    Estudo de prevalência de depressão e síndrome cerebral orgânica na população de idosos, Brasil

    Data from a prevalence study of Organic Cerebral Syndrome and Depression in an elderly population living in three boroughs of Rio de Janeiro city are presented. The methodological issues related to interrater and test-retest reliability are discussed and the cut-off point for the instrument adapted (BOAS) established. The prevalence rates in the three boroughs were found, respectively, to be: 5.9%, 9.8% and 29.8% for Organic Cerebral Syndrome and 20.9%, 23.0% and 36.8% for Depression. The prevalence rats have been adjusted using information on sensitivity and specificity for both diagnoses. Aspects of these differences are discussed in the light of national and international literature.São apresentados dados de um estudo de prevalência de síndrome cerebral orgânica e depressão em uma população de idosos em três distritos da cidade do Rio de Janeiro, RJ, Brasil, assim como discutem aspectos metodológicos relacionados com a confiabilidade interavaliadores e teste-reteste e estabelecimento do ponto de corte do instrumento utilizado (BOAS). As taxas de prevalência de síndrome cerebral orgânica variaram de 5,9%, 9,8% e 29,8% entre os distritos estudados, enquanto as taxas de Depressão variaram de 20,9%, 23,0% e 36,8%. Foram ainda calculadas as taxas de prevalência corrigidas pelos dados de sensibilidade e especificidade para ambos os diagnósticos.     Prevalência de consumo de bebidas alcoólicas e de alcoolismo em uma região metropolitana do Brasil

    Foi realizado Inquérito epidemiológico com o objetivo de estimar o uso de substâncias psicoativas e a prevalência de alcoolismo. Tomou-se como referência a população de maiores de 13 anos de idade de região administrativa da cidade do Rio de Janeiro, RJ (Brasil), da qual se extraiu uma amostra aleatória de 1.459 indivíduos. Os resultados referentes ao uso de bebidas alcoólicas e alcoolismo, identificados através do teste CAGE, mostraram: prevalência de 51% para o consumo de álcool e de 3% para alcoolismo, sendo 4,9% em homens e 1,7% em mulheres; maior proporção de consumidores de álcool e de alcoolistas entre homens de 30 e 49 anos; abstinência mais freqüente entre os viúvos, protestantes e indivíduos com níveis de renda inferiores.An epidemiological survey was carried out for the purpose of estimating the use of psychoactive substances and the prevalence of alcoholism. The target population consisted of people over 13 years old living in a district of Rio de Janeiro city - Brazil. A random sample of 1,459 people was researched. Data on the use of alcohol and on alcoholism are presented. The diagnosis of this latter, based upon the CAGE Test, showed that 51% used alcohol and 3% were suspected of alcoholism: 4.9% and 1.7% among men and women, respectively. The greatest prevalence of the use of alcohol and of alcoholism was found among men between 30 and 49 years of age. Abstinence from alcohol was more frequent among widowed, Evangelical and low-income groups

    Current practices in managing acutely disturbed patients at three hospitals in Rio de Janeiro-Brazil: a prevalence study

    BACKGROUND: The medical management of aggressive and violent behaviour is a critical situation for which there is little evidence. In order to prepare for a randomised trial, due to start in the psychiatric emergency rooms of Rio de Janeiro in 2001, a survey of current practice was necessary. METHODS: A seven day survey of pharmacological management of aggressive people with psychosis in the emergency rooms of all four public psychiatric hospitals in Rio de Janeiro, Brazil. RESULTS: In one hospital data were not available. Of the 764 people with psychosis attending these ERs, 74 were given IM medication for rapid tranquillisation (9.7%, 2.1/week/100,000). A haloperidol-promethazine mix (with or without other drugs) was used for the majority of patients (83%). CONCLUSION: The haloperidol-promethazine mix, given intramuscularly for rapid tranquilization, is prevalent in Rio, where it is considered both safe and efficient. However, scientific evaluation of all pharmacological approaches to rapid tranquilization of psychotic people is inadequate or incomplete and a randomized trial of IM haloperidol-promethazine is overdue

    Violence and post-traumatic stress disorder in Sao Paulo and Rio de Janeiro, Brazil: the protocol for an epidemiological and genetic survey

    Background: violence is a public health major concern, and it is associated with post-traumatic stress disorder and other psychiatric outcomes. Brazil is one of the most violent countries in the world, and has an extreme social inequality. Research on the association between violence and mental health may support public health policy and thus reduce the burden of disease attributable to violence. the main objectives of this project were: to study the association between violence and mental disorders in the Brazilian population; to estimate the prevalence rates of exposure to violence, post-traumatic stress disorder, common metal disorder, and alcohol hazardous use and dependence: and to identify contextual and individual factors, including genetic factors, associated with the outcomes.Methods/design: one phase cross-sectional survey carried out in São Paulo and Rio de Janeiro, Brazil. A multistage probability to size sampling scheme was performed in order to select the participants (3000 and 1500 respectively). the cities were stratified according to homicide rates, and in São Paulo the three most violent strata were oversampled. the measurements included exposure to traumatic events, psychiatric diagnoses (CIDI 2.1), contextual (homicide rates and social indicators), and individual factors, such as demographics, social capital, resilience, help seeking behaviours. the interviews were carried between June/2007 February/2008, by a team of lay interviewers. the statistical analyses will be weight-adjusted in order to take account of the design effects. Standardization will be used in order to compare the results between the two centres. Whole genome association analysis will be performed on the 1 million SNP (single nucleotide polymorphism) arrays, and additional association analysis will be performed on additional phenotypes. the Ethical Committee of the Federal University of São Paulo approved the study, and participants who matched diagnostic criteria have been offered a referral to outpatient clinics at the Federal University of São Paulo and Federal University of Rio de Janeiro

Rio de Janeiro, RJ, Brasil.The yellow fever 17D virus is attenuated and used for human vaccination. Two of its substrains, 17D-204 and 17DD, are used for vaccine production. One of the remarkable properties of this vaccine is limited viral replication in the host but with significant dissemination of the viral mass, yielding a robust and long-lived neutralizing antibody response. The vaccine has excellent records of efficacy and safety and is cheap, used as a single dose, and there are well-established production methodology and quality control procedures which include the monkey neurovirulence test (MNTV). The present study aims at a better understanding of YF 17DD virus attenuation and immunogenicity in the MNVT with special emphasis on viremia, seroconversion, clinical and histological lesions scores, and their intrinsic variability across the tests. Several MNVTs were performed using the secondary seed lot virus 17DD 102/84 totaling 49 rhesus monkeys. Viremia was never higher than the accepted limits established in international requirements, and high levels of neutralizing antibodies were observed in all animals. None of the animals showed visceral lesions. We found that the clinical scores for the same virus varied widely across the tests. There was a direct correlation between the clinical scores in animals with clinical signs of encephalitis and a higher degree of central nervous system (CNS) histological lesions, with an increase of lesions in areas of the CNS such as the substantia nigra, nucleus caudatus, intumescentia cervicalis, and intumescentia ventralis. The histological scores were shown to be less prone to individual variations and had a more homogeneous value distribution among the tests. Since 17DD 102/84 seed virus has been used for human vaccine production and immunization for 16 years with the vaccine being safe and efficacious, it demonstrates that the observed variations across the MNVTs do not influence its effect on humans

    Conhecimentos e atitudes relativas a SIDA/AIDS em uma população de favela do Rio de Janeiro

    Com o objetivo de avaliar o grau de conhecimento e atitudes relativos à SIDA/AIDS, realizou-se um inquérito numa amostra de moradores da Favela da Rocinha, Rio de Janeiro, com idades entre 13 e 45 anos. Optou-se pela metodologia denominada Rapid Survey Methodology, sobretudo pelo seu baixo custo e por permitir um conhecimento quase que imediato das questões de maior interesse. Embora a maior parte dos indivíduos da amostra tenha demonstrado um grau elevado de conhecimento no que se refere às formas de transmissão sexual do VIH, mais da metade destes referiu a picada de mosquito, a doação de sangue e o beijo na boca como meio de adquirir a SIDA/AIDS. Com relação ao uso de preservativos, apenas 57,6% dos entrevistados acreditaram tratar-se de um método seguro de prevenção, e somente 21,9% referiram utilizá-lo, ainda que de modo irregular
