    Appraisal of some methods of weed control during initial establishment of cocoa in a semi-deciduous forest zone of Ghana

    In appraising the effects of the quantum and quality of weed control required in establishment of young cocoa, six manual methods of controlling weeds were compared with chemical weed control at two stations of the Cocoa Research Institute of Ghana. Growth of cocoa seedlings after 2 years in the field was superior (P < 0.05) in plots which were clean-weeded two or four times per year or treated with paraquat four times per year than when plots were slashed two or four times per year. There was strong evidence to suggest that treatments which ensured a weed-free environment to the cocoa over a long period were favourable to girth increments in the seedlings. Death of seedlings during establishment was generally lower in plots which were either clean-weeded or treated with paraquat. Labour requirement for controlling weeds with paraquat was considerably lower than that required for the other treatments. Considering the equipment and chemical input, paraquat application was expensive compared to high slashing with or without clean-line weeding two times a year, or clean weeding two times a year; but by virtue of its efficacy, it is recommended for cocoa establishment. En évaluant les effets du quatum et de la qualité de désherbage exigé dans la culture de jeune cacao, six méthodes de désherbage manuel étaient comparées avec le désherbage chimique à deux stations de l'Institut de Recherche en cacao du Ghana. La croissance de semis de cacao, après deux années, dans le champ était supérieure (P £ 0.05 ) dans les lots qui étaient complètement désherbés 2 ou 4 fois/an ou traités avec le paraquat 4 fois/an que dans les lots qui étaient entaillés 2 ou 4 fois/an. Il y avait de nombreuses preuves qui laissent penser que les traitements qui assuraient un environnement sans mauvaise herbe pour le cacao sur une longue période étaient favorables aux augmentations de circonférence des semis. La mort de semis pendant la culture était dans l'ensemble plus faible dans les lots, qui étaient soit complètement désherbés soit traités avec le paraquat. L'exigence de main-d'oeuvre pour maîtriser les mauvaises herbes avec le paraquat était considérablement plus faible que celle exigée pour les autres traitements. En ce qui concerne les matériels et les intrants chimiques, l'application de paraquat était chère que les entailles élevées avec ou sans le désherbage complet en ligne deux fois par an ou le désherbage complet deux fois par an mais en vertu de son efficacité il est recommandé pour la culture de cacao.     Determinants of early initiation of breastfeeding in Ghana: a population-based cross-sectional study using the 2014 Demographic and Health Survey data.

    BACKGROUND:The World Health Organisation (WHO) recommends that breastfeeding should be initiated within the first hour of delivery followed by exclusive breastfeeding up to 6 months. This study examined the determinants of early initiation of breastfeeding in Ghana using data from the 2014 Ghana Demographic and Health Survey. METHODS:A sample size of 4219 was used for the study. Descriptive statistics was conducted to ascertain the proportion of children who had early initiation of breastfeeding after which binary logistic regression analysis was carried out. Results were presented using frequencies, percentages, unadjusted and adjusted odds ratios. Statistical significance was pegged at p<0.05. RESULTS:Children of first birth order [AOR = 0.71, CI = 0.61-0.84], those who were delivered by non-professionals [AOR = 0.51, CI = 0.30-0.88] and those whose mothers were Traditionalists [AOR = 0.65, CI = 0.46-0.92] and Mole-Dagbanis [AOR = 0.69, CI = 0.54-0.89] were less likely to go through early initiation of breastfeeding compared to those of 2-4 birth order, those who were delivered by health professionals, those whose mothers were Christians and Akan, respectively. Conversely, children born to mothers who read newspaper/magazine at least once a week were more likely to go through early initiation of breastfeeding, compared to those who never read newspaper/magazine [AOR = 1.40, CI = 1.01-1.95]. Children born to mothers who watched television less than once a week were more likely to go through early initiation of breastfeeding compared to those who watched television at least once a week [AOR = 1.40, CI = 1.01-1.95]. Finally, women from the Northern [AOR = 2.40, CI = [1.77-3.26] and Upper East regions [AOR = 2.57, CI = [1.86-3.56] practiced early initiation of breastfeeding compared to those from the Ashanti region. CONCLUSIONS:Empowering healthcare providers to be consistent in early breastfeeding initiation advocacy and effective community engagement on the need to embrace and practice early initiation of breastfeeding can improve the situation

    Beyond counting induced abortions, miscarriages and stillbirths to understanding their risk factors: analysis of the 2017 Ghana maternal health survey

    BackgroundInasmuch as induced abortions, miscarriages and stillbirths constitute common adverse pregnancy outcomes contributing to poor maternal health, there is paucity of literature about these in Ghana. We investigated the factors associated with induced abortions, miscarriages and stillbirths in Ghana.MethodsData derived from the 2017 Ghana Maternal Health Survey was used in this study. Women aged 15-49 constituted the target for the study. This study examined the relationship between socio-demographic characteristics and induced abortions, stillbirths and miscarriages. Subsequently, multivariable binary logistic regression models were fitted to investigate the factors associated with induced abortions, stillbirths and miscarriages at 95 % confidence interval (CI).ResultsThe prevalence of miscarriages, induced abortions and stillbirths in Ghana in 2017 were 10.8 %, 10.4 % and 2 % respectively. Induced abortions (12.9 %) and miscarriages (11.1 %) were found to be higher among urban residents whiles rural residents had more of stillbirths (2.1 %). Compared to women aged 15-24, those in all age categories had lower odds of experiencing induced abortions, with the lowest odds occurring among women aged 35-49 (AOR = 0.26, 95 % CI = 0.21-32). Conversely, women of all age categories had higher odds of experiencing miscarriages compared to those aged 15-24 with the highest odds among those aged 25-34 (AOR = 1.62, 95 % CI = 1.39-1.89). Women with at least primary education were more likely to experience miscarriages than those with no formal education, with those with higher level of education having the highest odds (AOR = 1.42, 95 % CI = 1.13-1.78). While the likelihood of induced abortions was lower among Muslims, compared to Christians (AOR = 0.65, 95 % CI = 0.52-0.82), the odds of miscarriages were higher among Muslims, compared to Christians (AOR = 1.31, 95 % CI = 1.13-1.52). Women with parity 1 or more were less likely to experience induced abortions, miscarriages and stillbirths compared to those with parity 0.ConclusionsOur study indicates that efforts to limit induced abortions, miscarriages and stillbirths in Ghana need to focus on the disparities in socio-demographic characteristics of women. Synergy between government health institutions and the private sector cannot be left out if much success can be achieved in efforts to subside the current prevalence of induced abortions, stillbirths and miscarriages confronting the country

    Missed Opportunities for the Diagnosis of Pulmonary Tuberculosis in Ashanti and Western Regions of Ghana; Polymerase Chain Reaction, A Better Tool

    The diagnosis of Pulmonary Tuberculosis (PTB) has mainly relied on sputum microscopy and culture. The use of molecular techniques such as the Polymerase Chain Reaction (PCR) and its overwhelming advantages compared with conventional diagnostic methodologies cannot be overemphasized. The main aim of this work was to compare the diagnostic sensitivities of microscopy, culture and PCR. Suspected PTB sputum samples were prospectively collected from six hospitals in the Ashanti and Western regions of Ghana. Microscopy was carried out on all samples at the field sites. Apart from culture and PCR, repeat sputum microscopy was carried out in the laboratories of the Kumasi Centre for Collaborative Research (KCCR). Out of the total 425 cases recruited for the study, 123 (29.0%) were smear positive on site in contrast to 275 (64.7%) positivity rate at KCCR. Regarding culture, 254 (59.9%) samples were culture positive whilst PCR technique using INS 1&amp;2 and PR 8&amp;9 primers were positive in 59.9 % and 56% of cases respectively. The proportion of missed positive cases of microscopy were 131 (51%) compared with culture and 1.1% missed cases when compared with PCR. The sensitivity and specificity of microscopy were 123/254 (48.4%) and 154/302 (50.99%). The study confirms the superiority of PCR in the diagnosis of PTB and indicates that a substantial proportion of PTB cases are missed when microscopy alone is used. In areas where the incidence of PTB is high and at referral hospitals, PCR can be done to augment the diagnosis of TB.Keywords: Pulmonary Tuberculosis, Sputum Microscopy, Culture, Polymerase Chain Reactio

    Effect Of Planting Device And Seed Sorting On Yield Of Maize

    ABSTRACT: Experiments were conducted at the CSIR-Crops Research Experimental station at Kumasi, Ghana, to determine the effects of seed sorting and planting device on yield of maize in the 2014 major growing season. Two jab planters were used to plant sorted and unsorted seeds. The control treatment was cutlass planting of unsorted seed. One of the jab planters was imported from China; and the other was fabricated in Ghana. The experimental design was a randomized complete block with 3 replications. Planting one hectare of maize with the Chinese planter took about 10 hours, 36 minutes; the local planter took 12 hours 39 minutes, whilst cutlass took 29 hours 36 minutes. Seed sorting and planting device had no significant effect on maize yield. However it was faster, cheaper and economically more viable to plant with jab planter than with cutlass

    Objective: Characterize mycobacterial species causing pulmonary tuberculosis (PTB) at the Korle-Bu Teaching Hospital in Ghana.Design: Sputum smear positive samples, two (2) from 70 patients diagnosed as having tuberculosis, after they had consented, were collected from the Korle-Bu Teaching Hospital Chest Clinic betweenJanuary and July 2003. Setting: Korle-Bu Teaching Hospital Chest Clinic, Accra. Results: Sixty-four mycobacterial isolates wereobtained and confirmed as members of Mycobacterium tuberculosis complex by colonial morphology and conventional biochemical assays. Fortyseven (73%) were M. tuberculosis, the human strain, 2 (3%) M. bovis, the bovine strain, 13 (20%) M. africanum I (West Africa type), and 2 (3%) M. africanum II (East Africa type). Conclusion: The results indicate that, there are various strains causing PTB at the Korle-BuTeaching Hospital and of great concern is M. bovis, which mostly causes extra-PTB in humans but found to cause PTB in this study. This calls for the need to conduct a nationwide survey using bothconventional and molecular techniques to characterize various mycobacterial species causing TB in Ghana. This will result in better understanding of the various strains circulating in the country andinform individual TB treatment regimen especially the inclusion or exclusion of pyrazinamide

    TIME Impact - a new user-friendly tuberculosis (TB) model to inform TB policy decisions.

    Tuberculosis (TB) is the leading cause of death from infectious disease worldwide, predominantly affecting low- and middle-income countries (LMICs), where resources are limited. As such, countries need to be able to choose the most efficient interventions for their respective setting. Mathematical models can be valuable tools to inform rational policy decisions and improve resource allocation, but are often unavailable or inaccessible for LMICs, particularly in TB. We developed TIME Impact, a user-friendly TB model that enables local capacity building and strengthens country-specific policy discussions to inform support funding applications at the (sub-)national level (e.g. Ministry of Finance) or to international donors (e.g. the Global Fund to Fight AIDS, Tuberculosis and Malaria).TIME Impact is an epidemiological transmission model nested in TIME, a set of TB modelling tools available for free download within the widely-used Spectrum software. The TIME Impact model reflects key aspects of the natural history of TB, with additional structure for HIV/ART, drug resistance, treatment history and age. TIME Impact enables national TB programmes (NTPs) and other TB policymakers to better understand their own TB epidemic, plan their response, apply for funding and evaluate the implementation of the response.The explicit aim of TIME Impact's user-friendly interface is to enable training of local and international TB experts towards independent use. During application of TIME Impact, close involvement of the NTPs and other local partners also builds critical understanding of the modelling methods, assumptions and limitations inherent to modelling. This is essential to generate broad country-level ownership of the modelling data inputs and results. In turn, it stimulates discussions and a review of the current evidence and assumptions, strengthening the decision-making process in general.TIME Impact has been effectively applied in a variety of settings. In South Africa, it informed the first South African HIV and TB Investment Cases and successfully leveraged additional resources from the National Treasury at a time of austerity.     Abnormal Visuo-vestibular Interactions in Vestibular Migraine: a Cross Sectional Study

    Vestibular migraine is amongst the commonest causes of episodic vertigo. Chronically, patients with vestibular migraine develop abnormal responsiveness to both vestibular and visual stimuli characterised by heightened self-motion sensitivity and visually-induced dizziness. Yet, the neural mechanisms mediating such symptoms remain unknown. We postulate that such symptoms are attributable to impaired visuo-vestibular cortical interactions, which in-turn disrupts normal vestibular function. To assess this, we investigated whether prolonged, full-field visual motion exposure, which has previously been shown to modulate visual cortical excitability in both healthy individuals and avestibular patients, could disrupt vestibular ocular reflex (VOR) and vestibular-perceptual thresholds of self-motion during rotations. Our findings reveal that vestibular migraine patients exhibited abnormally elevated reflexive and perceptual vestibular thresholds at baseline. Following visual motion exposure, both reflex and perceptual thresholds were significantly further increased in vestibular migraine patients relative to healthy controls, migraineurs without vestibular symptoms and patients with episodic vertigo due to a peripheral inner-ear disorder. Our results provide support for the notion of altered visuo-vestibular cortical interactions in vestibular migraine, as evidenced by vestibular threshold elevation following visual motion exposure
