    Bu çalışmada, 20. yüzyıl Amerikalı New York okulu bestecisi Morton Feldman’ın, besteci Bunita Marcus ile Türkiye ve İran’da yapmış oldukları geziler sonrası aldıkları kilim ve halıların Feldman’ın müzikal görüşlerinde ve notasyon biçimlerinde yapmış olduğu yenilikler incelenmiştir. Morton Feldman, müzik literatürüne pek çok yeni fikir ve yeni notasyon tekniği kazandırmıştır. Bu kazandırmış olduğu yeniliklerin kaynaklarından birinin de Türk ve İran halıları ile bağıntısının gün yüzüne çıkartılması, Feldman’ın müziğine ve nota diline getirdiği yeniliklerin irdelenmesi ile bu araştırmanın pek çok müzisyene yol gösterici olması amaçlanmıştır. Morton Feldman, Türkiye ve İran gezilerinde almış olduğu kilim ve halılardaki simetrik ve asimetrik desenlerden etkilenip, daha sonra “Spring of Chosroes”, “Why Patterns?”, “Patterns in a Chromatic Field”, “Crippled Symmetry” adlı eserleri bestelemiş ve müzik literatürüne yeni bir besteleme tekniği kazandırmıştır. Bu araştırma, Morton Feldman’ın adı geçen eserlerinin daha kolay anlaşılmasını, eserlerinin daha geniş müzisyen kitlelerce icra edilmesinin teşvikini sağlayacaktır.In this study, the innovations that the 20th century American New York school composer Morton Feldman has made in Feldman's musical views and notation styles, with the rugs and carpets they bought after their travels with the composer Bunita Marcus in Turkey and Iran were examined. Morton Feldman introduced many new ideas and new notation techniques to the music literature. It is aimed that this research will be a guide for many musicians by revealing one of the sources of the innovations it has brought, its relation with Turkish and Iranian carpets, and examining the innovations that Feldman brought to his music and musical notation. Morton Feldman was influenced by the symmetrical and asymmetrical patterns on the rugs and carpets he bought during his trips to Turkey and Iran, and later composed the works "Spring of Chosroes", "Why Patterns?", "Patterns in a Chromatic Field", "Crippled Symmetry" and brought a new meaning to the music literature composing technique. This research will provide an easier understanding of Morton Feldman's works and encourage the performance of his works by a wider audience of musicians

    Konjenital böbrek ve üriner kanal anomalisi bulunan çocukların klinik ve demografik özellikleri

    Objective: Congenital anomaly of kidney and urinary tract (CAKUT) includes functional and structural anomalies that can cause end-stage renal disease in children. Clinical and demographic characteristics of patients with CAKUT are evaluated in this study. Methods: The files of patients who were followed up with the diagnosis of CAKUT between November 2008 and June 2011 in Diyarbakır Children’s Hospital were reviewed retrospectively. Patient characteristics including age, gender, family history of CAKUT, consangineous marriage, and radiological imaging results were recorded. Results: The study was consisted of 232 patients 129 (55.6%) boys, and 103 (44.4%) girls. The mean age was 4.4 years. A family history of CAKUT was found in 15.2% of patients and 50.9% of parents had consanguineous marriage. Anomalies that were observed in the study included, ureteropelvic junction obstruction in 66 (28.5%), renal agenesis in 49 (21.1%), ectopic kidney in 25 (10.8%), horseshoe kidney in 25 (10.8%), multicystic dysplastic kidney in 20 (8.6%), vesicoureteral reflux in 20 (8.6%), polycystic kidney in 12 (5.2%), posterior urethral valve in 7 (3%), ureterovesical obstruction in 4 (1.7%), renal hypoplasia in 2 (0.9%), and fusion of kidneys in 2 (%0.9) patients. Renal scarring and chronic kidney disease was present in 32 (13.8%) and 7 (3%) patients, respectively. Conclusions: Childhood CAKUT can cause serious morbidities. Cases should be followed up carefully even from the antenatal period. Early intervention of cases requiring treatment will minimize the risk of permanent renal damage

    Femoral artery: An alternative cannulation localization in the angiographic treatment of dysfunctional arteriovenous fistulas. Single center experience

    Bu çalışma, 26-28 Şubat 2021 tarihlerinde Sapanca-İstanbul düzenlenen 13. Çapa Nefroloji Günleri Kongresi‘nde bildiri olarak sunulmuştur.Endovasküler girişimler, disfonksiyonel arteriovenöz fistüllerin (AVF) tedavisinde en sık kullanılan tedavi modaliteleri olsa da optimal ponksiyon yeri konusunda bir fikir birliği yoktur. Bu retrospektif, tek merkezli çalışmada, femoral arter yoluyla disfonksiyonel AVF'lere yönelik endovasküler girişimlerin kısa vadeli klinik başarı oranlarını ve komplikasyonlarını sunuyoruz. Ocak 2016 ile Aralık 2019 arasında AVF disfonksiyonu tanısı alan ve bu nedenle perkütan transluminal anjiyoplasti (PTA) uygulanan toplam 29 hemodiyaliz hastası bu çalışmaya dahil edildi. Tüm PTA'lar aynı deneyimli girişimsel kardiyolog tarafından yapıldı. Hastaların demografik, klinik verileri, birincil açıklık ve işlem komplikasyonlarına ilişkin veriler hastane veri tabanından ve/veya hastanın kendi hemodiyaliz merkezinin elektronik kayıtlarından elde edildi. İşlem sonrası “thrill”in saptanması, başarılı kanülasyon ve tedaviden hemen sonra yeterli hemodiyaliz yapılabilmesi klinik başarı olarak kabul edildi. Toplam 29 hemodiyaliz hastasına AVF disfonksiyonu nedeniyle PTA uygulandı. Hastaların ortanca yaşı 61 (IQR 55.0-68.0) olup, %72.4'ü erkekti. Hastaların %41'i diyabetikti. Ortanca AVF yaşı 44.0 (24.0-92.0) aydı. Jukstaanastomotik (%48.3) ve efferent ven darlığı (%37.9) AVF işlev bozukluğunun ana nedenleriydi. 27 hastaya paklitaksel salınımlı balon anjiyoplasti uygulandı. Girişimlerin klinik başarı oranı %93.1 idi. İki hastada femoral arter ponksiyon yerinde lokal hematom gelişmesi dışında diğer hastalarda başka bir majör veya minör komplikasyon gözlenmedi. Femoral arter yoluyla yapılan PTA girişimleri AVF disfonksiyonlarında etkili ve güvenli bir tedavi yöntemidir.Although endovascular interventions are the most commonly used treatment modalities in the treatment of dysfunctional AVFs, there is no consensus on the optimal puncture site. In this retrospective single-center study, we present the short-term clinical success rates and complications of our interventions for dysfunctional AVFs via the femoral artery. 29 hemodialysis patients who were diagnosed with AVF dysfunction between January 2016 and December 2019 and therefore underwent percutaneous transluminal angioplasty (PTA) were included in this study. All PTAs were performed by the same experienced interventional cardiologist. Demographic, clinical data of the patients, and data on primary patency and procedural complications used were obtained from the hospital database and electronic records of the patient's own hemodialysis center. Detection of "thrill" after the procedure, successful cannulation, and adequate hemodialysis immediately after the treatment was considered a clinical success. A total of 29 hemodialysis patients underwent PTA due to AVF dysfunction. The median age of the patients was 61 (IQR 55.0-68.0) and 72.4% were male. 41% of patients were diabetic. The Median AVF age was 44.0 (24.0-92.0) months. Juxtaanastomotic (48.3%) and efferent venous stenosis (37.9%) were the main causes of AVF dysfunction. Paclitaxel-released balloon angioplasty was performed on 27 patients. The clinical success rate of the procedure was 93.1%. Two patients developed a local hematoma at the femoral artery puncture site; no other major or minor complications were detected. PTA via the femoral artery is an effective and safe treatment modality for AVF dysfunctions
