EffektivitÀt von Canakinumab im klinischen Alltag bei Patienten mit Cryopyrin-assoziiertem periodischem Syndrom (CAPS)
Das Cryopyrin-assozierte periodische Syndrom (CAPS) ist eine autoinflammatorische Erkrankung. Die Ursache ist eine pathogene Mutation im NLRP3-Gen, die zu einer ĂŒbermĂ€Ăigen AusschĂŒttung von IL-1ÎČ fĂŒhrt. Klinisch fĂŒhrt dies zu einem Krankheitsspektrum von milden PhĂ€notypen bis hin zu schweren AusprĂ€gungen: das FCAS, das Muckle-Wells-Syndrom und die schwerste Form das NOMID/CINCA. Die Klinik zeichnet sich durch eine generelle EntzĂŒndungsaktivitĂ€t mit Symptomen wie Fieber, Ausschlag, Schwerhörigkeit, Amyloidose, Arthropathie und EntzĂŒndung des ZNS aus. Seit einigen Jahren ist mit Canakinumab eine gezielte und wirksame Therapie verfĂŒgbar und zugelassen. Die Ziele dieser Studie war es, die Behandlung mit Canakinumab und das Ansprechen darauf im klinischen Alltag zu beobachten. AuĂerdem sollte herausgefunden werden, welche Faktoren die EffektivitĂ€t von Canakinumab beeinflussen und wie sicher die Anwendung ist.
Es wurde eine retrospektive, nationale Studie an verschiedenen Behandlungszentren durchgefĂŒhrt. Patienten, die mindestens zwei Dosen Canakinumab erhielten, wurden in die Studie eingeschlossen. Es wurden Daten zu Demographie, Behandlung, EntzĂŒndungsparameter (u.a. SAA, CRP, S100, etc.), sowie zur klinischen AktivitĂ€t erhoben. Die Remission auf die Behandlung mit Canakinumab wurde mit Hilfe der KrankheitsaktivitĂ€t, CRP und/oder SAA- Werten bestimmt.
In dieser Studie wurden 68 Patienten untersucht. Von diesen waren 27 Kinder und 41 Erwachsene, mit einem medianen Alter von 25 Jahren und einem Follow-up-Median von 28 Monaten. Sowohl die KrankheitsaktivitĂ€t, als auch CRP und SAA zeigten einen deutlichen RĂŒckgang nach Therapiebeginn. Insgesamt 72 % der Patienten sprachen auf die Therapie an. Schwere PhĂ€notypen und Kinder sprachen dabei schlechter an bzw. benötigten höhere Dosierungen. Im Vergleich der beiden gröĂten Behandlungszentren konnten bei höherer Dosierung zusĂ€tzliche Patienten komplette Remission erreichen. Es konnte unabhĂ€ngig von der Höhe der Dosis ein positives Nebenwirkungsprofil festgestellt werden.
Die Konzentration auf die Anwendung im klinischen Alltag brachte auch Nachteile mit. So mussten sehr unterschiedliche Dokumentationswege fĂŒr Klinik, Labor und Therapie vereinheitlicht werden. Durch die RetrospektivitĂ€t und Behandlung im Alltag war keine vollstĂ€ndige Kontrolle der Rahmenbedingungen möglich. Diese Studie untersucht jedoch mit 68 Patienten die bisher gröĂte Kohorte ĂŒber einen lĂ€ngeren Zeitraum. Durch den Vergleich der unterschiedlichen Behandlungszentren ergeben sich âquasiâ-Kontrollgruppen. Auch wenn dies in Zukunft noch weiter untersucht werden sollte, lĂ€sst sich aus dem Ergebnissen schlieĂen, dass höhere Dosierungen von Canakinumab ohne groĂes Risiko weiterer Nebenwirkungen zu zusĂ€tzlichen Remissionen fĂŒhren. Hierdurch kann die LebensqualitĂ€t der Patienten verbessert und das Risiko von SpĂ€tfolgen gesenkt werden.
Die Ergebnisse aus dieser Studie zeigen, dass Ărzte die CAPS-Patienten behandeln, bei klinischer Notwendigkeit mehr Dosisanpassungen nach einem âTreat-to-targetâ-Vorgehen durchfĂŒhren sollten. Sowohl die EffektivitĂ€t, als auch die Sicherheit von Canakinumab lassen diese Empfehlung nach aktuellem Wissensstand zu
High-resolution surface plasmon imaging of gold nanoparticles by energy-filtered transmission electron microscopy
We demonstrate the imaging capabilities of energy-filtered transmission electron microscopy at high-energy resolution in the low-energy-loss region, reporting the direct image of a surface plasmon of an elongated gold nanoparticle at energies around 1 eV. Using complimentary model calculations performed within the boundary element method approach we can assign the observed results to the plasmon eigenmodes of the metallic nanoparticle
Comparison of EFTEM and STEM EELS plasmon imaging of gold nanoparticles in a monochromated TEM
We present and compare two different imaging techniques for plasmonic excitations in metallic nanoparticles based on high energy-resolution electron energy-loss spectroscopy in a monochromated transmission electron microscope. We demonstrate that a recently developed monochromated energy-filtering (EFTEM) approach can be used in addition to the well established scanning technique to directly obtain plasmon images in the energy range below 1 eV. The EFTEM technique is described in detail, and a double experiment performed on the same, triangular gold nanoparticle compares equivalent data acquired by both techniques, respectively
Determination of inner-shell cross-sections for EELS-quantification
Abstract. 2014 Quantitative microanalysis using the EELS technique requires the knowledge of partial scattering cross-sections to relate the measured intensities to the concentrations in the compounds. Partial scattering cross-sections can be determined either theoretically or empirically by means of standards. Cross-sections for K, L23 and M45 ionization edges can be calculated by the hydrogenic model and by the Hartree-Slater-model and with these models elements ranging from Li to W can be quantified. Alternatively, the ratio of two cross-sections (k-factor) can be used for quantification. These k-factors can be determined by measuring the appropriate ionization edges using a thin film standard, which has been done for K, L23, M45 and N45-edges. Scattering cross-sections which have been determined by these methods are compared and their impact on the reliability and accuracy of EELS-quantification is critically discussed. 1. Introduction. Quantitative Microanalysis Using Electron Energy-Loss Spectrometry: II. Compounds with Heavier Elements
Electron energy-loss spectrometry (EELS) in the TEM can be used
for the quantitative analysis of compounds including both light
and heavy elements at a submicrometre scale. However,
EELS-quantification can become complicated due to low
edge-to-background ratios, problems with multiple scattering in
case of thicker samples, inaccuracies due to background
extrapolation or edge-overlapping. All these problems can be
overcome by careful use of well known procedures. A further
problem of quantitative EELS is the need for precise partial
ionization cross-sections that are sometimes not well known. In
this work, we have quantified EELS-spectra of compounds with
known composition: , and . To demonstrate
the achievable accuracy with different models, the
quantifications have been calculated with theoretical cross-sections
(hydrogenic model and Hartree-Slater model) and experimentally
determined -factors. In every single case more accurate
quantification results could be obtained when experimentally
derived cross-sections (or -factors) were used, giving
concentration values that lie within 5 rel% of the nominal
composition
