    Time to publication for NIHR HTA programme-funded research: a cohort study

    ObjectiveTo assess the time to publication of primary research and evidence syntheses funded by the National Institute for Health Research (NIHR) Health Technology Assessment (HTA) Programme published as a monograph in Health Technology Assessment and as a journal article in the wider biomedical literature.Study designRetrospective cohort study.SettingPrimary research and evidence synthesis projects funded by the HTA Programme were included in the cohort if they were registered in the NIHR research programmes database and was planned to submit the draft final report for publication in Health Technology Assessment on or before 9 December 2011.Main outcome measuresThe median time to publication and publication at 30?months in Health Technology Assessment and in an external journal were determined by searching the NIHR research programmes database and HTA Programme website.ResultsOf 458 included projects, 184 (40.2%) were primary research projects and 274 (59.8%) were evidence syntheses. A total of 155 primary research projects had a completion date; the median time to publication was 23?months (26.5 and 35.5?months to publish a monograph and to publish in an external journal, respectively) and 69% were published within 30?months. The median time to publication of HTA-funded trials (n=126) was 24?months and 67.5% were published within 30?months. Among the evidence syntheses with a protocol online date (n=223), the median time to publication was 25.5?months (28?months to publication as a monograph), but only 44.4% of evidence synthesis projects were published in an external journal. 65% of evidence synthesis studies had been published within 30.0?months.ConclusionsResearch funded by the HTA Programme publishes promptly. The importance of Health Technology Assessment was highlighted as the median time to publication was 9?months shorter for a monograph than an external journal article

    Interplay between function and structure in complex networks

    We show that abrupt structural transitions can arise in functionally optimal networks, driven by small changes in the level of transport congestion. Our results offer an explanation as to why so many diverse species of network structure arise in Nature (e.g. fungal systems) under essentially the same environmental conditions. Our findings are based on an exactly solvable model system which mimics a variety of biological and social networks. We then extend our analysis by introducing a novel renormalization scheme involving cost motifs, to describe analytically the average shortest path across multiple-ring-and-hub networks. As a consequence, we uncover a 'skin effect' whereby the structure of the inner multi-ring core can cease to play any role in terms of determining the average shortest path across the network.Comment: Expanded version of physics/0508228 with additional new result

    Identification of diverse database subsets using property-based and fragment-based molecular descriptions

    This paper reports a comparison of calculated molecular properties and of 2D fragment bit-strings when used for the selection of structurally diverse subsets of a file of 44295 compounds. MaxMin dissimilarity-based selection and k-means cluster-based selection are used to select subsets containing between 1% and 20% of the file. Investigation of the numbers of bioactive molecules in the selected subsets suggest: that the MaxMin subsets are noticeably superior to the k-means subsets; that the property-based descriptors are marginally superior to the fragment-based descriptors; and that both approaches are noticeably superior to random selection

    Pre-COVID-19 physical activity status does not protect against reductions in post-covid-19 symptoms: a correlation relativistic analysis during the lockdown

    People who exhibit unhealthy lifestyle behaviours are at greater risk of severe disease outcomes, risk of hospitalisation and mortality when infected with COVID-19. Accordingly, it is suggested that those with higher levels of cardiorespiratory fitness and who engage in regular physical activity (PA) are associated with a reduced risk of adverse outcomes. Although improved physiological function may protect individuals against severe acute COVID-19 outcomes, it is unknown whether it offers protection against developing sustained symptom profile, known as post-acute COVID-19 syndrome or Long COVID. Affecting an estimated 2 million people in the UK and 144 million globally, Long COVID is challenging healthcare services with broader social and economic impacts. Accordingly, this project aimed to determine the impact of PA status on Long COVID. An online survey was developed Utilizing adapted versions of preexisting Patient Re-ports Outcome Measures (Qualtrics, Provo, Utah, United States). Participants self-reported PA status in line with the World Health Organisation guidelines and their pre- and post-COVID-19 health status and symptom profile. A Mann-Whitney U test was used to analyse between-group responses, and a Wilcoxon Signed Rank test was used to analyse within-group responses. The survey was completed by 381 participants, of which n=253 reported meeting or exceeding the recommended guidelines of PA. A significant difference was found between pre- and post-COVID-19 health, whilst a Mann-Whitney U test concluded that there was no significant difference between PA groups and post-COVID-19 health status. According to the results, increased PA and cardiorespiratory fitness might offer protection against severe disease outcomes in the acute phase of infection but this does not offer full protection against developing a long-term symptom profile and increased mechanistic understanding of the physiological determinants is needed to restore the pre-COVID-19 status and assist in the development of multi-disciplinary interventions

    Multistability of free spontaneously-curved anisotropic strips

    Multistable structures are objects with more than one stable conformation, exemplified by the simple switch. Continuum versions are often elastic composite plates or shells, such as the common measuring tape or the slap bracelet, both of which exhibit two stable configurations: rolled and unrolled. Here we consider the energy landscape of a general class of multistable anisotropic strips with spontaneous Gaussian curvature. We show that while strips with non-zero Gaussian curvature can be bistable, strips with positive spontaneous curvature are always bistable, independent of the elastic moduli, strips of spontaneous negative curvature are bistable only in the presence of spontaneous twist and when certain conditions on the relative stiffness of the strip in tension and shear are satisfied. Furthermore, anisotropic strips can become tristable when their bending rigidity is small. Our study complements and extends the theory of multistability in anisotropic shells and suggests new design criteria for these structures.Comment: 20 pages, 10 figure

    Acute COVID-19, the Lived Experience, and Lessons to Learn for Future Pandemics

    Objectives: The study aimed to increase the understanding of the lived experience of patients during the acute phase of a coronavirus disease 2019 (COVID-19) infection. Method: A Web-based survey was distributed through established patient and public engagement and involvement groups and networks, social media, and by means of word of mouth. The survey covered questions relating to patient demographics, COVID-19 diagnosis, symptom profile, and patient experience during acute COVID-19. Results: The findings demonstrate the varying symptom profiles experienced by people in the acute stage of COVID-19 infection, with participants sharing how they managed care at home, and/or accessed medical advice. Findings also highlight themes that people were concerned with being unable to receive care and believed they needed to rely heavily on family, with extreme thoughts of death. Conclusions: Although the urgent threat to public health has been negated by efficacious vaccines and enhanced treatment strategies, there are key lessons from the lived experience of COVID-19 that should be used to prepare for future pandemics and public health emergencies

    German Lexical Personality Factors: Relations with the HEXACO Model

    We correlated the scales of the HEXACO Personality Inventory (HEXACO-PI) with adjective scale markers of factors previously obtained in indigenous lexical studies of personality structure in the German language. Self-ratings obtained from a sample of 323 German participants showed a pattern of strong convergent and weak discriminant correlations, supporting the content-based interpretation of the German lexical factors in terms of the HEXACO dimensions. Notably, convergent correlations were strong for both the broader and the narrower variants of the Honesty-Humility factor as observed in German lexical studies. Also, convergent correlations for HEXACO Openness to Experience were, as expected, stronger for German adjectives describing a creative and intellectual orientation than for German adjectives describing intellectual ability.     Do Surgical Trials Meet the Scientific Standards for Clinical Trials?

    Unlike medications, the dissemination of surgical procedures into practice is not regulated. Before marketing, pharmaceutical products are required to be shown safe and efficacious in comparative clinical trials that use bias-reducing strategies designed to reduce the distortion of estimates of treatment effect by predispositions toward the investigational intervention or control. Unless an investigational device is involved, the corresponding process for surgical innovations is usually unregulated and therefore may not be based on adequate evidence. Given these differences, we sought to evaluate the state of clinical research on invasive procedures. We conducted a systematic review of publications from 1999 through 2008, which reported the results of studies evaluating the effects of invasive therapeutic procedures, focusing on trials that appeared to influence practice. Our objective was to determine what proportion of studies evaluating surgical procedures use a comparative clinical trial design and methods to control bias. This article reports our results and raises concerns about the methodologic, and therefore the ethical, quality of clinical research used to justify the implementation of surgical procedures into practice

    Intrusive dike complexes, cumulate cores, and the extrusive growth of Hawaiian volcanoes

    The Hawaiian Islands are the most geologically studied hot-spot islands in the world yet surprisingly, the only large-scale compilation of marine and land gravity data is more than 45 years old. Early surveys served as reconnaissance studies only, and detailed analyses of the crustal-density structure have been limited. Here we present a new chain-wide gravity compilation that incorporates historical island surveys, recently published work on the islands of Hawai‘i, Kaua‘i, and Ni‘ihau, and >122,000 km of newly compiled marine gravity data. Positive residual gravity anomalies reflect dense intrusive bodies, allowing us to locate current and former volcanic centers, major rift zones, and a previously suggested volcano on Ka‘ena Ridge. By inverting the residual gravity data, we generate a 3-D view of the dense, intrusive complexes and olivine-rich cumulate cores within individual volcanoes and rift zones. We find that the Hāna and Ka‘ena ridges are underlain by particularly high-density intrusive material (>2.85 g/cm3) not observed beneath other Hawaiian rift zones. Contrary to previous estimates, volcanoes along the chain are shown to be composed of a small proportion of intrusive material (<30% by volume), implying that the islands are predominately built extrusively
