    Scars of Nation: Surgical Penetration and the Ecuadorian State

    In Ecuador, middle‐class women, and increasingly more working‐class women, eagerly pay to be scarred. Cesarean sections carried out in private clinics leave a lateral scar—the mark of women not subject to the indignities of devalued public medical services. It is not citizenship per se that these women are after with their scars, since in Ecuador, citizenship, especially in the medical realm, is denigrated. Instead, the scar is a sign of a woman's ability to remain distinct from the governed masses who need to make citizenship claims for social services on state institutions. Scars and the bodies that carry them enact a racialized relationship to the nation. Browner bodies can withstand vaginal birth within the disciplines of public maternity care. When women pay for cesarean sections, the private scars make them whiter and more worthy of the nation.     WHAT GETS INSIDE: Violent Entanglements and Toxic Boundaries in Mexico City

    Entanglement is a key concept in contemporary anthropology and science and technology studies. By tracing the contingent and uncertain relations that endow objects with seemingly stable boundaries, entanglement allows us to see how such boundaries restrict our ability to know the world better. This article examines the concept of entanglement in the context of contemporary life in a working‐class Mexico City neighborhood, Colonia PerifĂ©rico, and a longitudinal environmental health project that studies the neighborhood’s residents. While entanglement has its uses, the entanglement of working‐class bodies with globalizing processes like NAFTA and the ongoing War on Drugs shows that the concept has its limits. For working‐class residents, life is already deeply entangled with chronic economic and political instability shaped through the violent ravages of transnational capital. Instead, I trace how residents in Colonia PerifĂ©rico secure stability through toxic boundaries that protectively keep out the disruptive effects of police and public health surveillance. Colonia PerifĂ©rico’s boundaries, which include a sewage‐filled dam, cement dust, and freeway exhaust, are clearly entangled with residents’ bodies. They get inside. These entanglements are the price paid for a remarkable stability, in which children can play on the streets and attentive care for drug‐addicted and disabled residents is part of everyday life.     Caretaker Government and the Evolution of Caretaker Conventions in New Zealand

    Since the financial crisis immediately following the 1984 general election, various efforts have been made to clarify the role and responsibilities of caretaker governments in New Zealand. The need to do so was given added urgency as a result of the referendum in 1993 in favour of proportional representation. This article examines the recent evolution of New Zealand's caretaker conventions and assesses their application following the first MMP election in late 1996. The article begins with a brief description of caretaker conventions in other parliamentary democracies. It then considers the operation of the caretaker conventions in New Zealand under the previous first-past-the-post electoral system, and discusses the measures taken in the early-to-mid 1990s to clarify these conventions in preparation for MMP. Having evaluated the conduct of government during the lengthy interregnum in late 1996, the article concludes with an analysis of some of the continuing policy issues generated by caretaker governments and outlines possible ways of reducing the frequency and duration of caretaker periods

    Changes in the redox state and endogenous fluorescence of in vivo human skin due to intrinsic and photo-aging, measured by multiphoton tomography with fluorescence lifetime imaging

    Ultraviolet radiation from solar exposure is a key extrinsic factor responsible for premature skin aging (i.e., photo-aging). Recent advances using in vivo multiphoton tomography (MPT) demonstrate the efficacy of this approach to assess intrinsic and extrinsic skin aging as an alternative to existing invasive techniques. In this study, we measured changes in epidermal autofluorescence, dermal collagen second harmonic generation (SHG), and the redox state of solar-exposed and solar-protected human skin by MPT with fluorescence lifetime imaging (MPT-FLIM). Twenty-four volunteers across four age categories (20 to 29, 30 to 39, 40 to 49, and 50 to 59 years old; six volunteers each) were recruited for MPT-FLIM imaging of the dorsal (solar-exposed; photo-damaged) and volar (solar-protected) forearm. We demonstrate a higher intensity of dermal collagen SHG within the volar forearm compared to dorsal solar-exposed skin. Redox imaging of each epidermal skin stratum by FLIM demonstrates an increase in fluorescence lifetime in the solar-exposed dorsal forearm that is more apparent in aged skin. The results of this study suggest the redox state of the viable epidermis is a key marker in assessing intrinsic and photo-damage skin aging, in combination with changes in autofluorescence and SHG

    Lessons learnt from prenatal exome sequencing

    BACKGROUND: Prenatal exome sequencing (ES) for monogenic disorders in fetuses with structural anomalies increases diagnostic yield. In England there is a national trio ES service delivered from two laboratories. To minimise incidental findings and reduce the number of variants investigated, analysis uses a panel of 1205 genes where pathogenic variants may cause abnormalities presenting prenatally. Here we review our laboratory's early experience developing and delivering ES to identify challenges in interpretation and reporting and inform service development. METHODS: A retrospective laboratory records review from 01.04.2020 to 31.05.2021. RESULTS: Twenty-four of 116 completed cases were identified as challenging including 13 resulting in difficulties in analysis and reporting, nine where trio inheritance filtering would have missed the diagnosis, and two with no prenatal diagnosis; one due to inadequate pipeline sensitivity, the other because the gene was not on the panel. Two cases with copy number variants identified were not detectable by microarray. CONCLUSIONS: Variant interpretation requires close communication between referring clinicians, with occasional additional examination of the fetus or parents and communication of evolving phenotypes. Inheritance filtering misses ∌5% of diagnoses. Panel analysis reduces but does not exclude incidental findings. Regular review of published literature is required to identify new reports that may aid classification

    Estimating uncertainty in density surface models

    This work was funded by OPNAV N45 and the SURTASS LFA Settlement Agreement, and being managed by the U.S. Navy’s Living Marine Resources program under Contract No. N39430-17-C-1982.Providing uncertainty estimates for predictions derived from species distribution models is essential for management but there is little guidance on potential sources of uncertainty in predictions and how best to combine these. Here we show where uncertainty can arise in density surface models (a multi-stage spatial modelling approach for distance sampling data), focussing on cetacean density modelling. We propose an extensible, modular, hybrid analytical-simulation approach to encapsulate these sources.     Levonorgestrel-releasing intrauterine system vs. usual medical treatment for menorrhagia: An economic evaluation alongside a randomised controlled trial

    Objective: To undertake an economic evaluation alongside the largest randomised controlled trial comparing Levonorgestrel-releasing intrauterine device ('LNG-IUS') and usual medical treatment for women with menorrhagia in primary care; and compare the cost-effectiveness findings using two alternative measures of quality of life. Methods: 571 women with menorrhagia from 63 UK centres were randomised between February 2005 and July 2009. Women were randomised to having a LNG-IUS fitted, or usual medical treatment, after discussing with their general practitioner their contraceptive needs or desire to avoid hormonal treatment. The treatment was specified prior to randomisation. For the economic evaluation we developed a state transition (Markov) model with a 24 month follow-up. The model structure was informed by the trial women's pathway and clinical experts. The economic evaluation adopted a UK National Health Service perspective and was based on an outcome of incremental cost per Quality Adjusted Life Year (QALY) estimated using both EQ-5D and SF-6D. Results: Using EQ-5D, LNG-IUS was the most cost-effective treatment for menorrhagia. LNG-IUS costs £100 more than usual medical treatment but generated 0.07 more QALYs. The incremental cost-effectiveness ratio for LNG-IUS compared to usual medical treatment was £1600 per additional QALY. Using SF-6D, usual medical treatment was the most cost-effective treatment. Usual medical treatment was both less costly (£100) and generated 0.002 more QALYs. Conclusion: Impact on quality of life is the primary indicator of treatment success in menorrhagia. However, the most costeffective treatment differs depending on the quality of life measure used to estimate the QALY. Under UK guidelines LNG-IUS would be the recommended treatment for menorrhagia.     The Momentum of Human Behavior in a Natural Setting

    Adults with mental retardation in a group home received popcorn or coffee reinforcers for sorting plastic dinnerware. In Part 1 of the experiment, reinforcers were dispensed according to a variable-interval 60-s schedule for sorting dinnerware of one color and according to a variable-interval 240-s schedule for sorting dinnerware of a different color in successive components of a multiple schedule. Sorting rates were similar in baseline, but when a video program was shown concurrently, sorting of dinnerware was more resistant to distraction when correlated with a higher rate of reinforcement. In Part 2 of the experiment, popcorn or coffee reinforcers were contingent upon sorting both colors of dinnerware according to variable-interval 60-s schedules, but additional reinforcers were given independently of sorting according to a variable-time 30-s schedule during one dinnerware-color component. Baseline sorting rate was lower but resistance to distraction by the video program was greater in the component with additional variable-time reinforcers. These results demonstrate that resistance to distraction depends on the rate of reinforcers obtained in the presence of component stimuli but is independent of baseline response rates and response-reinforcer contingencies. Moreover, these results are similar to those obtained in laboratory studies with pigeons, demonstrating that the determination of resistance to change by stimulus-reinforcer relations is not confined to controlled laboratory settings or unique to the pigeon

    Genetic Evidence for Selective Transport of Opsin and Arrestin by Kinesin-II in Mammalian Photoreceptors

    AbstractTo test whether kinesin-II is important for transport in the mammalian photoreceptor cilium, and to identify its potential cargoes, we used Cre-loxP mutagenesis to remove the kinesin-II subunit, KIF3A, specifically from photoreceptors. Complete loss of KIF3A caused large accumulations of opsin, arrestin, and membranes within the photoreceptor inner segment, while the localization of α-transducin was unaffected. Other membrane, organelle, and transport markers, as well as opsin processing appeared normal. Loss of KIF3A ultimately caused apoptotic photoreceptor cell death similar to a known opsin transport mutant. The data suggest that kinesin-II is required to transport opsin and arrestin from the inner to the outer segment and that blocks in this transport pathway lead to photoreceptor cell death as found in retinitis pigmentosa
