    Regeneração periodontal em lesões de furca – revisão da literatura

    ResumoO tratamento de dentes com envolvimento de furca é um procedimento complexo; no entanto, a evolução das diversas técnicas regenerativas tem proporcionado novos recursos e novas opções de tratamento para a eliminação destas lesões.ObjetivoA presente revisão bibliográfica pretende comparar a regeneração tecidular guiada com as proteínas de matriz de esmalte no tratamento de lesões de furca e avaliar um possível benefício na sua utilização conjunta.MétodosEnsaios clínicos randomizados com, pelo menos, 6 meses de duração e que utilizassem para o tratamento de lesões de furca pelo menos uma das 2 técnicas ou a combinação das 2.ResultadosForam incluídos 11 ensaios clínicos randomizados.ConclusãoDentro das limitações impostas pelo tipo de revisão efetuada, podemos concluir que ambas são eficazes no tratamento de lesões de furca de classe ii mandibulares e que a combinação das 2 não parece apresentar vantagens significativas na melhoria dos resultados clínicos.AbstractThe treatment of teeth with furcation involvement is a complex procedure; however, the evolution of regenerative techniques has provided several new features and new treatment options to eliminate these lesions.AimThis literature review aims to compare guided tissue regeneration with enamel matrix proteins in the treatment of furcation defects and evaluate a possible benefit in using them together.MethodsRandomized clinical trials with at least 6 months of duration and which use for the treatment of furcation defects, at least one of the two techniques or a combination of both.ResultsThe review includes 11 randomized clinical trials.ConclusionWithin the limitations imposed by the type of review undertaken, we can conclude that both are effective in the treatment of furcation defects in mandibular class II furcation and the combination of the two techniques does not seem to offer significant advantages in improving clinical outcomes

    Importância da imagem vídeo em ortodontia - Uma revisão da literatura

    ResumoIntroduçãoO objetivo deste estudo foi avaliar o interesse da captação de imagens faciais dinâmicas no diagnóstico e plano de tratamento em ortodontia.MétodosCom o recurso ao motor de busca PubMed, foram utilizadas as palavras-chave «vídeo AND orthodontics» e «dynamic analysis AND orthodontics». Os estudos que incluíam pacientes com defeitos congénitos, síndromes ou que tivessem sido previamente submetidos a tratamento ortodôntico-cirúrgico-ortognático foram excluídos.ResultadosNas 2 pesquisas, encontrámos 178 artigos. Após a análise de todos os resumos, foram incluídos 15 artigos, sendo que apenas 8 dos artigos eram ensaios clínicos randomizados.ConclusãoApesar das potencialidades apontadas à utilização de imagens vídeo no diagnóstico e plano de tratamento em ortodontia, estas devem funcionar como um complemento dos meios convencionais. No entanto, as funções educativa e motivacional parecem fazer sobressair esta tecnologia em comparação com os outros métodos.AbstractIntroductionThe aim of this study was to evaluate the interest of capturing dynamic facial images in the diagnosis and treatment planning in orthodontics.MethodsWith the use of the PubMed search engine, we used the keywords “video AND orthodontics” and “dynamic analysis AND orthodontics”. The studies including patients with birth defects, syndromes or patients who had been previously submitted to orthodontic-orthognathic surgery were excluded.ResultsFrom the 178 articles, after analyzing all the abstracts, 15 articles were included. Only 8 were randomized controlled trials.ConclusionDespite the potential indicated the use of video images in the diagnosis and treatment planning in orthodontics, they shall function as a complement to conventional means. However, educational and motivational functions appear to bring out this technology compared to other methods

    ResumoO objetivo principal desta revisão bibliográfica é sistematizar e determinar qual omaterial de enxerto ósseo de eleição no levantamento do seio maxilar atrófico pela técnica da janelalateral para posterior colocação de implantes dentários (segunda fase cirúrgica) em função da alturaganha de osso. Como objetivo secundário, tentamos avaliar qual o efeito do uso dos promotores decrescimento, da mistura dos diversos biomateriais com osso autólogo e a sobrevivência dos implantesdentários nos diferentes tipos de enxertos. A pesquisa foi realizada na base de dados Pubmed, utilizando-se o total de 22 artigos datados de 2005a 2011. As exclusões deveram-se aos artigos referirem-se a colocação dos implantes dentários numaetapa, a técnica utilizada não ser a janela lateral ou inadequado desenho do estudo.AbstractThe main objective of this literature review was to try with a simple and systematic observer which was the bone graft material of choice in sinus lift of atrophic maxilla for the technic of lateral window, to placing dental implants (second surgical phase) in according the gain of bone height. As asecond objective we try to understand what is the effect of the use of growth romoters, the mixture of various biomaterials whith autologous bone and the survival of dental implants in different types of grafts.     The posterior alveolar bone region of the maxilla has, in more than 40% of the cases, a height inferior to 4 mm, thus making rehabilitation with implants a clinical challenge. This focus literature review was carried out through an exhaustive literature search in PubMed, ScienceDirect, LILACS, Web of Science, Scielo and Cochrane Library. The aim was to illustrate and analyse various factors that should be accounted for while applying the maxillary sinus elevation technique.     Advantages of porcine xenograft over autograft in sinus lift: a randomised clinical trial

    This study aimed to compare the performance of intra-oral autologous bone grafts versus porcine xenografts in a two-step lateral window sinus lift. This split-mouth randomised controlled trial sequentially enrolled 12 patients with a 6-month follow-up. For each patient, a simultaneous randomised bilateral maxillary sinus lift was performed and filled with autologous bone from the mandible (control) or a porcine xenograft (test). A bone biopsy sample was collected during the implant placement for histological and histomorphometric analysis. CT scans were performed at the beginning and at the end of the trial to assess radiological evolution. A comparison of initial and six-month CT scans indicated statistically significant increases in bone level for both materials (7.8 ± 2.4 mm for autologous and 8.7 ± 2.2 mm for xenograft, p < 0.05), and there were no significant differences between the performance of the two materials over time (p = 0.26). The histological analysis showed various stages of the remodelling process and no cells or other signs of inflammation or infection were visible in both groups. The porcine xenografts presented similar results for the studied variables when compared to autologous bone, being a reasonable alternative for a sinus lift.publishe

    CT maxillary sinus evaluation: a retrospective cohort study

    Background: Proximity of the dental roots to the sinus floor makes dental disease a probable cause of maxillary sinusitis. The aim of this study was to find out if maxillary sinus pathologic changes were more prevalent in patients with dental disease and to evaluate the performance of computed tomography (CT) in analyzing and detecting apical periodontitis and other odontogenic causes on the maxillary sinusitis etiology in a Portuguese Caucasian population. Material and Methods: Retrospective cohort study. The total sample of 504 patients and their CT was included in this study. The patients were from a private dental clinic, specializing in oral surgery, where the first complaint was not directly related to sinus disease, but with dental pathology. For each patient, the etiological factors of maxillary sinusitis and the imaging CT findings were analyzed. All the axial, coronal and sagittal CT slices were evaluated and general data were registered. The latter was selected based on the maxillary sinus CT published literature. Results: 32.40% of patients presented normal sinus (without any etiological factor associated), 29.00% showed presence of etiological and imaging findings in the maxillary sinus, 20.60% had only imaging changes in the maxillary sinus and 18.00% of patients presented only etiological factors and no change in the maxillary sinus. Conclusions: Radiological imaging is an important tool for establishing the diagnosis of maxillary sinus pathology. These results indicate that the CT scan should be an excellent tool for complement the odontogenic sinusitis diagnosis
