Glaucoma agudo de ângulo fechado, bilateral, desencadeado pelo topiramato: relato de caso
This paper describes a topiramate induced acute bilateral angle-closure glaucoma. This rare adverse effect is an idiosyncratic reaction characterized by uveal effusion and lens forward displacement, leading to increased intraocular pressure and vision loss. We describe a 55 year-old white woman with migraine, spasmodic torticollis and essential tremor, who developed bilateral acute angle-closure glaucoma, one week after starting topiramate 25 mg/day. She was seen at the Ophthalmology Emergency Department of the Fundação João Penido Burnier (Campinas, SP, Brazil) with a 4 hours history of blurry vision, ocular pain and bright flashes vision. Slit lamp examination revealed moderate conjunctival injection and corneal edema, and shallow anterior chambers. Intraocular pressure was 48 mmHg in both eyes. Fundoscopic examination findings were normal. She was treated with timolol, brimonidine, dorzolamide, pilocarpine, prednisone acetate eye drops and acetazolamide. One hour after those measures, as the intraocular pressure was 30 mmHg, she received a manitol intravenous injection and the intraocular pressure normalized. After 24 hours an iridotomy with Yag laser was performed. Topiramate was discontinued and she was totally recovered after one week.Relato de um caso de glaucoma bilateral de ângulo fechado induzido pelo topiramato. Este raro efeito colateral é uma idiosincrasia causada por efusão uveal e deslocamento do cristalino para frente, causando aumento da pressão intraocular e perda visual. Descrevemos o caso de uma paciente de 55 anos com migrânea, torcicolo espasmódico e tremor essencial, que desenvolveu glaucoma bilateral de ângulo fechado uma semana após iniciar o uso de topiramato, 25 mg/dia. A paciente foi atendida no setor de Emergências Oftalmológicas da Fundação Penido Burnier (Campinas, SP, Brasil), com história de 4 horas de embaçamento visual, dor ocular e visão de flashes brilhantes. O exame com lâmpada de fenda revelou injeção conjuntival moderada, edema corneano e câmara anterior rasa em ambos os olhos. A pressão intraocular era de 48 mmHg bilateralmente e a fundoscopia era normal. Foi tratada com colírios de timolol, brimonidina, dorzolamida, pilocarpina e acetato de prednisona e acetazolamida via oral. Uma hora após essas medidas, a pressão intraocular era 30 mmHg, e a paciente recebeu uma injeção intravenosa de manitol, ocorrendo normalização da pressão intraocular após essa medida. Após 24 horas foi realizada iridectomia com Yag laser. O topiramato foi interrompido e ela se recuperou totalmente após uma semana.484
Vascular Parkinsonism: Analysis Of Seven Cases.
Neuroimaging studies of elderly individuals reveal alterations in the white matter that are incompatible with the patients parkinsonism, mistakenly classified as vascular parkinsonism (VP). This study was conducted on a population composed of 20 patients with Parkinsons disease (PD) whose neuroimaging exams revealed vascular alterations in the white matter and seven patients with VP in order to compare diagnostic criteria. Age at disease onset of patients with PD was 55+/-12 years and patients with VP it was 62+/-13 years. Twelve patients with PD and five patients with VP presented arterial hypertension; three patients with VP and two patients with PD presented gait impairment; all patients with VP presented rigidity and bradykinesia, six of them presented resting tremor; 19 patients with PD presented tremor and 19 of them presented rigidity, while 17 presented bradykinesia. When the symptoms and evolution of both diseases were compared, the vascular alterations in the white matter were considered unspecific. Since clinical symptoms are unspecific, a differential diagnosis requires neuroimaging, good response to levodopa and clinical evolution.64568-7
Spasmodic torticollis and frontal meningioma
A 57 year-old man developed a spasmodic torticollis with involuntary deviation of the head to the right-side. He had a left paramedian frontal meningioma. The association of spasmodic torticollis and other movement disorders has been reported with contralateral lesions in the basal ganglia. Its occurrence in association to a frontal lesion appears to be much less frequent suggesting a possible disorder of frontostriatal connections.Foi estudada o caso de um paciente de 57 anos com meningeoma frontal esquerdo, cuja única manifestação clínica foi torcicolo espasmódico contralateral. Há poucos casos relatados na literatura de lesões frontais levando a movimentos involuntários, sendo analisada a possibilidade de uma desconexão funcional entrei o córtex frontal e o complexo estriato-lentículo-talâmico.10210
Laryngeal Electromyography In Movement Disorders: Preliminary Data.
This study describes preliminary laryngeal electromyography (LEMG) data and botulinum toxin treatment in patients with dysphonia due to movement disorders. Twenty-five patients who had been clinically selected for botulinum toxin administration were examined, 19 with suspected laryngeal dystonia or spasmodic dysphonia (SD), 5 with vocal tremor, and 1 with Gilles de la Tourette syndrome (GTS). LEMG evaluations were performed before botulinum toxin administration using monopolar electrodes. Electromyography was consistent with dystonia in 14 patients and normal in 5, and differences in frequency suggesting essential tremor in 3 and Parkinson tremors in 2. The different LEMG patterns and significant improvement in our patients from botulinum toxin therapy has led us to perform laryngeal electromyography as a routine in UNICAMP movement disorders ambulatory.62741-
Larynx electromyography: study of the diagnostic contribution in 30 patients carrying vocal fold immobility
Introdução : Electromyography (EMG) is a technique developed and used in neurology for diagnosis and prognostic definition of neuromuscular diseases. Study design: Clinical prospective. Material and method: Thirty (30) patients with vocal fold immobility have been grouped according to the diagnostic hypothesis clinically established. Results: EMGL diagnosed peripherical neuropathic injury, central neuropathic injury or fixation of the cricoarterytenoideous in all patients who presented vocal fold immobility with no defined cause. In those patients carrying vocal fold immobility on account of mechanical traumatic cause, clinically so defined, EMGL confirmed peripheral neuropathic injury in 70% of the cases and for the remaining 30% of the cases, it determined other causes such as peripheral neuropathic of compression, myopia and fixation of the cricoarterytenoideous. Conclusion: EMGL confirmed a chronic neuropathic injury in those patients carrying vocal fold immobility by virtue of compression caused by a possible clinically defined tumor.Introdução: A eletromiografia (EMG) é utilizada para diagnóstico e prognóstico de doenças neuromusculares. Objetivo: avaliar a contribuição da EMG no diagnóstico da imobilidade de prega vocal. Forma de estudo: Clínico prospectivo. Material e Método: Para avaliação da contribuição diagnóstica trinta pacientes com imobilidade de prega vocal foram divididos em três grupos de acordo com a hipótese diagnóstica firmada clinicamente e submetidos a exame eletromiográfico de laringe. Resultados: A EMGL diagnosticou lesão neuropática periférica, lesão neuropática central ou fixação cricoarinetóidea em todos os casos de prega vocal imóvel sem causa definida. Nos casos de prega vocal imóvel por trauma mecânico definido clinicamente, a EMGL confirmou lesão neuropática periférica em 70% dos casos, e determinou outra causa em 30% (neuropatia por compressão, miopatia e fixação cricoaritenóidea). Nos pacientes com prega vocal imóvel por possível compressão tumoral definida clinicamente, a EMGL confirmou lesão neuropática crônica. Conclusão: A EMGL contribui para a precisão do diagnóstico da imobilidade de prega vocal.36937
Prevalência e fatores associados aos sintomas urinários na doença de Parkinson
The authors present a cross-sectional study involving 61 patients with idiopathic Parkinson's disease (PD) who were consecutively examined and compared to a control group with 74 subjects. Only patients who fulfilled the standard diagnostic criteria for PD and whose brain magnetic resonance imaging was normal were included. The objective of the study was to evaluate the prevalence of inferior urinary tract symptoms in PD and to study the possible association between clinical factors to urinary dysfunction. ln the patient group, 39.3% presented urinary symptoms when compared to 10.8% in the control group. All symptomatic patients presented irritative symptoms. The most common irritative symptom PD was nocturia, followed by frequency and urinary incontinence. Around 25% of the patients presented functional obstructive symptoms determined by the disease. The most frequent obstructive symptom was incomplete emptying of the bladder. Only the age of the patients and control group were correlated with urinary dysfunction.Trata-se de estudo clínico transversal, envolvendo 61 pacientes com doença de Parkinson idiopática (DP) examinados consecutivamente e comparados a 74 indivíduos do grupo controle. Foram incluídos apenas pacientes com DP que preencheram os critérios de diagnóstico estabelecidos. A ressonância magnética do encéfalo e a ultra-sonografia da próstata foram utilizadas como critérios de exclusão. O objetivo do estudo foi avaliar a prevalência de sintomas do trato urinário inferior na DP e verificar associações entre sexo, idade e fatores clínicos às disfunções urinárias. Sintomatologia urinária foi encontrada em 39,3% dos parkinsonianos e 10,8% do grupo controle. Todos os pacientes sintomáticos apresentavam sintomas irritativos e 25% destes tinham sintomas obstrutivos. A sintomatologia urinária não estava associada ao sexo, duração e gravidade da doença, hipotensão postural e sintomas ortostáticos, assim como com os sinais cardeais e tratamento com a levodopa.35936
