В статье представлены результаты анализа существующих моделей соглашений о разделении рисков (СРР), применяемых в зарубежных странах при возмещении затрат на лекарственное обеспечение населения. Оценены возможности использования СРР в Российской Федерации, с учетом существующего нормативно-правового обеспечения. Рассмотрена роль системы оценки медицинских технологий, как одного из элементов системы принятия решений, необходимого для внедрения СРР. Так же предложены конкретные изменения в существующее законодательство, которое необходимо произвести для успешного внедрения СРР в Российской Федерации

Mathematical Model of Speed Control System Induction Motor with the Purpose of Increasing its Service Life

В статье рассматривается замкнутая система регулирования частоты вращения асинхронного двигателя (АД), пропорционального регулятора и тиристорного преобразователя напряжения (ТПН). Разработана математическая модель регулирования частоты вращения АД, позволяющая снизить потери мощности в установившихся режимах и при допустимых параметрах переходных процессов для режимов холостого хода и малых нагрузок. Математическая модель АД с ТПН описывается в виде полиноминальных зависимостей от режимных параметров. Модель базируется на экспериментальных данных АД различной мощности. Разработана методика оценки эффективного использования электрической энергии в виде коэффициента мощности АД. Определен оптимизированный вариант формы регулировочной характеристики по критерию минимума затрат электрической энергии. Приведены экспериментальные и расчетные данные, даны практические рекомендации.The article deals with a closed system speed control of induction motor (IM), proportional controller and thyristor voltage converter (TVC). A mathematical model of regulation speed of IM, which reduces losses power in steady-state conditions and with permissible parameters of transients for idling and low load. The mathematical model of IM with TVC is described in the form polynomial dependencies on the regime parameters. The model is based on experimental data of IM of different power. Developed by methodology for assessing effective use electrical energy in the form of a power factor IM. An optimized form is defined by the criterion minimum electrical energy costs. Are presented experimental and calculated data are given practical recommendations

The review outlines the main legislative aspects of organization and financial support of health services for public institution employees inRussia. Specifically, the review describes the legal framework and regulations of healthcare for servicepersons and other staff within the Ministry of Defense of theRussian Federation. We discuss the financing mechanisms of the departmental health service in the Ministry of Defense. Conclusions are made on the need to improve the legislative base of financial and administrative support of healthcare for the military and associated personnel.В статье изложены основные аспекты правового обеспечения организации и финансирования медицинской помощи в рамках ведомственной медицины. Приведен обзор ведомственного законодательства Министерства обороны Российской Федерации (далее – Минобороны России) в контексте организации и финансирования оказания медицинской помощи военнослужащим и приравненным к ним по медицинскому обеспечению лицам. Представлен анализ финансирования ведомственной медицины Минобороны России. Сделаны выводы о необходимости разработки предложений по внесению изменений в законодательные и нормативные правовые акты с целью решения вопросов правового регулирования организации и финансирования медицинской помощи военнослужащим и лицам, приравненным к ним по медицинскому обеспечению

The article presents a review of the foreign approaches to the mechanisms of financing of medical help and innovative medical technologies with the description of the implemented financial mechanisms of stimulation of innovations in foreign systems of public healthcare. Since the system of payment for diagnosis-related groups (DRG) does not contribute to the implementation of innovations in the medical practice, the majority of countries apply additional mechanisms of financing of innovative technologies in public healthcare such as single and additional payments, and target financing that was initially not associated with a DRG model.В статье представлен обзор зарубежных подходов к механизмам финансирования медицинской помощи и применению инновационных медицинских технологий, с описанием используемых финансовых механизмов стимулирования инноваций в здравоохранении за рубежом. В связи с тем, что оплата по клинико-статистическим группам (КСГ) заболеваний не способствует внедрению инноваций в медицинскую практику, большинство стран применяют дополнительные механизмы финансирования инновационных технологий в здравоохранении, такие как отдельные и дополнительные платежи, целевое финансирование, изначально никак не связанные с моделью КСГ

Heating and melting of bulk lump materials

Porous materials, including bulk materials, are widely used for cleaning liquid and gaseous media from mechanical impurities, as heat insulating materials, working bodies in various technologies of mechanical engineering, power engineering, metallurgy, chemistry, petroleum chemistry, food industry, pharmacology, etc. The aim of the work is mathematical modeling of heat transfer in disperse bulk media, their melting, engineering assessment of a number of indicators essential in practice. A bulk disperse medium consisting of solid particles (lumps) of different shapes, sizes and composition, randomly arranged relative to each other, is considered. The article provides relationships for determining effective density, heat capacity, thermal conductivity, taking into account radiant heat transfer between particles (lumps) as applied to porous bulk materials. Using the obtained relationships, the model problem of heating and subsequent melting of the disperse medium under consideration is solved. The melting rate and the relaxation time during which the system consisting of melt and lumps comes to thermodynamic equilibrium are assessed. Though only one-dimensional thermal processes are considered in this paper, the proposed approach can be extended to two- and three-dimensional cases and applied for mathematical description of thermal behavior of nonmetallic porous materials. It is these materials that are mainly used for thermal insulation. Their behavior, especially in extreme, abnormal conditions is of great interest

Diagnosis-related groups and payments for the treatment of malignant neoplasms in the model of 2019

The article addresses the model of diagnosis-related groups (DRG) updated according to the new tariffs in the compulsory medical insurance. Especially emphasized are changes made in the DRG model of 2019, which resulted from the previous work on the development and revision of the clinical recommendations in oncology, as well as the regulation changes in the healthcare system. In addition, the article describes the functioning of the DRG model in 2018 and the payment for cancer care and also provides examples from the practice of chemotherapy. The modifications made in the 2019 model are carefully discussed in terms of: expanding the list of oncological diagnoses, creating and characterizing new DRG groups, updating the coding system and the structure of reference books, changing the Guidebook recommendations and the Instruction related to oncological groups. Clarifications are given regarding frequently asked questions on payments for the medical care in oncology within the current DRG model
