    Natural history of a visceral leishmaniasis outbreak in highland Ethiopia

    In May 2005, visceral leishmaniasis (VL) was recognized for the first time in Libo Kemken, Ethiopia, a highland region where only few cases had been reported before. We analyzed records of VL patients treated from May 25, 2005 to December 13, 2007 by the only VL treatment center in the area, maintained by Médecins Sans Frontières-Ethiopia, Operational Center Barcelona-Athens. The median age was 18 years; 77.6% were male. The overall case fatality rate was 4%, but adults 45 years or older were five times as likely to die as 5-29 year olds. Other factors associated with increased mortality included HIV infection, edema, severe malnutrition, pneumonia, tuberculosis, and vomiting. The VL epidemic expanded rapidly over a several-year period, culminating in an epidemic peak in the last third of 2005, spread over two districts, and transformed into a sustained endemic situation by 2007

    We investigate the radiatively induced Chern-Simons-like term in four-dimensional field theory at finite temperature. The Chern-Simons-like term is temperature dependent and breaks the Lorentz and CPT symmetries.     O superfosfato simples (SS) é uma alternativa aos adubos fosfatados que são utilizados via fertirrigação, como o ácido fosfórico, que provoca acidificação do bulbo úmido no solo, e o monofosfato de amônio purificado, que apresenta maior preço. No entanto, não se conhece o efeito de tal adubo no funcionamento de emissores, principalmente nos gotejadores, nem se é possível controlar o entupimento por SS por meio de aplicações de ácido. Assim, desenvolveu-se este trabalho com o objetivo de avaliar a possibilidade da utilização do superfosfato simples via gotejamento, como alternativa ao uso de outros adubos fosfatados, e a tentativa de controle do entupimento utilizando ácido nítrico. O trabalho foi desenvolvido em bancada de ensaios de gotejadores, localizada no Laboratório de Hidráulica da ESALQ/USP. Foram aplicadas doses de 50; 75 e 100% da solubilidade do superfosfato simples (SS), em 26 tubogotejadores de diferentes marcas comerciais, e ao fim de 360 horas de irrigação, foi aplicado ácido nítrico com pH 2, na tentativa de desentupir os gotejadores mais sensíveis. Diante dos resultados, foi possível inferir que o uso do superfosfato simples pode ser feito via fertirrigação, e o ácido nítrico, aplicado segundo esta metodologia, não proporcionou a desobstrução dos emissores.The super simple phosphate (SS) is an alternative to the phosphorous fertilizers which are used through fertigation, as the phosphoric acid, which triggers acidification of the wetted volume in the soil, and the monophosfato of purified ammonium, that has a higher price. However, the effect of such fertilizer in the emitters function, mainly in the drippers, is not known neither if it is possible to control the blockage for SS through acid applications. Thus, this study was developed in order to evaluate the possibility of the use of super simple phosphate trough drip irrigation, as an alternative to the use of other phosphorous fertilizers, and the attempt of controlling the blockage with nitric acid. The work was carried out in bench of drippers rehearsals, located at the Hydraulic laboratory in ESALQ-USP. Doses of 50; 75 and 100% of the super simple phosphate (SS) were applied in 26 emitter tubes with different commercial marks, and after 360 hours the nitric acid with a pH of 2 was applied, in the attempt of clearing the most sensitive drippers to the blockage. With the results it was possible to infer that: the use of super simple phosphate can be done by fertigation without causing damages to the system and the nitric acid, applied according to this methodology, did not provide the clearance of the emitters

    Black holes admitting a Freudenthal dual

    The quantised charges x of four dimensional stringy black holes may be assigned to elements of an integral Freudenthal triple system whose automorphism group is the corresponding U-duality and whose U-invariant quartic norm Delta(x) determines the lowest order entropy. Here we introduce a Freudenthal duality x -> \tilde{x}, for which \tilde{\tilde{x}}=-x. Although distinct from U-duality it nevertheless leaves Delta(x) invariant. However, the requirement that \tilde{x} be integer restricts us to the subset of black holes for which Delta(x) is necessarily a perfect square. The issue of higher-order corrections remains open as some, but not all, of the discrete U-duality invariants are Freudenthal invariant. Similarly, the quantised charges A of five dimensional black holes and strings may be assigned to elements of an integral Jordan algebra, whose cubic norm N(A) determines the lowest order entropy. We introduce an analogous Jordan dual A*, with N(A) necessarily a perfect cube, for which A**=A and which leaves N(A) invariant.     Establishment of the bacterial fecal community during the first month of life in Brazilian newborns

    OBJECTIVE: The establishment of the intestinal microbiota in newborns is a critical period with possible long-term consequences for human health. In this research, the development of the fecal microbiota of a group of exclusively breastfed neonates living in low socio-economic conditions in the city of São Paulo, Brazil, during the first month of life, was studied. METHODS: Fecal samples were collected from ten neonates on the second, seventh, and 30th days after birth. One of the neonates underwent antibiotic therapy. Molecular techniques were used for analysis; DNA was extracted from the samples, and 16S rRNA libraries were sequenced and phylogenetically analyzed after construction. A real-time polymerase chain reaction (PCR) was performed on the samples taken from the 30th day to amplify DNA from Bifidobacterium sp. RESULTS: The primary phylogenetic groups identified in the samples were Escherichia and Clostridium. Staphylococcus was identified at a low rate. Bifidobacterium sp. was detected in all of the samples collected on the 30th day. In the child who received antibiotics, a reduction in anaerobes and Escherichia, which was associated with an overgrowth of Klebsiella, was observed throughout the experimental period. CONCLUSION: The observed pattern of Escherichia predominance and reduced Staphylococcus colonization is in contrast with the patterns observed in neonates living in developed countries

    Embolic strokes of undetermined source: prevalence and patient features in the ESUS Global Registry

    Background: Recent evidence supports that most non-lacunar cryptogenic strokes are embolic. Accordingly, these strokes have been designated as embolic strokes of undetermined source (ESUS). Aims: We undertook an international survey to characterize the frequency and clinical features of ESUS patients across global regions. Methods: Consecutive patients hospitalized for ischemic stroke were retrospectively surveyed from 19 stroke research centers in 19 different countries to collect patients meeting criteria for ESUS. Results: Of 2144 patients with recent ischemic stroke, 351 (16%, 95% CI 15% to 18%) met ESUS criteria, similar across global regions (range 16% to 21%), and an additional 308 (14%) patients had incomplete evaluation required for ESUS diagnosis. The mean age of ESUS patients (62 years; SD = 15) was significantly lower than the 1793 non-ESUS ischemic stroke patients (68 years, p ≤ 0.001). Excluding patients with atrial fibrillation (n = 590, mean age = 75 years), the mean age of the remaining 1203 non-ESUS ischemic stroke patients was 64 years (p = 0.02 vs. ESUS patients). Among ESUS patients, hypertension, diabetes, and prior stroke were present in 64%, 25%, and 17%, respectively. Median NIHSS score was 4 (interquartile range 2–8). At discharge, 90% of ESUS patients received antiplatelet therapy and 7% received anticoagulation. Conclusions: This cross-sectional global sample of patients with recent ischemic stroke shows that one-sixth met criteria for ESUS, with additional ESUS patients likely among those with incomplete diagnostic investigation. ESUS patients were relatively young with mild strokes. Antiplatelet therapy was the standard antithrombotic therapy for secondary stroke prevention in all global regions
