Annex to Quirke et al. Quality assurance in pathology in colorectal cancer screening and diagnosis: annotations of colorectal lesions
Multidisciplinary, evidence-based European Guidelines for quality assurance in colorectal cancer screening and diagnosis have recently been developed by experts in a pan-European project coordinated by the International Agency for Research on Cancer. The full guideline document includes a chapter on pathology with pan-European recommendations which take into account the diversity and heterogeneity of health care systems across the EU. The present paper is based on the annex to the pathology chapter which attempts to describe in greater depth some of the issues raised in the chapter in greater depth, particularly details of special interest to pathologists. It is presented here to make the relevant discussion known to a wider scientific audience
A comparison of drug transport in pulmonary absorption models: isolated perfused rat lungs, respiratory epithelial cell lines and primary cell culture
Purpose: To evaluate the ability of human airway epithelial
cell layers and a simple rat isolated perfused lung (IPL) model to predict pulmonary drug absorption in rats in vivo.
Method: The permeability of seven compounds selected to
possess a range of lipophilicity was measured in two airway
cell lines (Calu-3 and 16HBE14o-), in normal human bronchial
epithelial (NHBE) cells and using a simple isolated perfused
lungs (IPL) technique. Data from the cell layers and
ex vivo lungs were compared to published absorption rates
from rat lungs measured in vivo.
Results: A strong relationship was observed between the logarithm of the in vivo absorption half-life and the absorption half-life in the IPL (r = 0.97; excluding formoterol). Good log-linear relationships were also found between the apparent first-order absorption rate in vivo and cell layer permeability with correlation coefficients of 0.92, 0.93, 0.91 in Calu-3, 16HBE14o- and NHBE cells, respectively.
Conclusion: The simple IPL technique provided a good prediction of drug absorption from the lungs, making it a useful method for empirical screening of drug absorption in the lungs. Permeability measurements were similar in all the respiratory epithelial cell models evaluated, with Calu-3 having the advantage for routine permeability screening purposes of being readily availability, robust and easy to culture
Chitosan Modification of Adenovirus to Modify Transfection Efficiency in Bovine Corneal Epithelial Cells
BACKGROUND: The purpose of this study is to modulate the transfection efficiency of adenovirus (Ad) on the cornea by the covalent attachment of chitosan on adenoviral capsids via a thioether linkage between chitosan modified with 2-iminothiolane and Ad cross-linked with N-[gamma-maleimidobutyryloxy]succinimide ester (GMBS). METHODOLOGY/PRINCIPAL FINDINGS: Modified Ad was obtained by reaction with the heterobifunctional crosslinking reagent, GMBS, producing maleimide-modified Ad (Ad-GMBS). Then, the chitosan-SH was conjugated to Ad-GMBS via a thioether bond at different ratios of Ad to GMBS to chitosan-SH. The sizes and zeta potentials of unmodified Ad and chitosan-modified Ads were measured, and the morphologies of the virus particles were observed under transmission electron microscope. Primary cultures of bovine corneal epithelial cells were transfected with Ads and chitosan-modified Ads in the absence or presence of anti-adenovirus antibodies. Chitosan modification did not significantly change the particle size of Ad, but the surface charge of Ad increased significantly from -24.3 mV to nearly neutral. Furthermore, primary cultures of bovine corneal epithelial cells were transfected with Ad or chitosan-modified Ad in the absence or presence of anti-Ad antibodies. The transfection efficiency was attenuated gradually with increasing amounts of GMBS. However, incorporation of chitosan partly restored transfection activity and rendered the modified antibody resistant to antibody neutralization. CONCLUSIONS/SIGNIFICANCE: Chitosan can provide a platform for chemical modification of Ad, which offers potential for further in vivo applications
Studies on p53, BAX and Bcl-2 protein expression and microsatellite instability in stage III (UICC) colon cancer treated by adjuvant chemotherapy: major prognostic impact of proapoptotic BAX
We evaluated the expression patterns of proapoptotic BAX, antiapoptotic Bcl-2 and p53, the proposed upstream effector of these molecules, as potential prognostic markers in UICC stage III colon cancer by immunohistochemical staining. To identify high-frequency microsatellite instability (MSI+) individuals, we performed single-strand conformation polymorphism-based analysis for BAT26. A total of 188 patients who had received 5-fluorouracil (5-FU)-based adjuvant chemotherapy (5-FU/folinic acid or 5-FU/levamisole) were enrolled. Median follow-up was 84.5 months. We found that BAX, Bcl-2 and p53 protein expressions were high or positive in 59, 70 and 50% of 188 cases, respectively. MSI+ tumours were detected in 9% of 174 evaluable patients. BAX or Bcl-2 was correlated with a higher degree of differentiation or left-sided tumours (P=0.01 or P=0.03, respectively); MSI was correlated with right-sided tumours (P<0.0001). In contrast to p53, Bcl-2, or MSI, low BAX, advanced pN category, low grade of differentiation and treatment with 5-FU/levamisole were univariately associated with poorer disease-free survival (DFS) (P=0.0005, P=0.001, P=0.005 and P=0.01, respectively) and poorer overall survival (OS) (P=0.002, P=0.0001, P=0.003 and P=0.02, respectively). Besides pN category and treatment arm, BAX was an independent variable related to both OS and DFS (P=0.003 and P=0.001, respectively). In both univariate and multivariate analysis, the p53−/BAX high in comparison with the p53+/BAX high subset conferred a significantly improved DFS (P=0.03 and P=0.03, respectively) as well as a marginally improved OS (P=0.07 and P=0.08, respectively). BAX protein expression may be of central significance for clinical outcome to 5-FU-based adjuvant chemotherapy in stage III colon cancer, and bivariate analysis of p53/BAX possibly may provide further prognostic evidence
Eyewitness Identification and the Accuracy of the Criminal Justice System
This article addresses the problem of eyewitness identification errors that can lead to false convictions of the innocent and false acquittals of the guilty. At the heart of our analysis based on signal detection theory is the separation of diagnostic accuracy—the ability to discriminate between those who are guilty versus those who are innocent—from the consideration of the relative costs associated with different kinds of errors. Application of this theory suggests that current recommendations for reforms have conflated diagnostic accuracy with the evaluation of costs in such a way as to reduce the accuracy of identification evidence and the accuracy of adjudicative outcomes. The Influence of International Law on the International Movement of Persons
Many migration theories identify ‘the law’ as a significant constraint on the international
movement of persons. While this constraint often operates through national migration
legislation, this study examines the influence of international law in shaping contemporary
patterns in the international movement of persons at the macro level. The analysis begins with an
examination of the long-established power of a State to regulate cross-border movement of
persons as an inherent attribute of State sovereignty, together with the accepted limitations on a
State’s power to control entry and exit. Yet, international law reaches well beyond the movement
of people across borders. The development of international human rights law has been a key
constraint on state action in the United Nations era by also regulating the treatment of migrants
within a State’s borders. The study considers how international law has responded to current
migration issues, including: protection of migrant women and children; suppression of
smuggling and trafficking of people; labour migration; and environmental migration. As in other
areas of international society, there has been a proliferation of institutions through which
international migration law is made and enforced. The most prominent among them are the
United Nations High Commissioner for Refugees (UNHCR) and the International Organization
for Migration (IOM), but the establishment of other entities with overlapping mandates has given
rise to calls for a new international migration regime based on streamlined institutional
arrangements. The study concludes that international law is an imperfect framework for
regulating the international movement of persons because it has developed in a piecemeal
fashion over a long time to deal with issues of concern at particular points in human history. Yet,
despite its shortfalls, international law and its associated institutions unquestionably play a most
important role in constraining and channeling state authority over the international movement of
persons
