    Emphysematous Pyelonephritis In Patient with Diabetes and Renal Cell Carcinoma, A case report

    Emphysematous pyelonephritis (EPN) usually happens in diabetes. It is reported a correlation between the Renal Cell Carcinoma (RCC) with diabetes. This is a rare case of PEN and RCC in diabetes which occur simultaneously. We present a case of 51 year old male referred with right flank mass and medical history of diabetic disease. Renal ultrasound revealed a solid tumor at the upper pole of the right kidney. Plain abdominal X-ray showed a lot of gas, fecal material, and patches of calcification in the right upper abdomen. On abdominal computed tomography found a collection of gas located in the upper right abdomen. An irregular solid tumor detected at the upper pole of the right kidney. The tumors was obtained at surgery and emphysematous pyelonephritis was found and then nephrectomy performed. Histopathological examination revealed a Renal Cell Carcinoma besides emphy-sematous pyelonephritis. The patient had no postoperative serious complications during his hospital stay and his symptoms resolved completely.     Mediastinal T-Cell Lymphoblastic Lymphoma: A Rare Case in a Limited Resources Setting

    Background: Lymphoblastic Lymphoma (LBL) is a rare neoplasm which represents 1-2% of all Non-Hodgkin’s Lymphomas (NHLs).Objectives: To present challenges in diagnostic of a rare case in limited resources settingCase Presentation: A 36-year-old male came to Dr. Soetomo hospital with shortness of breath, facial edema and chest pain. Physical examination revealed a decreased breath sound and dull percussion in the right chest. CT scan showed solid mass 10.4 x 5.9 x 14.6 cm in the anterior - medius of mediastinum. Fine needle aspiration biopsy guided by CT scan showed the spread of lymphoid cells, big in size, anaplastic, rounded and hyperchromatic nuclei. Core biopsy showed diffuse and monotonous pattern of pleomorphic cells, round to oval and hyperchromatic nuclei. Immunostaining were positive for CD3 and Tdt, high Ki67 proliferation index 98%, and negative for CD20.Conclusion: T-LBL is a rare neoplasm, that generally occurs in adolescents and young adults, and is most common in males. It is an aggressive neoplasm, which can involve mediastinal mass and cause superior vena cava syndrome. Immunostaining would be positive for Tdt, and variably express CD1a, CD2, CD3, CD4, CD5, CD7, and CD8. The prognosis was poor due to the aggressiveness and the relapse

    CORRELATION BETWEEN HORMONAL STATUS OF ESTROGEN RECEPTOR AND MALIGNANCY DEGREE OF INVASIVE DUCTAL BREAST CANCER

    Background: Globocan data in 2018 shows the cancer incidence in Indonesia. The highest case in women is breast cancer. The chance of survival for patients with high malignancy (Grade III) is only 11.86%, while for patients with low malignancy (Grade I) can reach 71.69%. Estrogen exposure is a risk factor for breast cancer. The hormone causes its effect through the estrogen receptor which is a core protein. Patients with positive receptor have 5 and 10 years survival rate which is better than patients with negative estrogen receptor. Objective: To identify correlation between hormonal status of estrogen receptor and malignancy degree of invasive ductal breast cancer. Materials and Methods: This research was an observational analytic study. The sample used was patients’ medical record data in 2015-2017 in Anatomic Pathology Unit, Central Laboratory Installation, Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. Results: The data used were 694 patients medical record in 2015-2017. Approximately 56 patients (86%) had hormonal status of positive estrogen receptor with grade I cancer, 180 patients (70%) were in grade II, and 217 patients (45%) were in grade III. Approximately 9 patients (14%) had hormonal status of negative estrogen receptor with grade I cancer, 71 patients (28%) were in grade II, and 161 patients (34%) were in grade III. The result using Chi-Square test found Chi-Square value of 27.413 with a significance of 0.000 (p <0.01). Conclusion: There was a positive correlation between the hormonal status of estrogen receptor and malignancy degree of invasive ductal breast cancer

    Risk Factor Profile and Quality of Life of Psoriasis Vulgaris

    Background: Psoriasis is an autoimmune human skin disease. It is a chronic and recurrent lesion characterized by erythema patches with well-defined borders with rough, multi-layered, and transparent scales with a significant negative impact on patients' quality of life. Several risk factors for psoriasis can decrease patient’ quality of life. Purpose: To determine the risk factor profile and quality of life of psoriasis patients at the dermatology and venereology outpatient clinic at Dr. Soetomo General Academic Hospital Surabaya.  Methods: This was a cross-sectional study involving new and control psoriasis patients. Their risk factors were evaluated using questionnaires, and their quality of life was measured using the DLQI questionnaires. The data was collected at the Dermatology and Venereology outpatient unit Dr. Soetomo General Academic Hospital Surabaya. Result: Of the 24 patients, the biggest risk factor for psoriasis patients was trauma/ Koebner phenomenon, as reported in 15 patients (62.5%), and 11 patients’ (45.8%) quality of life was affected. Conclusion: Trauma was the largest risk factor for psoriasis, and psoriasis has significantly affected patient’ quality of life

    ABSTRACT Cervical cancer is a malignant tumour that attacks the female genital area originating from epithelial metaplasia in the squamous protocol junction area. One method of diagnosis of cervical cancer is to do a Pap smear examination by taking a cervical cell smear from the woman’s cervix and observing its cell development. However, examination of cervical cancer from Pap smear results usually takes a long time. This is because medical practitioners still rely on visual observations in the analysis of the results of Pap smear so that the results are subjective. Therefore, we need a programme that can help the classification process in establishing a diagnosis of cervical cancer with high accuracy results. In this study, a cervical cancer classification program was developed using a combination of the Grey Level Co-occurrence Matrix (GLCM) and Extreme Learning Machine (ELM) methods. There are three classes of cervical cell images classified, namely adenocarcinoma, High Squamous Intraepithelial Lesion (HSIL) and Squamous Cell Carcinoma (SCC). From the results of the training program obtained an accuracy 100% and from the testing program obtained an accuracy of 80%

    Free radicals and inflammation in tumor microenvironment are important aspects of tumor biological activity. They activated NF-ÎşB p65 transcription factor pathway. HSP70 is chaperone protein that regulated host of imune response to cancer cells. NF-ÎşB p65 and HSP70 expression is associated with poor prognosis and new targeted therapy in colorectal adenocarcinoma.&nbsp; This study aims to analyze correlation between NF-ÎşB p65 expression in macrophages and HSP70 in tumor cells with various T stages of colorectal adenocarcinoma. This cross-sectional study was performed on 48 paraffin blocks from colorectal adenocarcinoma at the Anatomical Pathology Laboratory of Dr. Soetomo General Academic Hospital Surabaya from January 2015 to December 2019.&nbsp; The samples were divided into four groups based on T stages of colorectal adenocarcinoma. Immunohistochemical staining was performed to detect NF-ÎşB p65 expression in nucleus or cytoplasm of macrophages and HSP70 in cytoplasm of tumor cells. There was no significant difference of NF-ÎşB p65 expression percentage in macrophages at four groups (p=0.31). There was significant differences of HSP70 expression percentage in tumor cells at four groups (p=0.014). There was no correlation between NF-ÎşB p65 expression in macrophages and HSP70 expression in tumor cells with various T stages of colorectal adenocarcinoma (rs=0.05, p=0.97)

    Mediastinal lymphoma is a rare tumor. Approximately 10% of primary mediastinal lymphoma occur in mediastinum. In this study we look for sex and age characteristics, histopathology and immunohistochemical profile of mediastinal lymphoma. Data were collected from the Anatomical Pathology laboratory Dr. Soetomo archives from January 2016 to August 2018. Seventeen cases of mediastinal lymphoma were found predominantly in males (76.47 %). The age range was between 17 and 64 years old. Distribution of tumor type was as follows: Non-Hodgkin’s Lymphoma, B-cell type high grade (Ki67 &gt; 30%) 17.65 % and Non-Hodgkin’s Lymphoma, B-cell type low grade (Ki67 &lt;30%) 11.76%, Non-Hodgkin’s Lymphoma, T-cell type high grade 11.76%, Non-Hodgkin’s Lymphoma unknown subtype 35.29%, Classical Hodgkin’s Lymphoma (CHL) 17.65%, T-cell Lymphoblastic Lymphoma (T-LBL) 5.88%. Histologically, Non-Hodgkin’s Lymphoma showed a diffuse pattern consisting of anaplastic lymphoid cells, pleomorphic nuclei, coarse chromatin, thin cytoplasm between fibrotic stroma and the immunohistochemistry profile showed positive for CD20 in NHL B-cell type. CD3 were positive in Peripheral T-cell Lymphoma and T-Lymphoblastic Lymphoma (T-LBL). Classical Hodgkin’s Lymphoma showed large cells (reed-stenberg cells) and Hodgkin’s cells dispersed between an inflammatory background, CD30 were positive in CHL cases. Tdt was positive in T-LBL cases.&nbsp
