities.     Structural basis for inhibition of the epidermal growth factor receptor by cetuximab

    SummaryRecent structural studies of epidermal growth factor receptor (EGFR) family extracellular regions have identified an unexpected mechanism for ligand-induced receptor dimerization that has important implications for activation and inhibition of these receptors. Here we describe the 2.8 Å resolution X-ray crystal structure of the antigen binding (Fab) fragment from cetuximab (Erbitux), an inhibitory anti-EGFR antibody, in complex with the soluble extracellular region of EGFR (sEGFR). The sEGFR is in the characteristic “autoinhibited” or “tethered” inactive configuration. Cetuximab interacts exclusively with domain III of sEGFR, partially occluding the ligand binding region on this domain and sterically preventing the receptor from adopting the extended conformation required for dimerization. We suggest that both these effects contribute to potent inhibition of EGFR activation

    Development and evaluation of mosquito-electrocuting traps as alternatives to the human landing catch technique for sampling host-seeking malaria vectors

    Background The human landing catch (HLC) is the gold standard method for sampling host-seeking malaria vectors. However, the HLC is ethically questionable because it requires exposure of humans to potentially infectious mosquito bites. Methods Two exposure-free methods for sampling host-seeking mosquitoes were evaluated using electrocuting surfaces as potential replacements for HLC: (1) a previously evaluated, commercially available electrocuting grid (CA-EG) designed for killing flies, and (2) a custom-made mosquito electrocuting trap (MET) designed to kill African malaria vectors. The MET and the CA-EG were evaluated relative to the HLC in a Latin Square experiment conducted in the Kilombero Valley, Tanzania. The sampling consistency of the traps across the night and at varying mosquito densities was investigated. Estimates of the proportion of mosquitoes caught indoors (P i ), proportion of human exposure occurring indoors (π i ), and proportion of mosquitoes caught when most people are likely to be indoors (P fl ) were compared for all traps. Results Whereas the CA-EG performed poorly (<10 % of catch of HLC), sampling efficiency of the MET for sampling Anopheles funestus s.l. was indistinguishable from HLC indoors and outdoors. For Anopheles gambiae s.l., sampling sensitivity of MET was 20.9 % (95 % CI 10.3–42.2) indoors and 58.5 % (95 % CI 32.2–106.2) outdoors relative to HLC. There was no evidence of density-dependent sampling by the MET or CA-EG. Similar estimates of P i were obtained for An. gambiae s.l. and An. funestus s.l. from all trapping methods. The proportion of mosquitoes caught when people are usually indoors (P fl ) was underestimated by the CA-EG and MET for An. gambiae s.l., but similar to the HLC for An. funestus. Estimates of the proportion of human exposure occurring indoors (π i ) obtained from the CA-EG and MET were similar to the HLC for An. gambiae s.l., but overestimated for An. funestus. Conclusions The MET showed promise as an outdoor sampling tool for malaria vectors where it achieved >50 % sampling sensitivity relative to the HLC. The CA-EG had poor sampling sensitivity outdoors and inside. With further modification, the MET could provide an efficient and safer alternative to the HLC for the surveillance of mosquito vectors outdoors.     Outcomes after angiography with sodium bicarbonate and acetylcysteine

    Background: Intravenous sodium bicarbonate and oral acetylcysteine are widely used to prevent acute kidney injury and associated adverse outcomes after angiography without definitive evidence of their efficacy. Methods: Using a 2-by-2 factorial design, we randomly assigned 5177 patients at high risk for renal complications who were scheduled for angiography to receive intravenous 1.26% sodium bicarbonate or intravenous 0.9% sodium chloride and 5 days of oral acetylcysteine or oral placebo; of these patients, 4993 were included in the modified intention-to-treat analysis. The primary end point was a composite of death, the need for dialysis, or a persistent increase of at least 50% from baseline in the serum creatinine level at 90 days. Contrast-associated acute kidney injury was a secondary end point. Results: The sponsor stopped the trial after a prespecified interim analysis. There was no interaction between sodium bicarbonate and acetylcysteine with respect to the primary end point (P=0.33). The primary end point occurred in 110 of 2511 patients (4.4%) in the sodium bicarbonate group as compared with 116 of 2482 (4.7%) in the sodium chloride group (odds ratio, 0.93; 95% confidence interval [CI], 0.72 to 1.22; P=0.62) and in 114 of 2495 patients (4.6%) in the acetylcysteine group as compared with 112 of 2498 (4.5%) in the placebo group (odds ratio, 1.02; 95% CI, 0.78 to 1.33; P=0.88). There were no significant between-group differences in the rates of contrast-associated acute kidney injury. Conclusions: Among patients at high risk for renal complications who were undergoing angiography, there was no benefit of intravenous sodium bicarbonate over intravenous sodium chloride or of oral acetylcysteine over placebo for the prevention of death, need for dialysis, or persistent decline in kidney function at 90 days or for the prevention of contrast-associated acute kidney injury.     Association of central adiposity with psoriasis, psoriatic arthritis and rheumatoid arthritis: a cross-sectional study of the UK Biobank

    Objectives: To determine the independent association of central adiposity, assessed by waist circumference, with odds of psoriasis, PsA and RA prevalence after controlling for general adiposity (BMI). Methods: A cross-sectional study of UK Biobank participants aged 40–70 years was performed. Logistic regression was used to calculate the odds of psoriasis, PsA and RA occurrence compared with controls without these conditions by waist circumference, adjusting for covariates: age, sex, smoking status, socioeconomic deprivation and self-reported physical activity (Model 1), followed additionally by BMI (Model 2). Results: A total of 502 417 participants were included; 5074 with psoriasis (1.02%), 905 with PsA (0.18%), 5532 with RA (1.11%) and 490 906 controls without these conditions. Adjusted odds ratios (ORs) (Model 1) for psoriasis, PsA and RA, per S.D. (13.5 cm) higher waist circumference were 1.20 (95% CI 1.16, 1.23), 1.30 (95% CI 1.21, 1.39) and 1.21 (95% CI 1.17, 1.24), respectively (all P &lt; 0.001). These ORs remained significant after further adjustment for BMI (Model 2) in psoriasis [OR 1.19 (95% CI 1.12, 1.27), P &lt; 0.001] and RA [OR 1.19 (95% CI 1.12, 1.26), P &lt; 0.001], but not in PsA [OR 1.11 (95% CI 0.95, 1.29), P = 0.127]. Conclusion: Central adiposity as measured by waist circumference is associated with greater odds of psoriasis and RA prevalence after adjustment for confounders and for BMI. Our findings add support for central adiposity as a long-term clinically relevant component of these conditions
