    Old maps and the effectiveness of chitosan as antimicrobial agent

    [EN] In the case of tangible cultural heritage items made of natural fibres, biodegradation due to microorganisms can lead over time to undesirable deterioration, including physical, mechanical and chemical damage as well as aesthetic alteration of materials. In this study, the antifungal activity tests were performed on 3 old maps (on silk, on canvas and on paper) using the AATCC 30-2004 test method. Map samples were immersed in a chitosan solution (10 g/l) and evaluated using Scanning Electron Microscopy (SEM) with a Field Emission Scanning Electron Microscope (ULTRA 55, ZEISS). The process showed that, due to its antimicrobial effect, chitosan treatment is effective for removing external agents and microorganisms present on fibre surfaces. The technique is simple, efficient and the results indicated that chitosan can be transformed into a very good and cheap antimicrobial solution for the conservation and preservation of heritage objects.The research article was produced with equal scientific involvement of all authors.     Stress fracture of bilateral tibial metaphysis due to ceremonial march training: a case report

    Stress fractures are caused by repetitive microtraumas that occur during unusual or increased activities. Clinical suspicion is essential for the diagnosis. A twenty-years old soldier was presented with bilateral knee pain and restriction of knee movements after a period of training for ceremonial march. Although plain X-rays were normal, scintigraphy and MRI revealed stress fractures at metaphyseal region of both tibias. History of a patient presenting with persisting joint or bone pain after an unusual repetitive activity should be delicately inquired. Typical history, although pain might be localized to unusual sites, should raise the suspicion of a stress fracture

    Association between age at disease onset of anti-neutrophil cytoplasmic antibody-associated vasculitis and clinical presentation and short-term outcomes

    Objectives: ANCA-associated vasculitis (AAV) can affect all age groups. We aimed to show that differences in disease presentation and 6 month outcome between younger- A nd older-onset patients are still incompletely understood. Methods: We included patients enrolled in the Diagnostic and Classification Criteria for Primary Systemic Vasculitis (DCVAS) study between October 2010 and January 2017 with a diagnosis of AAV. We divided the population according to age at diagnosis: <65 years or ≥65 years. We adjusted associations for the type of AAV and the type of ANCA (anti-MPO, anti-PR3 or negative). Results: A total of 1338 patients with AAV were included: 66% had disease onset at <65 years of age [female 50%; mean age 48.4 years (s.d. 12.6)] and 34% had disease onset at ≥65 years [female 54%; mean age 73.6 years (s.d. 6)]. ANCA (MPO) positivity was more frequent in the older group (48% vs 27%; P = 0.001). Younger patients had higher rates of musculoskeletal, cutaneous and ENT manifestations compared with older patients. Systemic, neurologic,cardiovascular involvement and worsening renal function were more frequent in the older-onset group. Damage accrual, measured with the Vasculitis Damage Index (VDI), was significantly higher in older patients, 12% of whom had a 6 month VDI ≥5, compared with 7% of younger patients (P = 0.01). Older age was an independent risk factor for early death within 6 months from diagnosis [hazard ratio 2.06 (95% CI 1.07, 3.97); P = 0.03]. Conclusion: Within 6 months of diagnosis of AAV, patients >65 years of age display a different pattern of organ involvement and an increased risk of significant damage and mortality compared with younger patients

    Evaluation Of Patient Quality Of Life After Total Hip Arthroplasty

    Objectives: We evaluated the effect of total hip arthroplasty on patient quality of life. Methods: The study, included 30 patients (6 men, 24 women; mean age 62 years; range 36 to 82 years) undergoing total hip arthroplasty. The Medical Outcomes Study Short-Form 36-Item Health Survey (SF-36) was administered to the patients before and after 1.5 and 3 months of surgery. Special attention was given to provide the patients with sufficient information on surgery and postoperative rehabilitation program. Results: All the patients returned to their daily activities within six weeks postoperatively. SF-36 scores obtained after three months of surgery showed significant improvement in all SF-36 subscales compared to those obtained preoperatively and 1.5 months after surgery (p<0.05). Postoperative patient satisfaction was not correlated with sex, age, socioeconomic status, and education level of the patients. The presence of accompanying diseases or previous operations did not affect postoperative quality of life scores. Conclusion: Quality of life of patients increases substantially following total hip arthroplasty, with a corresponding increase in patient satisfaction.Wo

    Effects Of Exercise On Low Density Lipoprotein Receptor Related Protein 5 Gene Expression In Patients With Postmenopausal Osteoporosis

    Objectives: This study aims to investigate the effects of aerobic exercise on low density lipoprotein receptor related protein 5 (LRP5) gene messenger ribonucleic acid expression and evaluate the relationship between the clinical parameters and gene expression in patients with postmenopausal osteoporosis (OP). Patients and methods: Seven patients with postmenopausal OP (mean age 60.0 +/- 5.3 years; range 51 to 66 years) were included in the study. An exercise protocol/program consisting of treadmill exercising for 30 minutes three days a week for six weeks was performed at a moderate intensity. LRP5 gene expression levels were evaluated before the onset of the exercise program and then four hours after the end of the first session and 12th (fourth week) and 18th (sixth week) sessions of exercise. Results: Our results demonstrated variable changes in the LRP5 gene expression after the aerobic exercise sessions. Excluding one patient, the LRP5 gene expression levels showed a slight tendency to increase. In spite of this tendency, gene expression differences during the exercise sessions were not significant. Conclusion: Our results suggest that interindividual variations of LRP5 gene expression exist after moderate intensity aerobic exercises in patients with postmenopausal OP. Despite of this variability, LRP5 gene expression levels increased slightly, except in peripheral blood in one patient. Future studies with larger sample sizes and different sampling time/tissues are required to shed more light on the impact of exercise at molecular level in OP.WoSScopu

    Objectives: We evaluated the quality of life of female patients following total knee arthroplasty. Methods: The study included 50 women (mean age 67 years; range 52 to 84 years) who underwent bilateral total knee arthroplasty for primary osteoarthritis of the knee. All the patients were administered the Medical Outcomes Study Short Form-36 (SF-36) and the Knee Society Clinical Rating System (KSCRS) preoperatively and at six weeks, three and six months postoperatively. Particular attention was given to provide patients with sufficient information on surgery and postoperative rehabilitation program. Results: Compared to preoperative scores, all the subscales of both instruments showed significant improvement at six weeks and six months (p<0.05). After six weeks, however, only SF-36 physical function scores continued to improve significantly till the final assessment (p<0.05), whereas the other subscales reflected only maintenance of improvement. Conversely, a consistent significant improvement after six weeks was seen in the pain score of the KSCRS, while the function score representing only maintenance of improvement. Conclusion: Significant improvement is achieved in the quality of life of female patients within six weeks after total knee arthroplasty. It appears that, beyond six weeks, this improvement continues to be significant only in the physical function score of the SF-36 and pain score of the KSCRS.WoSScopu

    Comparison of GlideScope video laryngoscope and intubating laryngeal mask airway with direct laryngoscopy for endotracheal intubation

    The aim of this study was to determine whether GlideScope video laryngoscope (GVL) and intubating laryngeal mask airway (i-LMA) improve the intubation success rate and could be easily learned and performed by paramedic students when compared with the direct laryngoscopic (DL) method. The study was designed as a prospective randomized crossover trial that included 121 paramedic students. All participants were asked to intubate each Ambu Airway Management Trainer manikins after the lecture and demonstration. Successful intubation was defined as the passage of the tube through the vocal cord within 60 s. At the end of the study, a questionnaire survey was given to all participants about their preferences, and they were requested to define each method on an easy-difficult scale. Successful intubation was achieved by 95 students (78.5%) with DL, 112 students (92.6%) with i-LMA, and 111 students (91.7%) with GVL. Mean time of intubation was 25.06 +/- 14 s for DL, 22.32 +/- 12 s for i-LMA, and 22.63 +/- 10 s for GVL. Success rates of i-LMA and GVL were significantly higher compared with DL (P=0.005 and P=0.006, respectively). No significant difference was determined between i-LMA and GVL in terms of successful intubation (P>0.05). This study showed that GVL and i-LMA provided better intubation success rates and were easier for paramedic students when compared with the classic DL method.     Do ANCA-associated vasculitides and IgG4-related disease really overlap or not?

    Background: Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) and immunoglobulin G4-related disease (IgG4-RD) have some common features. The co-occurrence/concurrence of AAV and IgG4-RD was recently published by the collaborative European Vasculitis Study Group. First, we aimed to investigate ANCA positivity of our IgG4-RD cohort. Second, a literature review of co-occurrence/concurrence of AAV and IgG4-RD was done. Methods: Data of 62 patients with IgG4-RD in Hacettepe Vasculitis Center Database were used. Patient dataset was designed to include demographic data, clinical characteristics, imaging and IgG4-RD, AAV and ANCA test results. At the next step, we performed a systematic literature review in PUBMED database covering the time period from 1976 until April 2018. Relevant publications were searched using these MeSH terms ''IgG4-related disease and Anti-Neutrophil Cytoplasmic Antibody-Associated Vasculitis'', "IgG4-related disease and Eosinophilic Granulomatosis with Polyangiitis", "IgG4-related disease and Microscopic Polyangiitis" and "IgG4-related disease and Granulomatosis with Polyangiitis". Results: Three (10.3%) of 29 patients had low titer ANCA positivity. These three patients didn't have any findings of vasculitis and no granuloma was seen in biopsy. In the literature review, we found 17 cases had features of both IgG4-RD and AAV. These cases were re-evaluated according to the Comprehensive Diagnostic Criteria for IgG4-RD. ANCA were positive in 15 of 17 patients (88%). Conclusion: None of our IgG4-RD patients overlapped with AAV. Only two patients in the literature review seemed to be fully compatible with both diseases. Even though AAV and IgG4-RD share similar clinical features, we think this might be a co-occurrence instead of a histopathological link.PubMe
