    Fully endoscopic interlaminar and transforaminal lumbar discectomy: clinical results of 857 surgically treated patients

    Minimally invasive surgical techniques are becoming increasingly common in spinal surgery in an attempt to decrease tissue trauma during surgery, which in turn decreases post-operative pain and opioid use.The aim of this study was to investigate the clinical outcomes of a large group of patients with lumbar disc disease who underwent full endoscopic surgery. 857 patients who underwent fully endoscopic lumbar discectomy were investigated retrospectively. General demographics were evaluated in terms of mean operating time, mean length of hospital stay, mean time to return to work, complications and recurrences. Clinical outcomes were evaluated using visual analogue scale (VAS) for low back and leg pain, and Oswestry Disability Index (ODI) for functional assessment. The mean follow-up time was 36 months. The operation time ranged from 20 to 65 minutes (39 min on average), and there was no measurable intraoperative blood loss. Patients were mobilised on average 3.5 hours after the surgery. The length of hospital stay after the operation was 12–24 hours (18 hours on average). During postoperative follow-up, 19 patients required revision surgery due to recurrence (2.2%). There was one case of nerve root injury in which the patient recovered well following physical therapy, and 11 cases of paresthesia that gradually improved following 2–6 weeks of rehabilitation and treatment with pregabalin. All types of lumbar disc herniations are accessible via the full-endoscopic technique. Full-endoscopic discectomy, as a minimally invasive procedure, has the technical advantages of less paraspinal muscle dissection, less tissue trauma, less risk of spinal instability, and minimal blood loss

    The Effect of Air Pollution on Respiratory System Disease Admissions and Health Expenditures

    Objectives: Air pollution increases hospital admissions due to respiratory system diseases. This study aims to investi-gate health expenditures due to hospital admissions in Erzincan, Turkey.Methods: We acquired data on daily admissions of patients with respiratory system diseases from 2015 to 2016 in Erzincan from Mengücek Gazi Education and Research Hospital (MGERH). Concurrent air quality monitoring data were obtained from the Ministry of Environment and Urbanization, and meteorological data from General Directorate of Meteorology. Risk analyses were performed via the STATA® program, followed analyses of health expenditures based on patient treatment costs obtained from the Social Security Institution.Results: In Erzincan, a total of 78.793 patients (56.5% female, 43.5% male) presented to the emergency service and clinical departments of MGERH with respiratory complaints. The highest frequency diagnosis was upper respiratory system infections and most patients fell into the age groups of 15-34 and 45-64. A correlation was obtained between the amount of particulate matter 10 ?m and smaller, one of the air quality parameters monitored in Erzincan.Conclusion: Increases in air pollution levels similarly increase the number of hospital admissions secondary to respira-tory system diseases by 0.72%. According to Social Security Institution data, average per capita health expenditures related to these admissions were 44.338.98 USD

    Internet addiction, fatigue, and sleep problems among adolescent students: a large-scale study

    Aim: The aim of the present study was to examine the association between Internet Addiction (IA), fatigue, and sleep problems among university students. Methods: A total of 3,000 Turkish students aged 18 to 25 years were approached and 2,350 students (78.3%) participated in this cross-sectional study from April 2017 to September 2017 in public and private universities in Istanbul. Data were collected via a structured questionnaire including socio-demographic details, lifestyle and dietary habits, Internet Addiction Test (IAT), Fatigue Scale, and Epworth Sleepiness Scale [ESS]. Descriptive statistics, multivariate and factorial analyses were performed. Results: The overall prevalence of IA among the studied population was 17.7%. There were significant differences between gender, family income, father’s occupation, school performance, frequency and duration of watching television, physical activity, internet use duration, and sleep duration (all p<0.001). Significant differences were also found between participants with IA and those without IA in having headaches, blurred vision, double vision, hurting eyes, hearing problems, and eating fast food frequently (all p<0.001). Using multivariate regression analysis, the duration of internet use, physical and mental symptoms, headache, hurting eyes, tired eyes, hearing problems and ESS scores were significantly associated with (and primary predictors of) IA. Conclusion: The present study demonstrated that IA was associated with poor dietary habits, sleep problems, and fatigue symptoms
