    Identification of Vehicle Health Assurance Related Trends

    Trend analysis in aviation as related to vehicle health management (VHM) was performed by reviewing the most current statistical and prognostics data available from the National Transportation Safety Board (NTSB) accident, the Federal Aviation Administration (FAA) incident, and the NASA Aviation Safety Reporting System (ASRS) incident datasets. In addition, future directions in aviation technology related to VHM research areas were assessed through the Commercial Aviation Safety Team (CAST) Safety Enhancements Reserved for Future Implementations (SERFIs), the National Transportation Safety Board (NTSB) Most-Wanted List and recent open safety recommendations, the National Research Council (NRC) Decadal Survey of Civil Aeronautics, and the Future Aviation Safety Team (FAST) areas of change. Future research direction in the VHM research areas is evidently strong as seen from recent research solicitations from the Naval Air Systems Command (NAVAIR), and VHM-related technologies actively being developed by aviation industry leaders, including GE, Boeing, Airbus, and UTC Aerospace Systems. Given the highly complex VHM systems, modifications can be made in the future so that the Vehicle Systems Safety Technology Project (VSST) technical challenges address inadequate maintenance crew's trainings and skills, and the certification methods of such systems as recommended by the NTSB, NRC, and FAST areas of change

    Systems Analysis of NASA Aviation Safety Program: Final Report

    A three-month study (February to April 2010) of the NASA Aviation Safety (AvSafe) program was conducted. This study comprised three components: (1) a statistical analysis of currently available civilian subsonic aircraft data from the National Transportation Safety Board (NTSB), the Federal Aviation Administration (FAA), and the Aviation Safety Information Analysis and Sharing (ASIAS) system to identify any significant or overlooked aviation safety issues; (2) a high-level qualitative identification of future safety risks, with an assessment of the potential impact of the NASA AvSafe research on the National Airspace System (NAS) based on these risks; and (3) a detailed, top-down analysis of the NASA AvSafe program using an established and peer-reviewed systems analysis methodology. The statistical analysis identified the top aviation "tall poles" based on NTSB accident and FAA incident data from 1997 to 2006. A separate examination of medical helicopter accidents in the United States was also conducted. Multiple external sources were used to develop a compilation of ten "tall poles" in future safety issues/risks. The top-down analysis of the AvSafe was conducted by using a modification of the Gibson methodology.     Standardising Costs or Standardising Care? Qualitative Evaluation of the Implementation and Impact of a Hospital Funding Reform in Ontario, Canada

    Background  Since 2011, the Government of Ontario, Canada, has phased in hospital funding reforms hoping to encourage standardised, evidence-based clinical care processes to both improve patient outcomes and reduce system costs. One aspect of the reform – quality-based procedures (QBPs) – replaced some of each hospital’s global budget with a pre-set price per episode of care for patients with specific diagnoses or procedures. The QBP initiative included publication and dissemination of a handbook for each of these diagnoses or procedures, developed by an expert technical group. Each handbook was intended to guide hospitals in reducing inappropriate variation in patient care and cost by specifying an evidence-based episode of care pathway. We explored whether, how and why hospitals implemented these episode of care pathways in response to this initiative. Methods  We interviewed key informants at three levels in the healthcare system, namely individuals who conceived and designed the QBP policy, individuals and organisations supporting QBP adoption, and leaders in five case-study hospitals responsible for QBP implementation. Analysis involved an inductive approach, incorporating framework analysis to generate descriptive and explanatory themes from data. Results  The 46 key informants described variable implementation of best practice episode of care pathways across QBPs and across hospitals. Handbooks outlining evidence-based clinical pathways did not address specific barriers to change for different QBPs nor differences in hospitals’ capacity to manage change. Hospitals sometimes found it easier to focus on containing and standardising costs of care than on implementing standardised care processes that adhered to best clinical practices. Conclusion  Implementation of QBPs in Ontario’s hospitals depended on the interplay between three factors, namely complexity of changes required, internal capacity for organisational change, and availability and appropriateness of targeted external facilitators and supports to manage change. Variation in these factors across QBPs and hospitals suggests the need for more tailored and flexible implementation supports designed to fit all elements of the policy, rather than one-size-fits-all handbooks alone. Without such supports, hospitals may enact quick fixes aimed mainly at preserving budgets, rather than pursue evidence- and value-based changes in care management. Overestimating hospitals’ change management capacity increases the risk of implementation failure

    Three-Dimensional Microfluidic Tri-Culture Model of the Bone Marrow Microenvironment for Study of Acute Lymphoblastic Leukemia

    Acute lymphoblastic leukemia (ALL) initiates and progresses in the bone marrow, and as such, the marrow microenvironment is a critical regulatory component in development of this cancer. However, ALL studies were conducted mainly on flat plastic substrates, which do not recapitulate the characteristics of marrow microenvironments. To study ALL in a model of in vivo relevance, we have engineered a 3-D microfluidic cell culture platform. Biologically relevant populations of primary human bone marrow stromal cells, osteoblasts and human leukemic cells representative of an aggressive phenotype were encapsulated in 3-D collagen matrix as the minimal constituents and cultured in a microfluidic platform. The matrix stiffness and fluidic shear stress were controlled in a physiological range. The 3-D microfluidic as well as 3-D static models demonstrated coordinated cell-cell interactions between these cell types compared to the compaction of the 2-D static model. Tumor cell viability in response to an antimetabolite chemotherapeutic agent, cytarabine in tumor cells alone and tri-culture models for 2-D static, 3-D static and 3-D microfluidic models were compared. The present study showed decreased chemotherapeutic drug sensitivity of leukemic cells in 3-D tri-culture models from the 2-D models. The results indicate that the bone marrow microenvironment plays a protective role in tumor cell survival during drug treatment. The engineered 3-D microfluidic tri-culture model enables systematic investigation of effects of cell-cell and cell-matrix interactions on cancer progression and therapeutic intervention in a controllable manner, thus improving our limited comprehension of the role of microenvironmental signals in cancer biology

    Cross-taxonomic surrogates for biodiversity conservation in human-modified landscapes – A multi-taxa approach

    Cross-taxonomic surrogates are often used in conservation planning because inventorying large suites of taxa is either not feasible or too costly. However, cross-taxonomic surrogates are seldom tested rigorously using both correlational and representation-based approaches at the spatial scales at which conservation management occurs. Here, we evaluated the effectiveness of five ecologically contrasting taxa (birds, herpetofauna, wild bees, beetles, trees) as cross-taxonomic surrogates in native woodland patches within a heavily modified, farming and plantation-dominated landscape. We first compared species richness and compositional heterogeneity across taxa before testing for cross-taxonomic congruence using a correlative approach. We then quantified how well each taxon incidentally represented other taxa in their best patch sets, and the costs of doing so using a complementarity-based approach. We found significant pairwise associations between some taxa (birds, bees), but no single taxon was strongly correlated with all other taxa. Woodland patch sets prioritised for beetles represented other taxa best, followed by birds, but were the costliest and required the largest amount of woodland. This contrasted with patch sets prioritised for wild bees or herpetofauna, which achieved higher representation of other taxa at lower costs. Our study highlighted the influence of taxon-specific patterns of diversity and heterogeneity on how remnant vegetation patches should be prioritised for conservation, a consideration not immediately obvious in correlative analyses of surrogacy. Second, taxa that are not the most speciose (e.g. wild bees) can be efficient surrogates for other taxa, achieving higher incidental representation at lower costs. Thus, while species-rich taxa are ideal as surrogates for prioritising conservation, conservation planners should not overlook the potential of less speciose taxa such as bees, while considering the cost-effectiveness of surveying multiple different taxa.DLY is supported by the Lesslie Foundation and an Australian National University Postgraduate Scholarship.     Aviation Trends Related to Atmospheric Environment Safety Technologies Project Technical Challenges

    Current and future aviation safety trends related to the National Aeronautics and Space Administration's Atmospheric Environment Safety Technologies Project's three technical challenges (engine icing characterization and simulation capability; airframe icing simulation and engineering tool capability; and atmospheric hazard sensing and mitigation technology capability) were assessed by examining the National Transportation Safety Board (NTSB) accident database (1989 to 2008), incidents from the Federal Aviation Administration (FAA) accident/incident database (1989 to 2006), and literature from various industry and government sources. The accident and incident data were examined for events involving fixed-wing airplanes operating under Federal Aviation Regulation (FAR) Parts 121, 135, and 91 for atmospheric conditions related to airframe icing, ice-crystal engine icing, turbulence, clear air turbulence, wake vortex, lightning, and low visibility (fog, low ceiling, clouds, precipitation, and low lighting). Five future aviation safety risk areas associated with the three AEST technical challenges were identified after an exhaustive survey of a variety of sources and include: approach and landing accident reduction, icing/ice detection, loss of control in flight, super density operations, and runway safety

    Ice sheet record of recent sea-ice behavior and polynya variability in the Amundsen Sea, West Antarctica

Proxy records can extend these observations but require further development and validation. We compare methanesulfonic acid (MSA) and chloride (Cl–) concentrations from a new firn core from coastal West Antarctica with satellite-derived observations of regional sea-ice concentration (SIC) in the Amundsen Sea (AS) to evaluate spatial and temporal correlations from 2002–2010. The high accumulation rate (~39 g∙cm–2∙yr–1) provides monthly resolved records of MSA and Cl–, allowing detailed investigation of how regional SIC is recorded in the ice-sheet stratigraphy. Over the period 2002–2010 we find that the ice-sheet chemistry is significantly correlated with SIC variability within the AS and Pine Island Bay polynyas. Based on this result, we evaluate the use of ice-core chemistry as a proxy for interannual polynya variability in this region, one of the largest and most persistent polynya areas in Antarctica. MSA concentrations correlate strongly with summer SIC within the polynya regions, consistent with MSA at this site being derived from marine biological productivity during the spring and summer. Cl– concentrations correlate strongly with winter SIC within the polynyas as well as some regions outside the polynyas, consistent with Cl– at this site originating primarily from winter sea-ice formation. Spatial correlations were generally insignificant outside of the polynya areas, with some notable exceptions. Ice-core glaciochemical records from this dynamic region thus may provide a proxy for reconstructing AS and Pine Island Bay polynya variability prior to the satellite era.This research was supported by an award from the Department of Energy Office of Science Graduate Fellowship Program (DOE SCGF) to ASC, a James E. and Barbara V.     Causal Factors and Adverse Events of Aviation Accidents and Incidents Related to Integrated Vehicle Health Management

    Causal factors in aviation accidents and incidents related to system/component failure/malfunction (SCFM) were examined for Federal Aviation Regulation Parts 121 and 135 operations to establish future requirements for the NASA Aviation Safety Program s Integrated Vehicle Health Management (IVHM) Project. Data analyzed includes National Transportation Safety Board (NSTB) accident data (1988 to 2003), Federal Aviation Administration (FAA) incident data (1988 to 2003), and Aviation Safety Reporting System (ASRS) incident data (1993 to 2008). Failure modes and effects analyses were examined to identify possible modes of SCFM. A table of potential adverse conditions was developed to help evaluate IVHM research technologies. Tables present details of specific SCFM for the incidents and accidents. Of the 370 NTSB accidents affected by SCFM, 48 percent involved the engine or fuel system, and 31 percent involved landing gear or hydraulic failure and malfunctions. A total of 35 percent of all SCFM accidents were caused by improper maintenance. Of the 7732 FAA database incidents affected by SCFM, 33 percent involved landing gear or hydraulics, and 33 percent involved the engine and fuel system. The most frequent SCFM found in ASRS were turbine engine, pressurization system, hydraulic main system, flight management system/flight management computer, and engine. Because the IVHM Project does not address maintenance issues, and landing gear and hydraulic systems accidents are usually not fatal, the focus of research should be those SCFMs that occur in the engine/fuel and flight control/structures systems as well as power systems
