    Interventions for prevention of type 2 diabetes in relatives:A systematic review

    The relatives and partners of people with type 2 diabetes are at increased risk of developing type 2 diabetes. This systematic review examines randomized controlled trials, written in English that tested an intervention, which aimed to modify behaviors known to delay or prevent type 2 diabetes, among the relatives or partners of people with type 2 diabetes. Study quality was assessed using the Cochrane Collaboration’s tool for assessing risk of bias. Seven studies met the inclusion criteria. The majority of studies were at low risk of bias. Six studies tested an intervention in first-degree relatives of people with type 2 diabetes and one in partners. Intervention components and intervention intensity across studies varied, with those targeting diet and physical activity reporting the most significant changes in primary outcomes. Only one study did not observe significant changes in primary outcomes. There were three main recruitment approaches: advertising in the community, recruiting people through their relatives with diabetes, or identifying people as high risk by screening of their own health care contacts. Some evidence was found for potentially successful interventions to prevent type 2 diabetes among the relatives and partners of people with type 2 diabetes, although finding simple and effective methods to identify and recruit them remains a challenge. Future studies should explore the effect of patients’ perceptions on their family members’ behavior and capitalize on family relationships in order to increase intervention effectiveness

    Effects of a ruminally protected B vitamin supplement on milk yield and composition of lactating dairy cows

    It is not clear if B vitamins supplied to the small intestine of dairy cows from dietary and rumen microbial sources are provided in sufficient quantity to maximize animal performance. Our objective was to determine effects of adding a ruminally protected B vitamin blend supplement, containing biotin, folic acid, pantothenic acid and pyridoxine, to the diet of high producing dairy cows on their productivity. Two dairy facilities located in California (USA) were used, one with mid lactation Holstein cows (Experiment 1) and the other with early lactation Holstein cows (Experiment 2). In each Experiment, cows were randomly assigned to treatment in a 2×2 crossover design with 28 d (Experiment 1) or 35 d (Experiment 2) experimental periods. In Experiment 1, milk and milk fat yield were unaffected by treatment, although milk fat proportion was lower (37.1 versus 36.3 g/kg; P<0.01), but milk protein yield was higher (1.21 versus 1.24 kg/d; P=0.02) in cows fed B vitamins. In Experiment 2, milk (39.60 versus 40.46 kg/d; P=0.02), milk fat (1.40 versus 1.47 kg/d; P<0.01) and milk protein yield (1.10 versus 1.16 kg/d; P<0.01), as well as milk energy output (113.2 versus 117.8 MJ/d; P<0.01) were all higher with B vitamin feeding. Body condition score (BCS) increasedmore with B vitamin feeding in Experiment 2, but was unaffected in Experiment 1. Body locomotion score (BLS) increased with B vitamin feeding in both experiments (P=0.01 and < 0.01, respectively), possibly an indication of reduced locomotory ability.Overall, productivity of high producing lactating dairy cows responded positively to feeding a mixture of ruminally protected B vitamins, although differences in the extent of the positive responses between experiments perhaps suggests that early lactation cows, with lower DM intake to milk yield ratios, may be more responsive to ruminally protected B vitamins than mid lactation cows, with higher DM intake to milk yield ratios

    Is diagnosis of type 2 diabetes a “teachable moment”? A qualitative study

    Aims To explore the potential of a type 2 diabetes diagnosis to be a “teachable moment”. Methods Semi-structured interviews were conducted with 23 participants (10 people with type 2 diabetes, 13 relatives of people with type 2 diabetes) in Scotland, UK. They explored cognitive, emotional and behavioural changes following diagnosis of type 2 diabetes in oneself or in a relative. Data were analysed using Framework approach. Results Strong emotional responses are not always related to the occurrence of a teachable moment. Risk perception and outcome expectancy were found to be teachable moment factors for patients with type 2 diabetes and their offspring, but not their partners. Change in self-concept increases the likelihood of type 2 diabetes diagnosis to be a teachable moment for patients but not for relatives. In some cases, type 2 diabetes is perceived as incompatible with current roles thus hindering diabetes self-management. Relatives often engage in caring for patients and “policing” their behaviour but did not report perceived changes in social roles. Conclusions The study suggests that diagnosis of type 2 diabetes is a teachable moment for some patients and their relatives. These findings have implications for interventions to address diabetes self-management in patients and primary prevention in their relatives

    p-Type semiconducting properties in lithium-doped MgO single crystals

    The phenomenally large enhancement in conductivity observed when Li-doped MgO crystals are oxidized at elevated temperatures was investigated by dc and ac electrical measurements in the temperature interval 250-673 K. The concentration of ([Li]^{0}) centers (Li^{+} ions each with a trapped hole) resulting from oxidation was monitored by optical absorption measurements. Both dc and ac experiments provide consistent values for the bulk resistance. The electricalconductivity of oxidized MgO:Li crystals increases linearly with the concentration of ([Li]^{0}) centers. The conductivity is thermally activated with an activation energy of (0.70 +/- 0.01) eV, which is independent of the ([Li]^{0}) content. The \textit{standard semiconducting} mechanism satisfactorily explains these results. Free holes are the main contribution to band conduction as they are trapped at or released from the ([Li]^{0})-acceptor centers. In as-grown MgO:Li crystals, electrical current increases dramatically with time due to the formation of ([Li]^{0}) centers. The activation energy values between 1.3 and 0.7 eV are likely a combination of the activation energy for the creation of ([Li]^{0}) centers and the activation energy of ionization of these centers.     Micro-computed tomographic analysis of the radial geometry of intrarenal artery-vein pairs in rats and rabbits: Comparison with light microscopy

    We assessed the utility of synchrotron-radiation micro-computed tomography (micro-CT) for quantification of the radial geometry of the renal cortical vasculature. The kidneys of nine rats and six rabbits were perfusion fixed and the renal circulation filled with Microfil. In order to assess shrinkage of Microfil, rat kidneys were imaged at the Australian Synchrotron immediately upon tissue preparation and then post fixed in paraformaldehyde and reimaged 24 hours later. The Microfil shrank only 2-5% over the 24 hour period. All subsequent micro-CT imaging was completed within 24 hours of sample preparation. After micro-CT imaging, the kidneys were processed for histological analysis. In both rat and rabbit kidneys, vascular structures identified in histological sections could be identified in two-dimensional (2D) micro-CT images from the original kidney. Vascular morphology was similar in the two sets of images. Radial geometry quantified by manual analysis of 2D images from micro-CT was consistent with corresponding data generated by light microscopy. However, due to limited spatial resolution when imaging a whole organ using contrast-enhanced micro-CT, only arteries ≥100 and ≥60 μm in diameter, for the rat and rabbit respectively, could be assessed. We conclude that it is feasible and valid to use micro-CT to quantify vascular geometry of the renal cortical circulation in both the rat and rabbit. However, a combination of light microscopic and micro-CT approaches are required to evaluate the spatial relationships between intrarenal arteries and veins over an extensive range of vessel size

    Prediction of genetic merit for live weight and body condition score in dairy cows using routinely available linear type and carcass data

    peer-reviewedAccurate estimates of genetic merit for both live weight and body condition score (BCS) could be useful additions to both national- and herd-breeding programs. Although recording live weight and BCS is not technologically arduous, data available for use in routine genetic evaluations are generally lacking. The objective of the present study was to explore the usefulness of routinely recorded data, namely linear type traits (which also included BCS but only assessed visually) and carcass traits in the pursuit of genetic evaluations for both live weight and BCS in dairy cows. The data consisted of on-farm records of live weight and BCS (assessed using both visual and tactile cues) from 33,242 dairy cows in 201 commercial Irish herds. These data were complemented with information on 6 body-related linear type traits (i.e., stature, angularity, chest width, body depth, BCS, and rump width) and 3 cull cow carcass measures (i.e., carcass weight, conformation, and fat cover) on a selection of these animals plus close relatives. (Co)variance components were estimated using animal linear mixed models. The genetic correlation between the type traits stature, angularity, body depth, chest width, rump width, and visually-assessed BCS with live weight was 0.68, −0.28, 0.43, 0.64, 0.61, and 0.44, respectively. The genetic correlation between angularity and BCS measured on farm (based on both visual and tactile appraisal) was −0.79; the genetic and phenotypic correlation between BCS assessed visually as part of the linear assessment with BCS assessed by producers using both tactile and visual cues was 0.90 and 0.27, respectively. The genetic (phenotypic) correlation between cull cow carcass weight and live weight was 0.81 (0.21), and the genetic (phenotypic) correlation between cull cow carcass fat cover and BCS assessed on live cows was 0.44 (0.12). Estimated breeding values (EBV) for live weight and BCS in a validation population of cows were generated using a multitrait evaluation with observations for just the type traits, just the carcass traits, and both the type traits and carcass traits; the EBV were compared with the respective live weight and BCS phenotypic observations. The regression of phenotypic live weight on its EBV from the multitrait evaluations was 1.00 (i.e., the expectation) when the EBV was generated using just linear type trait data, but less than 1 (0.83) when using just carcass data. However, the regression changed across parities and stages of lactation. The partial correlation (after adjusting for contemporary group, parity by stage of lactation, heterosis, and recombination loss) between phenotypic live weight and EBV for live weight estimated using the 3 different scenarios (i.e., type only, carcass only, type plus carcass) ranged from 0.38 to 0.43. Although the prediction of phenotypic BCS from its respective EBV was relatively good when using just the linear type trait data (regression coefficient of 0.83 with a partial correlation of 0.22), the predictive ability of BCS EBV based on just carcass data was poor and should not be used. Overall, linear type trait data are a useful source of information to predict live weight and BCS with minimal additional predictive value from also including carcass data. Nonetheless, in the absence of linear type trait data, information on carcass traits can be useful in predicting genetic merit for mature cow live weight.     Process mapping of laser surface modification of AISI 316L stainless steel for biomedical applications

    A 1.5-kW CO2 laser in pulsed mode at 3 kHz was used to investigate the effects of varied laser process parameters and resulting morphology of AISI 316L stainless steel. Irradiance and residence time were varied between 7.9 to 23.6 MW/cm2 and 50 to 167 µs respectively. A strong correlation between irradiance, residence time, depth of processing and roughness of processed steel was established. The high depth of altered microstructure and increased roughness were linked to higher levels of both irradiance and residence times. Energy fluence and surface temperature models were used to predict levels of melting occurring on the surface through the analysis of roughness and depth of the region processed. Microstructural images captured by the SEM revealed significant grain structure changes at higher irradiances, but due to increased residence times, limited to the laser in use, the hardness values were not improved

    The liquid-vapor interface of an ionic fluid

    We investigate the liquid-vapor interface of the restricted primitive model (RPM) for an ionic fluid using a density-functional approximation based on correlation functions of the homogeneous fluid as obtained from the mean-spherical approximation (MSA). In the limit of a homogeneous fluid our approach yields the well-known MSA (energy) equation of state. The ionic interfacial density profiles, which for the RPM are identical for both species, have a shape similar to those of simple atomic fluids in that the decay towards the bulk values is more rapid on the vapor side than on the liquid side. This is the opposite asymmetry of the decay to that found in earlier calculations for the RPM based on a square-gradient theory. The width of the interface is, for a wide range of temperatures, approximately four times the second moment correlation length of the liquid phase. We discuss the magnitude and temperature dependence of the surface tension, and argue that for temperatures near the triple point the ratio of the dimensionless surface tension and critical temperature is much smaller for the RPM than for simple atomic fluids.Comment: 6 postscript figures, submitted to Phys.     Seizures after carotid endarterectomy: Hyperperfusion, dysautoregulation or hypertensive encephalopathy?

    AbstractObjectives: presentation, management and outcome following seizure after carotid endarterectomy (CEA). Materials and Design: prospective audit. Results: Eight patients (0.8%) suffered a seizure (three bilateral) <30 days following 949 CEAs. Seizure was not associated with age, gender or presentation. Seven were treated hypertensives but four had labile BP pre-operatively. Five had severe bilateral carotid disease and four had vertebral/subclavian stenoses. Six had a >50% drop in middle cerebral artery blood flow velocity (MCAV) with clamping. Only three had >100% increase in MCAV with flow restoration. Five required treatment for post-operative hypertension. Two suffered seizures <36 hrs of CEA, the remainder were at 3-8 days. All eight had significantly elevated blood pressure at onset of seizures. Four underwent immediate MCAV monitoring and each was elevated. Emergency CT scanning/autopsy showed normal scans (n = 3), white matter oedema (n = 3), oedema and diffuse haemorrhage (n = 1), intracranial haemorrhage (n = 1). Seven developed a post-ictal neurological deficit (stroke = 5, TIA = 2). Overall, two patients either died or suffered a disabling stroke. Conclusions: post-CEA seizure was associated with adverse outcome. Most were labile hypertensives with severe bilateral carotid/vertebral disease. MCAV changes suggested poor collateral recruitment, but no consistent pattern of early hyperperfusion emerged. It remains uncertain whether high MCAVs and severe hypertension after seizure onset are cause or effect. Clinicians treating these patients in acute medical units were generally unaware of the “post-CEA hyperperfusion syndrome” and tended to treat the hypertension less aggressively.Eur J Vasc Endovasc Surg 26, 39-44 (2003
