    The play behaviour of young blind children

    There are very few studies on developmental patterns of play in blind children. Those studies which do exist suggest that their play is not only delayed but also different in both qualitative and quantitative terms. Study 1 of this thesis gathered descriptive, cross-sectional data on the spontaneously-emerging patterns of play behaviour in 16 'educationally blind' children aged 1 year 4 months to 6 years. Study 2 extended Study 1 by gathering further play data and taking measures of concurrent developmental status, thereby allowing differences in play profiles to be related to both chronological age and current stage in cognitive, language, social, gross motor and fine motor development. All eight categories of play commonly found in sighted children were observed in the group of blind children who participated in these two studies: collaborative, exploratory, imitative, repetitive, constructive, functional, receptive and pretend play. However, frequency and duration of engagement varied between children and across age groups, reflecting both differences in individual developmental profiles and the affordance ofthe proffered toys. Many of the play behaviours identified correlated significantly with scores on the developmental measures taken. Children with low scores on sensori-motor understanding, verbal comprehension and expressive language were more involved in exploratory play, and significantly less involved in collaborative, constructive and fantasy play. Although an association between gross and fine motor abilities and functional play behaviour might have been expected, no significant correlations were found with scores on either of the motor measures. In contrast, constructive play, rarely seen in the younger children, was significantly correlated with fine motor abilities, language and social skills.The first two studies observed the children playing mainly with toys which were typically available to them in their nursery/school, and it was noted that certain toys appeared to appeal more to the blind child and to engage them differentially. Study 3 was therefore designed i) to allow a comparative investigation of play patterns when 'standard' versus 'blind-friendly' toys were made available, and ii) to provide longitudinal comparative data over an 18 month period on the development of play in blind versus typically-developing young children. Four 'educationally' blind and 4 typically-developing sighted children, matched for general cognitive status and aged 3 to 6 years, played with toys, books and art materials which either had or lacked tactile, olfactory and musical features. Six categories of play were examined, creative, exploratory, constructive, functional, receptive and pretend play. In all 6 categories, there were differences in play behaviour for both blind and sighted children when play took place under 'standard' or 'blind-friendly' conditions. The blind children typically performed less well in the 'standard' condition, producing less varied play, engaging with significantly less materials, and spending a greater length of time off-task than the sighted children. However, in the 'blind -friendly' condition these differences were not evident.Taken together, these findings suggest that the reported delays in the development of play in blind children may, in part at least, be due to the type of toys used in research studies or to the methods in which materials are presented to the children. Given the important role play is believed to have in development, the findings have implications for both educational practice and theories of atypical development

    Factors in assessing effectiveness of orientation programs for public welfare caseworkers; a group project

    A study done in 1968-1969 by students at Portland State University School of Social Work at the request of the Oregon State Public Welfare Commission Staff Development Division sought to devise an instrument for assessing the effectiveness of teaching the casework principles of Felix P. Biestek to casework trainees :in the public welfare\u27s orientation program. The test instrument developed was found to have low, but acceptable, internal reliability. Building en the previous year\u27s work, this 1969-1970 study sought to determine the validity of the test instrument by relating test scores to two measures of job performance, namely the latest supervisory civil service rating and a self-rating. Data was collected on thirty of the original test group. The test instrument was determined to be invalid on the basis of these assessments which used measures of total job performance as validating criteria. The study group concluded that the instrument should not be used by itself to determine the effectiveness of teaching casework principles to caseworkers in a public welfare orientation program. While the instrument was being tested, it was recognized that orientation training covers more than just Biestek’s casework principles. Other types of knowledge are also needed for caseworkers to perform effectively on their jobs. Consequently, the scope of the project was enlarged to include an exploration of other factors in caseworker development during orientation. To explore other factors, two instruments were used. One was a questionnaire developed by the group to obtain background information and to measure some attitudes of the caseworker toward his job and the welfare agency. The second was an instrument borrowed from the Oregon State Fish Commission for determining job satisfaction attitudes. The findings of the questionnaire indicated that informal training and supervision were important in caseworker development. The importance of supervision was reinforced by responses given to the survey of job satisfaction attitudes. The survey elicited complaints about bureaucratic agencies, i.e., the red tape, little use or trying of innovative methods, and poor communications within the agency and to the public. In view of the findings, the study group made six recommendations to the Oregon State Public Welfare Division regarding their orientation and staff development program. The study conclusions state that further research is needed (1) to define the casework job and then develop a test to measure a worker’s competency; (2) to develop tools to determine the social work attitudes, knowledge, and skills of the bachelor level service worker; and (3) to investigate use of the structured versus unstructured situation for teaching new caseworkers

    Outcomes of induction of labour in women with previous caesarean delivery:a retrospective cohort study using a population database

    <p>Background: There is evidence that induction of labour (IOL) around term reduces perinatal mortality and caesarean delivery rates when compared to expectant management of pregnancy (allowing the pregnancy to continue to await spontaneous labour or definitive indication for delivery). However, it is not clear whether IOL in women with a previous caesarean section confers the same benefits. The aim of this study was to describe outcomes of IOL at 39–41 weeks in women with one previous caesarean delivery and to compare outcomes of IOL or planned caesarean delivery to those of expectant management.</p> <p>Methods and Findings: We performed a population-based retrospective cohort study of singleton births greater than 39 weeks gestation, in women with one previous caesarean delivery, in Scotland, UK 1981–2007 (n = 46,176). Outcomes included mode of delivery, perinatal mortality, neonatal unit admission, postpartum hemorrhage and uterine rupture. 40.1% (2,969/7,401) of women who underwent IOL 39–41 weeks were ultimately delivered by caesarean. When compared to expectant management IOL was associated with lower odds of caesarean delivery (adjusted odds ratio [AOR] after IOL at 39 weeks of 0.81 [95% CI 0.71–0.91]). There was no significant effect on the odds of perinatal mortality but greater odds of neonatal unit admission (AOR after IOL at 39 weeks of 1.29 [95% CI 1.08–1.55]). In contrast, when compared with expectant management, elective repeat caesarean delivery was associated with lower perinatal mortality (AOR after planned caesarean at 39 weeks of 0.23 [95% CI 0.07–0.75]) and, depending on gestation, the same or lower neonatal unit admission (AOR after planned caesarean at 39 weeks of 0.98 [0.90–1.07] at 40 weeks of 1.08 [0.94–1.23] and at 41 weeks of 0.77 [0.60–1.00]).</p> <p>Conclusions: A more liberal policy of IOL in women with previous caesarean delivery may reduce repeat caesarean delivery, but increases the risks of neonatal complications.</p&gt

    Distribution of ancestral proto-Actinopterygian chromosome arms within the genomes of 4R-derivative salmonid fishes (Rainbow trout and Atlantic salmon)

    Comparative genomic studies suggest that the modern day assemblage of ray-finned fishes have descended from an ancestral grouping of fishes that possessed 12-13 linkage groups. All jawed vertebrates are postulated to have experienced two whole genome duplications (WGD) in their ancestry (2R duplication). Salmonids have experienced one additional WGD (4R duplication event) compared to most extant teleosts which underwent a further 3R WGD compared to other vertebrates. We describe the organization of the 4R chromosomal segments of the proto-rayfinned fish karyotype in Atlantic salmon and rainbow trout based upon their comparative syntenies with two model species of 3R ray-finned fishes. Results: Evidence is presented for the retention of large whole-arm affinities between the ancestral linkage groups of the ray-finned fishes, and the 50 homeologous chromosomal segments in Atlantic salmon and rainbow trout. In the comparisons between the two salmonid species, there is also evidence for the retention of large whole-arm homeologous affinities that are associated with the retention of duplicated markers. Five of the 7 pairs of chromosomal arm regions expressing the highest level of duplicate gene expression in rainbow trout share homologous synteny to the 5 pairs of homeologs with the greatest duplicate gene expression in Atlantic salmon. These regions are derived from proto-Actinopterygian linkage groups B, C, E, J and K. Conclusion: Two chromosome arms in Danio rerio and Oryzias latipes (descendants of the 3R duplication) can, in most instances be related to at least 4 whole or partial chromosomal arms in the salmonid species. Multiple arm assignments in the two salmonid species do not clearly support a 13 proto-linkage group model, and suggest that a 12 proto-linkage group arrangement (i.e., a separate single chromosome duplication and ancestral fusion/fissions/recombination within the putative G/H/I groupings) may have occurred in the more basal soft-rayed fishes. We also found evidence supporting the model that ancestral linkage group M underwent a single chromosome duplication following the 3R duplication. In the salmonids, the M ancestral linkage groups are localized to 5 whole arm, and 3 partial arm regions (i.e., 6 whole arm regions expected). Thus, 3 distinct ancestral linkage groups are postulated to have existed in the G/H and M lineage chromosomes in the ancestor of the salmonids

    Second MAYA Catalog of Binary Black Hole Numerical Relativity Waveforms

    Numerical relativity waveforms are a critical resource in the quest to deepen our understanding of the dynamics of, and gravitational waves emitted from, merging binary systems. We present 181 new numerical relativity simulations as the second MAYA catalog of binary black hole waveforms (a sequel to the Georgia Tech waveform catalog). Most importantly, these include 55 high mass ratio (q >= 4), 48 precessing, and 92 eccentric (e > 0.01) simulations, including 7 simulations which are both eccentric and precessing. With these significant additions, this new catalog fills in considerable gaps in existing public numerical relativity waveform catalogs. The waveforms presented in this catalog are shown to be convergent and are consistent with current gravitational wave models.     Protocol for a mixed-methods study to develop and feasibility test a digital system for the capture of Patient-Reported Outcomes (PROs) in patients receiving Chimeric Antigen Receptor T-cell (CAR-T) therapies (The PRO-CAR-T Study)

    Introduction: Chimeric antigen receptor (CAR) T-cell therapies are novel, potentially curative therapies for haematological malignancies. CAR T-cell therapies are associated with severe toxicities, meaning patients require monitoring during acute and postacute treatment phases. Electronic patient-reported outcomes (ePROs), self-reports of health status provided via online questionnaires, can complement clinician observation with potential to improve patient outcomes. This study will develop and evaluate feasibility of a new ePRO system for CAR-T patients in routine care. Methods and analysis: Multiphase, mixed-methods study involving multiple stakeholder groups (patients, family members, carers, clinicians, academics/researchers and policy-makers). The intervention development phase comprises a Delphi study to select PRO measures for the digital system, a codesign workshop and consensus meetings to establish thresholds for notifications to the clinical team if a patient reports severe symptoms or side effects. Usability testing will evaluate how users interact with the digital system and, lastly, we will evaluate ePRO system feasibility with 30 CAR-T patients (adults aged 18+ years) when used in addition to usual care. Feasibility study participants will use the ePRO system to submit self-reports of symptoms, treatment tolerability and quality of life at specific time points. The CAR-T clinical team will respond to system notifications triggered by patients’ submitted responses with actions in line with standard clinical practice. Feasibility measures will be collected at prespecified time points following CAR T-cell infusion. A qualitative substudy involving patients and clinical team members will explore acceptability of the ePRO system. Ethics and dissemination: Favourable ethical opinion was granted by the Health and Social Care Research Ethics Committee B(HSC REC B) (ref: 23/NI/0104) on 28 September 2023. Findings will be submitted for publication in high-quality, peer-reviewed journals. Summaries of results, codeveloped with the Blood and Transplant Research Unit Patient and Public Involvement and Engagement group, will be disseminated to all interested groups.
